Electrochemotherapy and heart function:Treatment in a patient with implantable cardioverter defibrillator/pacemaker

Electrochemotherapy(ECT) is a recently described therapy that relies on the permeation of cancer cell membranes by electrical pulses to enhance cytotoxic drug penetration. It has been successfully used in the treatment of primary and metastatic skin cancer. Systemic chemotherapy is the most commonly used therapeutic strategy, and the prevailing orientation calls for the administration of the maximum tolerated dose; however, considerable limitations exist including toxicities to healthy tissues and low achievable drug concentrations at tumor sites. We reported a case of an 83-years-old patient with a laterocervical metastasis of a squamous epidermoidal lip cancer. The patient had a complex medical history and an implantable cardioverter defibrillator(ICD)/pace-maker. The lesion was localized in the supraclavicular right side with a distance from the pace-maker/ICD about 5 cm, but the nodule was not deeply located. The ECT was performed un-der general anesthesia and particular attention we put on the interference with the functioning of the heart. The synchronization algorithm currently implemented in Clinoporator Vitae device coupled with the external triggering device Accu Sync proved to be effective in preventing external stimulation of the heart during the so-called vulnerable period of the ventricles. As a result all electroporation pulses in our study were delivered outside the vulnerable period and no heart arrhythmias or any other pathological morphological changes were observed. The safety of treatment was demonstrated also by absence of side effects during and after ECT.

Evolutionary Algorithm Based Approach for Modeling Autonomously Trading Agents

The autonomously trading agents described in this paper produce a decision to act such as: buy, sell or hold, based on the input data. In this work, we have simulated autonomously trading agents using the Echo State Network (ESNs) model. We generate a collection of trading agents that use different trading strategies using Evolutionary Programming (EP). The agents are tested on EUR/ USD real market data. The main goal of this study is to test the overall performance of this collection of agents when they are active simultaneously. Simulation results show that using different agents concurrently outperform a single agent acting alone.

Direct Experimental Evidence of the Statistical Nature of the Electron Gas in Superconducting Films

In an Nb film an alternate electrical current is partitioned at a Y-shaped obstacle into two splitted beams. The intensity-fluctuation correlation of the two beams (cross-correlation) and the intensity-fluctuation correlation of one beam (auto-correlation) are measured within a low-frequency bandwidth as a function of the incident beam intensity, at temperatures T above or below the temperature Tc of the superconductive transition. The results of these measurements reveal the statistical nature of the electron gas in the normal film and in the superconducting film. The conceptual scheme of the present experiment is a version of the Hanbury Brown and Twiss (HBT) experiment, here adopted for a gas of particles in a solid.

Phlogopite solid-solutions have a wide pressure-temperature(P-T)stability field and are ubiquitous in a wide variety of geological settings,from deep lithosphere magmatic environments to upper crust metamorphic *** composition represents therefore a valuable physical-chemical archive and may provide important information regarding its crystallization and the petrogenesis of the *** this paper we examine the phlogopite phenocrysts from the well-known Fort Regent mica-bearing lamprophyre minette from ***(Island of Jersey,UK).Phlogopite phenocrystals from lamprophyres generally show normal-step and continuous compositional zoning,however those from the Fort Regent minette show a peculiar texture characterized by dark brown high-Ti(average TiO_(2)≈8.5 wt.%)cores enveloped by euhedral low-to mid-amplitude zonation due to oscillatory contents in Ti,Fe and ***-barometry modelling based on biotite-only composition yields relatively high P-T estimates(T≈970±54℃at P≈0.73±0.13 GPa)for cores whereas lower values(T≈790±54℃at P≈0.29±0.13 GPa)are obtained for the outer *** temperatures(T≈1075±54℃)but extremely high and anomalous pressure values(P≈1.82±0.13 GPa)are obtained for the yellowish inner *** combination of electron micro probe(EMP)analysis and single-crystal infra-red(FTIR)imaging in the OH-stretching region shows that the exceptional and oscillatory Ti contents are due to the Ti-vacancy substitution,typical of crystallization and growth processes of HP/HT *** imaging provides additional insight for this process,confirming the dominant dioctahedral nature for the Ti-Fe-rich cores and outer *** of thermobaric estimates obtained from the phlogopite compositiononly model,based on the fine-scale compositional evolution,shows that pressure-temperature values from low-Ti high-Mg domains should be carefully evaluated because the substitution mechanisms during the dark mica growth are not univocal

Selecting which explanatory variables to include in a given score is a common difficulty, as a balance must be found between statistical fit and practical application. This article presents a methodology for constructing parsimonious event risk scores combining a stepwise selection of variables with ensemble scores obtained by aggregation of several scores, using several classifiers, bootstrap samples and various modalities of random selection of variables. Selection methods based on a probabilistic model can be used to achieve a stepwise selection for a given classifier such as logistic regression, but not directly for an ensemble classifier constructed by aggregation of several classifiers. Three selection methods are proposed in this framework, two involving a backward selection of the variables based on their coefficients in an ensemble score and the third involving a forward selection of the variables maximizing the AUC. The stepwise selection allows constructing a succession of scores, with the practitioner able to choose which score best fits his needs. These three methods are compared in an application to construct parsimonious short-term event risk scores in chronic HF patients, using as event the composite endpoint of death or hospitalization for worsening HF within 180 days of a visit. Focusing on the fastest method, four scores are constructed, yielding out-of-bag AUCs ranging from 0.81 (26 variables) to 0.76 (2 variables).

Feedback Control of Medication Delivery Device Using Machine Learning-Based Control Co-Design

Although the opioid crisis is a problem worldwide, recent emerging technology has the potential of curtailing the epidemic. By administering micro-doses of medication as needed, a feedback-driven medicine pump could lessen the highs and lows associated with the formation of an addiction. The focus of this study was to develop a feedback control loop for this pump that optimizes drug concentration in the bloodstream based on set criteria. In the process of optimization of the system, the mathematical model representing the system was analyzed to find an open loop transfer function. Using this function, a PID tuner was applied to set feedback control. Both machine learning (ML) and deep learning (DL) techniques are explored to act as a classifier that aids the pump in administering doses. The setpoint concentration of medication in the patient’s bloodstream was calculated to be 7.55 mg/ml this setpoint was the basis for steady state concentration of the transfer function. When a PID tuner was added to the feedback system, the plot was optimized to satisfy the design criteria of a rise time less than 25-minutes and no more than a 5% overshoot of the setpoint concentration. Naïve Bayesian (NB), Tree and support-vector machines (SVM) classifiers achieved the best classification accuracy of 100%. A DL network was successfully developed to predict patient class. This work is the theoretical basis for developing a feedback-driven medicine pump and an algorithm that can classify patients based on their body’s metabolism that will aid the doctor in formatting the medicine pump so that the patient is receiving the proper amount of medication.

A program of house dust sample collection and analysis has begun in Syracuse,New York,USA,in order to determine the feasibility of a geography-based exposure assessment for urban *** sampling program,and the protocols it employs,is described for two different types of wipe media,Ghost Wipes and Whatman *** results show that strong spatial patterns of floor dust loading(mg dust per square foot)can be observed for data aggregated at a spatial scale of about 1600 m(~2.5 km^(2)).Floor dust metal concentrations were similar to those found in other urban environments,with some regional *** median floor dust Pb concentration was~108 mg·kg^(-1)for this initial data set of~264 sampled residential locations,and varied from 50 to 1100 mg Pb·kg^(-1).
