Laparoscopic hysterectomy as optimal approach for 5400 grams uterus with associated polycythemia: A case report

BACKGROUND Uterine myomas are the most common benign gynecological soft tissue tumors in women but polycythemia associated with these tumors is *** is significantly associated with risk of venous thromboembolism(VTE),which is further increased in case of a large pelvic mass and *** we report the surgical challenges in the case of an obese patient with a giant fibromatous uterus and associated *** SUMMARY A 42-year-old obese woman was referred to our department for treatment for a huge fibromatous *** suffered of pelvic pressure and abdominal distension and reported severe dyspnea from several *** analyses including hemoglobin(Hb)19.2 g/dL and hematocrit(Hct)59.7%were indicative of *** blood gas analysis showed arterial oxygen pressure(pO2)of 81.5 *** computed tomography confirmed a giant fibromatous uterus measuring 28.2 cm×17 cm×25 *** reduce the thromboembolic risk,the patient underwent low molecular weight heparin,phlebotomy twice before surgery,and we opted for a laparoscopic *** uterus weighed 5400 g and the histology confirmed a diagnosis of *** did not find increased erythropoietin levels in the supernatant of the myomatous uterine *** were no *** postoperative day 1,the Hb and the Hct levels normalized to 13.3 g/dL and 41.7%,*** blood gas analysis after surgery showed a pO2 of 144.7 *** results suggested the diagnosis of myomatous erythrocytosis *** patient was discharged on the second postoperative day in very good condition with no *** We believe this is the first reported laparoscopic hysterectomy in a patient with a giant uterus,polycythemia and *** VTE risk factors warranted a laparoscopic approach.

Red wine and green tea reduce H pylori- or VacA-induced gastritis in a mouse model

AIM： To investigate whether red wine and green tea could exert anti-H pylori or anti-VacA activity in vivo in a mouse model of experimental infection. METHODS： Ethanol-free red wine and green tea concentrates were administered orally as a mixture of the two beverages to H pylori infected mice, or separately to VacA-treated mice. Gastric colonization and gastric inflammation were quantified by microbiological, histopathological, and immunohistochemical analyses.RESULTS： In H pylori-infected mice, the red wine and green tea mixture significantly prevented gastritis and limited the localization of bacteria and VacA to the surface of the gastric epithelium. Similarly, both beverages significantly prevented gastric epithelium damage in VacA-treated mice; green tea, but not red wine, also altered the VacA localization in the gastric epithelium. CONCLUSION： Red wine and green tea are able to prevent H pylori-induced gastric epithelium damage, possibly involving VacA inhibition. This observation supports the possible relevance of diet on the pathological outcome of Hpylori infection.

Absolute erythrocytosis,due to an increased production of red blood cells,becomes manifest with hemoglobin(Hb)levels above 165 or 160 g/L or with a hematocrit(HCT)above 51%and 48%in males and females,respectively.1 A minority of patients have polycythemia vera(PV);acquired secondary erythrocytosis frequently occurs due to appropriate or inappropriate erythropoietin(EPO)excess,while congenital secondary erythrocytosis may derive from genetic defects causing tissue *** erythrocytosis occurs in patients with genetic mutations of the oxygen-sensing pathway(VHL,PHD2,_HIF-1-alpha)or the erythropoietin receptor(EPOR)genes.2 Recently,HFE gene variants have been described in sporadic erythrocytoses.3 In a high proportion(about 70%)of patients with erythrocytosis,a specific etiology remains elusive despite extensive testing,and the diagnosis of idiopathic erythrocytosis(IE)is ruled out.

Rectal hepatoid carcinoma with liver metastases in a patient affected by ulcerative colitis

BACKGROUND:Hepatoid tumors(HTs)are rare extra-hepatic neoplasms with the histological features, biochemical profile and,sometimes,even clinical course of hepatocellular *** present a case of rectal hepatoid adenocarcinoma with metachronous liver metastases. METHODS:Four months after total procto-colectomy for a rectal adenocarcinoma(Astler-Coller C2),a 42-year-old man with ulcerative colitis showed hypoechoic masses in the hepatic parenchyma by abdominal ultrasonography. Carcinoembryonic antigen was normal,but alpha- fetoprotein was 32 000μg/***-needle biopsy revealed that liver masses were positive for hepatocellular carcinoma. The patient underwent left hepatectomy and alcoholisation of a small deep nodule in segment 8. RESULTS:Immunohistochemistry and albumin mRNA in situ hybridization suggested that the nodules were metastases of a *** patient was well during the first 6 months and refused any adjuvant *** died from liver failure 19 months after initial diagnosis. CONCLUSIONS:HT is a rare colon *** preoperative diagnosis of this tumor requires a high degree of suspicion,the availability of a panel of immunohistochemical markers,and a certain amount of *** prognosis is poor despite an aggressive andmultimodal therapeutic *** far,none of the hypotheses proposed about the origin and the biology of these tumors is convincing.

Recently,there has been a growing interest in integrated THz photonics for various applications in communications,spectroscopy and *** present a new integrated photonic platform based on active and passive elements integrated in a double-metal,high-confinement waveguide layout planarized with a low-loss *** extended top metallization keeps waveguide losses low while improving dispersion,thermal and RF properties,as it enables to decouple the design of THz and microwave ***-running on-chip quantum cascade laser combs spanning 800 GHz,harmonic states with over 1.1 THz bandwidth and RF-injected broadband incoherent states spanning over nearly 1.6 THz are observed using a homogeneous quantum-cascade active *** a strong external RF drive,actively mode-locked pulses as short as 4.4 ps can be produced,as measured by *** demonstrate as well passive waveguides with low insertion loss,enabling the tuning of the laser cavity boundary conditions and the cointegration of active and passive elements on the same THz photonic chip.
