Nutritional Status of Patients with Cutaneous Leishmaniasis from a Tropical Area of Bolivia, and Implications for Zinc Bioavailability

Macro and micronutrient deficiencies are a significant problem among people in rural areas in developing countries. Deficiencies may lead to an impaired immune system making the organism vulnerable to infections and diseases. In this paper, the dietary intake, anthropometric measurements, zinc and copper levels in serum, of patients with cutaneous leishmaniasis (CL) are compared with a group of healthy controls and reference values. Results showed no significant differences in most nutrient intake or anthropometrics between patients and controls. However, serum zinc level of patients (80 μg/dl) was significantly lower (P 0.001) than in controls (85 μg/dl), likely explained by the presence of leishmaniasis. The median serum zinc level in both groups was below the reference values, even though their median zinc intake met the zinc recommendations from WHO. Consequently, besides the presence of leishmaniasis, serum zinc levels may be compromised by inhibitory components in their diet, such as phytates, as it is shown by the molar ratio phytate:zinc (Phy:Zn) that was between 11 and 19, while 15 is the level said to compromise zinc status. There were significant (P 0.05) negative correlations between serum zinc and Phy:Zn, for patients (r = - 0.413) and controls (r = - 0.410). In conclusion this study shows that patients with CL in Chapare, Bolivia had low serum zinc levels due to the leishmaniasis per se and the decreased zinc bioavailability in their diet. CL infection was not in direct association with the nutritional status indicated by the anthropometric or dietary assessments. However, dietary intake showed 5 essential nutrients below the nutrient recommendation in both groups.

An easy,accurate and efficient procedure to create forest fire risk maps using the SEXTANTE plugin Modeler

To prevent, detect, and protect against forest fires, forest personnel need to define rules for determining forest fire risk. In Portugal, all municipalities must annually produce forest fire risk （FFR） maps. To produce more reliable FFR maps more easily, we developed an open source model using the Modeler plugin of SEXTANTE in the program QGIS version 2.0 Dufour. The model provides all the maps involved in the FFR model （susceptibility map, hazard map, vulnerability map, economic value map, and potential loss map） and was produced according to Portuguese Forest Authority＇s （AFN, Autoridade Florestal Nacional） rules for determining the FFR. This model was tested for the Portuguese municipality Santa Maria da Feira, where 40 % of the total municipality area falls in the category ＂very high＂ or ＂high＂ fire risk. The ＂very high＂ fire risk area is mainly classified as broad-leaved forest and has the steepest slopes （〉15 %）. The distance of burned areas to roads was also analyzed; the proportion of burned areas increased with increasing distance to the main roads. In addition, 92.6 % of the ＂high＂ and ＂very high＂ risk zones were located in areas with lower elevation. These results confirmed that forest fire is strongly influenced not only by environmental factors but also by anthropogenic factors. The procedure implemented here was compared with our open source application already available in QGIS and also to the same procedure implemented in GIS pro- prietary software. Although the results were obviously the same, the model developed here presents several advan- tages over the other two approaches. Besides being faster, it is easy to change the model parameters according to user needs （i.e., to the rules of different countries）, and can be modified and adapted to other variables and other areas to create risk maps for different natural phenomena （e.g., floods, earthquakes, landslides）. Using risk-benefit analysis to control Salmonella in chicken meat

We have created a risk-benefit analysis(RBA)model to assist in food safety decision-making by analyzing Salmonella control in Brazilian chicken ***,we described the issues in a risk profile and used a 5x5 matrix to rank the risks associated with *** then classified the magnitude of benefits and costs of control measures using another ***,we verified the beneficial effects of recommended control measures using Quantitative Microbiological Risk Assessment(QMRA).The RBA classified Salmonella contamination as risk 6,indicating that control measures should be taken in the short and medium *** also recommended the adoption of biosecurity measures on farms to reduce the prevalence of Salmonella in birds,better control of carcass washings and chiller tank management,and information placement on packages and campaigns to raise the awareness of the population about the need to control Salmonella contamination before *** the other hand,it did not recommend better controls at scalding and *** confirmed the beneficial effects of the recommended control *** example,as Salmonella prevalence in poultry increased from 4.04 per cent to 50 per cent,the risk of infection per serving also increased from 0.0080 to *** better controls in washings and chiller tank management did not affect the risk of infection,it reduced Salmonella counts on *** assume that the presence of Salmonella on carcasses was due to improper thermal processing or cross-contamination,which increased the risk from 0.0080 to *** RBA demonstrated the logic involved in the adoption of control measures,and this can be helpful in the risk management of food safety issues.

Cirrhosis in children and adolescents: An overview

Several conditions, especially chronic liver diseases, can lead to cirrhosis in children and adolescents. Most cases in clinical practice are caused by similar etiologies. In infants, cirrhosis is most often caused by biliary atresia and genetic-metabolic diseases, while in older children, it tends to result from autoimmune hepatitis,Wilson's disease, alpha-1-antitrypsin deficiency and primary sclerosing cholangitis. The symptoms of cirrhosis in children and adolescents are similar to those of adults. However, in pediatric patients, the first sign of cirrhosis is often poor weight gain. The complications of pediatric cirrhosis are similar to those observed in adult patients, and include gastrointestinal bleeding caused by gastroesophageal varices, ascites and spontaneous bacterial peritonitis. In pediatric patients, special attention should be paid to the nutritional alterations caused by cirrhosis, since children and adolescents have higher nutritional requirements for growth and development. Children and adolescents with chronic cholestasis are at risk for several nutritional deficiencies. Malnutrition can have severe consequences for both pre- and post-liver transplant patients. The treatment of cirrhosis-induced portal hypertension in children and adolescents is mostly based on methods developed for adults. The present article will review the diagnostic and differential diagnostic aspects of end-stage liver disease in children, as well as the major treatment options for this condition.

Anticardiolipin antibodies do not mediate macrovascular complications of type 2 diabetes

The relationship of anticardiolipin antibodies (ACA), markers of the antiphospholipid syndrome, with vascular complications of diabetes mellitus is polemic. This cross-sectional study assessed the frequency of IgG, IgM, and IgA ACA in type 2 diabetics with and without history of vascular events for the last 5 years, and in healthy controls. ACA were detected by enzyme immunoassay. A total of 73 type 2 diabetics (33 with history of vascular events) and 54 healthy controls were tested. Most diabetics were female (p = 0.003), and older than controls (p 0.09). ACA positivity rates were also similar when diabetics with and without history of vasculopathy were compared (p > 0.47). After adjusting for gender, age, hypertension, and smoking status, a weak but statistically insignificant association between IgM ACA and diabetics with vasculopathy was found (adjusted OR 2.7;95% CI 0.2 - 34.2;p = 0.441). Overall, levels of IgG (r = 0.25;p = 0.005) and IgM (r = 0.23;p = 0.010) ACA were associated with increasing age. In short, the frequency of a positive ACA test in type 2 diabetics (with or without previous macrovasculopathy) was not significant as compared to healthy controls. There was no association of ACA with vascular events in patients with type 2 diabetes.

Antidepressant Drugs Modulate Differentially Anti-inflammatory Lithium＇s Property： An in Vitro and in Vivo Study

Li （lithium）, a mood stabilizer has anti-inflammatory effect. However, in clinical practice, Li can be administered together with other antidepressants drugs, such as FLX （fluoxetine）, IMI （imipramine）, NOR （nortriptyline） and ESC （escitalopram）. As interaction between Li and these antidepressant drugs on inflammatory modulation has not been investigated yet, we performed an in vitro protocol using a non-human macrophage cell line. Oxidative and inflammatory markers, as well as cell cycle analysis and cytokine gene expressions were compared among treatments. An IR （inflammatory ratio） was calculated based on the following oxidative-inflammatory variables： nitric oxide, superoxide anion, reactive oxygen molecules, cytokines IL-113, IL-6, TNF-ct and IL-10. The in vitro calculated IR data were validated through an in vivo analysis of 154 human subjects with similar IR. Li and control cells presented similar IR values. FLX, NOR and IMI increased slightly IR values indicating some proinflammatory effect, whereas ESC decreased IR values indicating some anti-inflammatory effect. However, cells exposed to Li ＋ ESC triggered a proinflammatory response on macrophages. Thus, IR comparison results suggest that the Li anti-inflammatory effect is not universal and could be influenced by both basal macrophage-inflammatory state and interaction of other psychiatric drugs. These results could be useful to understand some inconsistencies observed in human studies involving Li and other psychiatric drugs.
