在过去的十年中,我们对于肺癌生物学和治疗的认识取得了显著进步。识别肺癌发生的关键驱动事件对其靶向治疗的发展作出了贡献,预示着肺癌的个性化医疗时代的到来。因此,病理分型和分子检测变得至关重要,而日益增加的检测需求往往是基于小的诊断标本。这使得国际肺癌研究学会/美国胸科学会/欧洲呼吸学会[The International association for the study of cancer(IASLC)/American thoracic society(ATS)/european respiratory society(eRS)]对小活检/细胞学肺癌标本的第一次结构分类进行了审视和推动,并且提出了肺腺癌的新分类。这些都提高了病理诊断的临床相关性,强调了现代外科病理学家作为多学科团队中成员的作用,在肺癌患者的临床试验和确定适当和及时的治疗中发挥了决定性作用。

Quantification of the expression of chitinolytic enzyme encoding genes ech30, ech42 and nag1 in Trichoderma atroviride P1 under varying growth conditions using a real-time RT-PCR assay

The quantitative expression and the regulation of chitinase-encoding genes ech30, ech42 and nag1 in Trichoderma atroviride P1 under varying growth conditions were investigated using real-time RT-PCR, principle component and multivariate analyses. Twelve media combinations including 0.1% and 3% glucose as carbon source and no (0 mmol/L), low (10 mmol/L) and high (100 mmol/L) ammonium acetate as nitrogen source combined with or without colloidal chitin at 3 time intervals and 2 replications were applied to current study. The real-time RT-PCR analysis showed that the expression of ech30, ech42 and nag1 was regulated by the interaction of nitrogen, glucose and chitin under different growth conditions. The highest and earliest expressions of ech30 were induced by glucose and nitrogen starvation i.e. 0.1% glucose and 10 mmol/L ammonium acetate in the growth media. This was also the case for ech42 and nag1 but at a relatively low level. In contrast, high (3%) glucose and high (100 mmol/L) ammonium acetate concentrations repressed the expression of all the genes studied. These results were confirmed by principle component and multivariate analyses. The effect of chitin on ech30, ech42 and nag1 expression varied depending on the concentrations of glucose and ammonium acetate.

Hydropower production is one.o. the greatest threats to fluvial e.o.ystems and freshwater *** that we have entered the Anthropocene,there is an opportunity to reflect on what might constitute a‘sustainable’Anthropocene in the context of hydropower and riverine fish *** elements of existing practices that promote favorable social-e.o.ogical outcomes(i.e.,‘bright spots’)is timely given that there are plans to expand hydropower capacity in previously undammed rivers,intensify dam deve.o.ment in some.o. the world's largest river systems,and re-license existing *** approach this from a pragmatic perspective:for the foreseeable future,hydropower will likely remain an important source.o. renewable *** offer support for moving toward a more‘sustainable’Anthropocene,we provide syntheses of best practices during the siting,design,construction,operation,and compensation phases of hydropower deve.o.ment to minimize impacts on inland *** each phase,we.o.fer positive examples(or what might be considered‘bright spots’)pertaining to some.o. the approaches described within our syntheses,acknowledging that these projects may not be viewed as without e.o.ogical and(or)societal detriment by all *** findings underscore the importance.o. protecting critical habitat and free-flowing river reaches through careful site selection and basinscale planning,infrastructure designs that minimize reservoir effects and facilitate safe passage.o. fish,construction of hydropower plants using best practices that minimize long-term damage.o.erating guidelines that mimic natural flow conditions,and compensation that is lasting,effective,inclusive,and locally *** from these‘bright spots’may require engagement of diverse stake.o.ders,professionals,and governments at scales that extend well be.o.d a given site,river,or even ***,environmental planning that integrates hydropower deve.o.ment into broader discussions

输血对消化道出血患者预后的影响

目的：消化道出血患者在我医疗中心常规先进行诊断/治疗性内镜检查；如抢救患者需输血6个单位以上则考虑外科手术治疗，本研究的目的在于研究消化道出血患者输血单位数与临床预后的关系。方法：本研究地1996年1月至2000年1月第81医疗组收治的消化道出血患者的资料进行了回顾性分析，在本研究中，全部病例均明确诊断上消化道或下消化道出血，且均接受了至少1个单位的红细胞成分输血。结果：本研究共分析35份完整的病历，病人年龄3岁-79岁，男女比例为4：1，平均输入压缩红细胞量为5.2个单位，8例患者输入量超过6个单位；4例接受外科手术治疗的病例中3例需输入量超过6个单位，这4个病例均属ASA分级的Ⅲe级。全部35例患者中7例死亡（20%），其中3例输入量超过6个单位。结论：消化道出血患者需输入量少于6个单位者死亡率（15%）低于超过6个单位的患者（38%），急诊外科手术治疗即使对于高危患者也是比较安全的。

目前,人类婴儿低初生重的总肠外营养(TPN)研究表明脂肪乳剂对于新生儿的营养是合适的,但不是最佳的选择。人类和猪的脂肪酸代谢非常相似,目前的研究主要集中在脂肪乳剂如何在TPN的临床作用。不同组成脂肪酸对新生仔猪进行7d处理,对于靠母乳替代物的仔猪而言,对组织脂肪酸的组成存在影响。在脂肪乳中,不同比例脂肪酸或母乳替代物对应的血浆、骨骼肌、皮下脂肪、肝脏、心脏、胰腺、胃及小肠的总血脂或脑磷脂呈正态的线性关系,而后者相关性最低。除大脑外,组织中的油酸或α-亚麻酸的比例与对应的脂肪乳或者母乳替代物相关,而亚麻酸的比例与研究的组织密切相关。除脑磷脂外,omegaven组中仔猪组织的二十碳五烯酸和二十二碳六烯酸含量更高。结论:除大脑外,血浆脂肪酸组成以及仔猪不同组织受全肠外营养乳剂中不饱和脂肪酸形态的影响极大。然而,脑磷脂脂肪酸的组成受日粮组成的影响要小,表明在生长的关键阶段有效的代谢是确保脂肪酸组成的关键。
