Nonopioid perioperative analgesia in head and neck cancer surgery:A systematic review

Objective:Management of postoperative pain after head and neck cancer surgery is a complex issue,requiring a careful balance of analgesic properties and side *** objective of this review is to discuss the efficacy and safety of multimodal analgesia(MMA)for these ***:Pubmed,cochrane,Embase,Scopus,and *** were systematically searched for all comparative studies of patients receiving MMA(nonsteroidal anti-inflammatory drugs(NSAIDs),acetaminophen,anticonvulsants,local anesthetics,and corticosteroids)for head and neck cancer *** primary outcome was additional postoperative opioid usage,and secondary outcomes included subjective pain scores,complications,adverse effects,and 30-day ***:A total of five studies representing 592 patients(MMA,n=275;non-MMA,n=317)met inclusion *** most commonly used agents were gabapentin,NSAIDs,and acetaminophen(n=221),NSAIDs(n=221),followed by corticosteroids(n=35),dextromethorphan(n=40),and local nerve block(n=19).Four studies described a significant decrease in overall postoperative narcotic usage with two studies reporting a significant decrease in hospital *** pain scores widely varied with two studies reporting reduced pain at postoperative day *** were no differences in surgical outcomes,medical complications,adverse effects,or 30-day mortality and readmission ***:MMA is an increasingly popular strategy that may reduce dependence on opioids for the treatment of postoperative pain.A variety of regimens and protocols are available for providers to utilize in the appropriate head and neck cancer patient.

A review of multi-chamber oscillating water column(OWc)device designs is *** significant variations of these devices are discussed,onshore OWc(OOWc)and a floating OWc(FOWc).The efficiency results of several theoretical studies based on low-and high-fidelity numerical models are presented and compared with the model scale ***,low-fidelity numerical models are very fast to run,but their accuracy is limited compared with high-fidelity numerical *** model experiments usually give results much more accurate than numerical models,but they need adequate facilities and are very *** the case of the OOWc,all models show a similar trend of total efficiency,but while the analytical model shows a maximum value of around 90%efficiency,the cFD model shows 60%,and the experiments only go up to 40%.The main reason is connected with the mathematical simplifications and assumptions that do not represent all the hydrodynamic and aerodynamic processes between the water,air,and *** the case of the FOWc,interestingly,the experimental results show a maximum efficiency of almost 100%,while the analytical model only predicts a maximum of 80%.The efficiency seems highly dependent on the heave motion resonance of the entire device,where the analytical model fails to predict this natural frequency.

Purpose:This study aimed to determine the impact of uttering the word"quiet"on clinical workload during the overnight otolaryngology call shift and understand the factors contributing to resident *** and Methods:A multicenter,single-blind,randomized-controlled trial was conducted.A total of 80 overnight call shifts covered by a pool of 10 residents were randomized to the quiet or to the control *** the start of shift,residents were asked to state aloud,"Today will be a quiet night"(quiet group)or"Today will be a good night"(control group).clinical workload,as measured by number of consults,was the primary *** measures included number of sign-out tasks,unplanned inpatient and operating room visits,number of phone calls and hours of sleep,and self-perceived ***:There was no difference in the number of total(P=0.23),nonurgent(P=0.18),and urgent(P=0.18)*** at signout,total phone calls,unplanned inpatient visits,and unplanned operating room visits did not differ between the control and quiet *** there were more unplanned operating room visits in the quiet group(29,80.6%)compared to the control group(34,94.4%),this was not found to be significant(P=0.07).The majority of residents reported feeling"not busy"during control nights(18,50.0%)compared to feeling"somewhat busy"during quiet nights(17,47.2%;P=0.42).conclusion:contrary to popular belief,there is no clear evidence that uttering the word"quiet"significantly increases clinical workload.

Annual forage legumes are important components of livestock production systems in East Texas and the southeastern US. Forage legumes contribute nitrogen (N) to cropping systems through biological N fixation, and their seasonal biomass production can be managed to complement forage grasses. Our research objectives were to evaluate both warm- and cool-season annual forage legumes as green manure for biomass, N content, ability to enhance soil organic carbon (SOc) and soil N, and impact on post season forage grass crops. Nine warm-season forage legumes (WSL) were spring planted and incorporated as green manure in the fall. Forage rye (Secale cereale L.) was planted following the incorporation of WSL treatments. Eight cool-season forage legumes (cSL) were fall planted in previously fallow plots and incorporated as green manure in late spring. Sorghum-sudangrass (Sorghum bicolor x Sorghum bicolor var. sudanense) was planted over all treatments in early summer after forage rye harvest and incorporation of cSL treatments. Sorghum-sudangrass was harvested in June, August and September, and treatments were evaluated for dry matter and N concentration. Soil cores were taken from each plot, split into depths of 0 to 15, 15 to 30 and 30 to 60 cm, and soil c and N were measured using combustion analysis. Nylon mesh bags containing plant samples were buried at 15 cm and used to evaluate decomposition rate of above ground legume biomass, including change in c and N concentrations. Mungbean (Vigna radiata L. [Wilczek]) had the highest shoot biomass yield (6.24 t DM ha-1) and contributed the most total N (167 kg∙ha-1) and total c (3043 kg∙ha-1) of the WSL tested. Decomposition rate of WSL biomass was rapid in the first 10 weeks and very slow afterward. Winter pea (Pisum sativum L. spp. sativum), arrow leaf clover (Trifolium vesiculosum Savi.), and crimson clover (Trifolium incarnatum L.) were the most productive cSL in this trial. Before the advent of cheap, synthetic fertilizers, legumes were commonly used as green manure crops for their ability to fix atmospheric nitrogen (N). A three-year study at Overton, TX examined legume integration into high-biomass sorghum (Sorghum bicolor L.) production systems on a Lilbert loamy fine sand recently cultivated after a fertilized bermudagrass [cynodon dactylon (L.) Pers.] pasture. In this split-split plot design, ‘Dixie’ crimson clover (Trifolium incarnatum L.) and ‘Iron and clay’ cowpea (Vigna unguiculata L.) were integrated into a high-biomass sorghum production system to evaluate impacts on N concentration, c concentration, and yield of high-biomass sorghum and their impacts on soil total N and soil organic carbon (SOc). Main plots were split into crimson clover green manure (cLGM) and winter fallow (FALL) followed by three sub-plots split into warm-season crop rotations: cowpea green manure (cPGM), cowpea-sorghum intercrop (cPSR), and sorghum monocrop (SORG). Three N fertilizer treatments (0, 45, 90 kg N∙ha−1) were randomized and applied as sub-sub plots. The cLGM increased (P sorghum biomass yield (16.5 t DM∙ha−1) 28% in year three but had no effect in the first two years. The cPSR treatment reduced sorghum yield up to 62% compared to SORG;whereas cPGM increased sorghum yield 56% and 18% the two years following cowpea incorporation. Rate of N fertilizer had no effect on sorghum biomass yield. Decrease in SOc and soil N over time indicated mineralization of organic N and may explain why no N fertilizer response was observed in sorghum biomass yield. cowpea showed strong potential as a green manure crop but proved to be too competitive for successful intercropping in high-biomass sorghum production systems.
