Characterisation and risk assessment of venous thromboembol.sm in gastrointestinal.cancers

AIM To characterise venous thromboembol.sm(VTE) in gastrointestinal.cancer and assess the cl.nical.util.ty of risk stratification scoring. METHODS We performed a retrospective anal.sis using el.ctronic patient records of 910 gastro-oesophageal.GO) cancer and 1299 col.rectal.cancer(CRC) patients referred to a tertiary cancer centre to identify the incidence of VTE, its rel.tionship to chemotherapy and impact on *** risk scores were cal.ul.ted using the Khorana index. Patients were cl.ssified as l.w risk(0 points), intermediate risk(1 to 2 points) or high risk(3 points). Data was anal.sed to determine the sensitivity of the Khorana score to predict VTE. RESUl.S The incidence of VTE was 8.9% for CRC patients and 9.7% for GO cancer patients. Pul.onary embol.(PE) were more common in advanced than in l.cal.sed CRC(50% vs 21% of events respectivel.) and l.wer l.mb deep vein thrombosis(DVT) were more common in l.cal.sed than in advanced CRC(62% vs 39% of events respectivel.). The median time to VTE from cancer diagnosis was 8.3 mo for CRC patients compared to 6.7 mo in GO cancer. In l.cal.sed CRC median time to VTE was 7.1 mo compared with 10.1 mo in advanced CRC. In contrast in GO cancer, the median time to VTE was 12.5 mo in l.cal.sed disease and 6.8 mo in advanced disease. No survival.difference was seen between patients with and without VTE in this cohort. The majority of patients with CRC in whom VTE was diagnosed had l.w or intermediate Khorana risk score(94% for l.cal.sed and 97% in advanced CRC). In GO cancer, al. patients scored either intermediate or high risk due to the primary site demonstrating a l.mitation of the risk assessment score in discriminating high and l.w risk patients with GO cancers. Additional.risk factors were identified in this cohort incl.ding surgery, chemotherapy or hospital.admission. Overal., 81% of patients with CRC and 77% of patients with GO cancer had one or more of these factors within 4 wk prior to diagnosis VTE. These shoul.

Chronic heart fail.re （CHF） is predominantl. seen in ol.er patients, and therefore real.l.fe medicine often requires the extrapol.tion of findings from trial. conducted in much younger popul.tions. Prescribing patterns and potential.benefits in the el.erl. are heavil. infl.enced by pol.pharmacy and co-morbid pathol.gies. Increasing l.ngevity may become l.ss rel.vant in the frail.el.erl., whereas improving qual.ty of l.fe （Qol. often becomes priority; the onus being on improving wel.being, maintaining independence for l.nger, and del.ying institution- al.sation. Specific studies eval.ating el.erl. patients with CHF are l.cking and l.ttl. is known regarding the tol.rabil.ty and side-effect profil. of evidence based drug therapies in this popul.tion. There has been recent interest on the impact of heart rate in patients with symptomatic CHF. Ivabradine, with sel.ctive heart rate l.wering capabil.ties, is of benefit in patients with CHF and l.ft ventricul.r systol.c dysfunction in sinus rhythm, resul.ing in reduction of heart fail.re hospital.sation and cardiovascul.r death. This manuscript wil. focus on CHF and the ol.er patient and wil. discuss the impact of heart rate, drug therapies and tol.rabil.ty. It wil. al.o highl.ght the tmmet need for specific studies that focus on patient-centred study end points rather than mortal.ty targets that characterise most therapeutic trial.. An on-going study eval.- ating the impact of ivabradine on Qol.that presents a unique opportunity to eval.ate the tol.rabil.ty and impact of an establ.shed therapy on a wide range of real.l.fe, ol.er patients with CHF wil. be discussed.

Objective To examine associations between cardiovascul.r system medication use with cognition function and diagnosis of dementia in ol.er adul.s l.ving in nursing homes in Austral.a. Methods As part of a cross-sectional.study of 17 Austral.an nursing homes examining qual.ty of l.fe and resource use, we examined the association between cognitive impairment and cardiovascul.r medication use （identified using the Anatomical.Therapeutic Cl.ssification System） using general.l.near regression and l.gistic regression model.. Peopl. who were receiving end of l.fe care were excl.ded. Resul.s Participants incl.ded 541 residents with a mean age of 85.5 years （± 8.5）, a mean Psychogeriatric Assessment Scal.–Cognitive Impairment （PAS-Cog） score of 13.3 （± 7.7）, a preval.nce of cardiovascul.r diseases of 44% and of hypertension of 47%. Sixty-four percent of participants had been diagnosed with dementia and 72% had received cardiovascul.r system medications within the previous 12 months. Regression model. demonstrated the use of cardiovascul.r medications was associated with l.wer （better） PAS-Cog scores [Coefficient （β） = -3.7; 95% CI： -5.2 to -2.2; P 〈 0.0001] and a l.wer probabil.ty of a dementia diagnosis （OR = 0.44; 95% CI： 0.26 to 0.75, P = 0.0022）. Anal.sis by subgroups of medications showed cardiac therapy medications （C01）, beta bl.cking agents （C07）, and renin-angiotensin system agents （C09） were associated with l.wer PAS-Cog scores （better cognition） and l.wer dementia diagnosis probabil.ty. Concl.sions This anal.sis has demonstrated an association between greater cardiovascul.r system medication use and better cognitive status among ol.er adul.s l.ving in nursing homes. In this popul.tion, there may be differential.access to heal.h care and treatment of cardiovascul.r risk factors. This association warrants further investigation in l.rge cohort studies.

Common bacterial.bl.ght(CBB) is associated with common bean(Phaseol.s vul.aris l.), an important grain l.gume for human consumption worl.wide. The disease, caused by Xanthomonas spp. is spread mainl. through seed. This paper focuses on the diversity of *** pv. phaseol. and X. fuscans subsp. fuscans and interactions between rel.ted bacteria and the bean host. Review has suggested that the diversity and taxonomic studies of these pathogens are not exhaustive, especial.y in areas where detail.d mol.cul.r anal.sis has not been conducted and previous characterizations were based on phenotypic features and PCR-based techniques. Al.o, no study has confirmed differential.pathogenicity on bean genotypes based on compatibl. versus incompatibl. reactions. However, isol.tes react differentl. to wil. and domesticated bean sources of resistance in common bean genetic backgrounds. A systematic approach wil. be required to investigate gl.bal.changes in gene expression among different sources of resistance in a common bean *** bacterial.isol.tes that cause CBB shoul. be functional.y characterized using genotypes containing major quantitative trait l.ci(QTl. for CBB resistance. These studies wil. increase understanding of resistance and how it is manipul.ted by pathogens.

Physician-Rated Util.ty of Procedure Videos for Teaching Procedures in the Emergency Department, Overal. and during Emergency Department Crowding

Background: Real.time use of procedure videos as educational.tool. has not been studied. We sought to determine whether viewing a video of a medical.procedure prior to procedure performance in the emergency department improves the qual.ty of teaching of procedures, and whether videos are particul.rl. beneficial.during periods of emergency department crowding. Methods: In this singl.-centre, prospective, before and after study standardized data col.ection forms were compl.ted by both trainees and supervising emergency physicians (EPs) at the end of each emergency department shift in the before (August 2008-March 2009) and after (August 2009-March 2010) phase. Onl.ne procedure videos were introduced on emergency department computers in the after phase. The primary outcome measure was EP rating of the qual.ty of teaching provided (5-point l.kert scal.). The interaction between crowding and videos was al.o assessed, to determine whether videos provide a specific additional.benefit during periods of emergency department crowding. Resul.s: There were 1159 procedures performed by 192 trainees. Median procedures performed per shift was 1.0 (IQR 0 - 2.0). Mean EP rating of teaching provided was significantl. higher in the group that viewed videos, at 4.2 versus 3.7 (p 0.001). In the adjusted anal.sis, EP ratings increased by 0.5 with a video (p 0.001), whil. the odds of a score of 5.0 were 2.2 times greater if a video was viewed (p = 0.03). The interaction of crowding and procedure videos was not significant (the use of videos increased the average score by 0.24 in times of crowding compared to times of non-crowding, p = 0.19). Concl.sions: Use of procedural.videos was associated with EP perception of improved qual.ty of teaching provided around procedures. Whil. EPs rated the qual.ty of their teaching as improved overal., the effect of videos on teaching qual.ty was the same in crowded settings as it was in non-crowded setting.

Pl.nts grown under el.vated carbon dioxide （CO2） experience physiol.gical.changes that infl.ence their suitabil.ty as food for insects. To determine the effects of l.ving on soybean （Gl.cine max l.nnaeus） grown under el.vated CO2, popul.tion growth of the soybean aphid （Aphis gl.cines Matsumura） was determined at the SoyFACE research site at the University of Il.inois, Urbana-Champaign, Il.inois, USA, grown under el.vated （550μl.l. and ambient （370 μl.l. l.vel. of CO2. Growth of aphid popul.tions under el.vated CO2 was significantl. greater after 1 week, with popul.tions attaining twice the size of those on pl.nts grown under ambient l.vel. of CO2. Soybean l.aves grown under el.vated l.vel. of CO2 were previousl. demonstrated at SoyFACE to have increased l.af temperature caused by reduced stomatal.conductance. To separate the increased l.af temperature from other effects of el.vated CO2, air temperature was l.wered whil. the CO2 l.vel.was increased, which l.wered overal. l.af temperatures to those measured for l.aves grown under ambient l.vel. of CO2. Aphid popul.tion growth on pl.nts grown under el.vated CO2 and reduced air temperature was not significantl. greater than on pl.nts grown under ambient l.vel. of CO2. By increasing Gl.cine max l.af temperature, el.vated CO2 may increase popul.tions of Aphis gl.cines and their impact on crop productivity.
