Fetal lung growth predicts the risk for early-life respiratory infections and childhood asthma

Background early-life respiratory infections and asthma are major health burdens during childhood.Markers predicting an increased risk for early-life respiratory diseases are sparse.Here,we identified the predictive value of ultrasound-monitored fetal lung growth for the risk of early-life respiratory infections and asthma.Methods Fetal lung size was serially assessed at standardized time points by transabdominal ultrasound in pregnant women participating in a pregnancy cohort.Correlations between fetal lung growth and respiratory infections in infancy or early-onset asthma at five years were examined.Machine-learning models relying on extreme gradient boosting regressor or classifier algorithms were developed to predict respiratory infection or asthma risk based on fetal lung growth.For model development and validation,study participants were randomly divided into a training and a testing group,respectively,by the employed algorithm.Results enhanced fetal lung growth throughout pregnancy predicted a lower early-life respiratory infection risk.Male sex was associated with a higher risk for respiratory infections in infancy.Fetal lung growth could also predict the risk of asthma at five years of age.We designed three machine-learning models to predict the risk and number of infections in infancy as well as the risk of early-onset asthma.The models’R2 values were 0.92,0.90 and 0.93,respectively,underscoring a high accuracy and agreement between the actual and predicted values.Influential variables included known risk factors and novel predictors,such as ultrasound-monitored fetal lung growth.Conclusion Sonographic monitoring of fetal lung growth allows to predict the risk for early-life respiratory infections and asthma.

Background Tuberculosis is one of the most significant infectious diseases for global public health.The reallocation of healthcare resources and the restrictions imposed by the COVID-19 pandemic have hindered access to TB diag-nosis and treatment.Increases in unfavorable outcomes of the disease have been observed in Brazil.The objective of this study was to analyze the spatial distribution of unfavorable TB treatment outcomes in Brazil before and during the pandemic.Methods An ecological study with spatial analysis was conducted with all 5569 municipalities in Brazil.All reported cases of tuberculosis between January 2010 and December 2021,as well as reported cases of COVID-19 from Febru-ary 2020 to December 2021,were included.The outcomes studied encompass loss to follow-up,drug-resistant tuber-culosis,and death.The Getis Ord GI*technique was employed to assess spatial association,and the Kernel density estimator was used to identify areas with concentrated increases or decreases in outcomes.Bivariate Local Moran's/was used to examine the spatial association between outcomes and cOVID-19 incidence.The study was approved by the Research ethics Committee of Ribeirao Preto Nursing School,University of Sao Paulo.Results There were 134,394 cases of loss to follow-up,10,270 cases of drug resistance,and 37,863 deaths.Clusters of high and low values were identified for all three outcomes,indicating significant changes in the spatial distribu-tion patterns.Increases in concentrations were observed for lost to follow-up cases in the Southeast,while reductions occurred in the Northeast,South,and Midwest.Drug-resistant tuberculosis experienced an increase in the Southern and Southeastern regions and a decrease in the Northeast and South.TB-related deaths showed notable concentra-tions in the Midwest,Northeast,South,and Southeast.There was an increase in high occurrence clusters for deaths after 2020 and 2021 in the Northeast.Conclusions The pandemic has brought additional challenges,emphasizing the importance of enhancing e

Neurodegenerative and neuropsychiatric diseases are increasingly affecting individuals’quality of life,thus increasing their cost to social and health systems.These diseases have overlapping mechanisms,such as oxidative stress,protein aggregation,neuroinflammation,neurotransmission impairment,mitochondrial dysfunction,and excitotoxicity.Currently,there is no cure for neurodegenerative diseases,and the available therapies have adverse effects and low efficacy.For neuropsychiatric disorders,such as depression,the current therapies are not adequate to one-third of the patients,the so-called treatmentresistant patients.So,searching for new treatments is fundamental.Medicinal plants appear as a strong alternative and complement towards new treatment protocols,as they have been used for health purposes for thousands of years.Thus,the main goal of this review is to revisit the neuroprotective potential of some of the most predominant medicinal plants(and one fungus)used in traditional Chinese medicine(TCM),focusing on their proven mechanisms of action and their chemical compositions,to give clues on how they can be useful against neurodegeneration progression.

The increasing share of renewable energy in the electricity grid and progressing changes in power consumption have led to fluctuating,and weather-dependent power flows.To ensure grid stability,grid operators rely on power forecasts which are crucial for grid calculations and planning.In this paper,a Multi-Task Learning approach is combined with a Graph Neural Network(GNN)to predict vertical power flows at transformers connecting high and extra-high voltage levels.The proposed method accounts for local differences in power flow characteristics by using an embedding Multi-Task Learning approach.The use of a Bayesian embedding to capture the latent node characteristics allows to share the weights across all transformers in the subsequent node-invariant GNN while still allowing the individual behavioral patterns of the transformers to be distinguished.At the same time,dependencies between transformers are considered by the GNN architecture which can learn relationships between different transformers and thus take into account that power flows in an electricity network are not independent from each other.The effectiveness of the proposed method is demonstrated through evaluation on two real-world data sets provided by two of four German Transmission System Operators,comprising large portions of the operated German transmission grid.The results show that the proposed Multi-Task Graph Neural Network is a suitable representation learner for electricity networks with a clear advantage provided by the preceding embedding layer.It is able to capture interconnections between correlated transformers and indeed improves the performance in power flow prediction compared to standard Neural Networks.A sign test shows that the proposed model reduces the test RMSe on both data sets compared to the benchmark models significantly.

Background Fanconi-Debré-de Toni syndrome(also known as Fanconi renotubular syndrome,or FRST)profoundly increased the understanding of the functions of the proximal convoluted tubule(PCT)and provided important insights into the pathophysiology of several kidney diseases and drug toxicities.Data sources We searched Pubmed and Scopus databases to find relevant articles about FRST.This review article focuses on the physiology of the PCT,as well as on the physiopathology of FRST in children,its diagnosis,and treatment.Results FRST encompasses a wide variety of inherited and acquired PCT alterations that lead to impairment of PCT reab-sorption.In children,FRST often presents as a secondary feature of systemic disorders that impair energy supply,such as Lowe's syndrome,Dent's disease,cystinosis,hereditary fructose intolerance,galactosemia,tyrosinemia,Alport syndrome,and Wilson's disease.Although rare,congenital causes of FRST greatly impact the morbidity and mortality of patients and impose diagnostic challenges.Furthermore,its treatment is diverse and considers the ability of the clinician to identify the correct etiology of the disease.Conclusion The early diagnosis and treatment of pediatric patients with FRST improve the prognosis and the quality of life.

Background:Age-related changes in brain structure may constitute the starting point for cerebral function alteration.Physical activity(PA)demonstrated favorable associations with total brain volume,but its relationship with cortical thickness(CT)remains unclear.We investigated the cross-sectional associations between PA level and CT in community-dwelling people aged 70 years and older.Methods:A total of 403 older adults aged 74.8±4.0 years(mean±SD)who underwent a baseline magnetic resonance imaging examination and who had data on PA and confounders were included.PA was assessed with a questionnaire.Participants were categorized according to PA levels.Multiple linear regressions were used to compare the brain CT(mm)of the inactive group(no PA at all)with 6 active groups(growing PA levels)in 34 regions of interest.Results:Compared with inactive persons,people who achieved PA at a level of 1500-1999 metabolic equivalent task-min/week(i.e.,about6-7 h of brisk walking for exercise) and those who achieved it at 2000-2999 metabolic equivalent task-min/week(i.e.,8-11 h of brisk walking for exercise)had higher CT in the fusiform gyrus and the temporal pole.Additionally,dose-response associations between PA and CT were found in the fusiform gyrus(B=0.011,Se=0.004,adj.p=0.035),the temporal pole(B=0.026,Se=0.009,adj.p=0.048),and the caudal middle frontal gyrus,the entorhinal,medial orbitofrontal,lateral occipital,and insular cortices.Conclusion:This study demonstrates a positive association between PA level and CT in temporal areas such as the fusiform gyrus,a brain region often associated to Alzheimer’s disease in people aged 70 years and older.Future investigations focusing on PA type may help to fulfil remaining knowledge gaps in this field.

Objective:Robot-assisted partial nephrectomy(RAPN)has become widely used for treatment of renal cell carcinoma and it is expanding in the field of complex renal masses.The aim of this systematic review was to analyze outcomes of RAPN for completely endophytic renal masses,large tumors(cT2-T3),renal cell carcinoma in solitary kidney,recurrent tumors,completely endophytic and hilar masses,and simultaneous and multiple tumors.Methods:A comprehensive search in the PubMed,Scopus,Web of Science,and Cochrane Central Register of Controlled Trials databases was performed in December 2022 for english language papers.The primary endpoint was to evaluate the role of RAPN in the setting of each category of complex renal masses considered.The secondary endpoint was to evaluate the surgical and functional outcomes.Results:After screening 1250 records,43 full-text manuscripts were selected,comprising over 8500 patients.Twelve and thirteen studies reported data for endophytic and hilar renal masses,respectively.Five and three studies reported outcomes for cT2-T3 and solitary kidney patients,respectively.Four studies focused on redo-RAPN for recurrent tumors.Two studies investigated simultaneous bilateral renal masses and five reports focused on multiple tumor excision in ipsilateral kidney.Conclusion:Over the past decade,evidence supporting the use of RAPN for the most challenging nephron-sparing surgery indications has continuously grown.Although limitations remain including study design and lack of detailed long-term functional and oncological outcomes,the adoption of RAPN for the included advanced indications is associated with favorable surgical outcomes with good preservation of renal function without compromising the oncological result.Certainly,a higher likelihood of complication might be expected when facing extremely challenging cases.However,none of these indications should be considered per se an exclusion criterion for performing RAPN.Ultimately,a risk-adapted approach should be employed.

Background This study aimed to estimate vaccine effectiveness(Ve)against omicron variant infection and severe corona virus disease 2019(COVID-19)in children aged 5–11 years hospitalized with acute respiratory syndrome.Methods A test-negative,case–control analysis was conducted from February 2022 to June 2022.We enrolled 6950 eligible children,including 1102 cases and 5848 controls.Ve was calculated after immunization with one and two doses of BNT162b2 or CoronaVac.The outcomes were hospitalization with acute respiratory symptoms and detection of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection and severe COVID-19.The adjusted odds ratio for the association of prior vaccination and outcomes was used to estimate Ve.Results For fully vaccinated children,the overall estimated Ve against hospitalization with SARS-CoV-2 infection was 42%[95%confidence interval(CI)26 to 54].Ve peaked at 29–42 days(67%,95%CI 40%to 82%)and then declined to 19%(95%CI,−20%to 45%)at 57–120 days after the second dose.The BNT162b2 vaccine had a similar Ve against hospitalization with SARS-CoV-2 infection(45%,95%CI,20 to 61)compared to the CoronaVac vaccine(40%,95%CI,17%to 56%).Among cases,56(5%)children died;53(94.6%)were not fully vaccinated.For cases,the two-dose schedule effectiveness against ICU admission,need for invasive ventilation,severe illness,and death were 10%(95%CI,−54%–45%),22%(95%CI−70%–68%),12%(95%CI,−62%–52%),and 16%(95%CI,−77%–75%),respectively.Conclusions For hospitalized children aged 5–11 years during the omicron-predominant period in Brazil,two doses of both vaccines had moderate effectiveness against hospitalization with acute respiratory symptoms and SARS-CoV-2 infection and offered limited protection against endpoints of COVID-19 severity.
