Prevention of hepatitis B reactivation in patients with hematologic malignancies treated with novel systemic therapies:Who and Why?

The risk of reactivation in patients with chronic or past/resolved hepatitis B virus(HBV)infection receiving chemotherapy or immunosuppressive drugs is a wellknown possibility.The indication of antiviral prophylaxis with nucleo(t)side analogue is given according to the risk of HBV reactivation of the prescribed therapy.Though the advent of new drugs is occurring in all the field of medicine,in the setting of hematologic malignancies the last few years have been characterized by several drug classes and innovative cellular treatment.As novel therapies,there are few data about the rate of HBV reactivation and the decision of starting or not an antiviral prophylaxis could be challenging.Moreover,patients are often treated with a combination of different drugs,so evaluating the actual role of these new therapies in increasing the risk of HBV reactivation is difficult.First results are now available,but further studies are still needed.Patients with chronic HBV infection[hepatitis B surface antigen(HBsAg)positive]are reasonably all treated.Past/resolved HBV patients(HBsAg negative)are the actual area of uncertainty where it could be difficult choosing between prophylaxis and pre-emptive strategy.

The effects of data aggregation on long-term projections of forest stands development

Forest management planning often relies on Airborne Laser Scanning(ALS)-based Forest Management Inventories(FMIs)for sustainable and efficient decision-making.Employing the area-based(ABA)approach,these inventories estimate forest characteristics for grid cell areas(pixels),which are then usually summarized at the stand level.Using the ALS-based high-resolution Norwegian Forest Resource Maps(16 m×16 m pixel resolution)alongside with stand-level growth and yield models,this study explores the impact of three levels of pixel aggregation(standlevel,stand-level with species strata,and pixel-level)on projected stand development.The results indicate significant differences in the projected outputs based on the aggregation level.Notably,the most substantial difference in estimated volume occurred between stand-level and pixel-level aggregation,ranging from-301 to+253 m^(3)·ha^(-1)for single stands.The differences were,on average,higher for broadleaves than for spruce and pine dominated stands,and for mixed stands and stands with higher variability than for pure and homogenous stands.In conclusion,this research underscores the critical role of input data resolution in forest planning and management,emphasizing the need for improved data collection practices to ensure sustainable forest management.

Assessment of groundwater quantity, quality, and associated health risk of the Tano river basin, Ghana

In the Tano River Basin,groundwater serves as a crucial resource;however,its quantity and quality with regard to trace elements and microbiological loadings remain poorly understood due to the lack of groundwater logs and limited water research.This study presents a comprehensive analysis of the Tano River Basin,focusing on three key objectives.First,it investigated the aquifer hydraulic parameters and the results showed signiﬁcant spatial variations in borehole depths,yields,transmissivity,hydraulic conductivity,and speciﬁc capacity.Deeper boreholes were concentrated in the northeastern and southeastern zones,while geological formations,particu-larly the Apollonian Formation,exhibit a strong inﬂuence on borehole yields.The study identiﬁed areas with high transmissivity and hydraulic conductivity in the southern and eastern regions,suggesting good groundwater avail-ability and suitability for sustainable water supply.Sec-ondly,the research investigated the groundwater quality and observed that the majority of borehole samples fall within WHO(Guidelines for Drinking-water Quality,Environmental Health Criteria,Geneva,2011,2017.http://www.***.int)limit.However,some samples have pH levels below the standards,although the groundwater generally qualiﬁes as freshwater.The study further explores hydrochemical facies and health risk assessment,highlighting the dominance of Ca–HCO3 water type.Trace element analysis reveals minimal health risks from most elements,with chromium(Cr)as the primary contributor to chronic health risk.Overall,this study has provided a key insights into the Tano River Basin’s hydrogeology and associated health risks.The outcome of this research has contributed to the broader understanding of hydrogeologi-cal dynamics and the importance of managing groundwater resources sustainably in complex geological environments.

Intelligent Energy Utilization Analysis Using IUA-SMD Model Based Optimization Technique for Smart Metering Data

Smart metering has gained considerable attention as a research focus due to its reliability and energy-efficient nature compared to traditional electromechanical metering systems. Existing methods primarily focus on data management,rather than emphasizing efficiency. Accurate prediction of electricity consumption is crucial for enabling intelligent grid operations,including resource planning and demandsupply balancing. Smart metering solutions offer users the benefits of effectively interpreting their energy utilization and optimizing costs. Motivated by this,this paper presents an Intelligent Energy Utilization Analysis using Smart Metering Data(IUA-SMD)model to determine energy consumption patterns. The proposed IUA-SMD model comprises three major processes:data Pre-processing,feature extraction,and classification,with parameter optimization. We employ the extreme learning machine(ELM)based classification approach within the IUA-SMD model to derive optimal energy utilization labels. Additionally,we apply the shell game optimization(SGO)algorithm to enhance the classification efficiency of the ELM by optimizing its parameters. The effectiveness of the IUA-SMD model is evaluated using an extensive dataset of smart metering data,and the results are analyzed in terms of accuracy and mean square error(MSE). The proposed model demonstrates superior performance,achieving a maximum accuracy of65.917% and a minimum MSE of0.096. These results highlight the potential of the IUA-SMD model for enabling efficient energy utilization through intelligent analysis of smart metering data.

Background Early-life respiratory infections and asthma are major health burdens during childhood.Markers predicting an increased risk for early-life respiratory diseases are sparse.Here,we identified the predictive value of ultrasound-monitored fetal lung growth for the risk of early-life respiratory infections and asthma.Methods Fetal lung size was serially assessed at standardized time points by transabdominal ultrasound in pregnant women participating in a pregnancy cohort.Correlations between fetal lung growth and respiratory infections in infancy or early-onset asthma at five years were examined.Machine-learning models relying on extreme gradient boosting regressor or classifier algorithms were developed to predict respiratory infection or asthma risk based on fetal lung growth.For model development and validation,study participants were randomly divided into a training and a testing group,respectively,by the employed algorithm.Results Enhanced fetal lung growth throughout pregnancy predicted a lower early-life respiratory infection risk.Male sex was associated with a higher risk for respiratory infections in infancy.Fetal lung growth could also predict the risk of asthma at five years of age.We designed three machine-learning models to predict the risk and number of infections in infancy as well as the risk of early-onset asthma.The models’R2 values were 0.92,0.90 and 0.93,respectively,underscoring a high accuracy and agreement between the actual and predicted values.Influential variables included known risk factors and novel predictors,such as ultrasound-monitored fetal lung growth.Conclusion Sonographic monitoring of fetal lung growth allows to predict the risk for early-life respiratory infections and asthma.

Variations in Soil Organic Matter Content in Cultivated and Uncultivated Calcareous Soils from the Mediterranean Island of Malta after 15 Years of Cultivation

The soils of Malta are calcareous and generally undeveloped. Organic matter (OM) in these soils is low and farmers are constantly urged to increase it. The objective of this study was to evaluate any temporal variation in soil OM after 15 years of cultivation, and determine whether soil series, soil depth, and cultivation influence variation. OM was determined in the topsoil and subsoil of 7 agricultural and 4 non-agricultural sites. The sites represented 7 different soil series that are present on the island. In sampling periods 1 (t = 0 years) and 2 (t =15 years), the OM content in the collective (all soil series) bulk (topsoil and subsoil) uncultivated soil was 3.9 % and 3.8 % respectively. This was significantly greater than that of the collective bulk cultivated soil (2.4% and 2.3%). The OM in the collective uncultivated topsoil was 5.4% and 5.2% in periods 1 and 2 and was significantly higher than that of the cultivated topsoil (2.5% in both periods). The OM content in the collective uncultivated subsoil was 2.3% and 2.5% in periods 1 and 2 respectively but only that measured in period 2 was significantly higher than that of the cultivated subsoil (2.2% in both periods). On an individual soil series basis, the OM in the uncultivated topsoils was significantly higher than that of their cultivated counterparts. The differences in the subsoils were not significant. Across the uncultivated soil series, OM was significantly higher in the topsoil than in the subsoil but in the cultivated soil series the differences between topsoil and subsoil were not significant. There was no significant difference in OM between the uncultivated soils of different series, but in the cultivated the OM content was higher in soils that were more mature. After 15 years, no significant change in OM occurred in both the collective cultivated and uncultivated bulk soils, the collective topsoil and subsoil, and in most of the individual series. Background Tuberculosis is one of the most significant infectious diseases for global public health.The reallocation of healthcare resources and the restrictions imposed by the COVID-19 pandemic have hindered access to TB diag-nosis and treatment.Increases in unfavorable outcomes of the disease have been observed in Brazil.The objective of this study was to analyze the spatial distribution of unfavorable TB treatment outcomes in Brazil before and during the pandemic.Methods An ecological study with spatial analysis was conducted with all 5569 municipalities in Brazil.All reported cases of tuberculosis between January 2010 and December 2021,as well as reported cases of COVID-19 from Febru-ary 2020 to December 2021,were included.The outcomes studied encompass loss to follow-up,drug-resistant tuber-culosis,and death.The Getis Ord GI*technique was employed to assess spatial association,and the Kernel density estimator was used to identify areas with concentrated increases or decreases in outcomes.Bivariate Local Moran's/was used to examine the spatial association between outcomes and cOVID-19 incidence.The study was approved by the Research Ethics Committee of Ribeirao Preto Nursing School,University of Sao Paulo.Results There were 134,394 cases of loss to follow-up,10,270 cases of drug resistance,and 37,863 deaths.Clusters of high and low values were identified for all three outcomes,indicating significant changes in the spatial distribu-tion patterns.Increases in concentrations were observed for lost to follow-up cases in the Southeast,while reductions occurred in the Northeast,South,and Midwest.Drug-resistant tuberculosis experienced an increase in the Southern and Southeastern regions and a decrease in the Northeast and South.TB-related deaths showed notable concentra-tions in the Midwest,Northeast,South,and Southeast.There was an increase in high occurrence clusters for deaths after 2020 and 2021 in the Northeast.Conclusions The pandemic has brought additional challenges,emphasizing the importance of enhancing e

Objective To investigate the in vivo immunomodulatory and anti-tumor mechanisms of the combined treatment of novel Four-Herb formula(4HF)and doxorubicin in triple-negative breast cancer(TNBC).Methods Murine-derived triple-negative mammary carcinoma cell line,4T1 cells,was cultured and inoculated into mouse mammary glands.Sixty-six mice were randomly assigned into 6 groups(n=11 in ench):naive,control,LD 4HF(low dose 4HF),HD 4HF(high dose 4HF),LD 4HF+D(low dose and doxorubicin),and D(doxorubicin).Apart from the naive group,each mouse received subcutaneous inoculation with 5×10^(5)4T1 cells resuspended in 100µL of normal saline in the mammary fat pads.Starting from the day of tumor cell inoculation,tumors were grown for 6 days.The LD and HD groups received daily oral gavage of 658 and 2,630 mg/kg 4HF,respectively.The LD 4HF+D group received daily oral gavage of 658 mg/kg 4HF and weekly intraperitoneal injection of doxorubicin(5 mg/kg).The D group received weekly intraperitoneal injections of doxorubicin(5 mg/kg).The treatment naive mice received daily oral gavage of 0.2 mL double distilled water and 0.1 mL normal saline via intraperitoneal injection once a week.The control group received daily oral gavage of 0.2 mL double-distilled water.The treatment period was 30 days.At the end of treatment,mice organs were harvested to analyze immunological activities via immunophenotyping,gene and multiplex analysis,histological staining,and gut microbiota analysis.Results Mice treated with the combination of 4HF and doxorubicin resulted in significantly reduced tumor and spleen burdens(P<0.05),altered the hypoxia and overall immune lymphocyte landscape,and manipulated gut microbiota to favor the anti-tumor immunological activities.Moreover,immunosuppressive genes,cytokines,and chemokines such as C-C motif chemokine 2 and interleukin-10 of tumors were significantly downregulated(P<0.05).4HF-doxorubicin combination treatment demonstrated synergetic activities and was most effective in activating the anti-tumor immune response(P<

Cosmetic or Dietary Vegetable Oils Sampled in the Cameroonian Market May Not Expose Consumers to Lipid Oxidation Products Generating Oxidative Stress and Inflammation

Vegetable oils are a source of energy, essential fatty acids, antioxidants and fat-soluble vitamins useful for human health care and development. These oils also contribute to organoleptic quality of their products’ derivatives. However, their chemical and physical properties can be modified by the mode of their extraction, storage and distribution. These modifications might negatively affect the nutritional quality of the oils. The goals of this study were to: sample different vegetable oils for cosmetic or dietary use marketed in Cameroon, and verify purity and oxidation states of each kind of oil through determination of its acidity, iodine, peroxide, saponification, refractive indexes and the conformity of the labeling. The carotene content, the level of polar components and specific absorbance were also determined. As the result, six oils namely palm, palm kernel, coconut, black cumin, peanut and shea butter were collected. Apart from labeling, chemicals and physicals parameters analyzed were generally in accordance with the Cameroonian and Codex Alimentarius standard. This study suggests that vegetable oils sampled in the Cameroonian market may not expose consumers to lipid oxidation products generating pathological oxidative stress and inflammation. However, efforts in application of existing standard need to be done as far as labeling are concerned.

Exclusive breastfeeding greater than 50%, success of education in a university hospital in Bogotá: Case-control study

BACKGROUND Maintenance rates of exclusive breastfeeding(EBF)worldwide are low,thus,one of the objectives of the summary of policies on breastfeeding(BF)in world nutrition goals for 2025 are that at least 50%of infants under six months of age receive EBF that year.The Objective of this study is to document the rates of EBF in children born in San Ignacio University Hospital(HUSI)and identify factors associated with maintenance.AIM To document the percentages of EBF in those that were born at HUSI and identify factors associated to their maintenance.METHODS This is a study of cases and controls in an analytic,retrospective cohort that took children born alive between January 2016 and January 2019 at HUSI located in the city of Bogotá,Colombia.RESULTS Receiving information about BF at HUSI was able to maintain EBF up until 4 mo(OR=1.65;95%CI:1.02-2.66).The presence of gynecologic and obstetric comorbidities(OR=0.32;95%CI:0.12-0.83),having mastitis(OR=0.56;95%CI:0.33-0.94),and receiving information from mass media(OR=0.52;95%CI:0.31-0.84)are factors associated with not maintaining EBF.CONCLUSION Receiving education at a Women-and Child-Friendly Institution was the only significant factor to achieve EBF until 4 mo,with a frequency greater than the one reported in the country,which matches multiple studies where counseling and individualized support on BF achieve this purpose.Knowledge about BF and early detection of obstetric/gynecologic complications must be strengthened among the healthcare staff in charge of mothers during post-partum.Additionally,strategies must be promoted to continue BF such as creating milk banks with the objective of increasing BF rates even when mothers return to work.
