Cost-Effectiveness Analysis of Renalof® versus Extracorporeal Shockwave Lithotripsy (ESWL) for the Treatment of Kidney Stones ≤ 1 cm in Nicaragua

Objectives: To assess the efficiency in terms of cost-effectiveness (CE) of oral Renalof® treatment versus extracorporeal shockwave lithotripsy (ESWL) in the treatment of kidney stones ≤ 1 cm in Nicaragua. Methods: A cost-effectiveness economic evaluation was carried out based on the results obtained in the randomised, prospective, observational, single-blind, prospective, phase 2 clinical trial. Cost-effectiveness and the incremental cost-effectiveness ratio (ICER) were calculated. Economic data were obtained from the Economics Department of clínica Senior in Managua, Nicaragua. The monetary cost was expressed in US dollars (USD). Results: Treatment with Renalof® yielded a CE of $1,323.08/% remission, while ESWL was $9,498.54/% remission. The ICER shows that, in order to achieve a high percentage of kidney stone remission with ESWL, an extra $4,734.70 per patient must be invested. Conclusions: The use of Renalof® is shown to be a more cost-effective option than ESWL. It is recommended for the treatment of kidney stones ≤ 1 cm in size.

Study of borohydride ionic liquids as hydrogen storage materials

Stability of borohydrides is determined by the localization of the negative charge on the boron *** liquids(ILs) allow to modify the stability of the borohydrides and promote new dehydrogenation pathways with a lower activation energy. The combination of borohydride and IL is very easy to realize and no expensive rare earth metals are required. The composite of the ILs with complex hydrides decreases the enthalpy and activation energy for the hydrogen desorption. The Coulomb interaction between borohydride and IL leads to a destabilization of the materials with a significantly lower enthalpy for hydrogen desorption. Here, we report a simple ion exchange reaction using various ILs, such as vinylbenzyltrimethylammonium chloride([VBTMA][cl]), 1-butyl-3-methylimidazolium chloride([bmim][cl]), and 1-ethyl-1-methylpyrrolidinium bromide([EMPY][Br]) with NaBH4 to decrease the hydrogen desorption temperature. Dehydrogenation of 1-butyl-3-methylimidazolium borohydride([bmim][BH4]) starts below 100℃. The quantity of desorbed hydrogen ranges between 2.4 wt% and 2.9 wt%, which is close to the theoretical content of hydrogen. The improvement in dehydrogenation is due to the strong amine cation that destabilizes borohydride by charge transfer.

Undernutrition in Adults Hospitalized Patients: Magnitude and Associated Factors at the University Hospital of Bobo-Dioulasso

Background: Despite numerous advances in medicine and clinical care, in developing countries, adult undernutrition is overlooked and not considered as a sufficient medical priority. The objective of this study was to identify the magnitude of adult undernutrition and its associated factors in hospitalized patients. Methods: Cross-sectional study, performed in the university hospital in Bobo-Dioulasso, from August to September 2016, in a sample of 244 adult individuals. Patients were included in the study consecutively to their admission. A structured questionnaire was applied consisting of anthropometric, clinical and dietary data. The patients were evaluated and dichotomized into undernourished and non-undernourished. The rate of undernutrtion was reported using and associated factors were tested using the unadjusted crude Odds ratios (COR) with 95% confidence interval (95% CI) and the Chi-square test. Results: Undernutrition occurred in 32.4% of the sample and at the univariate analysis, the variables associated with undernutrition were: recent and involuntary weight loss (COR = 2.58, 95% CI = 1.45 - 4.59, p Conclusion: Adult undernutrition is common in hospitalized patients. A proper early diagnosis and therapy need to be considered as medical priority of the entire healthcare team that treats adult’s inpatients.

EVOLUTION OF THE ELECTROCHEMICAL CHARACTERISTICS DURING PITTING CORROSION OF PURE ALUMINUM IN SODIUM CHLORIDE SOLUTION

The corrosion behavior of pure aluminum in neutral 3.0% （mass fraction） sodium chloride （Na-cl） solution has been studied using electrochemical impedance spectroscopy （EIS） measurement in conjunction with the scanning electron microscopy （SEM） technique. EIS information on the evolution of pitting corrosion over a period of 15 days has been obtained and analyzed with equivalent circuit technique. The results shown that, during the ensemble constant immersion time, two time constants involved, the high frequency one originated from the protective layer on the corroding surface while the low frequenO, one from the diffusion process or the corrosion reaction and so on. And there existed a period for oxide film to growth and thickening prior to the of the attacking of chloride ions to the substrate. Meanwhile, good relationship between EIS and the material corrosion type/severity has been obtained, which has been interpreted according to the characteristics of corrosion process such as auto acceleration of pitting corrosion and the protection of local anodic reaction to the area around them.

Reducing damage extent of epoxy coating on magnesium substrate by zr-enhanced PEO coating as an effective pretreatment

This research was undertaken to study the effect of zr-enhanced plasma electrolytic oxidation(PEO) as a pretreatment on the corrosion performance of epoxy coating applied on Magnesium in 3.5 wt.% Na cl solution. The parameters of delamination index along with coating damage index were extracted through electrochemical impedance spectroscopy(EIS) tests to determine how zr may affect the corrosion protection of duplex PEO/epoxy coated samples. Pull-off adhesion tests were also accomplished to form a better understanding of zr-enhanced PEO coating’ function. According to the obtained results, the presence of zr can reduce the damage to the coating system by almost twice.

The molten eutectic mixture of magnesium,sodium and potassium chlorides(Mg cl2–Na cl–Kcl) has inappreciable solubility for oxide ions,and can help disengage a carbon anode from the oxide ions generated at a metal oxide cathode,and effectively avoid carbon dioxide *** "disengaging strategy" was successfully demonstrated in electro-reduction of solid oxides of zirconium and *** has led to significantly higher current efficiency(93%),and lower energy consumption(1.4 k W h kg 1) in electrolysis of tantalum oxide to tantalum metal compared to the conventional electrolysis in molten calcium chloride(e.g.78% and 2.4 k W h/kg-Ta).

In eukaryotes, the RNase-Ⅲ Dicer often produces length/sequence microRNA （miRNA） variants, called ＂isomiRs＂, owing to intrinsic structural/sequence determinants of the miRNA precursors （pre-miRNAs）. In this study, we combined biophysics, genetics and biochemistry approaches to study Arabidopsis miR168, the key feedback regulator of central plant silencing effector protein ARGONAUTE1 （AGO1）. We identified a motif conserved among plant premiR168 orthologs, which enables flexible internal basepairing underlying at least three metastable structural configurations. These configurations promote alternative, accurate Dicer cleavage events generating length and structural isomiR168 variants with distinctive AGO sorting properties and modes of action. Among these isomiR168s, a duplex with a 22-nt guide strand exhibits strikingly preferential affinity for AGO10, the closest AGO1 paralog. The 22-nt miR168-AGO10 complex antagonizes AGO1 accumulation in part via ＂transitive RNAi＂, a silencing-amplification process, to maintain appropriate AGO1 cellular homeostasis. Furthermore, we found that the tombusviral P19 silencing-suppressor protein displays markedly weaker affinity for the 22-nt form among its isomiR168 cargoes, thereby promoting AGO10-directed suppression of AGOl-mediated antiviral silencing. Taken together, these findings indicate that structural flexibility, a previously overlooked property of premiRNAs, considerably increases the versatility and regulatory potential of individual MIRNA genes, and that some pathogens might have evolved the capacity or mechanisms to usurp this property.
