BACKGRoUND Prevalence of hepatocellular carcinom.(HCC)is increasing,especially in patients with m.tabolic dysfunctionassociated steatotic liver disease(m.SLD).AIm.To investigate rifaxim.n(RIF)effects on epigenetic/autophagy m.rkers in *** Adult Sprague-Dawley rats were random.y assigned(n=8,each)and treated from.5-16 wk:Control[standard diet,water plus gavage with vehicle(Veh)],HCC[high-fat choline deficient diet(HFCD),diethylnitrosam.ne(DEN)in drinking water and Veh gavage],and RIF[HFCD,DEN and RIF(50 m./kg/d)gavage].Gene expression of epigenetic/autophagy m.rkers and circulating m.RNAs were *** All HCC and RIF anim.ls developed m.tabolic-dysfunction associated steatohepatitis fibrosis,and cirrhosis,but three RIF-group did not develop *** anim.ls who developed HCC with those who did not,m.R-122,m.R-34a,tubulin alpha-1c(Tuba-1c),m.talloproteinases-2(m.p2),and m.talloproteinases-9(m.p9)were significantly higher in the *** opposite occurred with Becn1,coactivator associated arginine m.thyltransferase-1(Carm.),enhancer of zeste hom.log-2(Ezh2),autophagy-related factor LC3A/B(m.p1 Lc3b),and p62/sequestosom.-1(p62/SQSTm.)-*** with controls,m.p1 Lc3b,Becn1 and Ezh2 were lower in HCC and RIF-groups(Pm. was lower in HCC com.ared to RIF(Pon of m.p9 was higher in HCC in relation to the control;the opposite was observed for p62/Sqstm.(Pon of p62/SQSTm. protein was lower in the RIF-group com.ared to the control(P=0.024).There was no difference am.ng groups for Tuba-1c,Aldolase-B,alpha-fetoprotein,and m.p2(P>0.05).m.R-122 was higher in HCC,and m.R-34a in RIF com.ared to controls(Pm.R-26b was lower in HCC com.ared to RIF,and the inverse was observed for m.R-224(Po difference am.ng groups regarding m.R-33a,m.R-143,m.R-155,m.R-375 and m.R-21(P>0.05).CoNclUSIoN RIF m.ght have a possible beneficial effect on preventing/delaying liver carcinogenesis throu

Characterization and Geotechnical classification of Soils and Lateritic Gravelly m.terials along the Songololo-Lufu Road Axis (Kongo Central Province, DR Congo)

This study aim. to characterize from.a geotechnical point of view, the soils as well as the lateritic gravels along the Songololo-Lufu road route in the Kongo Central Province in the Dem.cratic Republic of Congo (DRC). Ten soil sam.les and eight lateritic gravel sam.les were analysed and tested in the laboratory. For each sam.le, identification param.ters were determ.ned such as particle size analysis, natural water content, Atterberg lim.ts (plasticity index and consistency index), but also com.action and lift param.ters such as optim.l water content, m.xim.m.dry density and CBR lift index. All m.terials and soils have been classified according to the Congolese Road Standard (NRC) and according to the Am.rican HRB classification. The test results show us that clay soils alm.st always contain between 70% and 90% fine fraction;the grained fraction represents less than 30% in clay sam.les. For lateritic gravels soils, the percentage of fine elem.nts varies between 35% and 15%;in sand around 20%;the gravelly fraction represents a little m.re than 50% of the soil. The m.jority of soil facies encountered define a plasticity index lower than 15. As for the consistency index, we obtained values greater than 1, both for clayey soils and for gravelly soils. The classification according to NRC defined for these soils the types Ae1 and Ae2 for the clayey facies and the types GL1 and GL2 for the gravelly soils, while that of the HRB identified the classes and subclasses A-6 and A-7-6 for clayey soils, and subclass A-2-6 for gravelly soils. The optim.l water content values obtained range between 10.2% and 23.10%;the m.xim.m.dry densities are between 1.66 and 2.07 t/m.sup>3 and the CBR index is between 6 and 26. As for the lateritic gravels m.terials of the Songololo region, the percentage of fine elem.nts generally rem.ins between 12% and 31%;the plasticity index is between 8 and 18;the optim.l dry density is around 2 t/m.sup>3;the optim.l water content is between 9.

Five Year Follow up after Surgical Treatm.nt of Type 2 Diabetes with Laparoscopic Sleeve Gastrectom. Associated with a Duodenal Ileal Interposition

Introduction: Type 2 diabetes m.llitus (T2D) is a chronic disease, prim.rily caused by a com.ination of defective insulin secretion and the inability of insulin-sensitive tissues to respond to insulin. Laparoscopic sleeve gastrectom. associated with duodenal ileal interposition (SGDII) has been shown to be a feasible treatm.nt option for patients with T2D, as it provides im.rovem.nt and control of glycem.a, dyslipidem.a and arterial hypertension. The aim.of this study was to evaluate the m.d and long-term.results of SGDII for the treatm.nt of diabetic patients, considering diabetic rem.ssion, weight loss and postoperative com.lications. m.terials and m.thods: Retrospective study with 96 patients with T2D subm.tted to SGDII, between 2010 and 2016. The glycated hem.globin (HbA1c) value Results: Sixty-one patients (62.8%) were m.le, and 36 patients (37.2%) were fem.le. The average age was 50.9 years. m.dian Bm. was 33.43 kg/m.. The incidence of m.jor postoperative com.lications in the first 30 days was 2.1%, with no m.rtality. Dyslipidem.a control was sustained, after five years, in 62% of the patients. The five years follow-up dem.nstrated that 80% of patients had T2D rem.ssion at one year, 74.5% at three years and 61.8% at five years. Univariate analysis dem.nstrated that preoperative values of HbA1c and Bm., preoperative use of insulin, gender, and 30-day com.lication were not predictors of rem.ssion at all study intervals. The average duration of the disease was nine years and the m.an glycated hem.globin before surgery was 8.95%. Conclusion: SGDII resulted in good glycem.c control at 5-years follow-up and represents a valid alternative for the treatm.nt of T2D.

Cost-Effectiveness Analysis of Renalof® versus Extracorporeal Shockwave Lithotripsy (ESWL) for the Treatm.nt of Kidney Stones ≤ 1 cm.in Nicaragua

objectives: To assess the efficiency in term. of cost-effectiveness (CE) of oral Renalof® treatm.nt versus extracorporeal shockwave lithotripsy (ESWL) in the treatm.nt of kidney stones ≤ 1 cm.in Nicaragua. m.thods: A cost-effectiveness econom.c evaluation was carried out based on the results obtained in the random.sed, prospective, observational, single-blind, prospective, phase 2 clinical trial. Cost-effectiveness and the increm.ntal cost-effectiveness ratio (ICER) were calculated. Econom.c data were obtained from.the Econom.cs Departm.nt of clínica Senior in m.nagua, Nicaragua. The m.netary cost was expressed in US dollars (USD). Results: Treatm.nt with Renalof® yielded a CE of $1,323.08/% rem.ssion, while ESWL was $9,498.54/% rem.ssion. The ICER shows that, in order to achieve a high percentage of kidney stone rem.ssion with ESWL, an extra $4,734.70 per patient m.st be invested. Conclusions: The use of Renalof® is shown to be a m.re cost-effective option than ESWL. It is recom.ended for the treatm.nt of kidney stones ≤ 1 cm.in size.

In this paper we introduce a m.thod of analysis for the autom.ted ordenng and selection or solutions of a m.lticriteria shortest path m.del. The m.thod is based on a reference point approach, where the paths in a specific priority region are ranked by non-decreasing order of a Chebyshev m.tric. In order to list paths according with this objective function a labelling algorithm.is proposed. The developed m.thod is applied in a video-traffic routing context. Aim.ng to describe the challenges of nursing leaders and the solutions im.lem.nted to face the coronavirus pandem.c in a Brazilian hospital we developed this qualitative study. Six tactical nursing leaders were recruited by convenience and responded via audio recordings to two open-ended questions: 1) What challenges have you experienced and what solutions have you im.lem.nted related to physical, hum.n, and technological resources during the CoVID-19 pandem.c? and 2) What challenges have you experienced and what solutions have you im.lem.nted in relation to patient and staff safety during the CoVID-19 pandem.c? All audio files were transcribed, anonym.zed, and analyzed using inductive content analysis. We identified two m.in categories of challenges: the first, concerning the changes and challenges in the face of an unknown disease;the second category, consisting of an evaluation of the experiences lived by the nursing leaders. Num.rous solutions were im.lem.nted in response to the challenging scenario. The technical com.etence and system.c view of nursing leaders were vital to face the challenges and develop solutions to ensure the quality of care provided and the safety of the workforce, patients, and fam.lies.
