Sewage introduction into rivers has altered the physical and chemical properties of waters and also the microbial metabolism. This study aimed to evaluate the Escherichia coli and nutrient concentrations in the Maratuãand Crumaú rivers (Santos Estuary, Brazil) during two periods with distinct magnitudes of freshwater runoff, verifying possible relation of abiotic changes with the microbial metabolism. Water sampling was performed in October/2012 (dry season) and January/2013 (rainy season) at two points in the Crumaú river (upstream and downstream zone) and one in the Maratuãriver (downstream zone). The water subsamples were obtained for E. coli and nutrient analyses while the velocity of water flow, water level, temperature, salinity, and dissolved oxygen were measured in situ. The E. coli concentrations were under the detection limit in the Maratuãdownstream during the dry season reaching a maximum value (1.47 × 104 CFU/100mL) in the Crumaú upstream during the rainy season. E. coli presented strong positive correlation with nutrients (ammoniacal-N and phosphate), evidencing the sewage source in the Crumaú upstream shown by this association. In both periods, the low oxygen saturation (100 μmol·L-1) indicated considerable predominance of heterotrophic metabolism in the Crumaú upstream. The low dissolved oxygen values in Crumaú River are corroborated to show a low self-depuration capacity in the rainy period due to maintenance of high nutrient and E. coli at two points in the Crumaú river (upstream and downstream zone) and one in the Maratuãriver (downstream zone). Besides, these results evidenced that the tendency of the metabolism changed from autotrophic to heterotrophic under high river flow events at this studied estuarine sector located at Santos estuarine complex.

Objective: To identify the main chemical classes of compounds from aqueous extract of Enterolobium contortisiliquum(E. contortisiliquum) seed bark and to evaluate its antibacterial activity, as well as its potential to increase the activity of antibiotics against strains of Staphylococcus aureus, Pseudomonas aeruginosa and Escherichia ***: Different classes of compounds in the aqueous extract of E. contortisiliquum were evaluated based on the visual changes in the coloration and the formation of precipitate after the addition of specific reagents. The antibacterial activity of the extract and its potential to increase of antibiotic activity of antibiotics drugs, gentamicin and norfloxacin was determined by using the microdilution ***: Our results demonstrated that the following secondary metabolites were presented in E. contortisiliquum seed bark: flavones, flavonols, xanthones, flavononols,chalcones, aurones, flavones and catechins. The extract itself had very low antibacterial activity against all bacterial strains tested(MIC ≥ 1024 μg/m L), but there was an increase in the antibiotic activity of gentamicin and norfloxacin when combined in the subinhibitory concentration(i.e., MIC/8).Conclusions: Our data suggests that E. contortisiliquum seed bark may be an alternative source for new drugs with the potential to increase antibiotic activity against different strains of bacteria.

AIM： To evaluate the safety and feasibility of bone marrow cell （BMC） transplantation in patients with chronic liver disease on the waiting list for liver transplantation. METHODS： Ten patients （eight males） with chronic liver disease were enrolled to receive infusion of autologous bone marrow-derived cells. Seven patients were classified as Child-Pugh B and three as Child-Pugh C. Baseline assessment included complete clinical and laboratory evaluation and abdominal MRI. Approximately 50 mL of bone marrow aspirate was prepared by centrifugation in a ficoll-hypaque gradient. At least of 100 millions of mononuclear-enriched BMCs were infused into the hepatic artery using the routine technique for arterial chemoembolization for liver tumors. Patients were followed up for adverse events up to 4 mo. RESULTS： The median age of the patients was 52 years （range 24-70 years）. All patients were discharged 48 h after BMC infusion. Two patients complained ofmild pain at the bone marrow needle puncture site. No other complications or specific side effects related to the procedure were observed. Bilirubin levels were lower at 1 （2.19 ± 0.9） and 4 mo （2.10 ± 1.0） after cell transplantation that baseline levels （238 ± 1.2）. Albumin levels 4 mo after BMC infusion （3.73 ± 0.5） were higher than baseline levels （3.47 ± 0.5）. International normalized ratio （INR） decreased from 1.48 （SD = 0.23） to 1.43 （SD = 0.23） one month after cell transplantation. CONclUSION： BMC infusion into hepatic artery of patients with advanced chronic liver disease is safe and feasible. In addition, a decrease in mean serum bilirubin and INR levels and an increase in albumin levels are observed. Our data warrant further studies in order to evaluate the effect of BMC transplantation in patients with advanced chronic liver disease.

This study investigated the effects on the reproductive system of balb-c mice exposed to water collected from different sources near to a deactivated fertilizer waste depositary. Eighty male mice were separated in four groups: Group A (mineral water);Group B (water from the water treatment station);Group C (water from Cubatao city);Group D (water from the waste depositary region). They were exposed to water since they were weaned until they reached sexual maturity, then they were coupled with females in reproductive age and after this mating time they were sacrificed. The evaluated parameters were testicle weight, sperm analysis, pregnancy rate, sex ratio of the offspring and Sertoli cell count. The analysis of the water did not show presence of pollutants in the Group “A” and Group “C” water. Group “B” showed low level of cadmium, 3.58 ± 0.50 μg/L. Group “D” showed the presence of PAH’s and high levels of lead (221 ± 16 μg/L), cadmiun (12.6 ± 1.2 μg/L) and mercury (5.3 ± 1.1 μg/L). The tests of Levene and Kolmogorov-Smirnov were employed to verify the homogeneity of the variances and the tack to the normal curve, respectively. ANOVA was used for parametric tests and Kruskall-Wallis was used for non-parametric tests, while Turkey tests were employed for multiple comparisons. There were no differences between groups in testicle weight, sperm analysis, pregnancy rate and Sertoli cell count. There was a significant reduction in sex-ratio of the offspring in Group B. This alteration cannot be explained by the cadmium levels in Group B water. In the present study we cannot associate the exposition to contaminated water from the waste depositary and reproductive alterations.

Objective: To compare the in vitro antiparasitic activity of aqueous extracts from Ziziphus joazeiro leaves and stem bark against Trypanosoma cruzi, Leishmania braziliensis, and Leishmania infantum, as well as to evaluate its cytotoxicity in mammalian cells, in addition to identifying the chemical composition of the extracts. Methods: Ziziphus joazeiro leaf and stem bark aqueous extracts were prepared by cold extraction maceration and subjected to ultra-efficient liquid chromatography coupled to a quadrupole/time of flight system. The susceptibility assays used Trypanosoma cruzi clB5 strains and promastigote forms of Leishmania braziliensis and Leishmania infantum for antiparasitic activity of the extracts. Moreover, mammalian fibroblasts NCTC clone 929 were used for cytotoxicity analysis. Results: Terpenoid compounds, flavonoids and phenolic acid were identified in extracts. The stem bark aqueous extracts presented more significant results in terms of antiparasitic activity compared with the leaf aqueous extracts, especially against Leishmania braziliensis and Leishmania infantum promastigote forms with an IC50 clusions: These preliminary results may contribute to the formulation of new therapeutic agents against this group of neglected diseases, so further investigations are required to delineate the mechanisms of action mainly of the aqueous extract of stem bark of Ziziphus joazeiro.

Risk Factors Associated with Acinetobacter baumannii Infections in Patients in an Intensive Care Unit of a Public Hospital in Paraná

To identify risk factors for A. baumannii infection in patients hospitalized in an Intensive Care Unit (ICU) of a tertiary public hospital in Paraná, Brazil, a retrospective paired case-control study (ratio 1:2) was conducted from January 2018 to December 2020. Patients in the case group were hospitalized in the ICU with A. baumanni (n = 68 cases) and were compared with patients in the control group, without infection by A. baumannii (n = 136). Both were matched by age (±10 years), sex, and ICU stay (±5 days). Conditional multiple logistic regression was used to determine statistically significant risk factors based on the results of bivariate analyses. Mortality was higher in infected (cases) than in non-infected patients (51.5% vs. 39.7%). The incidence and bacterial resistance increased annually. At bivariate analysis, cases had longer hospital stays (median 35 vs. 22 days, p A. baumannii and antimicrobial resistance. There is need for surveillance, and constant evaluation of control actions. Risk factors were colonization, previous hospitalization, and hospitalization time. This is essential for the decision-making of professionals and optimization of prevention, control, and therapeutic management actions.

Strategic Partnership Between Private Organizations and Universities： The Search for Regional Development Through Solutions for Hospital Management

The public power and the private enterprise, the main agents of national economic and social development, when cooperating strategically, promote increased functional efficiency of their activities. In order to highlight the relevance of such partnership, this article aims to analyze the potential of actions jointly developed by private organizations and Brazilian universities. The research problem is lined in the field of health, which hosted actions developed between the Federal University of Rio Grande do Norte （UFRN） and the Hospital Maternity Guiomar Fernandes （HMGF）, located in the city of Alexandria, in Rio Grande do Norte. The study emphasizes the shortcomings of this organization, the resolving power of the university in this scenario, and performs the analysis of the effects of this cooperation. The methodology used in this research was the case study in HMGF. With emphasis on fundraising and improving the informational management system, funding sources for projects of restructuring and expansion of the hospital were sought, as well as initiatives for developing hospital management softwares for small- and medium-sized organizations. As a result, the articulation between the university and the hospital provided the elaboration of a project for software development and the production of a project for physical restructuring and technological investment, aiming to expand the supply and quality of the hospital＇s services. The implementation of information systems, associated with investments in hospital infrastructure, providing increased efficiency in public assistance to the population, bringing both local and regional benefits, besides, providing the authorities with relevant data on the development of public policies.

BACKGROUND Metabolic syndrome is a multifactorial disease,and the gut microbiota may play a role in its ***,especially abdominal obesity,is associated with insulin resistance,often increasing the risk of type two diabetes mellitus,vascular endothelial dysfunction,an abnormal lipid profile,hypertension,and vascular inflammation,all of which promote the development of atherosclerotic cardiovascular *** To evaluate the outcomes of fecal microbiota transplantation(FMT)in patients with metabolic *** This was a randomized,single-blind placebo-controlled trial comparing FMT and a sham procedure in patients with metabolic *** selected 32 female patients,who were divided into eight groups of four patients *** of the patients were submitted to upper gastrointestinal *** each group,two patients were randomly allocated to undergo FMT,and the other two patients received saline *** patients were followed for one year after the procedures,during which time anthropometric,bioimpedance,and biochemical data were *** patients also had periodic consultations with a nutritionist and an *** primary end point was a change in the gut *** There was evidence of a postprocedural change in microbiota composition in the patients who underwent FMT in relation to that observed in those who underwent the sham ***,we found no difference between the two groups in terms of the clinical parameters *** There were no significant differences in biochemical or anthropometric parameters,between the two groups ***,there were significant postprocedural differences in the microbiota composition between the placebo *** date,clinical outcomes related to FMT remain uncertain.
