Introduction: The authors wanted to study the prevalence of ocular trauma in the ophthalmology unit of the CSREF of Kati. Patients and method: This is a retrospective study, covering the period from January to December 2015, carried out in the ophthalmology unit of the CSREF in Kati. Results: The sample consisted of 568 patients, or 6.82% of ophthalmological consultations. Children aged 0 to 14 years accounted for 154 (27.11%) of the trauma cases. Bruises constitute the main circumstance (58.1%) of eye trauma, followed by domestic accidents (15.6%). Closed globe ocular trauma was the main type of trauma (61.6%), followed by ocular adnexal trauma (30.1%) and open globe trauma (8.3%) in which the penetrating wound was the lesion, the least common (2.6%) but the most serious. Medical treatment was the most used therapeutic modality in 82.4% of cases, followed by surgery with 17.6% of cases. The evolution of the lesions after treatment was favorable in 85.6% of cases and unfavorable in 14.4% of cases. Complications were observed in 5.8% of cases and sequelae in 8.6% of cases. Conclusion: Ocular trauma constitutes an important reason for ophthalmological consultation at the CSREF of Kati. The severity of some of these lesions requires frequent recourse to surgery. The complexity of the treatment should encourage us to favor preventive measures.

Introduction: The severity of Sars-Cov-2 infection is associated with the development of acute respiratory distress syndrome (ARDS). The progression to ARDS appears to be driven by a major inflammatory mechanism potentially sensitive to corticosteroids. Observation: This article describes the case of an elderly patient was admitted to emergency departments for intense asthenia, accompanied by motor diarrhoea, dyspnoea with desaturation in ambient air, in a context of strong suspicion of infection linked to COVID-19. The article also reviews the existing literature on the diagnosis and treatment of this severe form of the disease. Conclusion: Corticosteroids, and in particular dexamethasone, have been shown to be effective in the management of patients with COVID-19, an oxygen-releasing disease.

Kinetics of 2,4-dichlorophenol dechlorination by Pd-Fe bimetallic nanoparticles in the presence of humic acid

The remediation of groundwater which contains chlorinated organic compounds (COCs) by nanoscale bimetallic catalysts has received increasing interest in recent years. This report presents the dechlorination of 2,4-dichlorophenol (2,4-DCP) by Pd-Fe bimetallic nanoparticles in the presence of humic acid (HA) to investigate the feasibility of using Pd-Fe for the in situ remediation of contaminated groundwater. Our experimental results indicated that HA had an adverse effect on the dechlorination of 2,4-DCP by Pd-Fe nanoparticles. The rate constant k values of 2,4-DCP dechlorination were 0.017, 0.013, 0.009, 0.006 and 0.004 min?1 for HA concentrations of 0, 5, 10, 15 and 20 mg/L, respectively. The relationship between HA dosage and k values can be described as a linear model.

In arid and semi-arid regions, the growth and development of cultivated plants, especially tomato (Solanum lycopersicum L.), are severely limited by water deficit. Thus, to cope with this constraint, the plant establishes symbiotic relationships with arbuscular mycorrhizal fungi (AMF) in the soil whose extension of the hyphae allows a better and deeper exploration;this notably improves the hydromineral nutrition of the plant. Therefore, the choice of fungal partner becomes crucial for the establishment of a crop in water-deficient soil. In this context, the contribution of AMF to the water stress tolerance of two varieties of tomato plants was assessed under semi-controlled conditions. Parameters, such as the mycorrhizal frequency, intensity of mycorrhization, relative mycorrhizal dependency, growth, and biochemical parameters (carbon, nitrogen, phosphorus, and proline contents) of plants subjected to three levels of water stress (T100, T70, and T30), were evaluated. The highest frequencies and intensities of mycorrhization and relative mycorrhizal dependencies were obtained with plants of the Xewel variety inoculated with Rhizophagus fasciculatus (F: 95.24%, 88.35%, and 13.64%;M: 40.52%, 37.52%, and 11.22%;D: 23.7%, 54.4%, and 78.82%) and in those of the Lady Nema variety inoculated with Claroideoglomus etunicatum (F: 95.12%, 87.01%, and 15.25%;M: 40.66%, 37.99%, and 11.42%;D: 19.27%, 57.01%, and 70.98%), respectively at water regimes of T100, T70 and T30. These same symbiotic couples recorded, at T30, the best survival rates (+ 40%) and the higher aerial (77% and 74%) and root dry weights (80% and 59%). Plants of the Xewel variety inoculated with R. fasciculatus recorded the highest contents of carbon (T70: 30.59% and T30: 21.55%) and phosphorus (T70: 0.18% and T30: 0.17%). Biermer’s Disease at the Donka National Hospital in Guinea—Epidemio-Clinical, Therapeutic and Evolutionary Aspect in the Internal Medicine Department

Introduction: Biermer’s disease is an autoimmune disease characterized by a lack of absorption of vitamin B12 in connection with the production of antibodies (A) destroying the intrinsic factor (IF) which allows the absorption of vitamin B12 (cobalamin). These clinical manifestations are polymorphic and severe in our context. The objective of this work is to identify the epidemiological-clinical, therapeutic and evolutionary characteristics of Biermer’s disease in Guinean population. Materials and methods: This was a retrospective of patient files followed for Biermer’s disease at the internal medicine department of Donka National Hospital from January 2012 to December 2021. Results: Eight patients were included including 5 women and 3 men. The average age of the patients was 48 years old. The diagnostic delay was 3.6 years on average. All our patients had bioclinical anemia (8 cases, i.e. 100%) followed by epigastralgia in 4 cases (50%), neurological damage such as sensitive polyneuropathy in 3 cases (37.5%). Four patients had acquired melanoderma (50%). Hypovitaminosis B12 was found in 4 patients. The myelogram performed in three patients (37.5%) found medullary megaloblastosis. One patient had Hashimoto’s disease associated with Biermer’s disease in endoscopy, (FOGD) found fundica trophy on macroscopy in 4 cases (50%). Treatment consisted of B12 vitamin therapy in all cases with a favorable clinical and biological outcome. Conclusion: Biermer’s disease remains common in Africa and is characterized at a younger age in addition to the severity of clinical and biological manifestations. The care consists of taking vitamin B12 which remains accessible in our context.

染料和苯酚是工业废水中常见的成分，全世界范围内其产量巨大，所以排放量也相当惊人。这种废水的大量存在日益对生态系统，陆地生物和水生生物构成威胁，所以它们吸引了人们的广泛关注，人们希望能够找到处理这些废水的有效方法以提高环境质量。本文的主要目的就是来验证臭氧氧化技术对这些废水处理的可行性和有效性。染料废水是一种高色度不易降解的有机废水，直接排放到水体，不但影响自然景观，而且将和其他污染物一起破坏环境，威胁人类健康，危害生态系统。随着纺织工艺的飞速发展，染料产生的污染情况日益严重。由于较高的色度，复杂的分子结构和含氮的键，普通的废水处理方法——如物化和生物处理，并不能达到一个较为满意的效果，于是吸引人们更多的目光，花更多的精力来寻找更为有效的技术和方法。在自然界中，腐烂的有机物如腐烂的蔬菜中以及煤炭中可发现苯酚。1834年第一次从煤焦油中提炼出来，到第一次世界大战前，都一直苯酚的主要来源。苯酚是中常见的污染物，许多工业过程中都会产生苯酚废水，所以在考虑环境问题时都要考虑到如何去除苯酚。由于其毒性和较高的需氧量(平均每克苯酚需要消耗2.4克氧气)以及难以生物降解，苯酚被认为是要优先考虑的污染物之一。臭氧是中强氧化剂，其分子有三个氧原子组成，在氧化处理饮用水和废水的时候，不会产生二次污染。臭氧可以单独使用，同时也可以和其他的处理单元相结合，如在本研究中与生物处理结合。臭氧氧化存在成本过高的缺点，单独使用臭氧完全处理废水就显得不现实，所以经常和其他的方法结合起来，但如何组合却是个值得研究的问题。本研究中臭氧与生物处理相结合就存在先后秩序的问题：臭氧氧化作为生物处理的预处理，可以增加有机废水的可生物降解性;若臭氧作为生物降解后的深度处理工艺，则可以保证去除废水的色度。前者，只要投加少量的臭氧就可以破坏大分子，从而增加了废水的生物降解性，后续的生物处理可以达到一个非常高的去除效果;而后者，色度的去除非常成功。本文研究了单独臭氧和臭氧与其他方法结合的技术来处理染料和含苯酚的废水，考察了臭氧前置和后置工艺处理染料和苯酚废水的各种因素影响：如污染物的初始浓度，温度，臭氧流量，溶液的pH值等。实验数据和理论数据进行了对比。实验结果表明：处理染料废水时，臭氧前置工艺并没有明显提高废水的BOD／COD比值，臭氧后置工艺则达到了较为满意的效果。当回流已处理过的废水后，却得到一个较高的CODcr和色度去除率。此外，还考察了UV与臭氧的联合工艺可以有效降解染料废水。处理苯酚废水中，臭氧前置工艺取得的效果要远远好于臭氧后置以及单独臭氧氧化。

Progressive Characteristics of HIV Infection in the Elderly in the Cohort of the Internal Medicine Department of the Points G University Hospital, Bamako. Mali

Few data are available on HIV infection in the elderly and they are an important population in our different cohorts in view of the improvement in the quality of care over the last ten years. Objective: to determine the clinical and immunovirological characteristics as well as the acceptance of antiretroviral drugs in this patient population. This is a cohort study of the files of PLHIV (People living with HIV) aged 50 years or more, followed in the Internal Medicine Department of the Point G University Hospital between January 2007 and December 2012. Results: Out of 161 HIV-infected patients in the cohort, 38 were aged 50 years or older, of whom 11 met the criteria and were included. The median age was 55 years, 90.9% of whom were in the [50 - 59 years] age group, with 63.6% being women (sex ratio = 0.57). At admission, 3 patients (27.3%) had prurigo and 2 (18.2%) had oral-pharyngeal candidiasis. At inclusion, 45.5% of patients were classified as WHO stage 2 and one as WHO stage 4. All were on ARVs, including 10 on 1st line HIV-1 and 1 on HIV-2. On triple therapy, the average weight gain was 5.1 kg at D15 and 6.84 kg at M6. However, at M12 there was a weight loss of 1.04 kg. The mean CD4 T cell gain was +102/mm3 at M6 and +188/mm3 at 12 months. At D0, mean viral load = 565024.75 copies/mm3 [99 - 1100000] in 4/11. At M12, two patients had undetectable viral loads. Conclusion: The prevalence of HIV in elderly subjects is certainly underestimated. Thanks to triple antiretroviral therapy, PLHIV are aging with HIV but screening is not systematically proposed during consultations in elderly subjects. It is necessary to reinforce communication about HIV at all ages.

Clinical and Epidemiological Aspects of Hepatocellular Carcinoma at the Internal Medicine Department of Point “G” Teaching Hospital in Mali

Liver cancer is the malignant transformation of liver cells. It develops in 90% of cases of cirrhosis, more rarely on chronic non-cirrhotic liver disease, and exceptionally in a healthy liver. This study aimed to investigate the clinical aspects of Hepatocellular Carcinoma (HCC). It was a retrospective descriptive study covering 10 years, focusing on HCC cases seen in outpatient and inpatient settings at the Internal Medicine Department. We recorded 153 cases out of 7021 patient records, resulting in a hospital frequency of 2.17%. The male-to-female ratio was 3.5. The mean age was 52.37 ± 14.34 years. The most common presenting complaint was pain in 16.3% of cases. A history of jaundice was found in 25.5% of cases. Alcohol consumption was observed in 15.38% of cases. The main physical sign found was hepatomegaly in 76% of cases. HBsAg was positive in 33.3% of cases. Alpha-fetoprotein levels were above 400 IU/ml in 50.81% of cases. Patients classified as CHILD PUGH A represented 39.72% of cases. Abdominal ultrasound revealed portal thrombosis associated with heterogeneous multinodular hepatomegaly in 11% of cases. Cytology confirmed HCC in four out of six patients who underwent the examination. We recorded 63 deaths out of 111 hospitalized patients. Complications included encephalopathy, hematemesis, and ascites in 48 patients. Hepatocellular carcinoma remains a significant public health issue. Its predominance in men and its occurrence in adults with factors such as viral infections and ethylism mean that prevention of this pathology could greatly reduce its incidence.

Risk Factors and Syndromic Management of Sexually Transmitted Infections among Sex Workers in the Kadiolo Circle

Sexually transmitted infections constitute a gateway for the human immunodeficiency virus by the presence of ulcerations and local inflammatory manifestations, thus increasing the risk of transmission. The aim of the study was to assess the sexual behavior and syndromic management of sexual infections among female sex workers in the Kadiolo circle. Material and method: This was a descriptive cross-sectional study carried out in the Kadiolo circle from January 1 to December 31, 2014. It focused on clandestine or posted sex workers, aged 15 and over. Results: We collected 465 sex workers aged 15 and 49, aged 26.6 on average and from Ivory Coast 52%, Nigeria 26.2% and Mali 9.7%. Educational attainment was higher with 2.4%. Posted sex workers represented 92.5% and housed in brothels with a stay of 6 months with 53.3%. They were single with 70.8% of cases and about 72.9% lived alone and without a spouse. The age at first sexual intercourse was between 15 - 19 years with 76.6% of cases. The number of first commercial sex was between 15 - 19 years (26%) and 20 - 24 years (45.8%). Usual condom use with regular sex partners was 22.1%. Condom use was almost compulsory at 89.7% with occasional clients or non-regular sexual partners. The average number of clients per day was five (5). The major reason for consultation was vaginal discharge 16.4% followed by abdominal pain. The human immunodeficiency virus test was not compulsory and only 86 workers or 18.5% accepted with a positive result of 11 people or 12.8% of cases. Conclusion: Sexually transmitted infections are common especially as they affect young people.

Introduction: Corneal opacities in children are relatively common. In children, it is urgent because of the risk of irreversible amblyopia. The causes of these blindnesses vary considerably according to the regions of the world. Chlorhexidine digluconate is a broad-spectrum antiseptic directed against gram-positive and gram-negative bacteria. It has been used as an antiseptic for more than 50 years, both in humans and in animals. However, despite these benefits, it can cause serious damage if applied to the eyes. Incidents of eye damage with 7.1% chlorhexidine have been reported to WHO across Africa. Observation: We report and write two cases of bilateral corneal blindness caused by accidental ocular use of chlorhexidine (HEXZ) GEL 7.1% in two newborns in the health district of Kati in MALI. The ophthalmological examination had revealed corneal opacity, conjunctival hyperaemia and limbic ischemia greater than 50% in 1 eye out of 4, corresponding to stage IV of the ROPPER HALL classification;2 eyes out of 4 were at stage III and 1 eye out of 4 was at stage I of the same classification. Conclusion: It is essential to ensure correct use of chlorhexidine digluconate gel 7%. It is an antiseptic which should not be used on the ocular and auditory mucous membranes, at the risk of most often permanent burns.
