PUrPOSE: Prostate brachytherapy is gaining wide popularity as an alternative t o resection for the treatment of locally advanced prostate cancer. rectal-ureth ral fistula after prostate brachytherapy is a r.r. but serious complication, and its incidence, presentation, risk factors, and clinical management have not bee n well described. METHODS: From January 1997 to October 2002, seven patients wit h rectal-urethral fistulas were referred to two institutions (Brigham and Women ’s Hospital andWest roxbury Veteran’s Administration Hospital) of a major teac hing referral center. Clinical presentation, risk factors, prostate staging, and clinical management were examined in a retrospective fashion. rESULTS: Seven re ctal-urethral fistulas developed from roughly 700 (1 percent) patients treated with prostate brachytherapy for prostate cancer. The average patient age was 67. 7 years, preimplant prostate-specific antigen was 7.1, and Gleason score was 3+3. Symptoms occurred at a mean of 27 .3 months after prostate brachytherapy was started and included anorectal pain ( 57 percent), clear mucous discharge (57 percent), diarrhea (43 percent), and rec tal ulceration (43 percent). Coronary artery disease was a common comorbidity (7 1 percent). Previous transurethral resection of prostate (28 percent) and pelvic irradiation or external beam radiation therapy (14 percent) were not associated with increased risk of rectal-urethral fistula. All patients underwent a diver ting colostomy (86 percent) or ileostomy (14 percent), and four patients went on to have definitive therapy. Definitive resection was performed between 5 and 43 months after diverting ostomy and was chosen on the basis of comorbid disease, quality of life, and degree of operation. Gastric cancer is a common neoplastic disease and, more precisely, is the third leading cause of cancer death in the world, with differences amongst geographic areas. The definition of advanced gastric cancer is still debated. Different stadiating systems lead to slightly different stadiation of the disease, thus leading to variations between the single countries in the treatment and outcomes. In the present review all the possibilities of treatment for advanced gastric cancer have been analyzed. Surgery, the cornerstone of treatment for advanced gastric cancer, is analyzed first, followed by an investigation of the different forms and drugs of chemotherapy and radiotherapy. New frontiers in treatment suggest the growing consideration for intraperitoneal administration of chemotherapeutics and combination of traditional drugs with new ones. Moreover, the necessity to prevent the relapse of the disease leads to the consideration of administering intraperitoneal chemotherapy earlier in the therapeutical algorithm.

The effect of the Al-6Ce-3La（ACL） on the microstructural behavior of the Al7075 was investigated. Materials were synthesized by mechanical alloying with variation in the ACL content and milling time. Products were characterized and studied in the as-milled condition and mechanically evaluated after sintering. The synergetic effect of milling time and ACL content in the modified materials led to a reduction in the particle size. results from electron microscopy showed a homogeneous dispersion of Ce/La phases up to 20 wt.% of ACL content after 10 h of milling. Mechanical evaluation under compressive test showed an improved performance for those alloys reinforced with 0.2 wt.% and 0.5 wt.% of ACL.

Does the incubator control mode influence outcomes of low-birth-weight neonates during the first days of life and at hospital discharge?

Background: The thermal environment surrounding neonates in closed incubators can be regulated via two different modes: skin servocontrol mode (SSC) and air temperature control mode (ATC). These produce different patterns of incubator air and infant body temperatures. Objective: To assess the effects of incubator control mode on clinical outcomes of low-birth-weight-infants during the first days of life and at hospital discharge. Methods: 52 low-birth-weight neonates were nursed over ten days in closed incubators functioning either with SSC mode (n = 29), or with ATC mode (n = 23). results: The anthropomorphic characteristics of the two groups of neonates were homogenous (gestational age = 29.4 ± 1.4 vs. 29.9 ± 1.2 weeks and birthweight = 1214 ± 347 vs. 1263 ±292 gin the SSC-group and the ATC-group, respectively) and the caregiving (energy and fluid intakes, ventilator assistance and drug administration) did not differ statistically. Daily means of incubator air temperature were similar in the SSC and the ATC-group, however, the SSC mode resulted in more variable incubator air temperature but more stable skin abdominal temperature whereas the reverse was found when using the ATC mode. Those differences had no impact on the body weight of the neonates or their clinical outcomes at hospital discharge which were not statistically different. Conclusion: The clinical outcomes do not differ depending on the incubator control mode after the first ten days of life and at hospital discharge.

Desert resource environments(***,food)are tied to limited,highly localized rainfall regimes which generate microgeographic variation in the life histories of ***,enhanced growth rates,reproduction and survivorship are observed in response to increased resource availability in a variety of desert plants and short-lived *** examined the thermal ecology and reproduction of US federally threatened Mojave desert tortoises(Gopherus agassizii),long-lived and large-bodied ectotherms,at opposite ends of a 250-m elevation-related rainfall cline within Ivanpah Valley in the eastern Mojave Desert,California,*** operative environments in both the upper-elevation,“cima,”and the lower-elevation,“Pumphouse,”plots corresponded with daily and seasonal patterns of incident solar *** received 22%more rainfall and contained greater perennial vegetative cover,which conferred 5°C-cooler daytime shaded *** a monitored average rainfall year,cima tortoises had longer potential activity periods by up to several hours and greater ephemeral *** resource availability in cima was associated with larger-bodied females producing larger eggs,while still producing the same number of eggs as Pumphouse ***,reproductive success was lower in cima because 90%of eggs were depredated versus 11%in Pumphouse,indicating that predatory interactions produced counter-gradient variation in reproductive success across the rainfall ***-use impacts on deserts(*** energy generation)are increasing rapidly,and conservation strategies designed to protect and recover threatened desert inhabitants,such as desert tortoises,should incorporate these strong ecosystem-level responses to regional resource variation in assessments of habitat for prospective development and mitigation efforts.

Tropospheric Circulation Variability over Central and Southern South America

Combined Empirical Orthogonal Function Analysis of low-level atmospheric circulation after filtering the synoptic scale was performed for the period 1981-2006 over Central and Southern South America. Modes with 40 and near 70 days frequency associated with swings in longitude of the South Pacific and South Atlantic Ocean heights were identified. Their extreme values were related to drought and to high anomalous precipitation associated to floods in South East South America (SESA). These modes were independent of other sources of variability of the Southern Hemisphere atmosphere, namely MJO (Madden-Julian Oscillation), ENSO (El Nino Southern Oscillation) and AAO (Antarctic Oscillation). Mode one, which in its positive phase has a circulation similar to the mean winter, has a trend that explains part of the winter warming observed since 1980’s in Central and Eastern Argentina. Variance was calculated for the intra-annual variability, the one associated to the inter-annual variability including trends and jumps, and that of the annual cycle. The partition of the total variance was roughly 70%, 10% and 20% respectively. This partition implies that predictability of the regional climate is more critically dependent on the predictability of the intra-annual variability than of the inter-annual variability.
