Objective:Gemcitabine plus nab-paclitaxel(GnP)is the standard first-line therapy for advanced pancreatic ductal adenocarcinoma(PDAC).S-1,an oral fluoropyrimidine derivative,as compared with gemcitabine,is non-inferior in terms of overall survival(OS)and is associated with lower hematologic toxicity.Accordingly,S-1 is a convenient oral alternative treatment for advanced PDAC.This study was aimed at comparing the efficacy and safety of gemcitabine plus S-1(GS)vs.GnP as first-line chemotherapy for advanced PDAC.Methods:Patients with advanced PDAC who received first-line GS or GnP at the Peking Union Medical College Hospital between March 2011 and November 2022 were evaluated.Results:A total of 300 patients were assessed,of whom 84 received GS and 216 received GnP.The chemotherapy completion rate was higher with GS than GnP(50.0%vs.30.3%,P=0.0028).The objective response rate(ORR)was slightly higher(14.3%vs.9.7%,P=0.35),and the median OS was significantly longer(17.9 months vs.13.3 months,P=0.0078),in the GS group than the GnP group.However,the median progression-free survival(PFS)did not significantly differ between groups.Leukopenia risk was significantly lower in the GS group than the GnP group(14.9%vs.28.1%,P=0.049).Conclusions:As first-line chemotherapy for advanced PDAC,the GS regimen led to a significantly longer OS than the GnP regimen.The PFS,ORR,and incidence of severe adverse events were comparable between the GS and GnP groups.

The Implementation Basis, Short-term Effects and Recommendations of the Comprehensive Two-child Policy Taking Shaanxi Province as an Example

Through the investigation of three cities in Shaanxi province, it is found that after the implementation of the "comprehensive two-child" policy, public service policies on birth, education and medical treatment, etc. have been rapidly improved on the original basis, which has reduced the cost of birth and rearing and encouraged the initiative of the masses to have children. However, under the current policies, the government can only play an "underpinning" role. There is a big gap between the government’s support and parents’ pursuit of improving the quality of their children, thus it is not enough to change the current population situation. The change of bearing concept, the constraint of economic conditions, the lack of childcare services, and the imbalance of educational resources are all important factors that restrict the two-child birth. The country should implement the policy of encouraging childbearing as soon as possible, popularize high-quality childcare institutions, increase the number of public kindergartens and focus on improving the quality of compulsory education, and create an atmosphere of encouraging childbearing in the whole society so as to gradually change people’s bearing concept.

Human Gut Microbiota and Gastrointestinal Cancer

There are significant differences between the implementation of the family planning policy and the effects. According to a survey of Shaanxi Province, the number of families with two children has reached 62% in some cities, and only 37.8% in some cities. This laid a different foundation for the implementation of the comprehensive two-child policy. The implementation of the family planning policy is strong and the effect is good. The comprehensive benefit of the two-child policy is higher than that of the policy-relaxed areas. The urban beneficiaries are more than the rural beneficiaries. And the staffs within the system are the main beneficiaries of the comprehensive two-child policy. The implementation of the comprehensive two-child policy in the surveyed area has achieved initial results in two years: the birth rate has increased by about one thousand points, and the sex ratio of the birth population has increased overall. In 2016 and 2017, the proportion of two children in newborns increased rapidly. In some areas, the proportion of two children exceeded one child. The phenomenon of childbirth is highlighted, and the normal work of the industry is affected, such as hospitals and schools, and the functions of the family planning department are not in place. Relevant departments need to make prejudgments and adjust relevant policies.

Human gut microbiota play an essential role in both healthy and diseased states of humans. In the past decade, the interactions between microorganisms and tumors have attracted much attention in the efforts to understand various features of the complex microbial communities, as well as the possible mechanisms through which the microbiota are involved in cancer prevention, carcinogenesis, and anti-cancer therapy. A large number of studies have indicated that microbial dysbiosis contributes to cancer susceptibility via multiple pathways. Further studies have suggested that the microbiota and their associated metabolites are not only closely related to carcinogenesis by inducing inflammation and immune dysregulation, which lead to genetic instability, but also inter- fere with the pharmacodynamics of anticancer agents. In this article, we mainly reviewed the influ- ence of gut microbiota on cancers in the gastrointestinal （GI） tract （including esophageal, gastric, colorectal, liver, and pancreatic cancers） and the regulation of microbiota by diet, prebiotics, pro- biotics, synbiotics, antibiotics, or the Traditional Chinese Medicine. We also proposed some new strategies in the prevention and treatment of GI cancers that could be explored in the future. We hope that this review could provide a comprehensive overview of the studies on the interactions between the gut microbiota and GI cancers, which are likely to yield translational opportunities to reduce cancer morbidity and mortality by improving prevention, diagnosis, and treatment.

Superhydrophobic materials have attracted much attention for their special wettability.In this study.magnesium oxysulfate(MOS)whiskers were surface modified by vinyltrimethoxysilane(VTMS)and prepared as superhydrophobic materials,which are expected to be widely used in self-cleaning,corro-sion prevention,and oil-water separation.The factors of silane concentration,hydrolysis time,reaction temperature,and reaction time were investigated.The superhydrophobic Mos whiskers were synthe-sized.SEM and XRD turned out that there were no apparent changes in the morphology and crystalli-zation behavior of whiskers before and after modification,while the surface was uniformly coated with a layer of non-crystal material,and the surface of the whiskers employed a chemical bond Si-O-Mg covalently connected.The thermogravimetric analysis ultimately demonstrated that surface modification was beneficial to the improvement of the thermal stability of MoS whiskers.Superhydrophobic MoS whiskers showed good compatibility with organic solvents through oil-water separation experiments,and demonstrated excellent self-cleaning performance.The methodology for the surface treatment of Mos whiskers to prepare superhydrophobic whiskers in this work may be extended for other whiskers or fillers,which may be promising for the preparation of superhydrophobic materials.

Objective:Different SSTR2 antagonists have been developed.This study aims to evaluate the impact of different peptides and chelators on the diagnostic performance of SSTR2 antagonists in well-differentiated NETs.Methods:In this prospective study,participants were equally randomized into two arms:Arm A,participants would undergo a whole-body 68Ga-NODAGA-LM3 PET/CT scan on the 1st day and 68Ga-DOTA-LM3 PET/CT scan on the 2nd day;Arm B,participants would undergo a whole-body 68Ga-NODAGA-LM3 PET/CT scan on the 1st day and 68Ga-NODAGA-JR11 PET/CT scan on the 2nd day.Biodistribution in normal organs,lesion detection ability,and tumor uptakes were compared within each Arm.Results:A total of 40 participants(age,49.5±13.4,21 men),20 in each arm,were recruited in the study.In Arm A,68Ga-DOTA-LM3 showed lower background.However,the lesion detection ability(overall lesion detected,445 versus 548,P=0.005)and the lesion uptake(overall lesions SUVmax,19.8±17.2 versus 35.3±28.8,P<0.001)was significantly lower than those of 68Ga-NODAGA-LM3.In Arm B,both 68Ga-NODAGA-LM3 and 68Ga-NODAGA-JR11 showed similar biodistribution and lesion uptake(SUVmax,28.5±23.8 versus 25.0±20.0,P<0.001)despite minor differences.The lesion detection ability was the same between these two tracers(overall lesion detected,503 versus 503).Conclusions:The diagnostic performance of SSTR2 antagonists was sensitive to chelators.Both 68Ga-NODAGA-LM3 and 68Ga-NODAGA-JR11 outperformed 68Ga-DOTA-LM3 with higher lesion uptake and detection ability,of which 68Ga-NODAGA-LM3 had marginally but significantly higher lesion uptake.

Emission factors, ozone and secondary organic aerosol formation potential of volatile organic compounds emitted from industrial biomass boilers

To evaluate the potential benefits of biomass use for air pollution control, this paper identified and quantified the emissions of major reactive organic compounds anticipated from biomass-fired industrial boilers. Wood pellets(WP) and straw pellets(SP) were burned to determine the volatile organic compound emission profiles for each biomass-boiler combination. More than 100 types of volatile organic compounds(VOCs) were measured from the two biomass boilers. The measured VOC species included alkanes, alkenes and acetylenes, aromatics, halocarbons and carbonyls. A single coal-fired boiler(CB) was also studied to provide a basis for comparison. Biomass boiler 1(BB1) emitted relatively high proportions of alkanes(28.9%–38.1% by mass) and alkenes and acetylenes(23.4%–40.8%),while biomass boiler 2(BB2) emitted relatively high proportions of aromatics(27.9%–29.2%)and oxygenated VOCs(33.0%–44.8%). The total VOC(TVOC) emission factors from BB1(128.59–146.16 mg/kg) were higher than those from BB2(41.26–85.29 mg/kg). The total ozone formation potential(OFP) ranged from 6.26 to 81.75 mg/m^3 with an average of 33.66 mg/m^3 for the two biomass boilers. The total secondary organic aerosol potential(SOAP) ranged from 61.56 to 211.67 mg/m^3 with an average of 142.27 mg/m^3 for the two biomass boilers.The emission factors(EFs) of TVOCs from biomass boilers in this study were similar to those for industrial coal-fired boilers with the same thermal power. These data can supplement existing VOC emission factors for biomass combustion and thus enrich the VOC emission inventory.

C4 plants possess better drought tolerance than C3 plants. However, Hedysarum scoparium, a C3 species, is dominant and widely distributed in the desert areas of northwestern China due to its strong drought tolerance. This study compared it with Haloxylon ammodendron, a C4 species, regarding the interactive effects of drought stress and different leaf–air vapor pressure deficits. Variables of interest included gas exchange, the activity levels of key C4 photosynthetic enzymes, and cellular anatomy. In both species, gas exchange parameters were more sensitive to high vapor pressure deficit than to strong water stress, and the net CO2 assimilation rate（A n） was enhanced as vapor pressure deficits increased. A close relationship between A n and stomatal conductance（g s） suggested that the species shared a similar response mechanism. In H. ammodendron, the activity levels of key C4 enzymes were higher, including those of phosphoenolpyruvate carboxylase（PEPC） and nicotinamide adenine dinucleotide phosphate-malate enzyme（NADP-ME）, whereas in H. scoparium, the activity level of nicotinamide adenine dinucleotide-malate enzyme（NAD-ME） was higher.Meanwhile, H. scoparium utilized adaptive structural features, including a larger relative vessel area and a shorter distance from vein to stomata, which facilitated the movement of water. These findings implied that some C4 biochemical pathways were present in H. scoparium to respond to environmental challenges.
