Engineering behaviour of in situ cored deep cement mixed marine deposits subjected to undrained and drained shearing

The deep cement mixing(DCM)is used to improve the capacity and reduce the settlement of the soft ground by forming cemented clay *** investigation on the mechanical behaviour of the DCM samples is limited to either laboratory-prepared samples or in-situ samples under unconfined *** this study,a series of drained and undrained triaxial shearing tests was performed on the in-situ cored DCM samples with high cement content to assess their mechanical *** is found that the drainage condition affects significantly the stiffness,peak and residual strengths of the DCM samples,which is mainly due to the state of excess pore water pressure at different strain levels,*** positive before the peak deviatoric stress and negative after the peak deviatoric stress,in the undrained *** slope of the failure envelope changes obviously with the confining pressures,being steeper at lower stress levels and flatter at higher stress *** strength parameters,effective cohesion and friction angle obtained from lower stress levels(c′0 andφ′0)are 400 kPa and 58°,respectively,which are deemed to be true for design in most DCM applications where the in-situ stress levels are normally at lower values of 50-200 ***,the computed tomography(CT)scanning system was adopted to visualize the internal structures of DCM *** is found that the clay pockets existing inside the DCM samples due to uneven mixing affect markedly their stress-strain behaviour,which is one of the main reasons for the high variability of the DCM samples.

Contact models that simulate rotational resistance at particle contacts have been proposed as a means to capture the effect of shape in DEM simulations. This contribution critically explores some key issues related to the implementation of rotational resistance models; these include the need for physically meaningful model parameters, the impact of the model on the overall numerical stability/critical time increment for the DEM model, model validation, and the assessment of model performance relative to real physical materials. The discussion is centred around a rotational resistance model that captures the resistance provided by interlocking asperities on the particle surface. An expression for the maximum permissible integration time step to ensure numerical stability is derived for DEM simulations when rotational resistance is incorporated. Analytical solutions for some single-contact scenarios are derived for model validation. The ability of this type of model to provide additional fundamental insight into granular material behaviour is demonstrated using particle-scale analysis of triaxial compression simulations to examine the roles that contact rollin~ and slidin~ have on the stability of stron：~ force chains.

Objective:To scrutinize the definitions of minimal invasive surgical therapy(MIST)and to investigate urologists’knowledge,attitudes,and practices for benign prostatic obstruction ***:A 36-item survey was developed with a Delphi *** on definitions of MIST and attitudes and practices of benign prostatic obstruction surgeries were *** were invited globally to complete the online *** was achieved when more than or equal to 70%responses were“agree or strongly agree”and less than or equal to 15%responses were“disagree or strongly disagree”(consensus agree),or when more than or equal to 70%responses were“disagree or strongly disagree”and less than or equal to 15%responses were“agree or strongly agree”(consensus disagree).Results:The top three qualities for defining MIST were minimal blood loss(n=466,80.3%),fast post-operative recovery(n=431,74.3%),and short hospital stay(n=425,73.3%).The top three surgeries that were regarded as MIST were Urolift®(n=361,62.2%),Rezum®(n=351,60.5%),and endoscopic enucleation of the prostate(EEP)(n=332,57.2%).Consensus in the knowledge section was achieved for the superiority of Urolift®,Rezum®,and iTIND®over transurethral resection of the prostate with regard to blood loss,recovery,day surgery feasibility,and post-operative *** in the attitudes section was achieved for the superiority of Urolift®,Rezum®,and iTIND®over transurethral resection of the prostate with regard to blood loss,recovery,and day surgery *** on both sections was achieved for EEP as the option with the better symptoms and flow improvement,lower retreatment rate,and better suitable for prostate more than 80 ***:Minimal blood loss,fast post-operative recovery,and short hospital stay were the most important qualities for defining ***®,Rezum®,and EEP were regarded as MIST by most urologists.

Women's experiences of formula feeding their infants:an interpretative phenomenological study

Objective: This study aimed to explore the experiences of women in the process of formula feeding their infants. The World Health Organization has emphasized the importance of breastfeeding for infant health. After decades of breastfeeding promotions,breastfeeding rates in Hong Kong have been rising consistently; however, the low continuation rate is alarming. This study explores women's experiences with formula feeding their infants, including factors affecting their decision to do ***: A qualitative approach using an interpretative phenomenological analysis(IPA) was adopted as the study design. Data were collected from 2014 to 2015 through individual in-depth unstructured interviews with 16 women, conducted between 3 and 12 months after the birth of their infant. Data were analyzed using ***: Three main themes emerged as follows:(1) self-struggle, with the subthemes of feeling like a milk cow and feeling trapped;(2) family conflict, with the subtheme of sharing the spotlight; and(3) interpersonal tensions, with the subthemes of embarrassment,staring, and innocence. Many mothers suffered various stressors and frustrations during breastfeeding. These findings suggest a number of pertinent areas that need to be considered in preparing an infant feeding ***: The findings of this study reinforce our knowledge of women's struggles with multiple sources of pressure, such as career demands, childcare demands, and family life after giving birth. All mothers should be given assistance in making informed decisions about the optimal approach to feeding their babies given their individual situation and be provided with support to pursue their chosen feeding method.

LOGISTICS SCHEDULING: ANALYSIS OF TWO-STAGE PROBLEMS

This paper studies the coordination effects between stages for scheduling problems where decision-making is a two-stage process. Two stages are considered as one system. The system can be a supply chain that links two stages, one stage representing a manufacturer; and the other, a distributor It also can represent a single manufacturer, while each stage represents a different department responsible for a part of operations. A problem that jointly considers both stages in order to achieve ideal overall system performance is defined as a system problem. In practice, at times, it might not be feasible for the two stages to make coordinated decisions due to (i) the lack of channels that allow decision makers at the two stages to cooperate, and/or (ii) the optimal solution to the system problem is too difficult (or costly) to achieve.Two practical approaches are applied to solve a variant of two-stage logistic scheduling problems. The Forward Approach is defined as a solution procedure by which the first stage of the system problem is solved first, followed by the second stage. Similarly, the Backward Approach is defined as a solution procedure by which the second stage of the system problem is solved prior to solving the first stage. In each approach, two stages are solved sequentially and the solution generated is treated as a heuristic solution with respect to the corresponding system problem. When decision makers at two stages make decisions locally without considering consequences to the entire system, ineffectiveness may result - even when each stage optimally solves its own problem. The trade-off between the time complexity and the solution quality is the main concern. This paper provides the worst-case performance analysis for each approach.

采用计算流体动力学(CFD)和离散单元法(DEM)建立描述水与颗粒相互作用的耦合算法,用以分析海底滑坡的动力学特性。通过颗粒堆积体坍塌试验验证了模型的有效性。模拟分析了不同初始高宽比的水下坍塌过程,并与空气中的坍塌过程进行详细对比。研究表明:该耦合算法可以较好地描述海底边坡的失稳、流动、堆积过程;水下滑坡体与水体会发生复杂的相互作用,在颗粒由垂直下落转为水平流出时,会在水中形成涡旋;在高宽比较大的情形下,水中涡旋会明显改变颗粒最终堆积的表面形态;水中滑坡的移动距离较小、堆积厚度较大、表面松散介质易受水流影响。

Indoleamine 2, 3-dioxygenase (IDO) is a rate-limiting enzyme for the tryptophan catabolism. In human and murine cells, IDO inhibits antigen-specific T cell proliferation in vitro and suppresses T cell responses to fetal alloantigens during murine pregnancy. In mice, IDO expression is an inducible feature of specific subsets of dendritic cells (DCs), and is important for T cell regulatory properties. However, the effect of IDO and tryptophan deprivation on DC func- tions remains unknown. We report here that when tryptophan utilization was prevented by a pharmacological inhibitor of IDO, 1-methyl tryptophan (1MT), DC activation induced by pathogenic stimulus lipopolysaccharide (LPS) or inflam- matory cytokine TNF-α was inhibited both phenotypically and functionally. Such an effect was less remarkable when DC was stimulated by a physiological stimulus, CD40 ligand. Tryptophan deprivation during DC activation also regu- lated the expression of CCR5 and CXCR4, as well as DC responsiveness to chemokines. These results suggest that tryptophan usage in the microenvironment is essential for DC maturation, and may also play a role in the regulation of DC migratory behaviors.

Ruptured hepatoblastoma with massive internal bleeding in an adult

Hepatoblastoma is the most common primary hepatic tumor of children. However, only a very few cases have been reported in adults. Most studies support treatment with chemotherapy followed by surgical resection. We present the first reported case of adult hepatoblastoma in Taiwan. A 52-year-old female suffered from sudden onset of abdominal pain and general weakness for *** bleeding with hemorrhagic shock was suspected and two massive lesions in both lobes of the liver with hemoperitoneum were noted from imaging *** resection of the larger left lobe tumor and radio-frequency ablation of the right smaller one were performed. The histopathology diagnosis was of a hepatoblastoma.

A ruptured recurrent small bowel gastrointestinal stromal tumour causing hemoperitoneum

Hemoperitoneum is a rare and potentially life-threatening complication of GIST. We reported a 54- year-old man who developed disseminated intra-abdominal recurrence from a previously resected gastrointestinal stromal tumour （GIST） of the small bowel, and the patient presented with hemoperitoneum. Emergent debulking surgery was performed. A high dose imatinib was prescribed. Despite the presence of residual disease, the patient was well clinically 8 months after the operation. Even though, there is no evidence to support the routine use of debulking surgery in the management of GIST. In our patient, disease progression after second line targeted therapy and the absence of alternative treatment options for spontaneous rupture and hemoperitoneum prompted us to treat the patient aggressively. Resection of the ruptured GIST was carried out for control of bleeding and to prevent recurrent bleeding in this patient with good surgical risks. During the treatment decision-making, the patient＇s general condition, the risk of surgery and the extent of dissemination were taken into consideration. In this patient who presented with spontaneous rupture of a small intestinal GIST, the novel use of targeted therapy and aggressive surgical treatment produced reasonably good survival outcome.

The characterization and dissolution performances of spray dried solid dispersion of ketoprofen in hydrophilic carriers

Solid dispersion is one of the most promising strategies to improve oral bioavailability of poorly soluble ***,there are inconsistent dissolution performances of solid dispersion reported which entails further *** this study,solid dispersions of ketoprofen in three hydrophilic carriers,*** K30,PVPVA 6:4 and PVA were prepared and *** characterization of the physical mixture of ketoprofen and carriers shows certain extent of amorphization of the *** result is coinciding to evaluation of drug-polymer interaction using ATR-FTIR whereby higher amorphization was seen in samples with higher drug-polymer *** scanning confirms that fully amorphous solid dispersion was obtained for SD KTP PVP K30 and PVPVA system whereas partially crystalline system was obtained for SD KTP ***,dissolution profiles of the solid dispersion had shown that degree of amorphization of KTP was not directly proportional to the dissolution rate enhancement of the solid dispersion ***,it is concluded that complete amorphization does not guarantee dissolution enhancement of an amorphous solid dispersion system.
