Objective:To identify the febrile characteristics and clinical presentations associated with fatality in hospitalized adult patients with dengue virus(DENV)***:A total of 289 adult hospitalized patients with laboratoryconfirmed DENV infections were examined,of which 22 were fatal and 267 were non-fatal.A comparison of the clinical and laboratory characteristics was retrospectively conducted of the deceased and surviving *** logistic regression and receiver operating characteristic curve analysis were performed to identify predictors of ***:Fatal patients exhibited significantly more comorbidities,particularly renal and cardiac comorbidities,and they were,in general,older than control individuals(Pdays before arriving in the Emergency Department(OR=5.34;95%CI:1.39–20.6),episode of hypotension in the Emergency Department(OR=6.95;95%CI:2.40–20.1),and comorbidity with congestive heart failure(OR=11.26;95%CI:2.31–54.79)were all significantly associated with inpatient fatality due to DENV *** ROC curve analysis indicated that the final prognostic model yielded an area under the curve of 0.87(95%CI:0.79–0.97)for ***:The aforementioned clinical findings may help clinicians predict fatality among adult inpatients with DENV infection.

Objective： Animal studies have demonstrated a lipid-modulating effect of yun-cai tea. However, little is known about the lipid-lowering effect in *** aim of this study was to evaluate the lipid lowering effects and safety of yun-cai tea in patients with elevated lipid levels in a human clinical trial. Methods： This was a 12-week, randomly assigned, parallel-group, double-blind, and placebo-controlled pilot clinical study. Sixty primary hyperlipidemia patients were included and randomly assigned to the yun-cai tea group （30 patients） and the placebo group （30 patients）, for 8 weeks of treatment and 4 weeks of follow-up. The primary endpoint was changes in plasma low-density lipoprotein-cholesterol （LDL-C） at 8 weeks. The secondary endpoints included total cholesterol （TC） and triglycerides （TG）. Results： Our results revealed no statistically significant differences in LDL-C and TC between the two groups. Despite the lack of a statistically significant difference in the level of TG between the two groups, a declining trend was noted. A significant reduction of TG was observed in the yun-cai tea group at week 8, compared to baseline （P=0.048）. The incidence of stomach discomfort, gastroesophageal reflux, diarrhea, and constipation was slightly higher in the yun-cai tea group. No other significant adverse events were found. Conclusions： It is unlikely that yun-cai tea used had a blood lipid reduction effect. Further larger scale clinical trials with a longer duration and larger dose are necessary.

AIM： To study the immunological effects of physiologica doses of purple sweet potato leaves （PSPL）. METHODS： The randomized crossover study （two periods, each lasting for 2 wk） involved 16 healthy non-smoking adults of normal weight. The 6-wk study consisted of a run-in （wk 1） PSPL diet （daily consumption of 200 g PSPL） or a control diet （low polyphenols, with the amount of carotenoids adjusted to the same level as that of PSPL）（wk 2-3）, washout diet （wk 4）, and switched diet （wk 5-6）. Fasting blood was collected weekly in the morning. T-lymphocyte function was assessed via the proliferation and secretion of immunoreactive cytokines. Salivary IgA secretion and the specific cytotoxic activities of cytotoxic T lymphocytes and natural killer （NK） cells were determined. RESULTS： The plasma 13-carotene level increased with time in both groups, while the plasma polyphenol level decreased in the control group, and no significant difference was detected between the two groups. Although p^asma polyphenol levels did not significantly increase in the PSPL group at the end of the study, they were significantly elevated in urine. PSPL consumption produced a significant increase in proliferation responsiveness of peripheral blood mononuclear cells （PBMC） and their secretion of immunoreactive IL-2 and IL-4. As well, lytic activity in NK cells was elevated in a time-dependent fashion. Salivary IgA secretion significantly decreased in control group after 2 wk, and returned to baseline following dietary switch to PSPL. CONCLUSION： Consumption of PSPL modulates various immune functions including increased proliferation responsiveness of PBMC, secretion of cytokines IL-2 and IL-4, and the lytic activity of NK cells. The responsible determinants of PSPL remain to be elucidated, as does the biological significance of the present observations.

AIM： To investigate acute transient hepatocellular injury in patients with cholelithiasis and cholecystitis but no evidence of choledocholithiasis.METHODS： The medical records of patients with cholelithiasis who underwent cholecystectomy between July 2003 and June 2007 were retrospectively reviewed. Imaging studies to detect common bile duct （CBD） stones were performed in 186 patients, who constituted the study population. Biochemical liver tests before and after surgery, and with the presence or absence of CBD stones were analyzed.RESULTS： In 96 patients with cholelithiasis and cholecystitis without evidence of CBD stones, 49 （51.0%） had an alanine aminotransferase level elevated to 2-3 times the upper limit of normal, and 40 （41.2%） had an elevated aspartate aminotransferase level. Similar manifestations of hepatocellular injury were, as would be expected, even more obvious in the 90 patients with CBD stones. These markers of hepatocellular injury resolved almost completely within 2 wk to 1 mo after cholecystectomy. Compared to 59 patients with histologically less severe cholecystitisin the group undergoing urgent surgery （total 74 patients）, the 15 patients with a gangrenous gallbladder had a higher mean level of total bilirubin （2.14 ± 1.27 mg/dL vs 2.66 ± 2.97 mg/dL, P 〈 0.001） and white cell count （9480 ± 4681/μL vs 12840 ± 5273/μL, P = 0.018）.CONCLUSION： Acute hepatocellular injury in cholelithiasis and cholecystitis without choledocholithiasis is mild and transient. Hyperbilirubinemia and leukocytosis may predict severe inflammatory changes in the gallbladder.

From ZINC database with a total of 1.8 million small molecules, four compounds are identified as prolyl hydroxylase 2 inhibitors through a virtual screening workflow that sequentially incorporates machine learning, molecular docking, and molecular dynamics. Among them, compound 103,(E)-5-(5-((2-(1Htetrazol-5-yl)hydrazineylidene)methyl)furan-2-yl)isoindoline-1,3-dione, promotes the migration and capillary tube formation capacity of human umbilical vein endothelial cells through enhancing the stability of hypoxia inducible factor-1α and increasing the level of vascular endothelial growth factor.

By means of chemical reduction,nanoparticles of platinum were deposited on the surface of multi walled carbon nanotubes (MWCNTs).The performance of hydrogen storage of as prepared MWCNTs decorated with platinum was *** results indicate that:(1) Hydrogen uptake is more quick and intense for decorated MWCNTs than that for not decorated ones at 10.931MPa and room *** saturation of hydrogen uptake of the former only lasts about 30min,while the latter needs about 150 min;(2) The amount of hydrogen uptake of decorated MWCNTs is about 1.13wt%, which is larger than that of not decorated ones(about 0.54wt%);(3) However,more than 37% hydrogen absorbed by decorated MWCNTs is chemisorbed.

AIM:To investigate the 10-year results of treating low rectal cancer by a single surgeon in one ***:From Oct 1998 to Feb 2009,we prospectively followed a total of 62 patients with cT2-4 low rectal cancer with lower tumor margins measuring at 3 to 6 cm above the anal *** patients received neoadjuvant chemoradiation(CRT) for 6 *** them,85% of the patients received 225 mg/m2/d 5-fluorouracil using a portable infusion *** whole pelvis received a total dose of 45 Gy of irradiation in 25 fractions over 5 *** interval from CRT completion to surgical intervention was planned to be approximately 6-8 *** mesorectal excision(TME) and routine defunctioning stoma construction were performed by one *** distal resection margin,circumferential resection margin,tumor regression grade(TRG) and other parameters were *** used TRG to evaluate the tumor response after neoadjuvant *** evaluated anal function outcomes using the Memorial Sloan-Kettering Cancer Center anal function scores after closure of the defunctioning ***:The median distance from the lower margin of rectal cancer to the anal verge was 5 cm:6 cm in 9 patients,5 cm in 32 patients,4 cm in 10 patients,and 3 cm in 11 *** receiving neoadjuvant CRT,45 patients(72.6%) had a cT3-4 tumor,and 21(33.9%) patients had a cN1-2 lymph node *** CRT,30 patients(48.4%) had a greater than 50% clinical reduction in tumor *** final pathology reports revealed that 33 patients(53.2%) had a ypT3-4 tumor and 12(19.4%) patients had ypN1-2 lymph node *** patients completed the entire course of neoadjuvant *** patients developed only Grade 1-2 toxicities during *** patients(21%) achieved a pathologic complete *** post-operative complications *** 90% of the defunctioning stomas were closed within 6 *** local recurrence rate was 3.2%.Pathologic lymph node involvement was the only prognostic factor predicting

Polymeric carbon nitride(PCN)has emerged as a promising candidate for photocatalytic hydrogen evolution,but its dependence on scarce and high-cost noble metal co-catalysts severely limits its extensive *** will be of great promise to develop non-noble metal single-atom co-catalysts with low-cost and high atom utilization to improve the photocatalytic performance over ***,single Ni atoms are successfully anchored onto carbon-vacant PCN nanosheets(CCN-SANi)via a two-step ammonia thermal treatment and photo-deposition *** calculations and experimental results demonstrate that the optical absorption property and the charge transfer ability of CCN-SANi have been significantly improved with the introduction of single Ni atoms to form Ni-N3 *** comparison to carbon-vacant PCN(CCN)loaded with Ni clusters,the obtained CCN-SANi exhibits 11.4 times increased photocatalytic performance,with the highest hydrogen evolution rate reaching 511μmol/(g·h),which is even 1.7 times higher than that of CCN loaded with Pt *** research proposes an inspiring and reliable strategy to design novel single-atom semiconducting polymers with electronic structures manipulated for efficient photocatalysis.
