Rationale Haematoma growth is common early after intracerebral haemorrhage(ICH),and is a key determinant of outcome.Tranexamic acid,a widely available antifibrinolytic agent with an excellent safety profile,may reduce haematoma growth.Methods and design Stopping intracerebral haemorrhage with tranexamic acid for hyperacute onset presentation including mobile stroke units(STOP-MSU)is a phase Ⅱ double-blind,randomised,placebo-controlled,multicentre,international investigator-led clinical trial,conducted within the estimand statistical framework.Hypothesis In patients with spontaneous ICH,treatment with tranexamic acid within 2 hours of onset will reduce haematoma expansion compared with placebo.Sample size estimates A sample size of 180 patients(90 in each arm)would be required to detect an absolute difference in the primary outcome of 20%(placebo 39%vs treatment 19%)under a two-tailed significance level of 0.05.An adaptive sample size re-estimation based on the outcomes of 144 patients will allow a possible increase to a prespecified maximum of 326 patients.Intervention Participants will receive 1 g intravenous tranexamic acid over 10 min,followed by 1 g intravenous tranexamic acid over 8 hours;or matching placebo.Primary efficacy measure The primary efficacy measure is the proportion of patients with haematoma growth by 24±6 hours,defined as either≥33%relative increase or≥6 mL absolute increase in haematoma volume between baseline and follow-up CT scan.Discussion We describe the rationale and protocol of STOP-MSU,a phase Ⅱ trial of tranexamic acid in patients with ICH within 2 hours from onset,based in participating mobile stroke units and emergency departments.

High glucose reduces Nrf2-dependent cRAGE release and enhances inflammasome-dependent IL-1βproduction in monocytes:the modulatory effects of EGCG

Soluble receptor for advanced glycation end products(sRAGE)acts as a decoy sequestering of RAGE ligands,thus preventing the activation of the ligand-RAGE axis linking human diseases.However,the molecular mechanisms underlying sRAGE remain unclear.In this study,THP-1 monocytes were cultured in normal glucose(NG,5.5 mmol/L)and high glucose(HG,15 mmol/L)to investigate the effects of diabetesrelevant glucose concentrations on sRAGE and interleukin-1β(IL-1β)secretion.The modulatory effects of epigallocatechin gallate(EGCG)in response to HG challenge were also evaluated.HG enhanced intracellular reactive oxygen species(ROS)generation and RAGE expression.The secretion of sRAGE,including esRAGE and cRAGE,was reduced under HG conditions,together with the downregulation of a disintegrin and metallopeptidase 10(ADAM10)and nuclear factor erythroid 2-related factor 2(Nrf2)nuclear translocation.Mechanistically,the HG effects were counteracted by siRAGE and exacerbated by siNrf2.Chromatin immunoprecipitation results showed that Nrf2 binding to the ADAM10 promoter and HG interfered with this binding.Our data reinforce the notion that RAGE and Nrf2 might be sRAGE-regulating factors.Under HG conditions,the treatment of EGCG reduced ROS generation and RAGE activation.EGCG-stimulated cRAGE release was likely caused by the upregulation of the Nrf2-ADAM10 pathway.EGCG inhibited HG-mediated NLRP3 inflammasome activation at least partly by stimulating sRAGE,thereby reducing IL-1βrelease.

Federated Feature Concatenate Method for Heterogeneous Computing in Federated Learning

Federated learning is an emerging machine learning techniquethat enables clients to collaboratively train a deep learning model withoutuploading raw data to the aggregation server. Each client may be equippedwith different computing resources for model training. The client equippedwith a lower computing capability requires more time for model training,resulting in a prolonged training time in federated learning. Moreover, it mayfail to train the entire model because of the out-of-memory issue. This studyaims to tackle these problems and propose the federated feature concatenate(FedFC) method for federated learning considering heterogeneous clients.FedFC leverages the model splitting and feature concatenate for offloadinga portion of the training loads from clients to the aggregation server. Eachclient in FedFC can collaboratively train a model with different cutting layers.Therefore, the specific features learned in the deeper layer of the serversidemodel are more identical for the data class classification. Accordingly,FedFC can reduce the computation loading for the resource-constrainedclient and accelerate the convergence time. The performance effectiveness isverified by considering different dataset scenarios, such as data and classimbalance for the participant clients in the experiments. The performanceimpacts of different cutting layers are evaluated during the model training.The experimental results show that the co-adapted features have a criticalimpact on the adequate classification of the deep learning model. Overall,FedFC not only shortens the convergence time, but also improves the bestaccuracy by up to 5.9% and 14.5% when compared to conventional federatedlearning and splitfed, respectively. In conclusion, the proposed approach isfeasible and effective for heterogeneous clients in federated learning.

Handheld 3D Scanner for Small Objects Using Kinect

This paper presents a handheld 3D vision-based scanner for small objects by using Kinect. It is different from the previous color-glove-based approaches which require segmenting the target object. First, we eliminate the noises and the outliers caused by holding hands. Second, we apply Kinect-fusion algorithm and truncated signed distance function （TSDF） to represent 3D surfaces. Third, we propose a modified integration strategy to eliminate the hand effect. Fourth, we take advantage of the parallel computation of GPUs for real-time operation. The major contributions of this paper are （1） the registration precision is improved, （2） the oflline amendment and loop closure operation are not required, and （3） concave 3D object reconstruction is feasible.

AIM:To investigate the prevalence of gallstone dis-ease(GSD) and to evaluate the risk of symptomatic GSD among diabetic patients.METHODS:The study was conducted by analyzing the National Health Research Institutes(NHRI) dataset of ambulatory care patients,inpatient claims,and the updated registry of beneficiaries from 2000 to 2008.A total of 615 532 diabetic patients without a prior history of hospital treatment or ambulatory care visits for symptomatic GSD were identified in the year 2000.Age-and gender-matched control individuals free from both GSD and diabetes from 1997 to 1999 were randomly selected from the NHIR database(n = 614 871).The incidence densities of symptomatic GSD were estimated according to the subjects' diabetic status.The distributions of age,gender,occupation,income,and residential area urbanization were compared between diabetic patients and control subjects using Cox proportion hazards models.Differences between the rates of selected comorbidities were also assessed in the two groups.RESULTS:Overall,60 734 diabetic patients and 48 116 control patients developed symptomatic GSD and underwent operations,resulting in cumulative operation rates of 9.87% and 7.83%,respectively.The age and gender distributions of both groups were similar,with a mean age of 60 years and a predominance of females.The diabetic group had a significantly higher prevalence of all comorbidities of interest.A higher incidence of symptomatic GSD was observed in females than in males in both groups.In the control group,females under the age of 64 had a significantly higher incidence of GSD than the corresponding males,but this difference was reduced with increasing age.The cumulative incidences of operations for symptomatic GSD in the diabetic and control groups were 13.06 and 9.52 cases per 1000 person-years,respectively.Diabetic men exhibited a higher incidence of operations for symptomatic GSD than did their counterparts in the control group(12.35 vs 8.75 cases per 1000 person-years).CONCLUSION:The association of diabet

Identifying survival protective factors for chronic dialysis patients with surgically confirmed acute mesenteric ischemia

BACKGROUND Mesenteric ischemia is significantly more common in end-stage kidney disease patients undergoing chronic dialysis than in the general population and is associated with high morbidity and mortality. However, reports on prognostic factors in this population are limited.AIM To elucidate the in-hospital outcomes of acute mesenteric ischemia in chronic dialysis patients and to analyze protective factors for survival.METHODS The case data of 426 chronic dialysis patients who were hospitalized in a tertiary medical center for acute mesenteric ischemia over a 14-year period were retrospectively reviewed. Of these cases, 103 were surgically confirmed, and the patients were enrolled in this study. A Cox regression analysis was used to evaluate the protective factors for survival.RESULTS The in-hospital mortality rate among the 103 enrolled patients was 46.6%.Univariate analysis was performed to compare factors in survivors and nonsurvivors, with better in-hospital outcomes associated with a surgery delay(defined as the time from onset of signs and symptoms to operation) y 1 of hospitalization, no resection of the colon, and a total bowel resection length ymphocyte counts, and lower C-reactive protein levels had better in-hospital outcomes. Following multivariate adjustment, a higher potassium level on day 1 of hospitalization(HR 1.71, 95%CI 1.19 to 2.46;P = 0.004), a lower neutrophil count(HR 0.91, 95%CI 0.84 to 0.99;P = 0.037) at 1 wk after admission, resection not involving the colon(HR 2.70, 95%CI 1.05 to 7.14;P = 0.039), and a total bowel resection length y associated with survival.CONCLUSION A surgery delay ysis patients with acute mesenteric ischemia.

Optimal design theories of tuned mass dampers with nonlinear viscous damping

Optimal design theory for linear tuned mass dampers （TMD） has been thoroughly investigated, but is still under development for nonlinear TMDs. In this paper, optimization procedures in the time domain are proposed for design of a TMD with nonlinear viscous damping. A dynamic analysis of a structure implemented with a nonlinear TMD is conducted first. Optimum design parameters for the nonlinear TMD are searched using an optimization method to minimize the performance index. The feasibility of the proposed optimization method is illustrated numerically by using the Taipei 101 structure implemented with TMD. The sensitivity analysis shows that the performance index is less sensitive to the damping coefficient than to the frequency ratio. Time history analysis is conducted using the Taipei 101 structure implemented with different TMDs under wind excitation. For both linear and nonlinear TMDs, the comfort requirements for building occupants are satisfied as long as the TMD is properly designed. It was found that as the damping exponent increases, the relative displacement of the TMD decreases but the damping force increases.

Experimental study of the separated joint of an underground pipeline rehabilitated by cured-in-place pipe

Reinforced concrete pipes(RCPs)were commonly used in older underground pipelines in Taiwan,China.However,their joints are prone to damage,including fracture and separation,from problems such as overloading,poor backfilling,and liquefaction.For the rehabilitation of such aged pipelines,trenchless methods are preferred to minimize the impact on transportation.The cured-in-place-pipe(CIPP)rehabilitation could be one of the most popular trenchless methods suitable for the cases with angular alignment and changing cross-section.However,according to the existing standards,no details are available on the performance of the rehabilitated parts of the damaged pipeline,including the effect of separated joint.This study designed and conducted laboratory testing on the composite pipe joint(the CIPP rehabilitated RCPs with a separated joint)with different scenarios,including radial and longitudinal loading tests.The radial test results indicated that,for a small separation distance,CIPP rehabilitation can restore the original function of a pipeline and improve its hoop strength.The longitudinal test revealed that the damage due to bending was controlled by the tensile strength of the liner material.The ultimate flexural strength decreased as the separation distance increased.However,pipelines may be subjected to additional loads due to flotation or subsidence,for which the longitudinal loading capacity is often the limiting parameter.For such conditions,it is necessary to confirm,by more research,whether the wall thickness is sufficient.

Real-Time Human Body Motion Capturing System

This paper proposes a human body motion capturing system using the depth images. It consists of three processes to estimate the human pose parameters. First, we develop a pixel-based body part classifier to segment the human silhouette into different body part sub-regions and extract the primary joints. Second, we convert the distribution of the joints to the feature vector and apply the regression forest to estimate human pose parameters. Third, we apply the temporal constraints mechanism to find the best human pose parameter with the minimum estimation error. In experiments, we show that our system can operate in real-time with sufficient accuracy.
