Benefit in physical function and quality of life to nonsurgical treatment of varicose veins: Pilot study

BACKGROUND Studies on varicose veins have focused its effects on physical function;however,whether nonsurgical treatments alter muscle oxygenation or physical function remains unclear.Moreover,the differences in such functions between individuals with varicose veins and healthy individuals remain unclear.AIM To investigate changes in physical function and the quality of life(QOL)following nonsurgical treatment of patients with varicose veins and determine the changes in their muscle oxygenation during activity.METHODS We enrolled 37 participants(those with varicose veins,n=17;healthy individuals,n=20).We performed the following measurements pre-and post-nonsurgical treatment in the varicose vein patients and healthy individuals:Calf muscle oxygenation during the two-minute step test,open eyes one-leg stance,30 s sit-to-stand test,visual analog scale(VAS)for pain,Pittsburgh sleep quality index,physical activity assessment,and QOL assessment.RESULTS Varicose veins patients and healthy individuals differ in most variables(physical function,sleep quality,and QOL).Varicose veins patients showed significant differences between pre-and post-nonsurgical treatment—results in the 30 sit-to-stand test[14.41(2.45)to 16.35(4.11),P=0.018],two-minute step test[162.29(25.98)to 170.65(23.80),P=0.037],VAS for pain[5.35(1.90)to 3.88(1.73),P=0.004],and QOL[39.34(19.98)to 26.69(17.02),P=0.005];however,no significant difference was observed for muscle oxygenation.CONCLUSION Nonsurgical treatment improved lower extremity function and QOL in varicose veins patients,bringing their condition close to that of healthy individuals.Future studies should include patients with severe varicose veins requiring surgery to confirm our findings.

BACKGROUND Venous adventitial cystic disease(VACD)is a rare disease characterized by cysts,filled with a gelatinous mucous substance similar to joint fluid,in the adventitia of blood vessels adjacent to the joints.It is often misdiagnosed as deep vein thrombosis(DVT),femoral varices,venous tumors,or lymphadenopathy.CASE SUMMARY A 69-year-old woman visited our hospital with a complaint of swelling in the right lower extremity.The patient was diagnosed with DVT and prescribed apixaban at an outpatient clinic.After 3 wk,the patient was hospitalized again because of sudden swelling in the right lower extremity.We diagnosed VACD and performed surgery for cyst removal as well as patch angioplasty and thrombectomy of the right common femoral vein.The patient received anticoagulants for 6 mo and has been doing well without recurrence for 1 year postoperatively.CONCLUSION Recurrent VACD requires complete removal of the connections to the joint cavity to prevent recurrence.

Acute limb ischemia after minimally invasive cardiac surgery using the ProGlide:A case series

BACKGROUND We report two cases of acute femoral artery occlusion following the use of ProGlide in minimally invasive cardiac surgery and insertion of large-bore catheters through the common femoral artery.This will add to the existing body of literature by highlighting the possible complications associated with the use of ProGlide and reiterate that the use of the sono-guided ProGlide skill will reduce the incidence of these complications.CASE SUMMARY A 78-year-old man underwent minimally invasive cardiac surgery for severe aortic valve stenosis.After the operation,the puncture site of the common femoral artery was closed using ProGlide.The next morning,after regaining consciousness,he complained of pain,motor weakness(grade 2),and coldness in the right lower extremity.A 65-year-old man underwent minimally invasive cardiac surgery for a large secundum atrial septal defect(5 cm×5 cm).After the operation,the puncture site of the common femoral artery was closed using ProGlide.After extubation,the patient complained of paresthesia of the right thigh.Both the patients underwent emergency surgery for acute occlusion of the common femoral artery.CONCLUSION If the sono-guided ProGlide skill is used,complications can be prevented,and ProGlide can be safely used.

Monitoring taste-inducing ions and molecules continuously in liquids or solutions is of great considerable matter for the realization of the electronic tongue(E-tongue).Particularly from the five major tastes,the highly selective,sensitive detection of Na^(+)in real-time is prioritized.Prioritization is due to the saltiness of food is the key ingredient in most meals.Nevertheless,existing Na^(+)detecting devices have relatively low performances of selectivity,sensitivity,and lack of on–off functions.Additionally,conventional devices significantly deteriorate in capac-ity due to repetitive usage or lifetime shortage by degradation of the sensing mate-rial.Herein,a graphene-based channel was rationally designed by the facile decoration of Calix[4]arene and Nafion to address this issue.They act as a receptor and a molecular sieve,respectively,to enhance selectivity and sensitivity and elon-gate the life expectancy of the device.This device was merged with a microfluidic channel to control the injection and withdrawal of solutions to fulfill dynamic on–off functions.The fabricated device has highly selective,sensitive Na^(+)detection properties compared to other 10 molecule/ionic species.Dynamic on–off functions of the device were available,also possesses a long lifespan of at least 220 days.Additionally,it can precisely discriminate real beverages containing Na^(+),which can be observed by principal component analysis plot.These features offer the possibility of ascending to a platform for E-tongues in near future.

Ammonia allows storage and transport of hydrogen over long distances and is an attractive potential hydrogen carrier.Electrochemical decomposition has recently been used for the conversion of ammonia to hydrogen and is regarded as a future technology for production of CO_(2)-free pure hydrogen.Herein,a heterostructural Pt-Ir dual-layer electrode is developed and shown to achieve successful long-term operation in an ammonia electrolyzer with an anion exchange membrane(AEM).This electrolyzer consisted of eight membra ne electrode assemblies(MEAs)with a total geometric area of 200 cm~2 on the anode side,which resulted in a hydrogen production rate of 25 L h~(-1).We observed the degradation in MEA performance attributed to changes in the anode catalyst layer during hydrogen production via ammonia electrolysis.Furthermore,we demonstrated the relationship between the ammonia oxidation reaction(AOR)and the oxygen evolution reaction(OER).

Experience of Mental Health Professionals Collaborating with Peer Supporters in a Community Mental Health Service Team

This study explored how mental health professionals collaborate with peer supporters with mental disabilities in a community mental health institution.From January 19 to February 23,2021,three 60 min interviews were conducted with six mental health professionals working at a Korean community center.The results were qualitatively analyzed and divided into four themes and eight categories.The four themes were the perceptions of and challenges in working with peer supporters with mental disabilities,conflict and confusion about working with peer supporters,forming partnerships with peer supporters,and policy support for peer supporters’job security.Participants reported vague anxiety about working with a peer supporter and difficulties with the trial-and-error process of adjusting to the role as challenging.Over time,however,they realized that they needed to make an effort to develop meaningful relationships with peer supporters and mental health professionals.Thus,through this study,we realized that there was a need to improve the system,such as building infrastructure for job stability for peer support workers and capacity building tailored to the mental disorders.Although peer supporters play various roles while working with mental health professionals,this study showed the possibility of mutual growth through communication and cooperation.These findings will help prepare systems necessary for collaboration between the two teams amidst the increasing institutionalization of peer support for mental disorders.

Introduction Antibiotic use for acute otitis media(AOM)is one of the major sources of antimicrobial resistance.However,the effective minimal antibiotic duration for AOM remains unclear.Moreover,guidelines often recommend broad ranges(5-10 days)of antibiotic use,yet the clinical impact of such a wide window has not been assessed.Methods We systematically searched PubMed/MEDLINE,Embase,Scopus,Web of Science,and Cochrane Library from database inception to 6 October 2021.Network meta-analysis was conducted on randomized controlled trials that assessed antibiotic treatment for AOM in children(PROSPERO CRD42020196107).Results For amoxicillin and amoxicillin-clavulanate,7-day regimens were noninferior to 10-day regimens in clinical responses[amoxicillin:risk ratio(RR)0.919(95%CI 0.820-1.031),amoxicillin-clavulanate:RR 1.108(0.957-1.282)],except for≤2 years.For the third-generation cephalosporins,7-day and 10-day regimens had similar clinical responses compared to placebo[7-day:RR 1.420(1.190-1.694),10-day:RR 1.238(1.125-1.362)compared to placebo].However,5-day regimens of amoxicillin-clavulanate and third-generation cephalosporins were inferior to 10-day regimens.Compared to amoxicillin,a shorter treatment duration was tolerable with amoxicillin-clavulanate.Conclusions Our findings indicated that 1O days of antibiotic use may be unnecessarily long,while the treatment duration should be longer than 5 days.Otherwise,5-day regimens would be sufficient for a modest treatment goal.Our findings revealed that the current wide range of recommended antibiotic durations may have influenced the clinical outcome of AOM,and a narrower antibiotic duration window should be re-established.

Self-adhesive pads in defi brillation or cardioversion: risks due to unfamiliarity and recommendations

During cardiopulmonary resuscitation(CPR),defibrillation,or cardioversion,the use of self-adhesive pads instead of conventional paddles has gradually become common.In particular,it was recommended during the COVID-19 pandemic,as it allows defibrillation or cardioversion to be performed without close contact between the rescuer and patient.[1,2]Self-adhesive pads can minimize the downtime of chest compressions before and after defibrillation and protect the patient from electric arc formation,skin burns,and fi re.Once the pads are attached,electrocardiographic rhythm analysis,defibrillation,synchronous cardioversion,and cardiovascular support are all possible.Unlike paddles,self-adhesive pads can save time because the application of conductive jelly,repeated positioning,and application of 10-12 kg of pressure on the chest are not needed.The European Resuscitation Council(ERC)Guidelines 2021 state that the use of self-adhesive pads is recommended in advanced cardiovascular life support(ACLS).[3]Nevertheless,some medical staff still tend to prefer the use of conventional paddles instead of self-adhesive pads in hospitals.

Objective To investigate the in vivo immunomodulatory and anti-tumor mechanisms of the combined treatment of novel Four-Herb formula(4HF)and doxorubicin in triple-negative breast cancer(TNBC).Methods Murine-derived triple-negative mammary carcinoma cell line,4T1 cells,was cultured and inoculated into mouse mammary glands.Sixty-six mice were randomly assigned into 6 groups(n=11 in ench):naive,control,LD 4HF(low dose 4HF),HD 4HF(high dose 4HF),LD 4HF+D(low dose and doxorubicin),and D(doxorubicin).Apart from the naive group,each mouse received subcutaneous inoculation with 5×10^(5)4T1 cells resuspended in 100µL of normal saline in the mammary fat pads.Starting from the day of tumor cell inoculation,tumors were grown for 6 days.The LD and HD groups received daily oral gavage of 658 and 2,630 mg/kg 4HF,respectively.The LD 4HF+D group received daily oral gavage of 658 mg/kg 4HF and weekly intraperitoneal injection of doxorubicin(5 mg/kg).The D group received weekly intraperitoneal injections of doxorubicin(5 mg/kg).The treatment naive mice received daily oral gavage of 0.2 mL double distilled water and 0.1 mL normal saline via intraperitoneal injection once a week.The control group received daily oral gavage of 0.2 mL double-distilled water.The treatment period was 30 days.At the end of treatment,mice organs were harvested to analyze immunological activities via immunophenotyping,gene and multiplex analysis,histological staining,and gut microbiota analysis.Results Mice treated with the combination of 4HF and doxorubicin resulted in significantly reduced tumor and spleen burdens(P<0.05),altered the hypoxia and overall immune lymphocyte landscape,and manipulated gut microbiota to favor the anti-tumor immunological activities.Moreover,immunosuppressive genes,cytokines,and chemokines such as C-C motif chemokine 2 and interleukin-10 of tumors were significantly downregulated(P<0.05).4HF-doxorubicin combination treatment demonstrated synergetic activities and was most effective in activating the anti-tumor immune response(P<

Ensemble Wind Power Prediction Interval with Optimal Reserve Requirement

Wind power prediction interval(WPPI)models in the literature have predominantly been developed for and tested on specific case studies.However,wind behavior and characteristics can vary significantly across regions.Thus,a prediction model that performs well in one case might underperform in another.To address this shortcoming,this paper proposes an ensemble WPPI framework that integrates multiple WPPI models with distinct characteristics to improve robustness.Another important and often overlooked factor is the role of probabilistic wind power prediction(WPP)in quantifying wind power uncertainty,which should be handled by operating reserve.Operating reserve in WPPI frameworks enhances the efficacy of WPP.In this regard,the proposed framework employs a novel bi-layer optimization approach that takes both WPPI quality and reserve requirements into account.Comprehensive analysis with different real-world datasets and various benchmark models validates the quality of the obtained WPPIs while resulting in more optimal reserve requirements.
