Reliability of the measurement of glenoid bone defect in anterior shoulder instability

Background:The size of the glenoid bone defect is an important index in selecting the appropriate treatment for anterior shoulder instability.However,the reliability of glenoid bone defect measurement is controversial.The purpose of the present study was to investigate the reliabilities of measurements of the glenoid bone defect on computed tomography and to explore the predisposing factors leading to inconsistency of these measurements.Methods:The study population comprised 69 consecutive patients who underwent surgery for recurrent anterior shoulder dislocation in Peking University Fourth School of Clinical Medicine from March 2016 to January 2017.The glenoid bone defect was measured by three surgeons on‘self-confirmed’and‘designated’3-D en-face views,and repeated after an interval of 3 months.Measurements included the ratio of the defect area to the best-fit circle area,and the ratio of the defect width to the diameter of the best-fit circle.The inter-and intra-observer reliabilities of the measurements were evaluated using intraclass correlation coefficients(ICCs).The maximum absolute inter-and intra-observer differences and the cumulative percentages of cases with inter-and intraobserver differences greater than these respective levels were calculated.Results:Almost all linear defect values were bigger than the areal defect values.The inter-observer ICCs for the areal defect were 0.557 and 0.513 in the‘self-confirmed’group and 0.549 and 0.431 in the‘designated’group.The inter-observer reliabilities for the linear defect were moderate or fair in the‘self-confirmed’group(ICC=0.446,0.374)and‘designated’group(ICC=0.402,0.327).The ICCs for intra-observer measurements were higher than those for inter-observer measurements.The respective maximum interand intra-observer absolute differences were 13.9%and 13.2%in the‘self-confirmed’group,and 15.8%and 9.8%in the‘designated’group.Conclusions:The areal measurement of the glenoid bone defect is more reliable than the linear measurement.The reliability of the glenoid defe

Electrocardiographic findings in an elderly patient before and after resolution of iatrogenic hyperkalemia

Hyperkalemia is a life-threatening electrolyte disorder that often occurs in patients with chronic kidney disease(CKD)and in those using potassium-sparing diuretics.Hyperkalemia can destabilize myocardial conduction by reducing the resting membrane potential,leading to increased cardiac depolarization,myocardial excitability,and arrhythmias,which can promote progress to ventricular fibrillation and asystole.[1]These patients often present with non-specific symptoms,such as fatigue and inappetence,or even sudden death.Determining the need for emergency therapy or less aggressive treatment is largely based on the patient’s electrophysiological presentation.In clinical settings,we need to differentiate hyperkalemia from hyperacute myocardial infarction,early repolarization,and pericarditis because of similarities in T-wave and ST-segment changes in the electrocardiogram(ECG).Here we present a case of hyperkalemia caused by amiloride,and discuss the ECG changes associated with an altered level of serum potassium.This case may help clinicians learn to recognize and manage patients with hyperkalemia.

Two different autorefractors for vision screening in children and adolescents

AIM:To compare the consistency of two autorefractors(Tianle RM-9000 and Topcon KR-800)for school-age myopia children,and to provide a basis for largescale data analysis and comparison.METHODS:The refractive error in 909 subjects(age 4–18y)were measured using both autorefractors without cycloplegia.The data were analyzed using Fourier decomposition and the correlation coefficients,intraclass correlation coefficients(ICC),and Bland-Altman limits of agreement(LoA)for each parameter were calculated.RESULTS:There was a strong correlation between the spherical equivalent(SE),sphere diopter(DS),and cylinder diopter(DC)readings of the Tianle RM-9000 and those of the Topcon KR-800,with correlation coefficient values of 0.98,0.98 and 0.83 and ICC values of 0.99,0.99 and 0.93,respectively.However,the correlation coefficients and ICC values of J0 and J45 were unreliable(R=-0.004,-0.034;both ICC<0.10).Bland-Altman analysis revealed that SE,DS,and DC measured by the Tianle RM-9000 were significantly biased toward myopia compared with the Topcon KR-800,and the mean differences were-0.072,-0.026,-0.091 D,respectively(all P<0.01).The minimum absolute value of the difference within the 95%LoA for SE,DS,and DC was 0.63 D,0.50 D,0.62 D,respectively;all these values were in the clinically acceptable range.For J0 and J45,the mean differences were close to zero(P=0.43,0.84);however,the 95%LoA were relatively wide(J0 SD:0.53;95%CI:-1.00,1.10;J45 SD:0.52;95%CI:-1.00,1.00).CONCLUSION:The two autorefractors are consistent with each other,as the differences in SE,DS,and DC were within the clinically acceptable range.Readers can compare the data measured by either device in different studies and use the two devices in the same study to generate a dataset that can be analyzed together.However,the J0 and J45 vectors are unreliable and should not be used to assess astigmatism.

An overview and perspective of identifying lunar craters

The spatiotemporal evolution of lunar impact craters is crucial for investigating lunar interior structure,internal and external dynamic processes,and interplanetary impact history.Advances in lunar crater identification are reviewed based on topography and gravity data,and the features and mechanisms of topographic or buried craters are analyzed regarding morphology,gravity anomalies,gravity gradients,and the underlying crust-mantle interface relief.Based on the compiled crater catalog,the early lunar impact flux and thermal evolution are further discussed according to the basin ages and their interior structures.For some ancient impact basins,the crater size-frequency distribution measurements revealed age discrepancies from previous studies,suggesting that the lunar late heavy bombardment event started at~3.95 Ga.The degraded bulge structures of the crust-mantle interface beneath mare basins reveal that these basins formed on the lunar crust surface under high-temperature conditions and underwent prolonged relaxation compared to highland basins.Finally,we reveal that the up-to-date identification of lunar buried craters remains inaccurate and incomplete,preventing us from accurately reconstructing lunar and interplanetary impact histories.Therefore,we propose that a gravity model constructed using localized orthogonal basis functions can be useful for identifying lunar craters.

Classifying Chinese Medicine Constitution Using Multimodal Deep-Learning Model

Objective:To develop a multimodal deep-learning model for classifying Chinese medicine constitution,i.e.,the balanced and unbalanced constitutions,based on inspection of tongue and face images,pulse waves from palpation,and health information from a total of 540 subjects.Methods:This study data consisted of tongue and face images,pulse waves obtained by palpation,and health information,including personal information,life habits,medical history,and current symptoms,from 540 subjects(202 males and 338 females).Convolutional neural networks,recurrent neural networks,and fully connected neural networks were used to extract deep features from the data.Feature fusion and decision fusion models were constructed for the multimodal data.Results:The optimal models for tongue and face images,pulse waves and health information were ResNet18,Gate Recurrent Unit,and entity embedding,respectively.Feature fusion was superior to decision fusion.The multimodal analysis revealed that multimodal data compensated for the loss of information from a single mode,resulting in improved classification performance.Conclusions:Multimodal data fusion can supplement single model information and improve classification performance.Our research underscores the effectiveness of multimodal deep learning technology to identify body constitution for modernizing and improving the intelligent application of Chinese medicine.

A Provable Positivity-Preserving Local Discontinuous Galerkin Method for the Viscous and Resistive MHD Equations

In this paper,we construct a high-order discontinuous Galerkin(DG)method which can preserve the positivity of the density and the pressure for the viscous and resistive magnetohydrodynamics(VRMHD).To control the divergence error in the magnetic field,both the local divergence-free basis and the Godunov source term would be employed for the multi-dimensional VRMHD.Rigorous theoretical analyses are presented for one-dimensional and multi-dimensional DG schemes,respectively,showing that the scheme can maintain the positivity-preserving(PP)property under some CFL conditions when combined with the strong-stability-preserving time discretization.Then,general frameworks are established to construct the PP limiter for arbitrary order of accuracy DG schemes.Numerical tests demonstrate the effectiveness of the proposed schemes.

Three-channel CMOS transimpedance amplifier for LiDAR sensor receiver

For time-of-flight(TOF)light detection and ranging(LiDAR),a three-channel high-performance transimpedance amplifier(TIA)with high immunity to input load capacitance is presented.A regulated cascade(RGC)as the input stage is at the core of the complementary metal oxide semiconductor(CMOS)circuit chip,giving it more immunity to input photodiode detectors.A simple smart output interface acting as a feedback structure,which is rarely found in other designs,reduces the chip size and power consumption simultaneously.The circuit is designed using a 0.5μm CMOS process technology to achieve low cost.The device delivers a 33.87 dB?transimpedance gain at 350 MHz.With a higher input load capacitance,it shows a-3 dB bandwidth of 461 MHz,indicating a better detector tolerance at the front end of the system.Under a 3.3 V supply voltage,the device consumes 5.2 mW,and the total chip area with three channels is 402.8×597.0μm2(including the test pads).

Excess soil salinity affects large regions of land and is a major hindrance to crop production worldwide.Therefore,understanding the molecular mechanisms of plant salt tolerance has scientific importance and practical significance.In recent decades,studies have characterized hundreds of genes associated with plant responses to salt stress in different plant species.These studies have substantially advanced our molecular and genetic understanding of salt tolerance in plants and have introduced an era of molecular design breeding of salt-tolerant crops.This review summarizes our current knowledge of plant salt tolerance,emphasizing advances in elucidating the molecular mechanisms of osmotic stress tolerance,salt-ion transport and compartmentalization,oxidative stress tolerance,alkaline stress tolerance,and the trade-off between growth and salt tolerance.We also examine recent advances in understanding natural variation in the salt tolerance of crops and discuss possible strategies and challenges for designing salt stress-resilient crops.We focus on the model plant Arabidopsis(Arabidopsis thaliana)and the four most-studied crops:rice(Oryza sativa),wheat(Triticum aestivum),maize(Zea mays),and soybean(Glycine max).

Clinical study on microscopic syndrome differentiation and traditional Chinese medicine treatment for liver stomach disharmony in chronic gastritis

BACKGROUND Chronic gastritis(CG)is a common gastrointestinal disorder characterized by inflammation of the stomach lining.Liver-stomach disharmony(LSD)syndrome is believed to contribute to CG symptoms.AIM To evaluate the efficacy and safety of microcosmic syndrome differentiation and Chinese herbal medicine(CHM)treatment in patients with CG and LSD syndrome.METHODS Sixty-four patients with CG and LSD syndrome were randomly divided into two groups:The treatment group received CHM based on microcosmic syndrome differentiation and the control group received conventional Western medicine.The treatment course lasted 12 wk.The primary outcome was improvement in dyspeptic symptoms,measured using the Nepean Dyspepsia Index.The secondary outcomes included the improvement rate of endoscopic findings,histopathological findings,and microcosmic syndrome scores and the incidence of adverse events.RESULTS After 12 wk of treatment,the treatment group showed significantly greater improvement in dyspeptic symptoms than the control group(93.75%vs 65.63%,P0.05).No serious adverse events were observed in either group.CONCLUSION Microcosmic syndrome differentiation and CHM treatment can effectively improve dyspeptic symptoms and endoscopic findings in patients with CG and LSD syndrome and have a good safety profile.Further studies with larger sample sizes and longer follow-up periods are required to confirm the long-term efficacy and mechanism of action of this treatment.
