Gastrointestinal stromal tumors in a cohort of Chinese patients in Hong Kong

AIM： To investigate the prevalence and clinical pattern of gastrointestinal stromal tumors （GISTs） in Hong Kong Chinese, and to assess the impact of introduction of CD117 on the disease incidence. METHODS： From the database of the Department of Pathology of Yan Chai Hospital, 47 patients, with GISTs from September 1995 to December 2003 were included in this study. Ten GISTs were diagnosed before the introduction of CD117. The clinical features, tumor characteristics, and treatment were analyzed. Factors predicting tumor related death or recurrence were studied with Cox proportional hazard model. RESULTS： The patients included 26 males and 21 females, with a mean age of 66.6 years （SD 13.1, range 29-87 years）. The estimated prevalence of GISTs was 13.4-15.6 per 100 000 people, with an annual inddence of 1.68-1.96 per 100 000 people. The annual incidence of GISTs before and after the introduction of CD117 was 1.1 per 100 000 people and 2.1 per 100 000 people respectively. Stomach （34 patients, 72.3%） was the most common location for the tumor, followed by the small intestine （8 patients, 17.0%）, esophagus （2 patients, 4.3%）, omentum （2 patients, 4.3%） and colon （1 patient, 2.1%）. Thirty-one patients （66%） had complete tumor resection. Eleven out of 16 deaths （23%） were tumor-related. The median survival time was 26 too. Five-year survival rate was 61.3%. The significant factors associated with tumor-related death or recurrence were incomplete resection, tumor size 5 cm or above, invasion to the adjacent organ or presence of metastasis, CONCLUSION： The incidence of GIST in Hong Kong is comparable to that in the United States but lower than that in Finland. The true incidence of GISTs could be underestimated before the introduction of CD117. Incomplete resection, tumor size 5 cm or above, invasion to the adjacent organ or presence of metastasis are factors predicting tumor-related death or recurrence.

Background: Transarterial chemoembolization(TACE) is the most commonly used adjuvant therapy for hepatocellular carcinoma(HCC) after curative resection. Responses to TACE are variable due to tumor and patient heterogeneity. We had previously demonstrated that expression of Granulin-epithelin precursor(GEP) and ATP-dependent binding cassette(ABC)B5 in liver cancer stem cells was associated with chemoresistance. The present study aimed to evaluate the association between GEP/ABCB5 expression and response to adjuvant TACE after curative resection for HCC. Methods: Patients received adjuvant TACE after curative resection for HCC and patients received curative resection alone were identified from a prospectively collected database. Clinical samples were retrieved for biomarker analysis. Patients were categorized into 3 risk groups according to their GEP/ABCB5 status for survival analysis: low(GEP-/ABCB5-), intermediate(either GEP +/ABCB5-or GEP-/ABCB5 +) and high(GEP +/ABCB5 +). Early recurrence(recurrence within 2 years after resection) and disease-free survival were analyzed. Results: Clinical samples from 44 patients who had followed-up for more than 2 years were retrieved for further biomarker analysis. Among them, 18 received adjuvant TACE and 26 received surgery alone. Patients with adjuvant TACE in the intermediate risk group was associated with significantly better overall survival and 2-year disease-free survival than those who had surgery alone( P = 0.036 and P = 0.011, respectively). Adjuvant TACE did not offer any significant differences in the early recurrence rate, 2-year disease-free survival and overall survival for patients in low and high risk groups. Conclusions: Adjuvant TACE can only provide survival benefits for patients in the intermediate risk group(either GEP +/ABCB5-or GEP-/ABCB5 +). A larger clinical study is warranted to confirm its role in patient selection for adjuvant TACE.

Impact of IPMOE on nursing tasks in the medical ward: A time-motion study

为探究动态剪切荷载作用下岩石的力学特性,采用冲击直剪方法,对立方体花岗岩试样进行了不同正应力和冲击速度条件下的动态剪切试验。分析了花岗岩的冲击剪切变形演化过程,讨论了正应力和冲击速度对花岗岩剪切变形和破坏模式的影响。然后,采用颗粒流程序模拟了花岗岩的冲击直剪试验,从细观角度探讨了花岗岩剪切变形破坏的演化机制。结果表明,冲击剪切荷载作用下,试样剪切变形过程中正应力持续变化,剪应力峰值对应的正应力可达到初始值的数倍。得到了回弹型和断裂型两类剪应力−剪切位移曲线,随着初始正应力的增大,峰值剪应力增大,峰值剪切位移和最大剪切位移减小;随着冲击速度的增大,峰值剪应力和最大剪切位移均增大。花岗岩的断裂包含主断裂面和多条翼裂纹,随正应力和冲击速度的增大,花岗岩的轮廓由起伏变得平直,断裂面由粗糙变得平坦,这与断裂后的摩擦滑移有关。花岗岩试样的起裂源于内部,并沿剪切方向扩展至试样边界,起裂方向与剪切面的夹角与接触力集中区的倾角一致。该研究可为岩石的动态剪切力学参数测试提供新的方法参考,也有助于揭示岩石冲击剪切断裂的动力诱灾机制。

Review of evolution of clinical, training and educational services and research program for autism spectrum disorders in Hong Kong

The evolution of a local fragmented model of services for children with autism in Hong Kong emerged gradually over the past three decades with lack of government funding or support. This had been due to increasing number of children with autism being detected and referred for earlier assessment. With increasing pressure from parents due to long waiting time for assessment and training services and the increasing polarization by mass media there had been a gradual increasing public awareness over the past five years. Though still highly fragmented in the availability of services, there is a growing "business model" available in the community due to increasing need and lack of public funding for support. There is a lack of strategic planning for medical diagnostic and management issues in Hong Kong. Our University of Hong Kong based Autism Research Program was pioneered in 1985 based on the increasing load of autism cases referred for assessment for other developmental problems and diagnosed as Autism in the Duchess of Kent Children's Hospital. As the first author has been the staff of the University of Hong Kong, this program flourished as a research based program. The benefits of early identification and intervention of autism spectrum disorder(ASD) had been increasingly recognized, and with the increased public awareness and increasing trend of earlier diagnosis, there has been a continuously high demand from parents for earlier assessment and training for children suspected to have ASD. This model had not received extra funding for this integrated program for research, teaching and training in autism. We had to apply for various donations and grants to support the development of this pioneer program. The research output and organization of forums for public education and awareness are reviewed. Intravenous fluid selection rationales in acute clinical management

BACKGROUND: Intravenous fluid(IVF) is commonly used in acute clinical management. This study aimed to review the choice and primary considerations in IVF prescriptions and to evaluate the adequacy of guidelines and trainings on it in the New Territories West Cluster(NTWC) of Hong ***: This is a descriptive study based on data collected from an online survey. Data were processed by SPSS for statistical analysis. This study focused on a general description and doctor-nurse between group comparison. Participants were asked the choice of IVF for nine acute clinical scenarios and provide reason. A 1–10 scale was used to assess the sufficiency of guideline, training and information, and time for revision on IVF ***: 0.9% sodium chloride was the most familiar IVF(36%), followed by 5% Dextrose solution(26%). In the nine scenarios, the most chosen IVF was 0.9% sodium chloride(37%–61%). There was significant difference in the choice of IVF between doctors and nurses in 7 cases. The second most chosen IVF for doctors was Plasma-Lyte A while that for nurses was Gelofusine. Departmental practice was the most chosen reason to account for the prescription. The adequacy of guideline, information and training, and time for revision was rated 5. Doctors had significantly more time at work than nurses to update knowledge in IVF prescription(5.41 versus 4.57).CONCLUSION: 0.9% sodium chloride was mostly chosen. The choice of IVF was mainly based on departmental practice. Adequacy of guideline, information and training, and time for revision on IVF prescription were average, indicating significant training deficit.

Preface: The chang'e-3 lander and rover mission to the Moon

The chang＇e-3 （CE-3） lander and rover mission to the Moon was an in- termediate step in China＇s lunar exploration program, which will be followed by a sample return mission. The lander was equipped with a number of remote-sensing instruments including a pair of cameras （Landing Camera and Terrain Camera） for recording the landing process and surveying terrain, an extreme ultraviolet camera for monitoring activities in the Earth＇s plasmasphere, and a first-ever Moon-based ultravi- olet telescope for astronomical observations. The Yutu rover successfully carried out close-up observations with the Panoramic Camera, mineralogical investigations with the VIS-NIR Imaging Spectrometer, study of elemental abundances with the Active Particle-induced X-ray Spectrometer, and pioneering measurements of the lunar sub- surface with Lunar Penetrating Radar. This special issue provides a collection of key information on the instrumental designs, calibration methods and data processing pro- cedures used by these experiments with a perspective of facilitating further analyses of scientific data from CE-3 in preparation for future missions.
