Sepsis heterogeneity

Background Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection,with extremely high mortality.Notably,sepsis is a heterogeneous syndrome characterized by a vast,multidimensional array of clinical and biologic features,which has hindered advances in the therapeutic field beyond the current standards.Data sources We used PubMed to search the subject-related medical literature by searching for the following single and/or combination keywords:sepsis,heterogeneity,personalized treatment,host response,infection,epidemiology,mortality,incidence,age,children,sex,comorbidities,gene susceptibility,infection sites,bacteria,fungi,virus,host response,organ dysfunction and management.Results We found that host factors(age,biological sex,comorbidities,and genetics),infection etiology,host response dysregulation and multiple organ dysfunctions can all result in different disease manifestations,progression,and response to treatment,which make it difficult to effectively treat and manage sepsis patients.Conclusions Herein,we have summarized contributing factors to sepsis heterogeneity,including host factors,infection etiology,host response dysregulation,and multiple organ dysfunctions,from the key elements of pathogenesis of sepsis.An in-depth understanding of the factors that contribute to the heterogeneity of sepsis will help clinicians understand the complexity of sepsis and enable researchers to conduct more personalized clinical studies for homogenous patients.

High-utility itemset mining(HUIM)can consider not only the profit factor but also the profitable factor,which is an essential task in data mining.However,most HUIM algorithms are mainly developed on a single machine,which is inefficient for big data since limited memory and processing capacities are available.A parallel efficient high-utility itemset mining(P-EFIM)algorithm is proposed based on the Hadoop platform to solve this problem in this paper.In P-EFIM,the transaction-weighted utilization values are calculated and ordered for the itemsets with the MapReduce framework.Then the ordered itemsets are renumbered,and the low-utility itemsets are pruned to improve the dataset utility.In the Map phase,the P-EFIM algorithm divides the task into multiple independent subtasks.It uses the proposed S-style distribution strategy to distribute the subtasks evenly across all nodes to ensure load-balancing.Furthermore,the P-EFIM uses the EFIM algorithm to mine each subtask dataset to enhance the performance in the Reduce phase.Experiments are performed on eight datasets,and the results show that the runtime performance of P-EFIM is significantly higher than that of the PHUI-Growth,which is also HUIM algorithm based on the Hadoop framework.

Dehydrogenation of propane(PDH)technology is one of the most promising on-purpose technologies to solve supply-demand unbalance of propylene.The industrial catalysts for PDH,such as Pt-and Cr-based catalysts,still have their own limitation in expensive price and security issues.Thus,a deep understanding into the structure-performance relationship of the catalysts during PDH reaction is necessary to achieve innovation in advanced high-efficient catalysts.In this review,we focused on discussion of structure-performance relationship of catalysts in PDH.Based on analysis of reaction mechanism and nature of active sites,we detailed interaction mechanism between structure of active sites and catalytic performance in metal catalysts and oxide catalysts.The relationship between coke deposition,co-feeding gas,catalytic activity and nanostructure of the catalysts are also highlighted.With these discussions on the relationship between structure and performances,we try to provide the insights into microstructure of active sites in PDH and the rational guidance for future design and development of PDH catalysts.

BACKGROUND Chronic gastritis (CG) is a common gastrointestinal disorder characterized by inflammation of the stomach lining. Liver-stomach disharmony (LSD) syndrome is believed to contribute to CG symptoms. AIM To evaluate the efficacy and safety of microcosmic syndrome differentiation and Chinese herbal medicine (CHM) treatment in patients with CG and LSD syndrome. METHODS Sixty-four patients with CG and LSD syndrome were randomly divided into two groups: The treatment group received CHM based on microcosmic syndrome differentiation and the control group received conventional Western medicine. The treatment course lasted 12 wk. The primary outcome was improvement in dyspeptic symptoms, measured using the Nepean Dyspepsia Index. The secondary outcomes included the improvement rate of endoscopic findings, histopathological findings, and microcosmic syndrome scores and the incidence of adverse events. RESULTS After 12 wk of treatment, the treatment group showed significantly greater improvement in dyspeptic symptoms than the control group (93.75% vs 65.63%, P 0.05). No serious adverse events were observed in either group. CONCLUSION Microcosmic syndrome differentiation and CHM treatment can effectively improve dyspeptic symptoms and endoscopic findings in patients with CG and LSD syndrome and have a good safety profile. Further studies with larger sample sizes and longer follow-up periods are required to confirm the long-term efficacy and mechanism of action of this treatment.

This article presents a modeling and simulation method for transient thermal analyses of integrated circuits(ICs)using the original and voltage-in-current(VinC)latency insertion method(LIM).LIM-based algorithms are a set of fast transient simulation methods that solve electrical circuits in a leapfrog updating manner without relying on large matrix operations used in conventional Simulation Program with Integrated Circuit Emphasis(SPICE)-based methods which can significantly slow down the solution process.The conversion from the thermal to electrical model is performed first by using the analogy between heat and electrical conduction.Since electrical inductance has no thermal equivalence,a modified VinC LIM formulation is presented which removes the requirement of the insertion of fictitious inductors.Numerical examples are presented,which show that the modified VinC LIM formulation outperforms the basic LIM formulation,in terms of both stability and accuracy in the transient thermal simulation of ICs.

In this work,high-manganese aluminium bronze CuMn_(13)Al_(7)samples were prepared by arc additive manufacturing technology.The phase composition,microstructure,and crystal structure of the high-manganese aluminium bronze CuMn_(13)Al_(7)arc additive manufactured samples were analysed using direct-reading spectrometer,metallographic microscope,scanning electron microscope,and transmission electron microscope.The micro-hardness tester,tensile tester,impact tester,and electrochemical workstation were also used to test the performance of the CuMn_(13)Al_(7)samples.By studying the microstructure and properties of the CuMn_(13)Al_(7)samples,it was found that preparation of the samples by the arc additive manufacturing technology ensured good forming quality,almost no defects,and good metallurgical bonding inside the sample.The metallographic structure(α+β+point phase)mainly comprises the following:the metallographic structure in the equiaxed grain region has an obvious grain boundaryα;the metallographic structure in the remelting region has no obvious grain boundaryα;the thermal influence on the metallographic structure produced a weaker grain boundaryαthan the equiaxed grain region.The transverse and longitudinal cross sections of the sample had uniform microhardness distributions,and the average microhardness values were 190.5 HV0.1 and 192.7 HV0.1,respectively.The sample also had excellent mechanical properties:yield strength of 301 MPa,tensile strength of 633 MPa,elongation of 43.5%,reduction of area by 58%,Charpy impact value of 68 J/cm^(2)at–20℃,and dynamic potential polarisation curve test results.Further,it was shown that the average corrosion potential of the sample was–284.5 mV,and the average corrosion current density was 4.1×10–3 mA/cm^(2).

As the promising catalysts for the water-gas shift(WGS)reaction,the activity of Au-CeO_(2) composites is susceptible to the aggregation size and electronic state of Au species.Previous reports were extensively focused on the discrepancy between nonmetallic Au and metallic Au nanoparticles,whereas the understanding of the authentic role of the isolated Au atoms was limited.Herein,we investigated the catalytic behavior and structural information over two types of Au/CeO_(2) catalysts,in which the predominant conjunctions were isolated Au1-CeO_(2) and Aun-CeO_(2),respectively.Based on comprehensive characterizations,particularly by in-situ Raman and in-situ DRIFTS,we found that the isolated Au atoms were responsible for enhancing the reducibility of the CeO_(2) matrix.The CO adsorption ability on the isolated Au sites was significantly inferior to clustered Au atoms,especially at relatively high temperatures(>200°C).As a result,the boosted O vacancy on the isolated Au1-CeO_(2) conjunctions could improve the H2O activation ability for the Au-CeO_(2) catalysts and demonstrate a comparable activity to the clustered Aun-CeO_(2) sites.This work might deepen understanding of the catalytic function for the isolated Au1 site within Au/CeO_(2) systems while catalyzing the WGS reaction.

Azomethine ylides are useful intermediates for the rapid construction of chiral N-containing compounds.However,its synthetic potential has not been fully developed due to the limited reaction models.In combination with synergistic catalysis and azomethine ylide chemistry,we have developed several types of novel catalytic system including Cu/Pd,Cu/Ir and PTC/Ir catalysis;which can convert readily-available azomethine ylides to various high-valued molecules such as unnatural a-amino acids,homoallylic amines and N-heterocycles.Compared with the traditional mono-catalysis,the synergistic catalyst system exhibits enhanced catalytic efficiency and chiral induction ability in many cases.In addition,we have demonstrated that these strategies could be applied in the construction of bioactive compounds and natural products.
