Long-term outcomes of endoscopic submucosal dissection and surgery for undifferentiated intramucosal gastric cancer regardless of size

BACKGROUND The clinical outcomes of endoscopic submucosal dissection(ESD)for undifferentiated(UD)intramucosal early gastric cancer(EGC)compared with those of surgery,regardless of lesion size,are not well ***,there is a concern regarding the treatment plan before and after ESD in cases of UD intramucosal EGC within expanded *** To evaluate clinical outcomes of ESD compared with those of surgery in UD intramucosal EGC patients regardless of tumor *** We enrolled patients with UD intramucosal EGC after ESD with complete resection or surgery from January 2005 to August 2020 who met the within or beyond expanded indications with lesion size>2 cm(the only non-curative factor).Overall,123 and 562 patients underwent ESD and surgery,*** propensity-score matching,clinical and long-term outcomes,i.e.,recurrencefree survival(RFS)and overall survival(OS),were *** multivariable Cox proportional hazard model with treatment modality and ESD indication was used to evaluate the recurrence *** After matching,119 patients each were finally enrolled in the ESD and surgery *** median length of hospital stay was shorter in the ESD group than surgery group(4.0 vs 9.0 days,Phowever,the ESD group had inferior RFS compared with the surgery group(P=0.031).ESD was associated with the risk of recurrence after initial treatment in all enrolled patients(hazard ratio,5.2;95%confidence interval:1.0-25.8,P=0.045).CONCLUSION Although OS was similar between the two groups,surveillance endoscopy was important for the ESD than for the surgery group because RFS was inferior and local recurrence was an issue.

We report a time-dependent quantum wavepacket theory employed to interpret the photoabsorption spectrum of the N20 molecule in terms of the nuclear motion on the upper 21A＇ and 11A＂ potential energy surfaces. The N2-O bond breaks upon excitation leading to dissociation. The total angular momentum is treated correctly taking into account the vector property of the electric field of the exciting radiation.

Unraveling the ties between irritable bowel syndrome and intestinal microbiota

Irritable bowel syndrome (IBS) is the most prevalent functional gastrointestinal disorder. It is a multifactorial disorder. Intestinal microbiota may cause the pathogenesis of IBS by contributing to abnormal gastrointestinal motility, low-grade inflammation, visceral hypersensitivity, communication in the gut-brain axis, and so on. Previous attempts to identify the intestinal microbiota composition in IBS patients have yielded inconsistent and occasionally contradictory results. This inconsistency may be due to the differences in the molecular techniques employed, the sample collection and handling methods, use of single samples that are not linked to fluctuating symptoms, or other factors such as patients’ diets and phenotypic characterizations. Despite these difficulties, previous studies found that the intestinal microbiota in some IBS patients was completely different from that in healthy controls, and there does appear to be a consistent theme of Firmicutes enrichment and reduced abundance of Bacteroides. Based on the differences in intestinal microbiota composition, many studies have addressed the roles of microbiota-targeted treatments, such as antibiotics and probiotics, in alleviating certain symptoms of IBS. This review summarizes the current knowledge of the associations between intestinal microbiota and IBS as well as the possible modes of action of intestinal microbiota in the pathogenesis of IBS. Improving the current level of understanding of host-microbiota interactions in IBS is important not only for determining the role of intestinal microbiota in IBS pathogenesis but also for therapeutic modulation of the microbiota.

Outcome after gastrectomy in gastric cancer patients with type 2 diabetes

AIM: To evaluate the prognosis of type Ⅱ diabetes mellitus (T2DM) after gastrectomy and related factors in gastric cancer patients. METhoDS: 403 gastric cancer patients with T2DM were studied, who underwent gastrectomy between May 2003 and September 2009. A review of medica records and telephone interviews was performed in this cross-sectional study. The factors included in the statistical analysis were as follows: gender, age, type of surgery, preoperative body mass index (BMI), current BMI, BMI reduction ratio, preoperative insulin or oral diabetic medicine requirement, follow-up duration, and current state of diabetes. Assessment of diabetes status after surgery was classified into four categories according to the change in hypoglycemic agents after surgery and present status of T2DM: resolution, improvement, same, and worse.(± 20.6 mo), preoperative BMI was 24.7 kg/m2 (± 3.0 kg/m2), and BMI reduction ratio was 9.8% (± 8.6%). After surgery, T2DM was cured in 58 patients (15.1%) and was improved in 117 patients (30.4%). According to the type of surgery, the BMI reduction ratio was signif icantly higher in the total gastrectomy and Roux-en-Y reconstruction group [14.2% ± 9.2% vs 9.2% ± 7.7% (Billroth Ⅱ group), P < 0.001] and signif icantly lower in the subtotal gastrectomy and Billroth Ⅰ reconstruction group [7.6% ± 8.0%, 9.2% ± 7.7% (Billroth Ⅱ group), P < 0.001]. The BMI reduction ratio, follow-up duration after surgery, type of surgery, extent of gastrectomy, and performance of duodenal bypass were signif icantly correlated to the course of T2DM (P < 0.05). The BMI reduction ratio was the most influential factor on T2DM status. In a subgroup analysis of patients with a BMI reduction ratio of 10% or less (n = 206), T2DM was cured in 15 (7.6%) patients and was improved in 57 (28.8%) patients after surgery, and only the duration of surgery was signif icantly correlated to T2DM status (P = 0.022).

Combined treatment of phacoemulsification and single-port limited pars plana vitrectomy in acute angle-closure glaucoma

AIM: To evaluate the efficacy of combined treatment of phacoemulsification(PE) and micro-incisional single-port transconjunctival limited pars plana vitrectomy(PPV) in acute angle-closure glaucoma(AACG).METhoDS: A retrospective study included 26 patients who underwent PE diagnosed with AACG. Among them, 16 patients(16 eyes) underwent PE alone, 10 patients(10 eyes) underwent combined limited vitrectomy and PE. Then we compared intraocular pressure(IOP), anterior chamber angle, anterior chamber depth, central corneal thickness and corneal endothelial cell count before and after surgery, and effective PE time during cataract ***: Effective PE time was shorter in the combined surgery group than in the single surgery group(P=0.040). There was no statistically significant difference in IOP and best-corrected visual acuity between the two groups postoperatively. At 6 mo postoperatively, there was no difference in the anterior chamber angle, anterior chamber depth, and central corneal thickness between two groups, but corneal endothelial cell count was higher in the combined surgery group than in the single surgery group(P=0.046). No complication such as vitreoretinal disease, endophthalmitis, bullous keratopathy was ***: Combined micro-incisional single-port transconjunctival limited PPV and PE are more effective and safer than PE alone because of less operation time and fewer complications for management of AACG.

Gastric cancer surgery in cirrhotic patients: Result of gastrectomy with D2 lymph node dissection

AIM： To explore the feasibility of performing gastrectomy with D2 lymphadenectomy in gastric cancer patients with liver cirrhosis. METhoDS： A total of 7 178 patients were admitted with a diagnosis of liver cirrhosis from January 1993 to December 2003. We reviewed the records of 142 patients who were diagnosed with liver cirrhosis and gastric adenocarcinoma during the same period. Gastrectomy with D2 lymph node dissection for carcinoma of the stomach was performed in 94 patients with histologically proven hepatic cirrhosis. RESULTS： All but 12 patients were dassified as Child＇s class A. Only 35 patients （37.2%） were diagnosed with cirrhosis before operation. Seventy-three patients underwent a subtotal gastrectomy （77.7%） and 21 patients （22.3%） underwent a total gastrectomy, each with D2 or more lymph node dissection. Two patients （3.8%） who had prophylactic intra-operative drain placement, died of postoperative complications from hepatorenal failure with intractable ascites. Thirty-seven patients （39.4%） experienced postoperative complications. The extent of gastric resection did not influence the morbidity whereas serum aspartate aminotransferase level （P = 0.011） and transfusion did （P = 0.008）. The most common postoperative complication was ascites （13.9%） followed by wound infection （10.6%）. CONCLUSION： We concluded that the presence of compensated cirrhosis, i.e. Child class A, is not a contraindication against gastrectomy with D2 or more lymph node dissection, when curative resection for gastric cancer is possible. Hepatic reserve and meticulous hemostasis are the likely determinants of operative prognosis.

We report on the measurements of nonlinear current-voltage characteristics of graphene fabricated by chemical vapor *** current-voltage characteristic is described by a power law with a superlinear dependence of the current on the voltage,and the nonlinearity depends on the carrier density and the excitation *** nonlinearity is strongest at the Dirac point and becomes weaker as the carrier density *** the Dirac point,we also observe a crossover to a much stronger nonlinear transport when the electric field increases above 104 V/m.

Silicate-doped hydroxyapatite and its promotive effect on bone mineralization

Bone defect is one of the most common diseases in clinic. Existing therapeutic approaches have encountered many problems, such as lack of autogenous allogeneic bone and immunological rejection to allogeneic implant. Synthetic hydroxyapatite (HA) provided solutions for bone repair, since the HA is the main inorganic component of animals’ bone. however, HA has good biocompatibility, but does not possess osteogenic capability, which is of significance for modern bone repair materials. Si is an essential trace element in bone tissue, and it has been demonstrated to be able to promote bone formation. Therefore, silicate-doped hydroxyapatite (Si-HA) may serve as a promising material for bone repair, and promote bone regeneration in the repair. The current review discusses development of Si-HA, focusing on its preparation and characterization, in vitro and in vivo evaluations of the material, positive effect of Si-HA on promoting bone formation in clinical applications, and molecular mechanism investigation of such promotive effect.

Objective:Benefits of adjuvant treatment in pT1 N1 gastric cancer(GC)remain ***,an effective biomarker for early GC is the need of the *** prognostic and predictive roles of single patient classifier(SPC)were validated in stageⅡ/Ⅲ*** this study,we aimed to elucidate the role of SPC as a biomarker for pT1 N1 ***:The present retrospective biomarker study(NCT03485105)enrolled patients treated for pT1 N1 GC between 1996 and 2012 from two large hospitals(the Y cohort and S cohort).For SPC,mRNA expression of four classifier genes(GZMB,WARS,SFRP4 and CDX1)were evaluated by real-time reverse transcription-polymerase chain reaction *** SPC was revised targeting pT1 stages and the prognosis was stratified as high-and lowrisk group by the expression of SFRP4,a representative epithelial-mesenchymal transition ***:SPC was evaluated in 875 patients(n=391 and 484 in the Y and S cohorts,respectively).Among 864 patients whose SPC result was available,41(4.7%)patients experience GC *** to revised SPC,254(29.4%)patients were classified as high risk[123(31.5%)and 131(27.1%)in the Y and S cohorts,respectively].The high risk was related to frequent recurrence in both Y and S cohort(log-rank P=0.023,Phort confirmed the high risk of revised SPC as a significant prognostic factor[hazard ratio(HR):4.402(2.293-8.449),P<0.001]of GC.A significant difference was not detected by SPC in the prognosis of patients in the presence and absence of adjuvant treatment(log-rank P=0.670).Conclusions:The present study revealed the revised SPC as a prognostic biomarker of pT1 N1 GC and suggested the use of the revised SPC for early-stage GC as like stageⅡ/Ⅲ.
