Changes in peak inspiratory flow rate and peak airway pressure with endotracheal tube size during chest compression

BACKGROUND:Adequate airway management plays an important role in high-quality cardiopulmonary resuscitation(CPR).Airway management is usually performed using an endotracheal tube(ETT)during CPR.However,no study has assessed the effect of ETT size on the fl ow rate and airway pressure during CPR.METHODS:We measured changes in peak inspiratory fl ow rate(PIFR),peak airway pressure(Ppeak),and mean airway pressure(Pmean)according to changes in ETT size(internal diameter 6.0,7.0,and 8.0 mm)and with or without CPR.A tidal volume of 500 mL was supplied at a rate of 10 times per minute using a mechanical ventilator.Chest compressions were maintained at a constant compression depth and speed using a mechanical chest compression device(LUCAS2,mode:active continuous,chest compression rate:102±2/minute,chest compression depth 2–2.5 inches).RESULTS:The median of several respiratory physiological parameters during CPR was significantly different according to the diameter of each ETT(6.0 vs.8.0 mm):PIFR(32.1 L/min[30.5–35.3]vs.28.9 L/min[27.5–30.8],P<0.001),Ppeak(48.84 cmH2O[27.46–52.11]vs.27.45 cmH2O[22.53–52.57],P<0.001),and Pmean(18.34 cmH2O[14.61–21.66]vs.13.66 cmH2O[8.41–19.24],P<0.001).CONCLUSION:The changes in PIFR,Ppeak,and Pmean were related to the internal diameter of ETT,and these values tended to decrease with an increase in ETT size.Higher airway pressures were measured in the CPR group than in the no CPR group.

BACKGROUND An impalement injury of the oral cavity is a common traumatic injury in children.In most cases,it is not accompanied by sequelae,but if foreign body residues are not found due to a minor injury,they may result in inflammatory responses and delayed vascular injuries in the surrounding tissues.Without early diagnosis and appropriate initial management,residual foreign bodies can cause serious complications and even mortality in some cases.CASE SUMMARY A 9-year-old boy suffered an intra-oral injury by a wooden chopstick,and the patient was discharged from the hospital after receiving conservative treatment for the injury.However,the patient was readmitted to the hospital due to intraoral bleeding,and since neck hematoma and right internal carotid artery pseudoaneurysm formation were detected on computed tomography,emergency surgery was performed.A remnant fragment of a wooden chopstick was found during the operation,and a delayed rupture of the internal carotid artery caused by the foreign body was also found.CONCLUSION The failure of early detection and diagnosis of a residual foreign body may result in delayed vascular rupture.

The purpose of the present research was to develop a suitable, simple, precise, accurate,robust, and reproducible RP-HPLC method for a reliable simultaneous quantification of docetaxel(DTX) and curcumin(CCM) in rat plasma samples using paclitaxel(PTX) as an internal standard. The samples were assayed by the Agilent 1260 Infinity HPLC instrument using a Capcell Pak C8 column(4.6 mm × 150 mm, 5 μm) under isocratic conditions. The mobile phase consisted of acetonitrile and triple distilled water(40/60, v/v) with a flow rate of 1.0 ml/min. The eluent was monitored at 230 nm for simultaneous measurement of curcumin and docetaxel. The method was validated by determining system suitability, selectivity, sensitivity, linearity, inter-day and intra-day precision, accuracy, robustness, and stability in accordance with the guidelines of the United States Food and Drug Administration(FDA).The developed chromatographic method proved to be simple, precise, accurate, robust and reproducible. Moreover, the samples showed stability at room temperature over a period of 48 h. Thus, this method would be employed for routine simultaneous quantification of docetaxel and curcumin in rat plasma samples.

Patient-reported ejaculatory function and satisfaction in men with lower urinary tract symptoms/benign prostatic hyperplasia

This study aimed to investigate perceived ejaculatory function/satisfaction before treatment for lower urinary tract symptoms （LUTS）/benign prostatic hyperplasia （BPH） and to identify associations between specific categories of ejaculatory dysfunctions （EjDs） and LUTS. A total of 1574 treatment-naive men with LUTS/BPH were included in this study. All patients underwent routine evaluation for LUTS/BPH including the International Index of Erectile Function and a 5-item questionnaire developed to assess ejaculatory volume/force/pain/satisfaction/latency time. Patients who had sexual intercourse over the past 4 weeks were classified as sexually active group. A total of 783 patients were categorized as sexually active group. Decreased ejaculatory volume and force were reported by 53.4% and 55.7% of 783 sexually active men, respectively, There was a strong correlation between ejaculatory volume and force. Ejaculatory pain/discomfort, premature ejaculation （PE）, and delayed ejaculation （DE） were reported in 41,0%, 16.3%, and 41.4% of the patients, respectively. Over 40.0% of men without decreased ejaculation volume/force were satisfied with ejaculatory function, whereas approximately 6.0% of men with decreased volume/force were satisfied with ejaculatory function. About 30.0% of men with decreased volume/force had orgasmic dysfunction, while approximately 10.0% of men without decreased volume/force did. Decreased ejaculatory volume or force was associated with LUTS severity after adjusting for other influential factors including testosterone level, erectile function, and prostate size on ultrasonography, but PE or DE or ejaculatory pain/discomfort was not. In conclusion, a considerable portion of men with LUTS/BPH appear to have a variety of EjDs. Ejaculatory volume/force and satisfaction/orgasm do not always appear to be concordant. Ejaculatory volume or force is independently associated with LUTS severity, whereas PE or DE or ejaculatory pain/discomfort is not.

Role of inhibiting LIM-kinase2 in improving erectile function through suppression of corporal fibrosis in a rat model of cavernous nerve injury

We evaluated whether LIM-kinase 2 inhibitor （LIMK2i） could improve erectile function by suppressing corporal fibrosis through the normalization of the Rho-associated coiled-coil protein kinase 1 （ROCK1）/LIMK2/Cofilin pathway in a rat model of cavernous nerve crush injury （CNCl）. Sixty 11-week-old male Sprague-Dawley rats were divided equally into five groups： sham surgery （S）, CNCl （I）, and CNCl treated with low-dose （L）, medium-dose （M）, and high-dose （H） LIMK2i. The L, M, and H groups were treated with a daily intraperitoneal injection of LIMK2i （2.5, 5.0, and 10.0 mg kg-1 body weight, respectively） for 1 week after surgery. The erectile response was assessed using electrostimulation at 1 week, postoperatively. Penile tissues were processed for Masson＇s trichrome staining, double immunofluorescence, and Western blot assay. Erectile responses in the H group improved compared with the I group, while the M group showed only partial improvement. A significantly decreased smooth muscle/collagen ratio and an increased content of fibroblasts positive for phospho-/IMK2 were noted in the I group. The M and H groups revealed significant improvements in histological alterations and the dysregulated LIMK2/Cofilin pathway, except for LIMK2 phosphorylation in the M group. The inhibition of LIMK2 did not affect the ROCK1 protein expression. The content of fibroblasts positive for phospho-LIMK2 in the H group returned to the level found in the S group, whereas it did not in the M group. However, the L group did not exhibit such improvements. Our data suggest that the inhibition of LIMK2, particularly with administration of 10.0 mg kg^-1 body weight LIMK2i, can improve corporal fibrosis and erectile function by normalizing the LIMK2/Cofilin pathway.

High-throughput computational-experimental screening protocol for the discovery of bimetallic catalysts

To accelerate the discovery of materials through computations and experiments,a well-established protocol closely bridging these methods is required.We introduce a high-throughput screening protocol for the discovery of bimetallic catalysts that replace palladium(Pd),where the similarities in the electronic density of states patterns were employed as a screening descriptor.Using first-principles calculations,we screened 4350 bimetallic alloy structures and proposed eight candidates expected to have catalytic performance comparable to that of Pd.Our experiments demonstrate that four bimetallic catalysts indeed exhibit catalytic properties comparable to those of Pd.Moreover,we discover a bimetallic(Ni-Pt)catalyst that has not yet been reported for H_(2)O_(2) direct synthesis.In particular,Ni_(61)Pt_(39) outperforms the prototypical Pd catalyst for the chemical reaction and exhibits a 9.5-fold enhancement in cost-normalized productivity.This protocol provides an opportunity for the catalyst discovery for the replacement or reduction in the use of the platinum-group metals.

Spinal CCL2 Promotes Central Sensitization, Long-Term Potentiation, and Inflammatory Pain via CCR2： Further Insights into Molecular, Synaptic, and Cellular Mechanisms

Mounting evidence supports an important role of chemokines, produced by spinal cord astrocytes, in promoting central sensitization and chronic pain. In particular, CCL2 （C-C motif chemokine ligand 2） has been shown to enhance N-methyl-D-aspartate （NMDA）-induced currents in spinal outer lamina II （Iio） neurons. However, the exact molecular, synaptic, and cellular mechanisms by which CCL2 modulates central sensitization are still unclear. We found that spinal injection of the CCR2 antagonist RS504393 attenuated CCL2- and inflammation-induced hyperalgesia. Single-cell RT-PCR revealed CCR2 expres- sion in excitatory vesicular glutamate transporter subtype 2-positive （VGLUT2＋） neurons. CCL2 increased NMDA- induced currents in CCR2＋/VGLUT2＋ neurons in lamina IIo; it also enhanced the synaptic NMDA currents evoked by dorsal root stimulation; and furthermore, it increased the total and synaptic NMDA currents in somatostatin- expressing excitatory neurons. Finally, intrathecal RS504393 reversed the long-term potentiation evoked in the spinal cord by C-fiber stimulation. Our findings suggest that CCL2 directly modulates synaptic plasticity in CCR2- expressing excitatory neurons in spinal lamina Iio, and this underlies the generation of central sensitization in patho- logical pain.

High-power terahertz radiation was observed to be emitted from a gas jet irradiated by 100-terawatt-class laser pulses in the laser-wakefield acceleration of electrons.The emitted terahertz radiation was characterized in terms of its spectrum,polarization,and energy dependence on the accompanying electron bunch energy and charge under various gas target conditions.With a nitrogen target,more than 4 mJ of energy Was produced at<10 THz with a laser-to-terahertz conversion efficiency of~0.15%.Such strong terahertz radiation is hypothesized to be produced from plasma electrons accelerated by the ponderomotive force of the laser and the plasma wakefelds on the time scale of the laser pulse duration and plasma period.This model is examined with analytic calculations and particle-in-cell simulations to better understand the generation mechanism of high-energy terahertz radiation in laser-wakefield acceleration.

The cell-type continuous electromagnetic radiation system is a demonstration device capable of generating high-power millimeter electromagnetic waves of a specific wavelength and observing their effects on living organisms.It irradiates a biological sample placed in a 30×30×50 cm^(3)cell with electromagnetic waves in the 3.15-mm-wavelength region(with an output of≥1 W)and analyzes the temperature change of the sample.A vacuum electronic device-based coupled-cavity backward-wave oscillator converts the electron energy of the electron beam into radiofrequency(RF)energy and radiates it to the target through an antenna,increasing the temperature through the absorption of RF energy in the skin.The system causes pain and ultimately reduces combat power.A cell-type continuous electromagnetic radiation system consisting of four parts—an electromagnetic-wave generator,a highvoltage power supply,a test cell,and a system controller—generates an RF signal of≥1 W in a continuous waveform at a 95-GHz center frequency,as well as a chemical solution with a dielectric constant similar to that of the skin of a living organism.An increase of 5°C lasting approximately 10 s was confirmed through an experiment.

Safety assessment of condensed phosphate intake from fishery and processed marine food products in Korea with respect to gender,age,and region

Condensed phosphates are used as food additives,especially in marine products and meat,to improve food quality.The import and consumption of fishery and processed marine food products in Republic of Korea have reportedly increased by~5 per cent annually.However,processed marine food products are often intentionally adulterated with excessive amounts of condensed phosphates to increase their weight.Excessive intake of condensed phosphates via consuming processed marine food products can lead to various adverse effects on human health due to anionic imbalance.Herein,we conducted a safety assessment of condensed phosphates in 14 types of fishery and processed marine food products in Korea for the first time.Subgroup analysis of various factors including gender,age,and region was also performed,and the risk level of exposure for each group was estimated.Safety assessments by age and gender indicated that infants were at the highest risk.In the regional safety assessment,Chungnam,the most inland region,showed the lowest risk.For both the general and the high-intake groups(95th percentile)in all classifications,the risk was lower(<20 per cent)than the international standard,and the phosphorus content of the 14 types of processed marine products in Korea was confirmed to be safe for human consumption.
