In obstetrics, the maternal immune system plays a critical role in the establishment and maintenance of a healthy pregnancy. The immune response of the placenta plays important role in determining a maternal vulnerability toward infectious diseases. The various adverse outcome, ranging from preterm birth, injury to the fetal brain cortex and fetal death are all documented in infected neonates. In obstetrics, viral infections can be either transmitted through the in-utero, intrapartum, and postpartum periods. Nevertheless, with the recent pandemic of coronavirus and some documented cases of a positively infected neonate, it is becoming more apparent that the mode of transmission of coronavirus during pregnancy needs to be better understood. This review focuses on revealing the possible modes of transmission of the coronavirus in obstetrics.

An intraligamentary pregnancy is an extremely rare form of ectopic pregnancy, with only a few cases reported. We report a 20-year-old woman who had an ongoing pregnancy at about 22 weeks gestation, consulted for recurrent urinary retention and severe lower abdominal pain of two days duration. Pelvic ultrasonography revealed an extrauterine gestation of 19 weeks and four days on the left adnexal region. Exploratory laparotomy was performed that found left intraligamentary pregnancy (unruptured) measured 20 - 25 cm across, attached posteriorly to the bowel (sigmoid colon) and extended to the pouch of douglas impinging on the bladder base. Resection of ectopic pregnancy was successfully performed, urine retention resolved, and the patient was discharged with no postoperative complications. Early recognition of this rare form of ectopic gestation is of considerable importance because of the risk of a life-threatening haemorrhagic complication necessitating emergency surgical intervention.

Aim: This study evaluated the impact of age on the hormonal profiles of women diagnosed with infertility in a Fertility Clinic in Abia State, South-East, Nigeria. Methodology: Subjects comprised of 200 females: 150 subjects and 50 controls, aged Result: The means ± SEM of serum anti-Mullerian hormones were 1602.44 ± 54.42 pg/ml (control), 848.06 ± 23.04 pg/ml (group 1), 26.74 ± 1.28 pg/ml (group 2), 10.37 ± 1.26 pg/ml (group 3) values for follicle stimulating hormone in the control subjects was 4.90 ± 0.22 mIU/ml, 12.59 ± 0.79 mIU/ml (group 1), 30.59 ± 1.31 mIU/ml (group 2), and 41.59 ± 1.59 miU/ml (group 3). Similarly, the mean ± SEM of luteinizing hormone of control, group 1, group 2 and group 3 were 5.01 ± 0.22 mIU/ml, 15.02 ± 1.13 mIU/ml, 42.71 ± 1.82 mIU/ml and 58.22 ± 2.62 mIU/ml respectively while for estadiol the values were 63.16 ± 1.95 pg/ml, 94.10 ± 5.56 pg/ml, 58.84 ± 4.01 pg/ml and 36.7 ± 1.59 pg/ml for control, group 1, group 2 and group 3 respectively. The mean ± SEM of FSH/LH ratio for the control and experimental subjects were 0.98 ± 0.01, 0.89 ± 0.02, 0.74 ± 0.03 and 0.75 ± 0.02 for control, group 1, group 2 and group 3 respectively. The comparison of the means showed significant difference (p Conclusion: The evaluation of levels of the hormonal parameters across the age ranges of the population studied shows that women within the control and experimental group 1 (<20 years and 20 - 29 years) have a better chance of achieving pregnancy naturally.

Dispersal Patterns of the Late Cretaceous to Early-Tertiary Sediments in the Southern Anambra Basin, Southeastern Nigeria

The dispersal patterns of the Late Cretaceous to Early Tertiary sediments in southern Anambra Basin were studied to delineate spatial facies distributions for improved mapping and hydrocarbon prospectivity. Seven lithofacies distinguished from eleven outcrop sections aided the interpretation of depositional environments for the different depositional units. Interpreted paleoenvironments ranged from estuarine/lagoonal to tidal delta and shallow marine depositional environments. Sequence stratigraphic interpretation of the deposits using vertical relationships between the delineated facies and foraminiferal and palynofacies assemblages and the interpreted environmental setting identified two complete sequences (SEQ-1 and -2) and two incomplete sequences (SEQ-3 and -4) in the study area. The sequiences are enveloped by three Type-1 Sequence Boundaries (SB), two Transgressive Surfaces of Erosion (TSE) and four Maximum Flooding Surfaces (MFS). The identified surfaces were used to delineate formation boundaries and as such would assist in the improved mapping of the Anambra Basin and adjoining depocenters. The massive sand units of the Highstand and Transgressive Systems Tracts constitute good (potential) reservoirs, while the transgressive shales would constitute seals for potential traps in the study area. The delineation of these petroleum system elements, especially those associated with the Transgressive and Highstand Systems Tracts has conferred great hydrocarbon prospectivity potential on the sedimentary sequences in the Basin. The absence of deep marine and slope complexes of the Lowstand Systems Tract (LST) in the interpreted sequences is indicative that the study area is the up-dip section of the Anambra Basin.

Introduction: Uric acid is a product of purine metabolism and elevated serum concentration are very common in, and linked with hypertension and chronic kidney disease, conditions associated with heavy health burden and cardiovascular complications particularly in sub Sahara Africa. An assessment of factors relating hyperuricemia to hypertension and chronic kidney disease would therefore be necessary as way of mitigating the poor quality of life, morbidity and mortality associated with these diseases in low income nations. Methods: A single centre, descriptive comparative study in which the demographic, clinical and laboratory data of hypertensive and non-dialyzed chronic kidney disease (CKD) patients were analyzed. Serum biochemical parameters with uric acid, hematocrit and urine dip strip protein were assessed. Predictors of hyperuricemia were determined using multivariate analysis. Results: One hundred and thirty nine hypertensives and 69 CKD were studied. The mean age of the participants was 54.3 ± 11.7 years, hypertensives (52.9 ± 15.7 years) and CKD (57.3 ± 16.1 years). Both groups had more males, P = 0.8. Majority (78.3%) of the CKD cohorts had stage 4 or 5 (non-dialyzed) disease. The systolic and diastolic blood pressure, creatinine and uric acid were lower in hypertension than in CKD, P = 0.07, P = 0.05, P < 0.001 and P = 0.004 respectively. The hematocrit, albumin and GFR were higher in HTN than CKD, P < 0.001, P < 0.001 and P < 0.001 respectively. The prevalence of hyperuricemia was 56.2%. The mean uric acid was 505.9 ± 23.6 mmol/L, 382 7 ± 10.5 mmol/L for hypertensive and 755.9 ± 14.8 mmol/L for CKD, P < 0.001. The prevalence of systolic HTN, proteinuria, hypoalbuminemia and anemia were 51%, 75%, 46% and 59%, and were higher in males.
