Effects of Total Intravenous Anesthesia and Static Aspiration Combined General Anesthesia on Postoperative Cognitive Function and Psychological State of Elderly Patients with Esophageal Cancer

Objective: To compare the effects of total intravenous anesthesia and static aspiration combined general anesthesia on postoperative cognitive function and psychological state of elderly esophageal cancer patients. Methods: From July 2020 to April 2021, 180 elderly patients who underwent radical esophageal cancer surgery in our hospital were randomly divided into 90 cases in the control group and 90 in the observation group. The control group used static aspiration compound general anesthesia, and the patients in the observation group used intravenous anesthesia to compare the cognitive function and psychological state of the two groups of patients. Results: There was no statistical difference in the cognitive function score of patients in the observation group 30 minutes before anesthesia, 1 h and 24 hours after anesthesia compared with that in the control group, P > 0.05;there was no statistical difference between the Hamilton Anxiety Scale (HAMA) scores 30 minutes before and 24 hours after anesthesia in the observation group compared with the control group, P > 0.05;the cognitive function score of patients in the observation group of 4 h after surgery and 12 h after operation was significantly higher than that of the control group;the HAMA scores of patients in the observation group of 1 h, 4 h and 12 h after surgery were significantly lower than that of the control group, P Conclusion: The application of total intravenous anesthesia in elderly patients with esophageal cancer surgery can reduce the impact of anesthesia on their cognitive function and psychological state, which is worth popularizing and applying in clinical practice.

Differentiation of neuromyelitis optica from multiple sclerosis in a cohort from the mainland of China

Background Although there were criteria for diagnosis of neuromyelitis optica （NMO） and multiple sclerosis （MS）,it is still difficult to differentiate NMO from MS,due to the overlapping clinical *** it is necessary to characterize clinical features of NMO and MS patients in the mainland of China,to simplify the process of disease diagnosis,and to identify criteria for the differential diagnosis of NMO and *** A total of 138 Chinese Han patients from the mainland of China including 73 NMO,60 MS and 5 MS-like patients with positive NMO-IgG were included in the *** records were reviewed retrospectively and the results of clinical examination,laboratory experiments,magnetic resonance imaging （MRI） and evoked potentials （EPs） were compared between NMO and MS *** addition,the relationship between the NMO-IgG serologic status and clinical characteristics were *** Compared with MS patients （1.3∶ 1.0）,more female prevalence was observed in NMO patients （4.2∶ 1.0; P=0.003）.There were also statistically significant differences in visual EPs,oligoclonal bands,brainstem lesions in MRI and longitudinally extensive spinal cord lesions （LESCLs） between NMO and MS *** lesions observed in brain MRI were found in 17.9% of MS patients,over 3.7 times higher than in NMO patients （4.8%,P=0.024）.When stratified NMO patients by NMO-IgG,LESCLs were found in 42.1% of NMO-IgG-negative NMO patients,over 3.5 times higher than in NMO-IgG-positive patients （11.9%,P=0.008）.Statistical difference was also observed in CD4＋/CD8＋ ratios between NMO-IgG-positive and-negative NMO *** Comprehensive analysis of MRI,laboratory and EPs data can facilitate differential diagnosis of MS and *** addition,the combination of LESCLs and brain MRI findings failing to satisfy MRI criteria for MS is highly sensitive and specific for NMO.

Application Problems and Countermeasures of External Diaphragm Pacer in Elderly Patients with Lung Cancer after Surgery

This article summarized the nursing problems and corresponding nursing strategies of 18 cases of external diaphragm pacemakers in the application of elderly patients with lung cancer after thoracic surgery. Timely psychological care, correct operation, close observation and infection prevention during treatment can effectively optimize the use of external diaphragm pacemakers, improve respiratory muscle function, and the quality of life after lung cancer surgery in elderly patients.

Perioperative Nursing Care for Patients with Lung Cancer Undergoing Total Pneumonectomy

Objectives: To explore the perioperative nursing care for lung cancer patients undergoing total pneumonectomy, and to promote their rehabilitation. Methods: We will provide preoperative assessment and education in the aspects of respiratory function, diet, and psychology to patients undergoing total pneumonectomy, and provide continuous postoperative management about posture, fluid, and respiratory to help patients diagnosed with lung squamous cell carcinoma and underwent total pneumonectomy to reduce their postoperative complications and improve their overall quality of life. Results: 78.9% of lung cancer patients who underwent total pneumonectomy achieved a better outcome and prognosis by receiving the above nursing program. Conclusion: With large lung cancer tumors, the surgical trauma is highly invasive and the resection is extensive. Management of the patient’s respiratory tract is the focus of perioperative nursing and the key to preventing the development of related postoperative complications.

Analysis of the Current Status and Influencing Factors of Caring Ability of Main Relatives and Caregivers of Patients with Esophageal Cancer

Objective: To investigate the caring ability of the main relatives of postoperative patients with esophageal cancer, and to analyze the influencing factors. Methods: From May 2019 to May 2021, 290 esophageal cancer patients and their main relatives were selected by convenience sampling method in a Grade III a cancer hospital in Guangzhou;the general data questionnaire and the Chinese version of the scale of care ability of relatives’ caregivers were used. Results: The total average score of caring ability of the main relatives’ caregivers after esophageal cancer operation was (32.99 ± 3.01). Multiple stepwise regression analysis showed that the age of the caregiver, the length of stay, the family income per capita and the experience of caring were the influencing factors. Conclusion: The caring ability of the main relatives of the patients with esophageal cancer after operation is moderate and low. The medical staff should improve the caring ability of the caregivers and take effective measures according to different influencing factors to improve the psychological state of the caregivers.

目的探讨舌鳞状细胞癌患侧颈部淋巴结Level Ⅳ转移的风险因素.方法收集北京大学口腔医学院＆#183;口腔医院口腔颌面外科2000年1月至2011年12月舌鳞状细胞癌同期行颈淋巴结清扫术及原发灶手术治疗的患者248例（255侧）,分别应用卡方检验及Logistic回归分析Level Ⅳ转移的影响因素,分析变量包括年龄、性别、生长方式、T分期、病理分级、Level Ⅲ转移、Level Ⅰ～Ⅲ的转移阳性淋巴结分区个数、Level Ⅰ～Ⅲ的转移淋巴结个数.结果颈淋巴结转移患者147例（152侧）,LevelⅣ转移21侧（21/255,8.2％）,LevelⅣ跳跃性转移2侧（2/255,0.8％）.卡方检验结果表明,年龄（P=0.020）、Level Ⅲ发生转移（P=0.000）、Level Ⅰ～Ⅲ的转移阳性淋巴结分区个数（≥2）（P=0.006）、Level Ⅰ～Ⅲ的转移淋巴结个数（≥3）（P=0.000）是独立的风险因素;Logistic回归分析显示,只有LevelⅢ发生转移（P=0.003）为Level Ⅳ转移的风险因素.结论 Level Ⅲ发生转移的舌癌患者的LevelⅣ转移风险增加.

目的：探讨口腔癌同侧颈部LevelⅣ淋巴结转移的影响因素。方法：收集北京大学口腔医院颌面外科2000年1月至2011年12月同期行颈淋巴清扫术及原发灶手术治疗的口腔癌患者624例（644侧）,采用χ2检验及Logistic回归分析LevelⅣ转移风险因素。结果：颈淋巴结转移患者334例（345侧）,LevelⅣ转移35侧（5.43%,35/644）,跳跃性转移4侧（0.62%,4/644）。p NLevelsⅠ~Ⅲ颈部的LevelⅣ转移率1.31%（4/303）,p N＋LevelsⅠ~Ⅲ颈部的LevelⅣ转移率为9.14%（31/341）,χ2检验表明p N＋LevelsⅠ~Ⅲ与LevelⅣ转移有关联。p N＋LevelⅢ、前三区的p N＋Level数3枚、前三区的阳性淋巴结数≥3枚的患者,p N＋LevelⅣ发生率分别为22.47%（20/89）、28.57%（10/35）、14.60%（20/137）。进一步应用Logistic回归分析前三区淋巴结转移状态与LevelⅣ转移关系,结果证实前三区的p N＋Level数为3枚（p N＋LevelⅠ＋p N＋LevelⅡ＋p N＋LevelⅢ）与LevelⅣ转移有关联。结论：对于口腔癌患者,LevelsⅠ~Ⅲ转移将增加LevelⅣ转移风险。

Pre- and Post-Surgical Health-Related Quality of life Evaluation of Esophageal Carcinoma Patients

For patients with esophageal carcinoma (ESCA), health-related quality of life (HRQoL) has now become an essential feature. To examine the quality of life of preoperative and postoperative ESCA patients, we used the European Organization for Research and Treatment of Cancer Quality of life Core Questionnaire (EORTC QLQ C-30) and the Quality of life Questionnaire Oesophageal 18 (QLQ-OES18). Using the EORTC QLQ-C30 and the QLQ-OES18 questionnaire, the analysis of the quality of life scores of 246 patients with oesophageal cancer who were operated on at the Sun Yat-sen University Cancer Centre during the period 2013 to 2015 was carried out. Differences between pre- and post-surgical EORTC QLQ C-30 and QLQ-OES18 scores were examined using the Student’s t-test. Patients’ global health status (QoL) decreased significantly one month after the operation but gradually recovered within a year. In terms of the role function, the emotional function, the cognitive function, and the perception and function variants, EORTC QLQ-C30 and QLQ-OES18 scores increased statistically significantly, as did clinical signs variables such as exhaustion, nausea, vomiting, pain, sleeplessness, decreased appetite, stomach pain, and economic hardship. After surgery, there was an improvement in functional and symptom domains in esophageal carcinoma patients. EORTC QLQ-C30 and QLQ-OES18 can be used to assess the HRQoL before and after surgical procedures.
