Updates on treatment of irritable bowel syndrome

Irritable bowel syndrome （IBS） is a highly prevalent gastrointestinal disorder characterized by abdominal pain and discomfort in association with altered bowel habits. It is estimated tD affect 10%-15% of the western population, and has a large impact on quality of life and （in）direct healthcare costs. IBS is a multifactorial disorder involving dysregulation within the brain-gut axis, and it is frequently associated with gastrointestinal motor and sensory dysfunction, enteric and central nervous system irregularities, neuroimmune dysregulation, and postinfectious inflammation. As with other functional medical disorders, the treatment for IBS can be challenging. Conventional therapy for those with moderate to severe symptoms is largely unsatisfactory, and the development of new and effective drugs is made difficult by the complex pathogenesis, variety of symptoms, and lack of objective clinical findings that are the hallmark of this disorder. Fortunately, research advances over the past several decades have provided insight into potential mechanisms responsible for the pathogenesis of IBS, and have led to the development of several promising pharmaceutical agents. In recent years there has been much publicity over several of these new IBS medications （alosetron and tegaserod） because of their reported association with ischemic colitis and cardiovascular disease. while these agents remain available for use under restricted prescribing programs, this highlights the need for continued development of safe and effective medication for IBS. This article provides a physiologicallybased overview of recently developed and frequently employed pharmaceutical agents used to treat IBS, and discusses some non-pharmaceutical options that may be beneficial in this disorder.

Comparative analysis of empirical and deep learning models for ionospheric sporadic E layer prediction

Sporadic E(Es)layers in the ionosphere are characterized by intense plasma irregularities in the E region at altitudes of 90-130 *** they can significantly influence radio communications and navigation systems,accurate forecasting of Es layers is crucial for ensuring the precision and dependability of navigation satellite *** this study,we present Es predictions made by an empirical model and by a deep learning model,and analyze their differences comprehensively by comparing the model predictions to satellite RO measurements and ground-based ionosonde *** deep learning model exhibited significantly better performance,as indicated by its high coefficient of correlation(r=0.87)with RO observations and predictions,than did the empirical model(r=0.53).This study highlights the importance of integrating artificial intelligence technology into ionosphere modelling generally,and into predicting Es layer occurrences and characteristics,in particular.

Repeated hydrogen peroxide dosing briefly reduces cyanobacterial blooms and microcystin while increasing fecal bacteria indicators in a eutrophic pond

This study explored the effects of H_(2)O_(2)on Cyanobacteria and non-target microbes using fluorometry,microscopy,flow cytometry,and high throughput DNA sequencing of the 16S rRNA gene during a series of mesocosm and whole-ecosystem experiments in a eutrophic pond in NY,*** addition of H_(2)O_(2)(8 mg/L)significantly reduced Cyanobacteria concentrations during a majority of experiments(66%;6 of 9)and significantly increased eukaryotic green and unicellular brown algae in 78%and 45%of experiments,*** heterotrophic bacteria declined significantly following H_(2)O_(2)addition in all experiments,bacteria indicative of potential fecal contamination(Escherichia coli,Enterococcus,fecal coliform bacteria)consistently and significantly increased in response to H_(2)O_(2),evidencing a form of‘pollution swapping’.H_(2)O_(2)more effectively reduced Cyanobacteria in enclosed mesocosms compared to whole-ecosystem *** whole-pond H_(2)O_(2)applications over a twoyear period temporarily reduced cyanobacterial levels but never reduced concentrations below bloom thresholds and populations always rebounded in two weeks or *** bacterial phyla of Cyanobacteria,Actinobacteria,and Planctomycetes were the most negatively impacted by H_(2)O_(2).Microcystis was always reduced by H_(2)O_(2),as was the toxin microcystin,but Microcystis remained dominant even after repeated H_(2)O_(2)*** H_(2)O_(2)favored the growth of eukaryotic algae over potentially harmful Cyanobacteria,the inability of H_(2)O_(2)to end cyanobacterial blooms in this eutrophic waterbody suggests it is a non-ideal mitigation approach in high biomass ecosystems and should be used judiciously due to potential negative impacts on non-target organisms and promotion of bacteria indicative of fecal contamination.

Emission properties of nanolasers during the transition to lasing

This review addresses ongoing discussions involving nanolaser experiments,particularly those related to thresholdless lasing or few-emitter devices.A quantum-optical(quantum-mechanical active medium and radiation field)theory is used to examine the emission properties of nanolasers under different experimental *** active medium is treated as inhomogeneously broadened semiconductor quantum dots embedded in a quantum well,where carriers are introduced via current *** are made between a conventional laser and a nanolaser with a spontaneous emission factor of unity,as well as a laser with only a few quantum dots providing the *** is found that the combined exploration of intensity,coherence time,photon autocorrelation function and carrier spectral hole burning can provide a unique and consistent picture of nanolasers in the new regimes of laser operation during the transition from thermal to coherent ***,by reducing the number of quantum dots in the optical cavity,a clear indication of non-classical photon statistics is observed before the single-quantum-dot limit is reached.

Characterisation of dissolved organic matter in stormwater using high-performance size exclusion chromatography

Understanding the complexity of dissolved organic matter（DOM）in stormwater has drawn a lot of interest,since DOM from stormwater causes not only environmental impacts,but also worsens downstream aquatic quality associated with water supply and *** study introduced and employed high-performance size exclusion chromatography（HPSEC）coupled with an ultraviolet–visible（UV–vis）diode array detector to assess changes in stormwater-associated DOM *** DOM was also analysed in relation to storm event characteristics,water quality and spectroscopic *** tools were used to determine the correlations within DOM and water quality *** showed that dissolved organic carbon（DOC）and UV absorbance at 254 nm（UV（254））as conventional DOM parameters were found to be correlated well to the changes in stormwater quality during each of the three storm events *** detector wavelengths（210and 254 nm）and their ratio（A（210）/A（254））were found to provide additional information on the physiochemical properties of stormwater-associated *** study indicated that A（210）/A（254） is an important parameter which could be used to estimate the DOM proportions of functional groups and conjugated carbon *** study provided also an understanding of stormwater quality constituents through assessing variability and sensitivity for various parameters,and the additional information of rainfall characteristics on runoff quality data for a better understanding of parameter correlations and influences.

Hydrolyzed Al(Ⅲ) clusters:Speciation stability of nano-Al_(13)

Pure nano-All3 and aggregates at various concentrations were prepared to examine the particle size effect of coagulation with inorganic polymer flocculant. The property and stability of various species formed were characterized using Infrared, 27Al-NMR, photo correlation spectroscopy （PCS）, and Ferron assay. Results showed that concentration and temperature exhibited different roles on the stability of Al13. The quantity of Alb species analyzed by ferron assay in the initial aging period corresponded well with that of Al13, which has been confirmed in a dimension range of 1-2 nm by PCS. Al13 solutions at high concentrations （0.5-2.11 tool/L） were observed to undergo further aggregation with aging. The aggregates with a wide particle size distribution would contribute to the disappeared/decreased Al13 basis on the eTA1-NMR spectrum, whereas a part of Al13 would still remain as Alb. At low concentrations, All3 solution was quite stable at normal temperature, but lost its stability quickly when heating to 90~C.

Endoscopic ultrasound(EUS)is an important part of modern gastrointestinal endoscopy and now has an integral role in the diagnostic evaluation of pancreatic ***,as EUS technology has advanced,it has increasingly become a therapeutic procedure,and the prospect of multiple applications of interventional EUS for the pancreas is truly on the near ***,this review focuses on the established diagnostic and therapeutic roles of EUS that are used in current clinical *** particular,the diagnostic evaluation of acute pancreatitis,chronic pancreatitis,cystic pancreatic lesions and solid masses of the pancreas are *** newer enhanced imaging modalities of elastography and contrast enhancement are evaluated in this *** main therapeutic aspects of pancreatic EUS are then considered,namely celiac plexus block and celiac plexus neurolysis for pain control in chronic pancreatitis and pancreas cancer,and EUS-guided drainage of pancreatic fluid collections.

Improved hemostasis with major hepatic resection in the current surgical era

Background: Major hepatic resection, predominantly performed for oncologic intent, is a complex procedure with the potential for severe intraoperative hemorrhage. The current surgical era has the ability to improve hemostasis throughout the performance of major hepatic resections which decreases blood transfusions and the detrimental effects associated with transfusion. we evaluated hemostasis and outcomes in the current surgical era of performing hepatic resections. Methods: Utilizing the American College of Surgeons (ACS) National Surgical Quality Improvement Program (NSQIP) database all major hepatic resections performed between 2012 and 2016 were analyzed in regards to hemostasis. Hemostasis was evaluated by the need for and magnitude of blood transfusions. Additional perioperative variables (including operative time, length of hospital stay, and mortality rates) were analyzed to assess for outcomes with hemostasis. The NSQIP results were compared to previous publications involving major hepatic resections to detect improvement in hemostasis and outcomes in the current surgical era. Results: A total of 22777 major hepatic resections met the inclusion criteria for analysis in the NSQIP database. An additional 21198 cases were compiled within the selected publications for comparative analysis. The transfusion rate in the current surgical era was 13.3% versus 38.7% in the previous era ( P = 0.0001). when a transfusion was required in the current surgical era there was a two-fold reduction in the number of units transfused (1.5 U vs. 3.8 U, P = 0.0001). Statistically significant improvements in operative time and length of hospital stay were presented within the current surgical era ( P = 0.0001). when a transfusion was required there was an increased relative risk score of 7 for mortality (4.9% vs. 0.7%, P = 0.0001), however, improvement in mortality rates did not reach statistical significance across surgical eras (1.3% vs. 4.0%, P = 0.0001).
