Acute kidney injuries (AKI) are a common problem encountered by internists in the inpatient and outpatient setting. Uncovering the etiology becomes important in tailoring treatment. Point-of-care ultrasound (POCUS), when used in conjunction with a patient’s history and physical exam, can quickly identify if hydronephrosis is present and shed light on the underlying etiology. In this article, we discuss how incorporating POCUS into the initial assessment of patients with an AKI can reduce the number of unnecessary and expensive formal renal ultrasounds and potentially expedite clinically appropriate treatment. the purpose of this review is to examine the benefits, reliability, and feasibility of POCUS to further evaluate and manage patients with AKI. An extensive review of the literature was performed and found POCUS to be a reliable and realistic method for internists to incorporate into their assessment of patients with AKI. In this article, we also provide instruction on how to perform a POCUS exam of the kidneys and how to identify hydronephrosis. Furthermore, we discuss the challenges we face and ideas for further practice including the emergence of hand-held ultrasounds.

Somatic activating mutations in the epidermal growth factor receptor(EGFR)are one of the most common oncogenic drivers in cancers such as non-small-cell lung cancer(NSCLC),metastatic colorectal cancer,glioblastoma,head and neck cancer,pancreatic cancer,and breast ***-targeted agents against EGFR signaling pathways have shown robust clinical efficacy,but patients inevitably experience acquired *** immune checkpoint inhibitors(ICIs)targeting PD-1/PD-L1 have exhibited durable anti-tumor responses in a subset of patients across multiple cancer types,their efficacy is limited in cancers harboring activating gene alterations of *** studies have demonstrated that upregulation of new B7/CD28 family members such as B7-H3,B7x and HHLA2,is associated with EGFR signaling and may contribute to resistance to EGFR-targeted therapies by creating an immunosuppressive tumor microenvironment(tME).In this review,we discuss the regulatory effect of EGFR signaling on the PD-1/PD-L1 pathway and new B7/CD28 family member *** these interactions may inform combination therapeutic strategies and potentially overcome the current challenge of resistance to EGFR-targeted *** also summarize clinical data of anti-PD-1/PD-L1 therapies in EGFR-mutated cancers,as well as ongoing clinical trials of combination of EGFR-targeted therapies and anti-PD-1/PD-L1 immunotherapies.

Herbal medicines are popular natural medicines that have been used for *** use of alternative medicines continues to expand rapidly across the *** World Health Organization suggests that quality assessment of natural medicines is essential for any therapeutic or health care applications,as their therapeutic potential varies between different geographic origins,plant species,and *** of herbal medicines based on a limited number of secondary metabolites is not an ideal *** quality should be considered based on a complete metabolic profile,as their pharmacological activity is not due to a few specific secondary metabolites but rather a larger group of bioactive compounds.A holistic and integrative approach using rapid and nondestructive analytical strategies for the screening of herbal medicines is required for robust *** this study,a rapid and effective quality assessment system for geographical traceability,species,and variety-specific authenticity of the widely used natural medicines turmeric,Ocimum,and Withania somnifera was investigated using Fourier transform near-infrared(Ft-NIR)spectroscopy-based metabolic *** different geographical origins of turmeric,five different Ocimum species,and three different varieties of roots and leaves of Withania somnifera were studied with the aid of machine learning *** good discrimination(R^(2)>0.98,Q^(2)>0.97,and accuracy=1.0)with sensitivity and specificity of 100%was achieved using this metabolic fingerprinting *** study demonstrated that Ft-NIR-based rapid metabolic fingerprinting can be used as a robust analytical method to authenticate several important medicinal herbs.

Rate-transient analysis(RtA)has been widely applied to extract estimates of reservoir/hydraulic fracture ***,the majority of RtA techniques can lead to misdiagnosis of reservoir/fracture information when the reservoir exhibits reservoir heterogeneity and multiphase flow *** work proposes a practical-yet-rigorous method to decouple the effects of reservoir heterogeneity and multiphase flow during tLF,and improve the evaluation of reservoir/fracture properties.A new,general,semi-analytical model is proposed that explicitly accounts for multiphase flow,fractalbased reservoir heterogeneity,anomalous diffusion,and pressure-dependent fluid *** is achieved by introducing a new Boltzmann-type transformation,the exponent of which includes reservoir heterogeneity and anomalous *** order to decouple the effects of reservoir heterogeneity and multiphase flow during tLF,the modified Boltzmann variable allows the conversion of three partial differential equations(PDE's)(i.e.,oil,gas and water diffusion equations)into ordinary differential equations(ODE's)that are easily solved using the Runge-Kutta(RK)method.A modified time-power-law plot is also proposed to estimate the reservoir and fracture properties,recognizing that the classical square-root-of-time-plot is no longer valid when various reservoir complexities are exhibited *** the slope of the straight line on the modified time-power-law plot,the linear flow parameter can be estimated with more ***,because of the new Boltzmann-type transformation,reservoir and fracture properties can be derived more efficiently without the need for defining complex pseudo-variable *** the new semi-analytical model,the effects of multiphase flow,reservoir heterogeneity and anomalous diffusion on rate-decline behavior are *** the case of approximately constant flowing pressure,multiphase flow impacts initial oil rate,which is a funct

Chronic infection with Helicobacter pylori cagA-positive strains is the strongest risk factor for gastric *** cagA gene product,CagA,is delivered into gastric epithelial cells via the bacterial type IV secretion *** CagA then undergoes tyrosine phosphorylation at the Glu-Pro-Ile-tyr-Ala(EPIYA)motifs in its C-terminal region and acts as an oncogenic scaffold protein that physically interacts with multiple host signaling proteins in both tyrosine phosphorylation-dependent and-independent *** of CagA using in vitro cultured gastric epithelial cells has indicated that the nonphysiological scaffolding actions of CagA cell-autonomously promote the malignant transformation of the cells by endowing the cells with multiple phenotypic cancer hallmarks:sustained proliferation,evasion of growth suppressors,invasiveness,resistance to cell death,and genomic *** expression of CagA in mice leads to in vivo oncogenic action of CagA without any overt *** in vivo oncogenic activity of CagA is further potentiated in the presence of chronic *** Helicobacter pylori infection triggers a proinflammatory response in host cells,a feedforward stimulation loop that augments the oncogenic actions of CagA and inflammation is created in CagA-injected gastric *** that Helicobacter pylori is no longer colonized in established gastric cancer lesions,the multistep nature of gastric cancer development should include a“hit-and-run”process of CagA ***,acquisition of genetic and epigenetic alterations that compensate for CagA-directed cancer hallmarks may be required for completion of the“hit-and-run”process of gastric carcinogenesis.
