Hepatocellular carcinoma(HCC)is the most common liver-related complication seen in patients with non-alcoholic fatty liver disease(NAFLD)and non-alcoholic steatohepatitis(NASH)(1).NASH is strongly associated with type 2 diabetes mellitus(t2DM)but it has also been apparent for a number of years that t2DM is associated with an increased risk of HCC independent of the presence of NASH(2).A 2006 meta-analysis of retrospective cohort studies and case-control studies demonstrated an association between t2DM and an increased risk of HCC(3).International guidelines advocate surveillance for HCC in patients with NASH who have progressed to cirrhosis(4).An important clinical question is whether t2DM confers an increased risk of HCC once patients with NASH have progressed to *** a recent study by Yang and colleagues(5),the authors reported strong evidence that t2DM is an independent risk factor for HCC in patients with NASH-related *** and colleagues performed a retrospective analysis of a prospectively registered research dataset of patients with NASH-related cirrhosis.

Background and Aims:Previous studies have reported that the single nucleotide polymorphisms(SNPs)of SAMM50-rs738491,PARVB-rs5764455 and PNPLA3-rs738409 are associated with nonalcoholic fatty liver disease(NAFLD).However,no studies have examined the effect of interactions between these three genotypes to affect liver disease *** assessed the effect of these three SNPs on nonalcoholic steatohepatitis(NASH)and also examined the gene-gene interactions in a Chinese population with biopsy-confirmed ***:We enrolled 415 consecutive adult individuals with biopsy-proven *** logistic regres-sion analysis was undertaken to test associations between NASH and SNPs in SAMM50-rs738491,PARVB-rs5764455 and ***-gene interactions were ana-lyzed by performing a generalized multifactor dimensionality reduction(GMDR)***:the mean±standard deviation age of these 415 patients was 41.3±12.5 years,and 75.9%were *** with SAMM50-rs738491 tt,PARVB-rs5764455 AA or PNPLA3-rs738409 GG genotypes had a higher risk of NASH,even after adjustment for age,sex and body mass *** analysis showed that the combination of all three SNPs was the best model for predicting ***,the odds ratio of the haplotype t-A-G for predicting the risk of NASH was nearly three times higher than that of the haplotype ***:NAFLD patients carrying the SAMM50-rs738491 tt,PARVB-rs5764455 AA or PNPLA3-rs738409 GG genotypes are at greater risk of *** three SNPs may synergistically interact to increase susceptibility to NASH.

Objective:to review follow-up imaging after equivocal bone scans in men with castration resistant prostate cancer(CRPC)and examine the characteristics of equivocal bone scans that are associated with positive follow-up ***:We identified 639 men from five Veterans Affairs Hospitals with a technetium-99m bone scan after CRPC diagnosis,of whom 99(15%)had equivocal *** with equivocal scans were segregated into“high-risk”and“low-risk”subcategories based upon wording in the bone scan *** follow-up imaging(bone scans,computed tomography[Ct],magnetic resonance imaging[MRI],and X-rays)in the 3 months after the equivocal scan were *** were compared between patients with a positive *** follow-up imaging after an equivocal bone ***:Of 99 men with an equivocal bone scan,43(43%)received at least one follow-up imaging test,including 32/82(39%)with low-risk scans and 11/17(65%)with high-risk scans(p=0.052).Of follow-up tests,67%were negative,14%were equivocal,and 19%were *** those who underwent follow-up imaging,3/32(9%)low-risk men had metastases vs.5/11(45%)high-risk men(p=0.015).Conclusion:While 19%of all men who received follow-up imaging had positive follow-up imaging,only 9%of those with a low-risk equivocal bone scan had metastases versus 45%of those with *** preliminary findings,if confirmed in larger studies,suggest follow-up imaging tests for low-risk equivocal scans can be delayed while high-risk equivocal scans should receive follow-up imaging.

this paper presents an extensive survey of the most commonly used tools for diagnosing unbalanced flow in the atmosphere, namely the Lagrangian Rossby number, Psi vector, divergence equation, nonlinear balance equation, generalized omega-equation, and departure from fields obtained by potential vorticity (PV) inversion. the basic thoery, assumptions as well as implementation and limitations for each of the tools are all discussed. these tools are applied to high—resolution mesoscale model data to assess the role of unbalanced dynamics in the generation of a mesoscale gravity wave event over the East Coast of the United States. Comparison of these tools in this case study shows that these various methods agree to a large extent with each other though they differ in details. Primary biliary cholangitis(PBC) is an autoimmune cholestatic liver disease with multiple debilitating complications. Osteoporosis is a common complication of PBC resulting in frequent fractures and leading to significant morbidity in this population, yet evidence for effective therapy is lacking. We sought to summarize our current understanding of the pathophysiology of osteoporosis in PBC, as well as current and emerging therapies in order to guide future research directions. A complete search with a comprehensive literature review was performed with studies from Pub Med, EMBASE, Web of Science, Cochrane database, and the Countway Library. Osteoporosis in PBC is driven primarily by decreased bone formation, which differs from the increased bone resorption seen in postmenopausal osteoporosis. Despite this fundamental difference, current treatment recommendations are based primarily on experience with postmenopausal osteoporosis. trials specific to PBC-related osteoporosis are small and have not consistently demonstrated a benefit in this population. As it stands, prevention of osteoporosis in PBC relies on the mitigation of risk factors such as smoking and alcohol use, as well as encouraging a healthy diet and weight-bearing exercise. the primary medical intervention for the treatment of osteoporosis in PBC remains bisphosphonates though a benefit in terms of fracture reduction has never been shown. this review outlines what is known regarding the pathogenesis of bone disease in PBC and summarizes current and emerging therapies.

A systematic investigation of stem cell-derived neural interfaces can facilitate the discovery of the molecular mechanisms behind cell behavior in neurological disorders and accelerate the development of stem cell-based ***,high-throughput investigation of the cell-type-specific biophysical cues associated with stem cell-derived neural interfaces continues to be a significant obstacle to *** this end,we developed a combinatorial nanoarray-based method for high-throughput investigation of neural interface micro-/nanostructures(physical cues comprising geometrical,topographical,and mechanical aspects)and the effects of these complex physical cues on stem cell fate ***,by applying a machine learning(ML)-based analytical approach to a large number of stem cell-derived neural interfaces,we comprehensively mapped stem cell adhesion,differentiation,and proliferation,which allowed for the cell-type-specific design of biomaterials for neural interfacing,including both adult and human-induced pluripotent stem cells(hiPSCs)with varying genetic *** short,we successfully demonstrated how an innovative combinatorial nanoarray and ML-based platform technology can aid with the rational design of stem cell-derived neural interfaces,potentially facilitating precision,and personalized tissue engineering applications.

Many experimental cancer vaccines are exploring toll-like receptor agonists （tLRas） such as CpG, a DNA motif that agonizes toll-like receptor 9 （tLR9）, to trigger immune responses that are potent and molecularly-specific. the ability to tune the immune response is especially important in the immunosuppressive microenvironments of tumors. Because tLR9 is located intracellularly, CpG must be internalized by immune cells for functionality. Polyplexes can be self- assembled through electrostatics using DNA （anionic） condensed by a positively charged carrier. these structures improve cell delivery and have been widely explored for gene therapy. In contrast, here we use cationic poly （^-amino esters）（PBAEs） to assemble polyplexes from CpG as an adjuvant to target and improve immune stimulation in cells and mouse models. Polyplexes were formed over a range of PBAE：CpG ratios, resulting in a library of complexes with increasingly positive charge and stronger binding as PBAE：CpG ratio increased. Although higher PBAE：CpG ratios exhibited improved CpG uptake, lower ratios of PBAE：CpG--which condensed CpG more weakly, activated DCs and tumorspecific t cells more effectively. In a mouse melanoma model, polyplexes with lower binding affinities improved survival more effectively compared with higher binding affinities. these data demonstrate that altering the polyplex interaction strength impacts accessibility of CpG to tLRs in immune cells. thus, physiochemical properties, particularly the interplay between charge, uptake, and affinity, play a key role in determining the nature and efficacy of the immune response generated. this insight identifies new design considerations that must be balanced for engineering effective immunotherapies and vaccines.
