AIM To review surgical outcomes for patients undergoing pancreatectomy after proton therapy with concomitant capecitabine for initially unresectable pancreatic *** From April 2010 to September 2013,15 patients with initially unresectable pancreatic cancer were treated withproton therapy with concomitant capecitabine at 1000 mg orally twice daily. All patients received 59.40 Gy(RBE) to the gross disease and 1 patient received 50.40 Gy(RBE) to high-risk nodal targets. There were no treatment interruptions and no chemotherapy dose reductions. Six patients achieved a radiographic response sufficient to justify surgical exploration,of whom 1 was identified as having intraperitoneal dissemination at the time of surgery and the planned pancreatectomy was aborted. Five patients underwent resection. Procedures included:Laparoscopic standard pancreaticoduodenectomy(n = 3),open pyloris-sparing pancreaticoduodenectomy(n = 1),and open distal pancreatectomy with irreversible electroporation(IRE) of a pancreatic head mass(n = 1). RESULTS The median patient age was 60 years(range,51-67). The median duration of surgery was 419 min(range,290-484),with a median estimated blood loss of 850 cm^3(range,300-2000),median ICU stay of 1 d(range,0-2),and median hospital stay of 10 d(range,5-14). Three patients were re-admitted to a hospital within 30 d after discharge for wound infection(n = 1),delayed gastric emptying(n = 1),and ischemic gastritis(n = 1). Two patients underwent R0 resections and demonstrated minimal residual disease in the final pathology specimen. One patient,after negative pancreatic head biopsies,underwent IRE followed by distal pancreatectomy with no tumor seen in the specimen. Two patients underwent R2 resections. Only 1 patient demonstrated ultimate local progression at the primary site. There is now strong evidence that the paternal contribution to offspring phenotype at fertilisation is more than just DNA. However, the identity and mechanisms of this nongenetic inheritance are poorly understood. One of the more important questions in this research area is： do changes in sperm DNA methylation have phenotypic consequences for offspring？ We have previously reported that offspring of obese male rats have altered glucose metabolism compared with controls and that this effect was inherited through nongenetic means. Here, we describe investigations into sperm DNA methylation in a new cohort using the same protocol. Male rats on a high-fat diet were 30% heavier than control-fed males at the time of mating （16-19 weeks old, n = 14/14）. A small （0.25%） increase in total 5-methyl-2＇-deoxycytidine was detected in obese rat spermatozoa by liquid chromatography tandem mass spectrometry. Examination of the repetitive fraction of the genome with methyI-CpG binding domain protein-enriched genome sequencing （MBD-Seq） and pyrosequencing revealed that retrotransposon DNA methylation states in spermatozoa were not affected by obesity, but methylation at satellite repeats throughout the genome was increased. However, examination of muscle, liver, and spermatozoa from male 27-week-old offspring from obese and control fathers （both groups from n = 8 fathers） revealed that normal DNA methylation levels were restored during offspring development. Furthermore, no changes were found in three genomic imprints in obese rat spermatozoa. Our findings have implications for transgenerational epigenetic reprogramming. They suggest that postfertilization mechanisms exist for normalising some environmentally-induced DNA methylation changes in sperm cells.

In this report, we demonstrate a novel technique for the microscopic patterning of gold by combining the photoreduction of Au^(Ⅲ)Br_(2)^(-)to Au^(Ⅰ)Br_(2)^(-)and the electrochemical reduction of Au^(Ⅰ)Br_(2)^(-)to elemental gold in a single step within solution. While mask-based methods have been the norm for electroplating, the adoption of direct laser writing for flexible, real-time patterning has not been widespread. Through irradiation using a 405 nm laser and applying a voltage corresponding to a selective potential window specific to Au^(Ⅰ)Br_(2)^(-), we have shown that we can locally deposit elemental gold at the focal point of the laser. In addition to demonstrating the feasibility of the technique, we have collected data on the kinetics of the photoreduction reaction in ethanol and have deduced its rate law. We have confirmed the selective deposition of Au^(Ⅰ)Br_(2)^(-) within a potential window through controlled potential electrolysis experiments and through direct measurement on a quartz crystal microbalance. Finally, we have verified local deposition through scanning electron microscopy.

目的:研究背阔肌肌皮穿支血管在皮肤内的走行和分布面积,探索背阔肌穿支皮瓣的切取范围和预测该穿支皮瓣的成活面积。方法:(1)选用10具新鲜尸体,采用改良氧化铅-明胶灌注技术进行动脉灌注。解剖胸背动脉穿支血管(口径大于0.5mm)。分别拍摄X线片以显示胸背动脉在背阔肌内和其穿支血管皮肤内的形态和分布。测定穿支血管的数量、口径及其供应区域的面积。(2)在解剖研究基础上应用了游离胸背动脉穿支皮瓣修复肢体和躯干皮肤缺损共6例。结果:发现胸背动脉发出3～6支肌皮穿支胸背动脉血管供应皮肤。其中最大的胸背动脉穿支起自外侧支,位于腋后襞下6～8cm。其以下从外侧支发出的穿支数可多达3个,每间隔1.5～4cm发出穿支。每个穿支斜行3～5cm穿过肌肉达皮肤。穿支动脉口径约为0.3～0.6mm,均有两条伴行静脉。临床应用胸背动脉穿支皮瓣6例,皮瓣最大面积为20cm×12cm,最小为15cm×8cm。皮瓣全部成活,随访8～14个月,皮瓣外形均满意,无需二次手术修薄皮瓣。供区隐蔽,无背阔肌功能障碍。结论:本研究为预测胸背动脉穿支皮瓣的成活面积及安全可靠地切取该穿支皮瓣提供解剖依据。

目的:对全身皮肤血管区域进行定性和定量分析,确定全身皮肤穿支血管的位置、数量、口径、穿支蒂的长度、类型、来源血管以及穿支所供应皮肤的面积,为穿支皮瓣提供血管解剖基础。方法:选用10具新鲜尸体,采用改良氧化铅-明胶灌注技术进行动脉灌注。将每个口径大于0.5mm的穿支血管进行解剖并记录。拍摄X线片以显示皮肤内血管的形态和分布。定量数据分析包括全身各部位的穿支血管的数量、口径、类型及其供应区域的面积。结果:发现全身128支起源血管发出440支穿支供应皮肤。其中肌皮穿支与肌间隔穿支之比为3:2。穿支平均直径为0.7mm。穿支血管的分布规律如下:①躯干皮肤的血供主要来自肌皮穿支,这些穿支在皮肤内的走行距离和分布范围大于肢体皮肤的穿支。②肢体皮肤的血供主要来自肌间隙穿支血管,主要分布在深筋膜表面,皮神经和浅静脉周围穿支之间形成链式血管吻合。③单位面积的穿支数量与皮肤的移动程度成反比,穿支的口径大小和穿支在皮肤内走行距离与皮肤移动度成正比,与穿支的供应面积成正比。结论:本研究提供详细的皮肤穿支血管解剖数据,为临床设计应用穿支皮瓣提供解剖学依据。

This article provides a review of tropical cyclone(TC)surface wind estimation from an operational forecasting ***,we provide a summary of operational forecast center practices and historical ***,we discuss current and emerging objective estimates of TC surface winds,including algorithms,archive datasets,and individual algorithm strengths and weaknesses as applied to operational TC surface wind forecast *** review leads to recommendations about required surface coverage—an area covering at least 1100 km from center of TC at a 2-km resolution in the inner-core,and at a frequency of at least once every 6 *** is enough coverage to support a complete analysis of the TC surface wind field from center to the extent of the 34-kt(17 m s^(-1))winds at 6-h *** also suggest future designs of TC surface wind capabilities include funding to ensure near real-time data delivery to operators so that operational evaluation and use are feasible within proposed ***,we suggest that users of archived operational wind radii datasets contact operational organizations to ensure these datasets are appropriate for their needs as the datasets vary in quality through time and space,even from a single organisation.

Background:The transition metal copper enhances amyloidβaggregation and neurotoxicity,and in models of concomitant amyloid and tau pathology,copper also promotes tau *** it is not clear if the effects of environmental copper upon tau pathology are dependent on the presence of pathological amyloidβ,we tested the effects of copper overload and complexing in disease models which lack pathological amyloidβ.Methods:We used cell culture and transgenic murine models to test the effects of environmental copper on tau *** used oral zinc acetate as a copper lowering agent in mice and examined changes in blood and brain metals through inductively coupled plasma mass *** effects of copper lowering were assessed with morris water maze and novel object recognition *** in tau phosphorylation were examined by phosphorylation specific antibodies on Western ***:In human neuroblastoma cells,excess copper promoted tau phosphorylation and a copper complexing agent,tetrathiomolybdate,attenuated tau *** a transgenic mouse model expressing wild type human tau,copper-lowering by oral zinc suppressed plasma and brain levels of copper,and resulted in a marked attenuation of tau *** significant changes in behavior were observed with copper lowering,but a trend to improved recognition of the novel object was observed in zinc acetate treated ***:We propose that reduction of brain copper by blocking uptake of copper from the diet may be a viable strategy for modulating tau pathology in Alzheimer’s *** potential benefits of this approach are tempered by the absence of a behavioral benefit and by the health risks of excessive lowering of copper.

目的比较微创左胸廓内动脉冠状动脉搭桥术和经皮冠状动脉支架术作为初始治疗方法治疗左前降支孤立性病变的结果。设计将已发表文献中的随机和非随机的比较性研究进行荟萃分析。数据来源 Embase,Medline,Cochrane,Google Scholar 以及 Health Technology Assessmentdatabases(1966-2005)。综述方法检索和找出以上数据库中采用这两种方法初始治疗孤立性左前降支狭窄的所有比较性研究,并提取以下内容:研究设计,人口学特征,冠心病的严重程度,心血管疾病的高危因素以及有关的治疗结果。结果本荟萃分析包括了来自8个研究小组的12项研究(1952例患者):其中1项回顾性研究,1项非随机前瞻性研究,6项随机前瞻性研究。随机临床试验的荟萃分析显示经皮冠状动脉支架术组有相对较高的心绞痛复发率(比数比2.62,95%可信区间1.32～5.21),主要冠状动脉和脑血管事件发生率较高(2.86,1.62～5.08),需要再次血运重建率亦较高(4.63,2.52～8.51)。在最大随访终点时,两种治疗方法在心肌梗死、脑卒中或死亡率上无显著性差异。结论与经皮冠状动脉支架术相比,微创左胸廓内动脉搭桥术治疗左前降支孤立性病变的中期并发症发生率较低。
