Liver Fibrosis Helps to Distinguish Autoimmune Hepatitis from DILI with Autoimmune Features: A Review of Twenty Cases

Background and Aims:Drug-induced liver inj.ry with autoimmune features(AI-DILI)mimics the clinical presentation,and laboratory and pathologic features of idiopathic autoimmune hepatitis(AIH).We aimed to identify histopathologic hallmarks to differentiate these ***:All liver biopsies archived for the past 10 years were reviewed retrospectively to identify cases of recently detected liver inj.ry associated with predominantly lymphoplasmacytic interphase hepatitis,positive markers for liver autoimmunity,and negative tests for viral *** cases were divided into AIH(n=12)or AI-DILI(n=8)*** qualitative evaluation of necroinflammatory changes and liver fibrosis were performed according to the Scheuer scoring *** densities were determined using Imagej.V1.51t,National Institutes of Health,Bethesda,MD,USA).Fibrosis was assessed on Masson trichrome-stained slides,and collagen deposition was estimated following a protocol of color ***:Necroinflammatory changes as well as densities(portal and lobular)of neutrophils and eosinophils,intracellular cholestasis,and regenerative changes did not differ between the two groups(P≥0.05).Neutrophil densities but not eosinophils showed a positive correlation with the severity of hepatocellular damage(r=0.6and 0.58,*** aminotransferase,P<0.05).Ceroidladen macrophages but not histiocytic aggregates appeared to be more common in AI-DILI(P<0.05).AIH patients presented more often with evidence of chronic damage,including higher scores of fibrosis and collagen deposition,in comparison to AI-DILI(P<0.05).Conclusions:Although there is no histologic feature pathognomonic for AI-DILI or AIH,advanced stages of liver fibrosis can be used to support the diagnosis of AIH in some *** diagnosis of AI-DILI requires follow-up and demonstration of complete remission after drug withdrawal with no need for immunosuppression.

Non-alcoholic fatty liver disease: a narrative review of genetics

Non-alcoholic fatty liver disease(NAFLD) is now the most common cause of chronic liver diseases worldwide. It encompasses a spectrum of disorders ranging from isolated hepatic steatosis to nonalcoholic steatohepatitis(NASH),fibrosis, cirrhosis, and hepatocellular carcinoma. One of the key challenges in NAFLD is identifying which patients will progress. Epidemiological and genetic studies indicate a strong pattern of heritability that may explain some of the variability in NAFLD phenotype and risk of progression. To date, at least three common genetic variants in the PNPLA3, TM6 SF2, and GCKR genes have been robustly linked to NAFLD in the population. The function of these genes revealed novel pathways implicated in both the development and progression of NAFLD. In addition,candidate genes previously implicated in NAFLD pathogenesis have also been identified as determinants or modulators of NAFLD phenotype including genes involved in hepatocellular lipid handling, insulin resistance,inflammation, and fibrogenesis. This article will review the current understanding of the genetics underpinning the development of hepatic steatosis and the progression of NASH. These newly acquired insights may transform our strategy to risk-stratify patients with NAFLD and to identify new potential therapeutic targets.

Football is the most popular sport worldwide and is associated with a high inj.ry rate, most of which are the result of trauma from player contact. Ankle inj.ries are among the most commonly diagnosed inj.ries in the game. The result is reduced physical activity and endurance levels, lost game time, and considerable medical cost. Sports medicine professionals must employ the correct diagnostic tools and effective treatments and rehabilitation protocols to minimize the impact of these inj.ries on the player. This review examines the diagnosis, treatment, and postoperative rehabilitation for common football inj.ries of the ankle based on the clinical evidence provided in the current literature.

P2X7 purinoceptor promotes survival or cytotoxicity depending on extraceUular adenosine triphosphate （ATP） stimulus Intensity controlling its ion channel or P2X7-dependent large pore （LP） functions. Mechanisms governing this operational divergence and functional idiosyncrasy are ill-understood. We have discovered a feedback loop where sustained activation of P2X7 triggers release of active matrix metalloproteinase 2 （MMP-2）, which halts ion channel and LP responses via the MMP-2-dependent receptor cleavage. This mechanism operates in cells as diverse as macrophages, dystrophic myoblasts, P2X7-transfected HEK293, and human tumour cells. Given that serum-born MMP-2 activity also blocked receptor functions, P2X7 responses in vivo may decrease in organs with permeable capillaries. Therefore, this mechanism represents an Important fine-tuning of P2X7 functions, reliant on both cell-autonomous and extraneous factors. Indeed, it allowed evasion from the ATP-induced cvtotoxicity in macrophages and human cancer ceils with high P2X7 expression levels. Finally, we demonstrate that P2X7 ablation eliminated getatinase activity in inflamed dystrophic muscles in vivo. Thus, P2X7 antagonists could be used as an alternative to highly toxic MMP inhibitors in treatments of inflammatory diseases and cancers.

An experimental study of elastic properties of dragonfly-like flapping wings for use in biomimetic micro air vehicles(BMAVs)

This article studies the elastic properties of several biomimetic micro air vehicle（BMAV）wings that are based on a dragonfly *** are a new class of unmanned micro-sized air vehicles that mimic the flapping wing motion of flying biological organisms（e.g.,insects,birds,and bats）.Three structurally identical wings were fabricated using different materials：acrylonitrile butadiene styrene（ABS）,polylactic acid（PLA）,and *** wing frame structures were fabricated from these materials and then a nanocomposite film was adhered to them which mimics the membrane of an actual *** wings were then attached to an electromagnetic actuator and passively flapped at frequencies of 10-250 Hz.A three-dimensional high frame rate imaging system was used to capture the flapping motions of these wings at a resolution of 320 pixels x 240 pixels and 35000 frames per *** maximum bending angle,maximum wing tip deflection,maximum wing tip twist angle,and wing tip twist speed of each wing were measured and compared to each other and the actual dragonfly *** results show that the ABS wing has considerable flexibility in the chordwise direction,whereas the PLA and acrylic wings show better conformity to an actual dragonfly wing in the spanwise *** studies have shown that the aerodynamic performance of a BMAV flapping wing is enhanced if its chordwise flexibility is increased and its spanwise flexibility is ***,the ABS wing（fabricated using a 3D printer） shows the most promising results for future applications.

The Universal Soil Loss Equation model is often used to improve soil resource conservation by monitoring and forecasting soil *** study tested a novel method to determine the cover and management factor(C)of this model by coupling the leaf area index(LAI)and soil basal respiration(SBR)to more accurately estimate a soil erosion map for a typical region with red soil in Hetian,Fuj.an Province,*** spatial distribution of the LAI was obtained using the normalized difference vegetation index and was consistent with the LAI observed in the field(R^2=0.66).The spatial distribution of the SBR was obtained using the Carnegie-Ames-Stanford Approach model and verified by soil respiration field observations(R^2=0.51).Correlation analyses and regression models suggested that the LAI and SBR could reasonably reflect the structure of the forest canopy and understory vegetation,***,the C-factor was reconstructed using the proposed forest vegetation structure factor(Cs),which considers the effect of the forest canopy and shrub and litter layers on reducing rainfall *** feasibility of this new method was thoroughly verified using runoff plots(R2=0.55).The results demonstrated that Cs may help local governments understand the vital role of the structure of the vegetation layer in limiting soil erosion and provide a more accurate large-scale quantification of the C-factor for soil erosion.

ABCD^(2) risk score does not predict the presence of cerebral microemboli in patients with hyper-acute symptomatic critical carotid artery stenosis

Introduction:ABCD^(2) risk score and cerebral microemboli detected by transcranial Doppler(TCD)have been separately shown to the predict risk of recurrent acute *** studied whether ABCD2 risk score predicts cerebral microemboli in patients with hyper-acute symptomatic carotid artery *** and methods:We studied 206 patients presenting within 2 weeks of transient ischaemic attack or minor stroke and found to have critical carotid artery stenosis(≥50% ).86 patients(age 70±1(SEM:years),58 men,83 Caucasian)had evidence of microemboli;72(84% )of these underwent carotid endarterectomy(CEA).120 patients(age 72±1 years,91 men,113 Caucasian)did not have microemboli detected;102(85% )of these underwent *** were analysed using X2 and Mann–Whitney U tests and receiver operating characteristic(ROC)***:140/206(68% :95% CI 61.63 to 74.37)patients with hyper-acute symptomatic critical carotid stenosis had an ABCD2 risk score≥*** was no significant difference in the NICE red flag criterion for early assessment(ABCD^(2) risk score≥4)for patients with cerebral microemboli versus those without microemboli(59/86 vs 81/120 patients:OR 1.05 ABCD2 risk score≥4(95% CI 0.58 to 1.90,p=0.867)).The ABCD2 risk score wasj.sting for pre-neurological event antiplatelet treatment(APT),area under the curve(AUC)of ROC for ABCD2 risk score showed no prediction of cerebral microemboli(no pre-event APT,n=57:AUC 0.45(95% CI 0.29 to 0.60,p=0.531);preevent APT,n=147:AUC 0.51(95% CI 0.42 to 0.60,p=0.804)).Conclusions:The ABCD2 score did not predict the presence of cerebral microemboli or carotid disease in over one-quarter of patients with symptomatic critical carotid artery *** the basis of NICE guidelines(refer early if ABCD2≥4),assessment of high stroke risk based on ABCD2 scoring may lead to inappropriate delay in urgent treat

Muscle energy technique for chronic obstructive pulmonary disease:A feasibility study

Obj.ctive:This study aimed to investigate the feasibility of implementing a manual therapy technique(muscle energy technique,MET)protocol in a hospital pulmonary rehabilitation(PR)program for patients with moderate to severe chronic obstructive pulmonary disease(COPD).Methods:Participants aged 40 years and over,with moderate to severe COPD,were recruited into this 12-week *** primary outcome measures were feasibility(acceptability of the intervention and attendance/adherence to the trial)and safety(adverse events,AEs).All participants received the MET and PR *** and assessors were ***-standardized MET was delivered on 6 occasions(a maximum of once per week)at the hospital directly before a PR *** undertook PR sessions as per the hospital program at a frequency of two days per week for 8 *** were contacted 4 weeks after their final MET treatment via a telephone call to assess acceptability of the ***:Thirty-three participants were enrolled,with a median age of 74 years(range 45-89 years).The median number of MET sessions that participants attended was 5(range O-6)out of a possible 6 sessions(83%attendance).At follow-up,participants overwhelmingly enj.yed the MET treatment with some subj.ctively reporting improved *** were no maj.r AEs related to the intervention,with the maj.rity of AEs classified as expected events related to COPD ***:It is feasible to implement a manual therapy protocol using MET as an adj.nct to PR in a hospital *** rates were satisfactory and there were no AEs related to the MET component of the intervention.

Osteoporosis in primary biliary cholangitis

Primary biliary cholangitis(PBC) is an autoimmune cholestatic liver disease with multiple debilitating complications. Osteoporosis is a common complication of PBC resulting in frequent fractures and leading to significant morbidity in this population, yet evidence for effective therapy is lacking. We sought to summarize our current understanding of the pathophysiology of osteoporosis in PBC, as well as current and emerging therapies in order to guide future research directions. A complete search with a comprehensive literature review was performed with studies from Pub Med, EMBASE, Web of Science, Cochrane database, and the Countway Library. Osteoporosis in PBC is driven primarily by decreased bone formation, which differs from the increased bone resorption seen in postmenopausal osteoporosis. Despite this fundamental difference, current treatment recommendations are based primarily on experience with postmenopausal osteoporosis. Trials specific to PBC-related osteoporosis are small and have not consistently demonstrated a benefit in this population. As it stands, prevention of osteoporosis in PBC relies on the mitigation of risk factors such as smoking and alcohol use, as well as encouraging a healthy diet and weight-bearing exercise. The primary medical intervention for the treatment of osteoporosis in PBC remains bisphosphonates though a benefit in terms of fracture reduction has never been shown. This review outlines what is known regarding the pathogenesis of bone disease in PBC and summarizes current and emerging therapies.
