Differences in phenotype,homing properties and suppressive activities of regulatory T cells induced by epicutaneous,oral or sublingual immunotherapy in mice sensitized to peanut

Allergen-specific immunotherapy has been proposed as an attractive strategy to actively treat food allergy using the following three different immunotherapy routes:oral(OIT),sublingual(SLIT)and epicutaneous(EPIT)immunotherapy.Regulatory T cells(Tregs)have been shown to have a pivotal role in the mechanisms of immunotherapy.The aim of this study was to compare the phenotype and function of Tregs induced in peanut-sensitized BALB/c mice using these three routes of treatment.We show that although EPIT,OIT and SLIT were all able to effectively desensitize peanut-sensitized mice,they induced different subsets of Tregs.Foxp3+Tregs were induced by the three treatment routes but with greater numbers induced by EPIT.EPIT and OIT also increased the level of LAP+Tregs,whereas SLIT induced IL-10+cells.The suppressive activity of EPIT-induced Tregs did not depend on IL-10 but required CTLA-4,whereas OIT acted through both mechanisms and SLIT was strictly dependent on IL-10.Moreover,the three routes influenced the homing properties of induced Tregs differently,with a larger repertoire of chemokine receptors expressed by EPIT-induced Tregs compared with OIT-and SLIT-induced cells,resulting in different protective consequences against allergen exposure.Furthermore,whereas OIT-or SLIT-induced Tregs lost their suppressive activities after treatment was discontinued,the suppressive activities of EPIT-induced Tregs were still effective 8 weeks after the end of treatment,suggesting the induction of a more long-lasting tolerance.In summary,EPIT,OIT and SLIT mediated desensitization through the induction of different subsets of Tregs,leading to important differences in the subsequent protection against allergen exposure and the possible induction of tolerance.

Lifestyle factors, such as weight and nutritional status may affect male fertility, including sperm fertilization ability. The objective of this retrospective study was to evaluate the association between body mass index （BMI） and sperm-zona pellucida binding ability assessed according to the zona binding （ZB） test, which has been described to be a relevant diagnostic tool for the prediction of in vitro fertilization （IVF） ability. Three hundred and six male patients from couples diagnosed with primary idiopathic or mild male factor infertility were included. Correlations between BMI and semen parameters according to ZB test indices were assessed, together with frequencies of positive and negative tests across the BMI categories. In this selected population, BMI was not related to conventional semen parameters or sperm quality assessed according to the ability of spermatozoa to bind to the zona pellucida. The previously described poor outcomes of IVF procedures in cases of male obesity could be due to other sperm defects, such as alterations of sperm capacitation or acrosome reaction. The link between male BMI and biological outcomes during IVF procedures, such as fertilization rates, should be further evaluated.

Almitrine for COVID-19 critically ill patients – a vascular therapy for a pulmonary vascular disease: Three case reports

BACKGROUND Several reports with clinical,histological and imaging data have observed the involvement of lung vascular function to explain the severe hypoxemia in coronavirus disease 2019(COVID-19)patients.It has been hypothesized that an increased pulmonary blood flow associated with an impairment of hypoxic pulmonary vasoconstriction is responsible for an intrapulmonary shunt.COVID-19 may lead to refractory hypoxemia(PaO_(2)/FiO_(2) ratio below 100 mmHg)despite mechanical ventilation and prone positioning.We hypothesized that the use of a pulmonary vasoconstrictor may help decrease the shunt and thus enhance oxygenation.CASE SUMMARY We report our experience with three patients with refractory hypoxemia treated with almitrine to enhance oxygenation.Low dose almitrine(Vectarion®;Servier,Suresnes,France)was started at an infusion rate of 4μg×kg/min on a central line.The PaO_(2)/FiO_(2) ratio and total respiratory system compliance during almitrine infusion were measured.For the three patients,the PaO_(2)/FiO_(2) ratio time-course showed a dramatic increase whereas total respiratory system compliance was unchanged.The three patients were discharged from the intensive care unit.The intensive care unit length of stay for patient 1,patient 2 and patient 3 was 30 d,32 d and 31 d,respectively.Weaning from mechanical ventilation was performed 13 d,18 d and 15 d after almitrine infusion for patient 1,2 and 3,respectively.We found no deleterious effects on the right ventricular function,which was similar to previous studies on almitrine safety.CONCLUSION Almitrine may be effective and safe to enhance oxygenation in coronavirus disease 2019 patients.Further controlled studies are required.
