Diagnosis and Objective Pain Assessment of Traditional Chinese Medicine May Be Useful to Demonstrate Specific Effects of Acupuncture in Low Back Pain: A Prospective, Randomized, Controlled and Single Blinded Pre-study

Objective: To prove specific effects of acupuncture on chronic pain. It was speculated that the potential specific effectiveness of acupuncture could be better shown in more properly designed studies. Therefore, diagnoses of both Western medicine and traditional Chinese medicine (TCM) were used as inclusion criteria to allocate acupoints more precisely to the complaints of the patients. Secondly, objectively measurable parameters of pain relief in addition to usual Visual Analogue Scale (VAS) were chosen to quantify the effects of acupuncture. Methods: The study was prospective, randomized, controlled and single-blinded. Eighteen patients with chronic back pain and TCM diagnosis of a Taiyang/Yangming syndrome received one single session of acupuncture chosen according to TCM diagnosis or acupuncture on points outside the meridian system (controls). We evaluated pain via VAS and increased mobility via inclinometry of the back. Results: Although the sample size was small, there was a statistically significant improvement in pain and mobility in the intervention group (n=13), but not in the control group (n=5). The calculated sample size adequate power was lower for inclinometry than for VAS, indicating that VAS is less probable to discriminate acupuncture effects. Conclusion: Objectively measurable physical parameters such as the angle of flexion before and after acupuncture (inclinometry) may be more suitable to measure pain relief than subjective assessment by VAS in acupuncture studies. TCM diagnosis may be a helpful inclusion criterion in studies on acupuncture, so as to potentially allocate interventions better to the complaints of patients.

This paper presents a digital model for the powder metallurgical(PM)production chain of high-performance sintered gears based on an integrated computational materials engineering(ICME)*** and finite element methods(DEM and FEM)were combined to describe the macroscopic material response to the thermomechanical loads and process conditions during the entire production *** microstructural evolution during the sintering process was predicted on the meso-scale using a Monte-Carlo *** effective elastic properties were determined by a homogenization method based on modelling a representative volume element(RVE).The results were subsequently used for the FE modelling of the heat treatment *** the development of multi-scale models,it was possible obtain characteristics of the microstructural *** predicted hardness and residual stress distributions allowed the calculation of the tooth root load bearing capacity of the heat-treated sintered gears.

Vertical power flow forecast with LSTMs using regular training updatestrategies

The strong growth of renewable energy sources as well as the increasing amount of volatile energy consumptionis leading to major challenges in the electrical grid. In order to ensure safety and reliability in the electricitygrid, a high quality of power flow forecasts for the next few hours are needed. In this paper we investigateforecasts of the vertical power flow at transformer between the medium and high voltage grid. Forecastingthe vertical power flow is challenging due to constantly changing characteristics of the power flow at thetransformer. We present a novel approach to deal with these challenges. For the multi step time series forecastsa Long-Short Term Memory (LSTM) is used. In our presented approach an update process where the model isretrained regularly is investigated and compared to baseline models. The model is retrained as soon as asufficient amount of new measurements are available. We show that this retraining mostly captures changesin the characteristic of the transformer that the model has not yet seen in the past and therefore cannot bepredicted by the model without an update process. To give more weight to recent data, we examined differentstrategies in terms of the number of epochs and the learning rate. We show that our new approach significantlyoutperforms the investigated baseline models. On average, we achieved an improvement of about 8% with theregular update process compared to the approach without update process.

Glycolytic Shunts Replenish the Calvin-Benson-Bassham Cycle as Anaplerotic Reactions in Cyanobacteria

The recent discovery of the Entner-Doudoroff(ED)pathway as a third glycolytic route beside Embden-Meyerhof-Parnas(EMP)and oxidative pentose phosphate(OPP)pathway in oxygenic photoautotrophs requires a revision of their central carbohydrate *** this study,unexpectedly,we observed that deletion of the ED pathway alone,and even more pronounced in combination with other glycolytic routes,diminished photoautotrophic growth in continuous light in the cyanobacterium Synechocystis *** ***,we found that the ED pathway is required for optimal glycogen catabolism in parallel to an operating Calvin-Benson-Bassham(CBB)*** is counter-intuitive that glycolytic routes,which are a reverse to the CBB cycle and do not provide any additional biosynthetic intermediates,are important under photoautotrophic ***,observations on the ability to reactivate an arrested CBB cycle revealed that they form glycolytic shunts that tap the cellular carbohydrate reservoir to replenish the *** together,our results suggest that the classical view of the CBB cycle as an autocatalytic,completely autonomous cycle that exclusively relies on its own enzymes and C02 fixation to regenerate ribulose-1,5-bisphosphate for Rubisco is an *** propose that in common with other known autocatalytic cycles,the CBB cycle likewise relies on anaplerotic reactions to compensate for the depletion of intermediates,particularly in transition states and under fluctuating light conditions that are common in nature.

Predictors of long-term outcome in patients with biopsy proven inflammatory cardiomyopathy

Background The objective of this study was to identify prognostic indicators in patients with inflammatory cardiomyopathy （iCM） on endomyocardial biopsy （EMB）. Methods and results Between 2007 and 2011 all consecutive patients with diagnosed with iCM at EMB were retrospectively analyzed. The combined primary endpoint （EP）（1E°P） was cardiac death, aborted sudden cardiac death/appropriate implantable cardioverter defibrillator （ICD） shock, progressive heart failure requiring left venWicular assist device （LVAD） implantation and heart transplantation. 503 patients （mean age 58 ° 12 years, 73% male） were available for analysis. Genomes of cardiotrophic viruses were detected in 396 patients （79%） and immuno-histochemical signs of inflammation were present in 223 individuals （44%）. After 3.6 ° 2.4 years of follow-up, cardiac mortality was 3.0% （n = 14） and a total of 8.6% （n = 40） reached the primary endpoint. Independent predictors for the 1 °EP were： age 〉 50 years, presence and duration （〈 28 days） of symptomatic heart failure. A risk stratification approach based on the results of the multivariate analysis demonstrated that absence of signs and/or symptoms of congestive heart failure in younger （〈 50 years） patients with longer （〉 28 days） duration of disease appear to have an excellent prognosis with 100% survival and no events during follow-up The presence of all above mentioned independent risk factors results in an 1°EP occurrence of 35.9%. Conclusions Symptoms of heart fail- ure, short duration of disease, and older age are indicators of poor outcome in patients with iCM.

目的:本研究探讨通过测量微循环相关指标客观衡量针刺效果的可能性,中医寒证针刺治疗前毛细血管灌注状态的意义,以及微循环相关指标在未来中医诊断中的意义。方法:本研究为前瞻性、非对照、不设盲临床研究。研究共纳入32例股骨骨折手术后的老年患者。这些患者均接受施于胃经梁丘穴的"豹斑点刺"针刺治疗。针刺治疗前后使用白光光谱学及激光多普勒测量微循环相关指标如血流速率、血流量、血红蛋白含量、氧饱和度等。结果:股骨骨折后,根据血流量的不同,患者被分为两组,即高灌注组和低灌注组。低灌注组的毛细血管血流量及血流速率在针刺治疗后明显增加,而高灌注组则无明显改变。如果不考虑患者在针刺治疗前的灌注状态,则统计分析表明所有患者针刺治疗前后的灌注状态均无明显改变,因此针刺的作用可能被患者的异质性所掩盖。结论:微循环相关指标对于客观衡量针刺的治疗效果及描述针刺治疗前的营养性功能以均衡比较组之间的差异有重要意义。在本试验中,局部寒证(腿部的毛细血管低灌注)可以通过针刺补气血的胃经梁丘穴得到显著改善,而在热证组(毛细血管高灌注)未见明显改变。未来的研究可以通过将中医营养参数测量的营养状态与中医诊断中的主要证候相联系而得到更多结论。

Three-dimensional visualization of planta clathrin-coated vesicles at ultrastructural resolution

Biological systems are the sum of their dynamic three-dimensional(3D)***,it is critical to study biological structures in 3D and at high resolution to gain insights into their physiological *** microscopy of metal replicas of unroofed cells and isolated organelles has been a key technique to visualize intracellular structures at nanometer ***,many of these methods require specialized equipment and personnel to complete ***,we present novel accessible methods to analyze biological structures in unroofed cells and biochemically isolated organelles in 3D and at nanometer resolution,focusing on Arabidopsis clathrin-coated vesicles(CCVs).While CCVs are essential trafficking organelles,their detailed structural information is lacking due to their poor preservation when observed via classical electron microscopy protocols ***,we establish a method to visualize CCVs in unroofed cells using scanning transmission electron microscopy tomography,providing sufficient resolution to define the clathrin coat ***,the samples are prepared directly on electron microscopy grids,removing the requirement to use extremely corrosive acids,thereby enabling the use of this method in any electron microscopy ***,we demonstrate that this standardized sample preparation allows the direct comparison of isolated CCV samples with those visualized in ***,to facilitate the high-throughput and robust screening of metal replicated samples,we provide a deep learning analysis method to screen the“pseudo 3D”morphologies of CCVs imaged with 2D ***,our work establishes accessible ways to examine the 3D structure of biological samples and provide novel insights into the structure of plant CCVs.

Optical probing of ultrafast laser-induced solid-to-overdense-plasma transitions

Understanding the solid target dynamics resulting from the interaction with an ultrashort laser pulse is a challenging fundamental multi-physics problem involving atomic and solid-state physics,plasma physics,and laser *** of the initial interplay of the underlying processes is essential to many applications ranging from lowpower laser regimes like laser-induced ablation to high-power laser regimes like laser-driven ion *** the properties of the so-called pre-plasma formed as the laser pulse’s rising edge ionizes the target is complicated from the theoretical and experimental point of view,and many aspects of this laser-induced transition from solid to overdense plasma over picosecond timescales are still open *** the one hand,laser-driven ion acceleration requires precise control of the pre-plasma because the efficiency of the acceleration process crucially depends on the target properties at the arrival of the relativistic intensity peak of the *** the other hand,efficient laser ablation requires,for example,preventing the so-called“plasma shielding”.By capturing the dynamics of the initial stage of the interaction,we report on a detailed visualization of the pre-plasma formation and ***-thin diamond-like carbon foils are shown to transition from solid to plasma during the laser rising edge with intensities<10^(16)W/cm^(2).Single-shot near-infrared probe transmission measurements evidence sub-picosecond dynamics of an expanding plasma with densities above 10^(23)cm^(−3)(about 100 times the critical plasma density).The complementarity of a solid-state interaction model and kinetic plasma description provides deep insight into the interplay of initial ionization,collisions,and expansion.

In the phloem cap region o i Arabidopsis plants,sulfur-rich cells(S-cells)accumulate>100 mM glucosinolates(GLS),but are biosynthetically *** source and route of S-cell-bound GLS remain *** this study,using single-cell sampling and scanning electron microscopy with energy-dispersive X-ray analysis we show that two GLS importers,NPF2.10/GTR1 and NPF2.11/GTR2,are critical for GLS accumulation in S-cells,although they are not localized in the *** of GLS levels in S-cells in multiple combinations of homo-and heterografts o lg t r l gtr2,biosynthetic null mutant and wild-type plants indicate that S-cells accumulate GLS via symplasmic connections either directly from neighboring biosynthetic cells or indirectly to non-neighboring cells expressing GTR1/*** sources and transport routes exist for different types of GLS,and vary depending on the position of S-cells in the inflorescence *** on these findings,we propose a model illustrating the GLS transport routes either directly from biosynthetic cells or via GTR-mediated import from apoplastic space radially into a symplasmic domain,wherein the S-cells are the ultimate ***,we observed accumulation of the cyanogenic glucoside defensive compounds in high-turgor cells in the phloem cap of Lotus japonicus,suggesting that storage of defensive compounds in high-turgor cells may be a general mechanism for chemical protection of the phloem cap.
