Introduction: Tra.sition of ca.e stra.egies ha.e shown to improve qua.ity.of lives of hea.t fa.lure pa.ients, but it ha. little implemented in clinica. settings. Objective: To eva.ua.e context-specific perceived ba.riers a.d experiences of hea.t fa.lure pa.ients during their a.mission a.d a.ter they.were discha.ged from hospita.. methods: a.cross-sectiona. qua.ita.ive study.wa. conducted a.ong 13 hea.t fa.lure pa.ients a. two la.ge referra. hospita.s in northwestern Ta.za.ia. In-depth interviews a.ong hea.t fa.lure pa.ients, in line with the Consolida.ed Criteria.for Reporting Qua.ita.ive resea.ch checklist, were used to collect da.a. Interviews were a.dio recorded, tra.scribed, a.d tra.sla.ed into English. Results: Three key.ba.riers were identified, a. well a. possible solutions tha. could improve the tra.sition of ca.e for hea.t fa.lure pa.ients. These include strengthening hea.thca.e provider communica.ions, orga.izing medica.ion ma.a.ement, a.d a.sisting with follow-up a.pointments. Conclusion: The ba.riers identified a.e rea. a.d cha.lenging in clinica. resource- limited settings. Findings suggest they.ca. be overcome when rea.istic a.d ta.lor-ma.e interventions a.e in pla.e.

The pa.er is ba.ed on a.study.whose objective is to provide a. understa.ding of the extent to which tra.itiona. knowledge a.d indigenous institutions for na.ura. resource governa.ce rema.n releva.t to solving current la.d degra.a.ion issues a.d how they.a.e integra.ed in forma. policy.process in kilima.ja.o Region. Da.a.collection for this study.combined qua.ita.ive a.d qua.tita.ive methods. a.tota. of 221 individua.s from households were interviewed using a.structured questionna.re;41 in-depth interviews a.d 24 focus group discussions were held. Findings indica.e tha. the community.a.knowledges tha. there is tra.itiona. knowledge a.d indigenous institutions rega.ding susta.na.le la.d ma.a.ement. However, a.a.eness of the tra.itiona. knowledge a.d pra.tices va.ied between districts. Rura.-ba.ed districts were found to be more a.a.e a.d therefore pra.ticed more of tra.itiona. knowledge tha. urba. ba.ed districts. Va.ia.ions in la.dsca.e fea.ures such a. proneness to drought, la.dslides a.d soil erosion ha.e a.so a.tra.ted va.ia.le responses a.ong the communities rega.ding tra.itiona. knowledge a.d indigenous pra.tices of susta.na.le la.d ma.a.ement. In a.dition, men were found to ha.e more keen interest in conserving the la.d tha. women a. well a. involvement in other tra.itiona. pra.tices of susta.na.le la.d ma.a.ement. This is due to the fa.t tha., customa.ily. it is men who inherit a.d own la.d. This, a.ong other fa.tors, could ha.e limited the integra.ion of tra.itiona. knowledge a.d indigenous institutions in villa.e by.la.s a.d overa.l policy.process. The pa.er concludes by.recommending tha. tra.itiona. knowledge a.d indigenous institutions for susta.na.le la.d ma.a.ement should be promoted a.ong the y.unger genera.ions so a. to ca.ture their interest, a.d ensure tha. successful pra.tices a.e effectively.integra.ed into the na.iona. policies a.d stra.egies.

Specifica.ly.a.grea.er effect wa. seen in migra.ne pa.ients trea.ed by.ma.sa.e with rela.a.ion when compa.ed to a.upuncture. No psy.hologica. fa.tors were found to predict response to either trea.ment. a. the end of the study. 13% of pa.ients were significa.tly.more worried tha. there ma.0}be a.more serious ca.se underly.ng their hea.a.he despite rea.sura.ce a.d a. improvement in their hea.a.he scores.
