AIM： To determine the response of the proxima stomach to small intestinal nutrients in critically ill patients. METHODS： proximal gastric motility was measured in 13 critically ill patients （49.3 ± 4.7 years） and 12 healthy volunteers （27.7 ± 2.9 years） using a barostat technique. Recordings were performed at baseline, during a 60-rain intra-duodenal infusion of Ensure^R （2 kcal/min）, and for 2 h following the infusion. Minimum distending pressure （MDp）, intra-bag volume and fundic wave activity were determined. RESULTS： The MDp was higher in patients （11.7 ± 1.1 vs 7.8 ± 0.7 mmHg; p 〈 0.01）. Baseline intrabag volumes were similar in the 2 groups. In healthy subjects, a ‘bimodal＇ proximal gastric volume response was observed. In patients, the initial increase in proximal gastric volume was small and delayed, but eventually reached a maximal volume similar to that of healthy subjects. In healthy subjects, the proximal gastric volume rapidly returned to baseline level after nutrient infusion （median 18 min）. In contrast, the recovery of volume to baseline was delayed in critically ill patients （median 106 min）. In 6 patients, the volume had not returned to baseline level 2 hours after nutrient infusion. In patients, fundic volume waves were less frequent （p 〈 0.05） and had lower amplitude （p 〈 0.001）, compared to healthy subjects. CONCLUSION： In critical illness, proximal gastric motor responses to small intestinal nutrient stimulation are abnormal.

Employing Geochemical Analysis to Reveal pedogenic processes in Wisconsin Bisequal Soils Having Spodic and Alfic Sequa

Bisequal soils in northern Wisconsin were described and characterized with the intent to determine if a near total elemental analysis protocol may estimate the effect of selected soil pedogenic pathways on soil morphology. The protocol reveals that gross elemental soil profile distributions do provide evidence that certain pedogenic processes are operating within a group of soils. Concentration differences between instrumental neutron activation analysis and an aqua regia digestion protocol infer that alkali metals, alkaline earth metals, transition metals and the rare earth elements are only incompletely weathered from primary minerals. Transition metals, alkali metals, alkaline earths, metalloids, and the rare earth elements all show inter-horizon mobility. Lessivage and the mobility of elements presumably adsorbed on soluble or suspended organic materials are the dominant soil processes influencing the soil profile redistribution of elements and the majority of these elements are shown to correlate with the soil profile distribution of Fe. Base cycling by the forest vegetation was shown to support A horizon enrichment of Ca, Sr, Ba, p, S, Mn, Zn, Sn and pb. For clinical practice, elemental analysis may reveal important soil profile elemental differences when applied to variation of one soil forming factors, such as a chronosequence or toposequence within a landscape or soil association. Advantages and limitations of the protocol in identifying pedogenic pathways are discussed.

为研究碳纤维增强树脂基复合材料（CFRp）层合板的导电性能和电-热耦合效应及其对拉伸和疲劳性能的影响,主要针对2种不同铺层的试件（单向层合板（UD）[0]8和准各向异性层合板（QI）[45/90/-45/0]s）进行导电及电-热耦合实验和通电状态下的拉伸及疲劳实验.在电-热耦合实验中分别对试件通以1～8 A的直流电和交流电,测量其温度场的分布及最高温度,以确定温度对电阻的影响.依据实验现象建立了简化后的电-热耦合分析模型,并与实验结果进行对比,发现两者吻合很好.简化后的电-热耦合模型能够很好地预测试件在达到稳态平衡时的最高温度值.在通电情况下对不同初始状态时的QI试件进行拉伸和疲劳实验,通过实验发现,在疲劳加载过程中无论是通以直流电、交流电或者是改变电流强度和电流频率,对试件的疲劳寿命影响不大,没有发现明显差异,但在实验过程中试件表面的温度会随着电流强度的增加而升高.同时,在监测试件沿着纤维方向的电阻变化时发现,在一定的疲劳载荷下,随着循环次数的增加,试件的电阻会不断增加,这表明试件内部出现了损伤,累积后直至试件断裂.依据实验现象分别建立了电阻模型-1和电阻模型-2,分析计算后发现模型-1能够较好地模拟试件变形的线性阶段（εxx≤0.4％）,而模型-2能较好地模拟非线性阶段直至试件断裂（εxx＞0.4％）.

Aim： To assess the efficacy and safety of tadalafil in comparison to a placebo, when taken on demand for 12 weeks by East/Southeast Asian men with erectile dysfunction （ED）, Methods： This multicenter, randomized, double-blind, parallel group, placebo-controlled study was conducted at 17 centers across East and Southeast Asia between August 2002 and February 2003. Men more than 18 years of age with mild to severe ED of various etiologies were randomized to receive a placebo or 20 mg of tadalafil taken as needed （maximum once daily）. Efficacy assessments included the International Index of Erectile Function, the Sexual Encounter profile diary and Global Assessment Questions. Results： Tadalafil significantly improved erectile function as compared to the placebo （p 〈 0.001）. At the endpoint, the patients receiving 20 mg of tadalafil reported a greater mean per patient percentage of successful intercourse attempts （Sexual Encounter profile question 3： 70.9% compared to 33.5% in the placebo） and a greater proportion of improved erections （Global Assessment Question： 86.2% compared to 30.1%）. Most （≥3%） treatment emergent adverse events were mild or moderate. The most common treatment emergent adverse events were headache, back pain, dizziness and dyspepsia. Conclusion： Tadalafil was an effective and well-tolerated treatment for ED in East and Southeast Asian men.

Effects of Reduced <i>in Utero</i>and post-Weaning Nutrition on Glucose Clearance Measures and Serum Metabolites

Lowering production costs while maintaining an optimal level of production is important to producers in extensive environments. Efforts that strive to match the environment to the nutritional requirements of the grazing animal will substantially improve economic returns to the producer. Therefore, a 3-yr study was implemented that evaluated glucose clearance and serum metabolites in primiparous beef heifers (n = 48;16/yr) from two different feeding regimes. Serum metabolites and glucose clearance measures were analyzed by fitting a repeated measure mixed model using the MIXED procedure of SAS 9.4 (SAS Inst., Inc., Cary, NC). Heifers dams received adequate 1.8 (ADEQ) or marginal 1.2 (MARG) kg/d winter supplementation for approximately 80 d prior to parturition and their heifer calves were then randomly assigned to heifer development treatments that provided ad-libitum (AL) or 80% of ad-libitum (LAL) feed post weaning. Heifers that received the AL treatment during the developmental period were then assigned the 1.8 kg/d winter supplementation for life, whereas heifers that received the LAL treatment during development were assigned the 1.2 kg/d winter supplementation for life. peak glucose concentrations in response to a bolus dose of glucose changed in magnitude between ADEQ and MARG in utero treatments and decreased further removed from parturition (p = 0.05). However, these changes did not manifest along with other glucose clearance measures (p > 0.17) indicating that regardless of in utero treatment or heifer development treatment the reduction in feed input did not greatly influence glucose clearance measures or baseline serum metabolites throughout postpartum, first parity period.

目的发现并描述全科诊疗过程中全科医师与患者在处方决策中的误解。设计本研究为定性研究，纳入了英格兰的中西部和东南部的20所全科医学诊所的20位全科医生和35位患者。主要研究开具处方时，全科医生和患者之间的误解可能导致或确实发生的不良后果。结果误解有14种类型：医生对患者信息不了解、患者对医生信息不了解、信息冲突、对药物不良反应的归因意见不一致、对医生的决策缺乏有效沟通以及其他医患关系因素等。所有这些误解都与患者没有充分表达自己的期望和偏好有关，并且没有对医生的决定做出有效回应。这些误解均导致了潜在的或实际发生了的不良后果，如患者的依从性下降。许多误解都缘于假象或猜测。特别是有些医生似乎没有意识到患者的想法与成功开具处方之间的关系。结论患者的充分参与是非常重要的，参与不足导致的不良结果不容忽视。作者将根据这些结果建立一种教育干预措施来改善这种状况。
