Informal Caregiving and Disaster Risk Reduction:A Scoping Review

Informal caregivers are a population currently in the shadows of disaster risk reduction(DRR),and yet essential to the provision of healthcare *** scoping review explored the literature to understand issues related to informal caregiving and promising practices to support resilience for *** guidelines for scoping review as outlined by Tricco et al.(2016),relevant publications were identified from five major databases—Medline,Embase,PubMed,Web of Science,and *** studies referenced informal caregiving and disasters for a variety of population groups including children,people with disabilities or chronic illnesses,and older *** were excluded if they discussed formal caregiving services(for example,nursing),lacked relevance to disasters,or had insufficient discussion of informal ***,21 articles met the inclusion criteria and were fully *** themes were identified:(1)the need for education and training in DRR;(2)stressors around medication and supply issues;(3)factors affecting the decision-making process in a disaster;(4)barriers leading to disaster-related problems;and(5)factors promoting *** areas of strategic action and knowledge gaps are *** informal caregivers do not feel adequately prepared for *** the important role of informal caregivers in healthcare provision,preparedness strategies are essential to support community resilience for those requiring personal care *** understanding and mobilizing assets to support the resilience of informal caregivers,we also support the resilience of the greater healthcare system and the community,in disaster contexts.

The Sendai Framework for Disaster Risk Reduction 2015–2030 underlines the importance of Science and Technology(S&T) and S&T networks for effective disaster risk reduction(DRR). The knowledge of existing S&T networks and their exact role in DRR,however, is limited. This opinion piece initiates a discussion on the role of S&T networks in the implementation of the Sendai Framework. The article highlights that current practice is oriented towards a narrative that emphasizes the potential of S&T for DRR and stresses a collaborative approach delivered through networks. But a true understanding of whether and how S&T networks can mobilize and enable S&T for DRR is missing. We call for a review of existing S&T networks for DRR and the development of good practice guidelines on S&T networks for DRR. This review should include knowledge on how to overcome common challenges and maximize the benefits, along with a framework for successful evaluation of such *** knowledge would provide much needed guidance for existing and emerging networks.

Determination of selected semi-volatile organic compounds in water using automated online solid-phase extraction with large-volume injection/gas chromatography/mass spectrometry

A rapid,sensitive,and cost-effective analyticalmethod was developed for the analysis of selected semivolatileorganic compounds in *** method used anautomated online solid-phase extraction technique coupledwith programmed-temperature vaporization large-volumeinjection gas chromatography/mass *** samples were extracted by using a fully automatedmobile rack system based on x-y-z robotic techniquesusing syringes and disposable 96-well extraction *** method was validated for the analysis of 30 semivolatileanalytes in drinking water,groundwater,andsurface *** a sample volume of 10 mL,the linearcalibrations ranged from 0.01 or 0.05 to 2.5μg·L^(-1),and themethod detection limits were less than 0.1μg·L^(-1).For thereagent water samples fortified at 1.0μg·L^(-1)and2.0μg·L^(-1),the obtained mean absolute recoveries were70%-130%with relative standard deviations of less than20%for most *** the drinking water,groundwater,and surface water samples fortified at 1.0μg·L^(-1),theobtained mean absolute recoveries were 50%-130%withrelative standard deviations of less than 20%for *** new method demonstrated three advantages:1)no manipulation except the fortification of surrogatestandards prior to extraction;2)significant cost reductionassociated with sample collection,shipping,storage,andpreparation;and 3)reduced exposure to hazardous solventsand other *** a result,this new automatedmethod can be used as an effective approach for screeningand/or compliance monitoring of selected semi-volatileorganic compounds in water.

Liver surgery in cirrhosis and portal hypertension

The prevalence of hepatic cirrhosis in Europe and the United States, currently 250 patients per 100000 inhabitants, is steadily increasing. Thus, we observe a significant increase in patients with cirrhosis and portal hypertension needing liver resections for primary ormetastatic lesions. However, extended liver resections in patients with underlying hepatic cirrhosis and portal hypertension still represent a medical challenge in regard to perioperative morbidity, surgical management and postoperative outcome. The Barcelona Clinic Liver Cancer classification recommends to restrict curative liver resections for hepatocellular carcinoma in cirrhotic patients to early tumor stages in patients with Child A cirrhosis not showing portal hypertension. However, during the last two decades, relevant improvements in preoperative diagnostic, perioperative hepatologic and intensive care management as well as in surgical techniques during hepatic resections have rendered even extended liver resections in higher-degree cirrhotic patients with portal hypertension possible. However, there are few standard indications for hepatic resections in cirrhotic patients and risk stratifications have to be performed in an interdisciplinary setting for each individual patient. We here review the indications, the preoperative risk-stratifications, the morbidity and the mortality of extended resections for primary and metastatic lesions in cirrhotic livers. Furthermore, we provide a review of literature on perioperative management in cirrhotic patients needing extrahepatic abdominal surgery and an overview of surgical options in the treatment of hepatic cirrhosis.

Contrasting oxidation states of low-Ti and high-Ti magmas control Ni-Cu sulfide and Fe-Ti oxide mineralization in Emeishan Large Igneous Province

Magmatic Ni-Cu-(PGE) sulfide and Fe-Ti oxide deposits in plume-related large igneous provinces(LIPs)are commonly related to low-Ti and high-Ti series magmas, respectively, but the major factors that control such a relationship of metallogenic types and magma compositions are unclear. Magma fOcontrols sulfur status and relative timing of Fe-Ti oxide saturation in mafic magmas, which may help clarify this issue. Taking the Emeishan LIP as a case, we calculated the magma fOof the high-Ti and low-Ti picrites based on the olivine-spinel oxygen barometer, and the partitioning of V in olivine. The obtained fOof the high-Ti series magma(FMQ + 1.1 to FMQ + 2.6) is higher than that of the low-Ti series magma(FMQ-0.5to FMQ + 0.5). The magma fOof the high-Ti and low-Ti picrites containing Fo > 90 olivine reveals that the mantle source of the high-Ti series is likely more oxidized than that of the low-Ti series. The results using the ’lambda REE’ approach show that the high-Ti series may have been derived from relatively oxidized mantle with garnet pyroxenite component. The S contents at sulfide saturation(SCSS) of the two series magmas were calculated based on liquid compositions obtained from the alpha Melts modeling, and the results show that the low-Ti series magma could easily attain the sulfide saturation as it has low fOwith S being dominantly as S. In contrast, the oxidized high-Ti series magma is difficult to attain the sulfide saturation, but could crystallize Fe-Ti oxides at magma MgO content of ~7.0 wt.%. Thus, contrasting magma fOof low-Ti and high-Ti series in plume-related LIPs may play an important role in producing two different styles of metallogeny.

Quantification of skid resistance seasonal variation in asphalt pavements

The present study deals with the quantification of skid resistance seasonal variation in asphalt ***,skid resistance data were collected twice per year(after"wet"and"dry"seasons)along four trial sections of an urban highway with a GripTester system(during a six-year field experiment).For the estimation of a percentage reference change(%)between wet and dry skid resistance data,a methodology is developed based on distribution fitting of the percentage change of skid resistance index due to seasonal variation *** outcome of the analysis is applied and successfully validated through a set of skid resistance data collected from an interurban highway pavement section with different traffic and environmental *** such the developed methodology can be considered as a promising tool for accommodating the seasonal variation of skid ***,the resulting percentage reference change(%)enables the estimation of skid resistance level for the dry period based on the wet period ***,it seems that the proposed quantification process may constitute a practical yet efficient tool for road authorities and highway operators on how to estimate the effect of seasonal variation on skid resistance levels and schedule well-ahead necessary actions for enhancing driving safety.

Autonomous vehicles wheel wander: Structural impact on flexible pavements

Autonomous vehicles(AVs), including trucks, are already available and their adoption is coming at a rapid pace. Despite the fact that this new transportation technology will travel on existing pavement infrastructure assets, the study and investigation of the AVs structural impact on pavements has not reached the same level of attention and maturity as other related research areas. Considering the extensive investment on pavement construction and maintenance, the necessity to fully understand these impacts in the long term, is becoming evident. The present paper presents and discusses currently published relevant research and findings on the quantification of the wheel wander potential impact,both negative and positive, offering potential insightful future areas of enquiry to help mold and shape future research for this emerging field. The paper focuses on the impact of the AVs zero lateral wheel wander on both new flexible pavement design and the damage accumulation within existing flexible pavement structures. Other wheel wander distributions(normal, uniform, etc.) are investigated as well. The outcome and findings are that the AVs zero lateral wheel wander has a definable negative structural impact on flexible pavement structures in comparison to current human-driven(non-autonomous) vehicles that tend to follow a normal lateral wheel wander distribution. However, under certain defined conditions(optimal wheel wander distribution), the pavement service life can be potentially extended.

Risk of Cardiovascular Events in Patients with Primary Biliary Cholangitis-Systematic Review

Background and Aims:Hypercholesterolemia is a common finding in patients with primary biliary cholangitis(PBC)and is a well-defined risk factor for cardiovascular ***,studies have been mixed on whether PBC patients do,in fact,have higher cardiovascular *** aim of this study is to review the current literature and provide an evidence-based assessment of cardiovascular risk in PBC ***:We performed a systematic literature search on PubMed re-garding patients with PBC and cardiovascular events from the database inception to july 1,2017.A total of 33 articles ful-filling our inclusion criteria were ***:The majority of the studies evaluated yielded no statistically significant dif-ference in cardiovascular disease in the PBC population com-pared to the general ***,some reports found a statistically significantly increase in coronary artery *** studies have looked at the specific lipid profile of pa-tients with PBC with *** these lipid ab-normalities differ by stage of disease,there is evidence to suggest that the specific lipid profile in PBC may have lower atherogenicity than in patients with hypercholesterolemia without *** looking at patients with PBC with other risk factors for cardiovascular disease,such as hypertension and metabolic syndrome,have consistently found a higher risk for cardiovascular disease in these *** treat-ment is effective in reducing lipid levels and possibly improv-ing endothelial inflammation in patients with PBC with ***:There is not enough evidence to suggest an increased risk of cardiovascular dis-ease in patients with PBC with hypercholesterolemia,except for those individuals with concomitant features of metabolic *** patients with PBC with no additional cardiovas-cular risk factors,individual risk/benefit discussion on lipid-lowering treatment should be considered.

Liver transplantation for malignancy:current treatment strategies and future perspectives

In 1967,Starzl et al performed the first successful liver transplantation for a patient diagnosed with *** the following,liver transplantation was considered ideal for complete tumor resection and potential cure from primary hepatic *** reports of liver transplantation for primary and metastatic liver cancer however showed disappointing results and the strategy was soon ***1996,Mazzaferro et al introduced the Milan criteria,offering liver transplantation to patients diagnosed with limited hepatocellular *** then,liver transplantation for malignant disease is an ongoing subject of preclinical and clinical *** this context,several aspects must be considered:(1)Given the shortage of deceased-donor organs,long-term overall and disease free survival should be comparable with results obtained in patients transplanted for nonmalignant disease;(2)In this regard,living-donor liver transplantation may in selected patients help to solve the ethical dilemma of optimal individual patient treatment vs organ allocation justice;and(3)Ongoing research focusing on perioperative therapy and antiproliferative immunosuppressive regimens may further reduce tumor recurrence in patients transplanted for malignant disease and thus improve overall *** present review gives an overview of current indications and future perspectives of liver transplantation for malignant disease.

Modeling nonstationary extreme wave heights in present and future climates of Greek Seas

In this study the generalized extreme value （GEV） distribution function was used to assess nonstationarity in annual maximum wave heights for selected locations in the Greek Seas, both in the present and future climates. The available significant wave height data were divided into groups corresponding to the present period （1951-2000）, a first future period （2001-2050）, and a second future period （2051-2100）. For each time period, the parameters of the GEV distribution were specified as functions of time-varying covariates and estimated using the conditional density network （CDN）. For each location and selected time period, a total number of 29 linear and nonlinear models were fitted to the wave data, for a given combination of covariates. The covariates used in the GEV-CDN models consisted of wind fields resulting from the Regional Climate Model version 3 （RegCM3） developed by the International Center for Theoretical Physics （ICTP） with a spatial resolution of 10 km ×10 km, after being processed using principal component analysis （PCA）. The results obtained from the best fitted models in the present and future periods for each location were compared, revealing different patterns of relationships between wind components and extreme wave height quantiles in different parts of the Greek Seas and different periods. The analysis demonstrates an increase of extreme wave heights in the first future period as compared with the present period, causing a significant threat to Greek coastal areas in the North Aegean Sea and the Ionian Sea.
