Background and objective: Nasal obstruction is the most frequent presentation to the otolaryngologist all over the world. Symptoms are multisystem involvement such as headache, epistaxis, ophthalmological, otological, upper and lower respiratory tract infections;and many more. Septoplasty is commonly performed for nasal septal deviation as a primary indication of nasal airway obstruction. Physical examination, anterior rhinoscopy and nasal endoscopy are considered the gold standard for detecting septal deviation. Computed tomography (CT) can also be used to evaluate the nasal septum, anatomical variants and unveils the associated sinonasal diseases. This study aimed to find out the advantages of CT in endoscopic septoplasty for all out benefits to the patients. Patients and methods: 168 patients of nasal septal deviation were selected from 2015 to 2018 in the Bangladesh Medical and Popular Medical College Specialized Hospital. Age ranged between 10 years to 70 years of age. CT scanning in both coronal, axial and sagittal sections had been done in all patients after proper history taking, physical examination, anterior rhinoscopy and nasal endoscopy. Endoscopic septoplasty alone and/or other ancillary procedures were performed in all 168 patients under general anesthesia. Results: Amongst 168 patients, male was 116 (69%) and female was 52 (31%) with a ratio 2.2:1. Maximum 58 (35%) reported in 31 to 40 years and 48 (29%) was found in 21 to 31 years of age. Hypertrophied inferior turbinate (HIT) 54 (32%) and 48 (29%) concha bullosa (CB) were associated with 168 septal deviation. Associated pathologies like maxillary antral cyst 68 (40%) and maxillary sinusitis 62 (37%) were documented. Conclusion: CT scanning unveils the abnormal anatomic variants and associated sinonasal pathologies along with nasal septal deviation. Background: By the dawn of this modern era of science, the prime challenge of physician is cardiovascular disease (CVD). The most important modifiable risk factors of CVDs are unhealthy diet, physical inactivity and tobacco use. The effects of unhealthy diet and physical inactivity include abnormal blood lipid, obesity and hypertension. We tried to evaluate and correlate the pattern of lipid profile in obese and non-obese hypertensive patients. Objectives: This study was conducted at medicine department of Cumilla Medical College Hospital. The principal aim was to evaluate the lipid profile in obese and non-obese adult hypertensive patients. Methodology: During this cross sectional analytical study, a total of 100 adult hypertensive patients were taken by purposive sampling. Among them 50 (group 1) patients were taken those were obese and 50 (group 2) patients taken those were non-obese according to BMI measurement on operational definition. Diagnosis of hypertension would be established with the help of ambulatory BP measurements two occasions few minutes apart. The staging of hypertension was done according to JNC7 Criteria. Morning blood samples were taken after 8 - 12 hours of fasting and lipid profiles were done on authentic laboratories. The laboratory values were interpreted according to the operational definition of dyslipidaemia. The ethical research and review committee approved the study protocol and signed informed consent was obtained from the participants. Penetrating injuries to the subclavian arteries as well as post traumatic pseudoaneurysm involving Subclavian artery (SCA) are very much uncommon. We present one case of a 21 year-old male sustained a physical assault with a post stab injury left subclavian artery pseudoaneurysm. He was referred to our facility, two days after sustaining a penetrating chest trauma over left infraclavicular area of the chest. The presentation, diagnostic procedures and surgical approach for management of this very rare injury are discussed.

Evaluation of land-use pattern change in West Bhanugach Reserved Forest,Bangladesh,using remote sensing and GIS techniques

A study was conducted to investigate the land-use pattern change over a period of 18 years （1988-2006） by using remote sens- ing and Geographical Information System （GIS） technologies, in the West Bhanugach Reserved Forest, a hill forest, in Sylhet Forest Divi- sion of Bangladesh, The images were processed using ERDAS Imagine software. Both supervised and unsupervised approaches were ap- plied and ground control points were collected using a GPS. Maps were prepared using GIS software. Results showed that vegetation cover drastically decreased from the year 1988 to 1996 （l 826 ha to l 714.85 ha）, but increased gradually from the year 1996 to 2006 （l 714.85 ha to l 847.83 ha） due to the initiation of co-management practice involving local communities. Change in bare land was inversely proportionate to the amount of vegetation cover changes unless any other land-uses were converted into bare land. The area of water bodies increased from the year 1988 to 1996 （307.67 ha to 379.53 ha）, but decreased from the year 1996 to 1997, then remained invariabile from the year 1997 to 2006. Some recommendations were also made for applying the RS and GIS techniques to study the land-use pattern change in the Bhanugach Reserved Forest and to create a GIS data base for the study area.

The use of bilateral internal thoracic arteries to treat coronary artery disease is very less despite of improved long-term survival. In this case report, a patient presented with TVD has been managed with anaortic off-pump complete arterial revascularization (OPCABG) by using composite LIMA RIMA Y Sequential 5 grafts to achieve complete arterial revascularization successfully. This type of operative procedure is technically difficult in a resource poor country like Bangladesh. However, this has been done by a group of young cardiac surgeon. During follow up, the patient is in a good condition.

Antidepressant,anxiolytic and anti-nociceptive activities of ethanol extract of Steudnera colocasiifolia *** leaves in mice model

Objective:To estimate the antidepressant,anxiolytic and antinociceptive activities of ethanol extract of Steudnera colocasiifolia ***(***)***:Swiss albino mice treated with 1%Tween solution,standard drugs and ethanol extract of ***,respectively,were subjected to the neurological and antinociceptive *** tail suspension test and forced swimming test were used for testing antidepressant activity,where the parameter is the measurement of immobility *** activity was evaluated by hole board ***-nociceptive potential of the extract was also screened for centrally acting analgesic activity by using formalin induced licking response model and acetic acid induced writhing test was used for testing peripheral analgesic ***:Ethanol extract of *** significantly decreased the period of immobility in both tested models(tail suspension and forced swimming models)of antidepressant *** the hole board model,there was a dose dependant(at 100 and 200 mg/kg)and a significant increase in the number of head dipping by comparing with control(1%Tween solution)(P<0.05 and P<0.001).In formalin induced licking model,a significant inhibition of pain compared to standard diclofenac sodium was observed(P<0.05 and P<0.001).In acetic acid induced test,there was a significant reduction of writhing response and pain in mice treated with leaves extract of *** at 200 mg/kg body weight(P<0.05 and P<0.001).Conclusions:The results proofed the prospective antidepressant,anxiolytic and antinociceptive activities of ethanol extract of *** leaves.

Surgical Management for a Broken Delivery Shaft in <i>Vivo</i>during PCI Following Acute Myocardial Infarction in a Complex LAD Artery Lesion: A Case Report and Literature Review

Stuck up and fracture of coronary angioplasty hardware are unwonted complications of percutaneous coronary interventions (PCI) for which surgical retrieval and management is once in a while required. We present one case of a 59-year-old diabetic, a hypertensive gentleman who attended the emergency room (ER) with central chest pain for 2 hours. After evaluation of the patient by physical and noninvasive diagnostic tests, he was diagnosed as a case of acute myocardial infarction (AMI) with stable hemodynamic. For the aforementioned findings, the patient was treated first with a thrombolytic agent in ER and then shifted to Cardiology Department for monitoring and further coronary evaluation by coronary angiography (CAG). CAG revealed essentially single vessel disease (SVD) with complex left anterior descending (LAD) artery lesion, so PCI was attempted but failed with an unfortunate incidence of the broken delivery shaft and left in vivo. Immediate decision making and surgical management for retrieval of lost angioplasty device and correction coronary lesion with revascularization save the patient from grave complication. All the series of events and management approaches of this very complex coronary artery lesion are discussed in this article.

Yield and Profit from New and Old Wheat Varieties Using Certified and Farmer-Saved Seeds

Wheat is a strategic crop for food security in Pakistan with predominance of smallholder farmers. Farmer-saved seed （FSS） is used nearly by 75% of farmers for wheat production. Frequent variety or seed replacement is uncommon even among large-scale farmers, and certified seed （CS） is mostly bought to verify the value of new varieties. Replacing old and obsolete varieties by new high yielding disease resistant varieties is key to transferring new genetic gains to farmers. For the first time in Pakistan, on-farm factorial experiments involving seven new and five old wheat varieties and their corresponding CS and FSS were conducted. A total of 49 farmers representing major wheat cropping patterns throughout the country participated in these trials in the 2014 and 2015 wheat growing seasons. Analysis of variance revealed that there was highly significant difference between wheat varieties and between seed classes. New variety ＋ CS gave 33.8% more grain yield as well as higher marginal return over farmers＇ variety ＋ FSS. Grain yield and returns from new variety ＋ FSS and farmers＇ variety ＋ CS were at par. Scientific knowledge generated in this research demonstrated that use of CS ＆new wheat varieties is best option, while growing new varieties with FSS is a second choice for the advantages accruing from their built-in genetic traits of economic importance over growing CS of old and obsolete varieties. Having more flexible seed system will help accelerate the delivery of new genetic gains to farmers＇ fields.

Background: Emergence agitation (EA) is a common phenomenon observed in pediatric patients following general anesthesia. This study aimed to assess the efficacy of propofol and fentanyl in preventing EA and to compare their associated complications or side effects. Methods: This prospective randomized observational comparative study was conducted at Dhaka Medical College Hospital from July 2013 to June 2014. The study aimed to evaluate the effects of propofol and fentanyl on EA in children aged 18 to 72 months undergoing circumcision, herniotomy, and polypectomy operations. Ninety children were included in the study, with 45 in each group. Patients with psychological or neurological disorders were excluded. Various parameters including age, sex, weight, American Society of Anesthesiologists (ASA) class, duration of anesthesia, Saturation of Peripheral Oxygen (SPO2), heart rate (HR), respiratory rate (RR), Pediatric Anesthesia Emergence Delirium (PAED) score, duration of post-anesthesia care unit (PACU) stay, incidence of laryngospasm, nausea, vomiting, and rescue drug requirement were compared between the two groups. Results: Age, sex, weight, ASA class, and duration of anesthesia were comparable between the two groups. Perioperative SpO2 and HR were similar in both groups. However, the PAED score was significantly higher in the fentanyl group during all follow-ups except at 30 minutes postoperatively. The mean duration of PACU stay was significantly longer in the fentanyl group. Although the incidence of laryngospasm was higher in the fentanyl group, it was not statistically significant. Conversely, nausea or vomiting was significantly higher in the fentanyl group. The requirement for rescue drugs was significantly higher in the fentanyl group compared to the propofol group. Conclusion: Both propofol and fentanyl were effective in preventing emergence agitation in pediatric patients undergoing various surgical procedures under sevoflurane anesthesia. However, propo

A new N-acetylsulfanilylation series of uridine have been synthesized in good yield using direct acylation method and afforded the 5’-O-N-acetylsulfanilyluridine. In order to obtain newer products, the 5’-O-N-acetylsulfanilyluridine derivative was further transformed to a series of 2’,3’-di-O-acyl derivatives containing a wide variety of functionalities in a single molecular framework. The chemical structures of the newly synthesized compounds were confirmed on the basis of their FTIR, 1H-NMR spectroscopy, physicochemical properties and elemental analysis. All the synthesized uridine derivatives were tested for their in vitro antibacterial activity against six human pathogenic bacterial strains and for comparison standard antibiotic Ampicillin was also determined. The study revealed that the selectively acylated deriva-tives 5’-O-N-acetylsulfanilyl-2’,3’-di-O-lauroyluridine and 5’-O-N-acetylsulfanilyl-2’,3’-di-O-pivaloyluridine showed highest inhibition against Staphylococcus aureus and Bacillus cereus, respectively. We also observed that the introduction of hexanoyl, decanoyl, lauroyl, myristoyl and pivaloyl groups, the antibacterial functionality of the compound uridine increases. Another noteworthy observation was that the uridine derivatives were found comparatively more effective against Gram-positive microorganisms than those of Gram-negative microorganisms. In addition, the test chemicals were also tested for cyto-toxicity by brine shrimp lethality bioassay and compounds showed different rate mortality with different concentrations.
