Nonalcoholic steatohepatitis in nonalcoholic fatty liver disease patients of Bangladesh

AIM: To explore the prevalence and risk factors for nonalcoholic steatohepatitis (NASH) in nonalcoholic fatty liver disease (NAFLD) patients. METHODS: We have included 493 patients with sonographic evidence of a fatty change, and 177 of these individuals were evaluated and confirmed after liver biopsy. The exclusion criteria consisted of significant alcohol abuse (< 20 g daily), evidence of hepatitis B and C, evidence of drug-induced fatty liver disease and other specific liver diseases such as hemochromatosis, Wilson’s disease or autoimmune liver disease. The patients were assessed for metabolic syndrome, and biochemical, anthropometric and histopathological evaluations were carried out. The degree of disease activity in the NAFLD patients was evaluated using the NAFLD Activity Score. The data were analyzed by SPSS, version 16.0. RESULTS: Females predominated among the study participants (250, 57.0%), and the mean age was 40.8 ± 10.2 years. The numbers of overweight, obeseⅠ and obese Ⅱ patients were 58 (13.2%), 237 (53.9%) and 93 (21.2%), respectively. However, there were 422 (96.2%) centrally obese patients. NASH was absent in 10 (5.6%) cases, borderline in 92 (52.6%) cases and present in 75 (42.4%) cases. The presence of diabetes could significantly (P = 0.001) differentiate NASH from simple steatosis. The following parameters did not influence the development of NASH: age, sex, basal metabolic index, waist circumference, serum high-density lipoprotein, triglyceride, insulin resistance index, hypertension and metabolic syndrome. The serum gammaglutamyl transpeptidase (GGT) level was significantly higher (P = 0.05, 51.7 ± 32.8 and 40.4 ± 22.6 U/L) in the NASH patients, with a sensitivity of 45% and a specificity of only 68%. The serum alanine aminotransferase and aspartate aminotransferase levels were not able to predict NASH. CONCLUSION: Females were the predominant sufferers of NAFLD in Bangladesh. Dear Editor,Arboviruses of medical importance are maintained in nature in enzootic cycles between haematogenous vectors and susceptible vertebrate hosts(Huang et al.,2019).The rapid increase in human populations around the globe and the associated urbanization are creating irreversible damage to the ecosystem,giving rise to many problems including emergence and intensification of the vector borne diseases(Sutherst,2004).Among vector borne diseases,mosquito-borne arboviruses including dengue virus(DENV),West Nile virus(WNV),Japanese Encephalitis virus(JEV)and Zika virus(ZIKV)are rapidly emerging in the affected regions of the world(Palmer et al.,2011).

Trends and Spatial Heterogeneity of Climate Extremes in the Southwestern Region of Bangladesh

This study attempts to investigate the climatic variability in the southwestern area of Bangladesh focusing on temperature and precipitation extremes. Long-term daily records of temperature and precipitation from six weather stations spread over the study area were analyzed. A total of 20 yearly indices were calculated and testified for the trends with the Mann–Kendal test and Sen’s estimator of slope at a 95% confidence interval. Moreover, one-way ANOVA and Tukey–Kramer post-hoc tests were applied to verify the spatial heterogeneity of the climatic indices among the meteorological stations. Findings revealed that Khulna is the worst affected area with significant changes in 12 climatic indices. Jessore and Faridpur followed Khulna with significant changes in10 and 9 indices, respectively. Among the indices, significant changes in TN10p(frequencies of daily minimum temperature event that falls below the 10th percentile value), TX10p(frequencies of daily maximum temperature event that falls beyond the 10th percentile value), CSDI(cold spell duration indicator), and CDD(consecutive dry day) are the most common phenomenon in the studied region. Five out of six stations have been experiencing significant increasing trends for TN10p. That indicates an overall increase of extreme cold day frequencies in the area. Khulna station showed shreds of evidence of extreme cold temperature instances with a statistically significant fall in the minimum value of the yearly minimum temperatures(TNn). Besides,Khulna also experienced a statistically significant fall in the minimum value of the yearly maximum temperatures. Consistently, the area showed a significant trend of wider cold ***, the extremely warm day frequencies have also been increasing significantly for the Khulna, Jessore, and Faridpur areas. Application of GEE Models for Assessing Maternal Health Complications

Bangladesh, a developing country, gained success towards the fifth-millennium development goals target of reducing its maternal mortality ratio by three quarters by 2015, but yet worked more on it for further reduction of maternal mortality. In this light, though Bangladesh is committed to the sustainable development goals target of reducing its maternal mortality ratio to be reduced from 170 to 105 per 100,000 live births, the scope of research on this issue is limited because the maternal morbidity data is scarce in Bangladesh. In this paper, the prospective data on maternal morbidity in rural Bangladesh (collected by BIRPERHT) have been employed to trace out the high-risk and life-threatening factors associated with pregnancy-related complications. The subject-specific generalized estimating equations (SS-GEE) model with random effect structure is used for multivariate binary data for the repeated observations. The findings indicate that the risk of suffering from pregnancy complications is higher for high economic status, lower age at marriage, not visited for medical check-ups, outside home workers, and having miscarriage or abortion. Comparing the SS-GEE model with other correlation structures and relative efficiency factors, the SS-GEE model with random effect structure is well fitted for the prospective repeated observation data.

Social Business in Bangladesh: A Study on Grameen Danone Shokti+ Yogurt

Social business is becoming popular worldwide as a sustainable and innovative solution to tackling pressing social problems. The Grameen Danone Food Ltd. (GDFL) Shokti+ yogurt business was chosen as a social business case study. The study described GDFL Shokti+ yogurt production, pricing, and distribution process, elucidated consumers’ perception of the Shokti+ yogurt, identified the major problems faced by the consumers, and proposed probable solutions to popularize the yogurt among the consumers. The study’s objectives were achieved using descriptive statistics based on a quantitative survey of 60 consumers. The study found that Shokti+ yogurt is distributed primarily through four channels: modern model, urban trade, rural trade, and direct trade. The study revealed that non-poor households consume more Shokti+ yogurt than poor households. The findings showed that about 78% and 95% of respondents were satisfied with the yogurt quality and availability, respectively. About 17% of respondents perceived that Shokti+ yogurt is superior to regular yogurt. However, about 58% of respondents claimed that the price is higher than the amount and quality offered. Excessive liquidity, increased price, and lack of trust in the nutritional quality of yogurt were some major problems identified by the consumers. The study proposed certain recommendations such as maintaining the consistency of the yogurt, increasing the amount of yogurt sold, making the yogurt more affordable for poor consumers, and raising awareness about its nutritional benefits for popularizing Shokti+ yogurt in Bangladesh.

Low Ca^2＋ Content and Ca^2＋/Na^＋ Ratio in Leaf Tissues Determine Salinity Tolerance in Spanish Type Groundnut （Arachis hypogeal L.）

The study was performed with seven groundnut varieties/genotypes and F1s derived from crossing in all possible combinations without reciprocal among the mentioned varieties/genotypes. The objective was to assess whether low Ca2＋ content and Ca2＋/Na＋ ratio of leaf tissue or stem tissue determine salinity tolerance in terms of economic yield （kernel yield） in groundnut. It revealed that the varieties, “Binachinabadam-6”, “Binachinabadam-5” and the F1 G2 × G3 were most tolerant based on kernel yield under 8 dS/m and 10 dS/m salinity stresses. These two tolerant varieties and the F1 also showed lower Ca2＋ and Ca2＋/Na＋ ratios in leaf tissue, which indicated lower Ca2＋ and Ca2＋/Na＋ ratio of leaf tissue determined salinity tolerance in terms of kernel yield in Spanish type groundnut. These findings could be applied in future plant breeding applications for screening salt tolerant Spanish type groundnut genotypes.
