Minimizing the risk of community spread of COVID-19 via institutional quarantine of high-risk travelers with serial viral RNA testing: A successful experience from Macao SAR, China

Macao,a special administrative region(SAR)of the People’s Republic of China,is located in southern China and shares the border with China's Mainland.It is the most densely populated region in the world,with a population of 667400 and a total land area of 32.9 square kilometers in 2019.Since the first case diagnosed on January 22,2020,there was a total of 45 laboratory-confirmed coronavirus disease 2019(COVID-19)cases in Macao,of which 43 patients(96%)were imported cases.To date,all patients had been discharged successfully from Centro Hospitalar Conde de São Januário,a designated hospital to manage all COVID-19 patients in Macao.Eventually,no patient died,and no local community outbreak was noted.This opinion review describes the underlying factors that could have contributed to the successful experience in Macao SAR,China,which include the following:(1)Early implementation of containment measures;(2)Large-scale quarantine using hotel rooms to reduce the risk of a local outbreak;and(3)Multidisciplinary co-operation and transparency of information to the public.Although the successful experience in Macao SAR,China,may not be generalized to other regions,it should not be unreasonable to be well prepared with sufficient logistic support to conduct timely containment and early detection of episodic cases to prevent the backsliding of COVID-19 outbreak.

Background and Objective:Hong Kong,like many parts of Asia,faces a high burden of hepatocellular carcinoma(HCC)caused by high endemic rates of hepatitis B virus infection.Hong Kong clinicians have developed a high level of expertise in HCC treatment across surgical,transarterial,ablative,radiotherapeutic and systemic modalities.This publication summarizes the latest evidence-based recommendations on how these modalities should be used.Methods:In two meetings held in 2020,a multidisciplinary panel of surgeons,oncologists and interventional radiologists performed a narrative review of evidence on the management of HCC,with an emphasis on treatment of HCC not amenable to surgical resection.Close attention was paid to new evidence published since the previous version of these statements in 2018.Key Content and Findings:The expert panel has formulated 60 consensus statements to guide the staging and treatment of unresectable HCC.Since the previous version of these statements,considerable additions have been made to the recommendations on use of targeted therapies and immunotherapies because of the large volume of new evidence.Conclusions:Our consensus statements offer guidance on how to select HCC patients for surgical or non-surgical treatment and for choosing among non-surgical modalities for patients who are not candidates for resection.In particular,there is a need for more evidence to aid physicians in the selection of second-line systemic therapies,as currently most data are limited to patients with disease progression on first-line sorafenib.

Background: Transarterial chemoembolization(TACE) is the most commonly used adjuvant therapy for hepatocellular carcinoma(HCC) after curative resection. Responses to TACE are variable due to tumor and patient heterogeneity. We had previously demonstrated that expression of Granulin-epithelin precursor(GEP) and ATP-dependent binding cassette(ABC)B5 in liver cancer stem cells was associated with chemoresistance. The present study aimed to evaluate the association between GEP/ABCB5 expression and response to adjuvant TACE after curative resection for HCC. Methods: Patients received adjuvant TACE after curative resection for HCC and patients received curative resection alone were identified from a prospectively collected database. Clinical samples were retrieved for biomarker analysis. Patients were categorized into 3 risk groups according to their GEP/ABCB5 status for survival analysis: low(GEP-/ABCB5-), intermediate(either GEP +/ABCB5-or GEP-/ABCB5 +) and high(GEP +/ABCB5 +). Early recurrence(recurrence within 2 years after resection) and disease-free survival were analyzed. Results: Clinical samples from 44 patients who had followed-up for more than 2 years were retrieved for further biomarker analysis. Among them, 18 received adjuvant TACE and 26 received surgery alone. Patients with adjuvant TACE in the intermediate risk group was associated with significantly better overall survival and 2-year disease-free survival than those who had surgery alone( P = 0.036 and P = 0.011, respectively). Adjuvant TACE did not offer any significant differences in the early recurrence rate, 2-year disease-free survival and overall survival for patients in low and high risk groups. Conclusions: Adjuvant TACE can only provide survival benefits for patients in the intermediate risk group(either GEP +/ABCB5-or GEP-/ABCB5 +). A larger clinical study is warranted to confirm its role in patient selection for adjuvant TACE.

Fusarium wilt,a disease caused by Fusarium oxysporum f.sp batatas(Fob)is an important disease in sweet potato production.Using endophytic bacteria for biological control of sweet potato diseases is one of the important ways.A Bacillus subtilis with antagonistic effect on Fusarium wilt of sweet potato was isolated from soil by confrontation culture.According to the biological characteristics,16S rDNA sequence analysis,and physiological and biochemical analysis,the Bacillus subtilis HAAS01 was named.A pot experiment was conducted for the biological control experiment of strain HAAS01,and the endogenous hormone content,antioxidant enzyme activity,soluble protein content,and related gene expressions of sweet potato plants were detected.The results showed that the HAAS01 strain could promote the production of endogenous hormones and resist the infection of plant diseases together with defensive enzymes and upregulation of related gene expressions.In summary,Bacillus subtilis HAAS01 was effective in controlling Fusarium wilt of sweet potato and has potential for application and development.

APPL2 Negatively Regulates Olfactory Functions by Switching Fate Commitments of Neural Stem Cells in Adult Olfactory Bulb via Interaction with Notch1 Signaling

Adult olfactory neurogenesis plays critical roles in maintaining olfactory functions.Newly-generated neurons in the subventricular zone migrate to the olfactory bulb(OB) and determine olfactory discrimination,but the mechanisms underlying the regulation of olfactory neurogenesis remain unclear.Our previous study indicated the potential of APPL2(adaptor protein,phosphotyrosine interacting with PH domain and leucine zipper 2) as a modulating factor for neurogenesis in the adult olfactory system.In the present study,we report how APPL2 affects neurogenesis in the OB and thereby mediates olfactory discrimination by using both in vitro neural stem cells(NSCs) and an in vivo animal model-APPL2 transgenic(Tg) mice.In the in vitro study,we found that APPL2 overexpression resulted in NSCs switching from neuronal differentiation to gliogenesis while APPL2 knockdown promoted neurogenesis.In the in vivo study,APPL2 Tg mice had a higher population of glial cells and dampened neuronal production in the olfactory system,including the corpus callosum,OB,and rostral migratory stream.Adult APPL2 Tg mice displayed impaired performance in olfactory discrimination tests compared with wild-type mice.Furthermore,we found that an interaction of APPL2 with Notch1 contributed to the roles of APPL2 in modulating the neurogenic lineage-switching and olfactory behaviors.In conclusion,APPL2 controls olfactory discrimination by switching the fate choice of NSCs via interaction with Notch1 signaling.

2-11 Stellar -decay Rate of 59Fe and Its Impact on the 60Fe Nucleosynthesis

60Fe is a long-lived nucleus (T1=2=2.62×106 a) which is mainly synthesized in the Carbon-shell burning ofmassive stars. It still could be observed nowadays after being ejected to the space after massive star ends its life assupernova. Along with another long-lived nucleus 26Al (T1=2=7.17×105 a) which is synthesized in the similar stars,the observation of their decay could provide the information of stellar evolution. From 2002-2005 the INTEGRALsatellite with detector obtained 60Fe/26Al flux ratio in our Galaxy to be 0.148(60)[1]. It's significantly smallerthan the theoretical prediction 0.45[2], and indicated that the theory need to be improved to increase the 60Fe yieldor decrease 26Al yield. 60Fe is produced by neutron capture reactions: 58Fe(n, )59Fe and 59Fe(n, )60Fe. Thecompetition between -decay of 59Fe and its neutron capture plays an important role in 60Fe synthesis path. In thepresent work, the impact on the 60Fe synthesis of the -decay process in stellar environment is studied.

Hepatitis B virus （HBV） infects more than 350 million .people worldwide, leading to chronic hepatitis,cirrhosis and hepatocellular carcinoma.＇ In recent years, the treatment of chronic hepatitis B （CHB） has been improved by the availability of nucleoside analogues （NAs）, such as lamivudine （lam）, adefovir （ADV）, and entecavir （ETV）.2 NAs target HBV reverse transcriptase （RT）, thus inhibiting viral replication and leading to virological, biochemical, and histological improvement in most patients. Unfortunately, the efficacy of NAs is limited by the selection of resistant viruses during the course of therapy, and sequential NA monotherapy can promote multi-drug resistance.

Sampled-data control through model-free reinforcement learning with effective experience replay

Reinforcement Learning(RL)based control algorithms can learn the control strategies for nonlinear and uncertain environment during interacting with it.Guided by the rewards generated by environment,a RL agent can learn the control strategy directly in a model-free way instead of investigating the dynamic model of the environment.In the paper,we propose the sampled-data RL control strategy to reduce the computational demand.In the sampled-data control strategy,the whole control system is of a hybrid structure,in which the plant is of continuous structure while the controller(RL agent)adopts a discrete structure.Given that the continuous states of the plant will be the input of the agent,the state–action value function is approximated by the fully connected feed-forward neural networks(FCFFNN).Instead of learning the controller at every step during the interaction with the environment,the learning and acting stages are decoupled to learn the control strategy more effectively through experience replay.In the acting stage,the most effective experience obtained during the interaction with the environment will be stored and during the learning stage,the stored experience will be replayed to customized times,which helps enhance the experience replay process.The effectiveness of proposed approach will be verified by simulation examples.
