Giant choledocholithiasis treated by mechanical lithotripsy using a gastric bezoar basket

Mechanical lithotripsy （ML） is usually considered as a standard treatment option for large bile duct stones. However, it is impossible to retrieve oversized stones because the conventional lithotripsy basket may not be able to grasp the stone. However, there is no es- tablished endoscopic extraction method for such gi- ant stone removal. We describe a case of successful extraction of a 4-cm large stone using a gastric bezoar basket. A 78-year-old woman had suffered from upper abdominal pain for 20 d. Contrast-enhanced computed tomogram revealed a 4-cm single stone in the distal common bile duct （CBD）. Endoscopic stone retraction was decided upon and endoscopic papillary balloon dilation was performed using a large balloon. An at- tempt to capture the stone using a standard lithotripsy basket failed due to the large stone size. Subsequently, we used a gastric bezoar basket to successfully capturethe stone. The stone was fragmented into small pieces and extracted. The stone was completely removed after two sessions of endoscopic retrograde cholangio- pancreatography; each of which took 30 rain. No com- plications occurred during or after the procedure. The patient was fully recovered and discharged on day 11 of hospitalization. ML using a gastric bezoar basket is a safe and effective retrieval method in select cases, and is considered as an alternative nonoperative option for the management of difficult CBD stones.

视网膜色素上皮65 (retinal pigment epithelium 65, RPE65)基因在视循环中起重要作用，当RPE65基因发生突变时，视锥、视杆细胞功能丧失，严重的患者会视力丧失。为构建RPE65基因突变动物疾病模型，本研究利用转录激活因子样效应物核酸酶(transcription activator-like effector nucleases,TALENs)基因编辑技术，以比格犬(Canis lupus familiaris)为实验动物，针对RPE65基因第3号和第7号外显子构建表达载体，结合显微操作创建RPE65基因编辑犬。通过临床观察、常规眼科检查、眼底照相(fundus photography, FP)、光学相干断层扫描(optical coherence tomography, OCT)和全视野视网膜电图(electroretinography, ERG)对幼犬视功能进行表型分析检测。T7E1酶切和基因测序结果显示，最终获得2只RPE65基因编辑雄性幼犬。眼科表型分析表明，RPE65基因编辑幼犬眼底血管稀疏纤细；ERG结果显示，光感受器细胞的电位活动a波、b波振幅降低，峰时延长，提示视网膜功能受损。本研究利用TALEN基因编辑技术构建的RPE65基因编辑幼犬存在视功能障碍，可为RPE65基因突变诱导的视网膜疾病及相关研究提供参考，还为治疗手段的开发、临床药物的筛选提供良好的动物疾病模型。

Survival Analysis of Stage Ⅳ Metastatic Gastric Cancer Patients Treated with HangAm-Plus

Objective：To evaluate the efficacy of HangAm-Plus（HAP）on stageⅣmetastatic gastric cancer by analyzing the treated patients’overall survival ***：Following the study eligibility,overall survival and one year survival rate of 44 stageⅣmetastatic gastric cancer patients who visited EastWest Cancer Center（EWCC）were *** study consisted of two arms：HAP treatment only（n=18）and combined treatment of concurrent conventional chemotherapy and HAP（n=26）.Patient characteristics by gender,age,surgical intervention,Eastern Cooperative Oncology Group（ECOG）score,treatment duration（〈4 weeks or≥4 weeks）,and lines of the chemotherapy received were *** related side effects were also ***：The median survival of combined group was longer（10.0 months）than that of HAP group（5.1 months）.One-year survival rate of combined treatment group and HAP group was 38.5%±9.5%and 33.3%±11.1%,respectively（P〉0.05）.One-year survival rate of those received more and less than 4-week treatment was 57.1%±18.7%and 8.3%±8.0%,respectively（P=0.001）.Conclusions：The study supports the safety and potential efficacy of HAP treatment in prevention of chemo-related side effects for stageⅣmetastatic gastric cancer treated with conventional *** studies are needed to investigate and confirm the results before applying the treatment in clinical settings.

E dible bird’s nest(EBN)hydrolysates have been proven to exhibit enhanced ***,being a macromolecule,fractions with different molecular weights would have different properties and ***,this research was aimed to determine the chemical properties and antioxidant activities of freezedried(EBN_(FD))and spray-dried EBN(EBN_(SD))hydrolysates fractionated using gel permeation chromatography(GPC).Overall,two well-separated fractions were identified(EBN_(fFD1),EBN_(fFD2),EBN_(fSD1) and EBN_(fSD2)).EBNFD demonstrated significantly higher(P≤0.05)peptide(3.6%),total carbohydrate(27.7%)and sialic acid(18.2%)contents than that of *** trend was observed in low molecular weight fractionates(EBN_(fFD2) and EBN_(fSD2)).Meanwhile,the first fractionates(EBN_(fFD1) and EBN_(fSD1))exhibited significantly higher(P≤0.05)hydroxyl radical(·OH)scavenging *** transform infrared(FTIR)spectroscopy demonstrated that all EBN fractionates have similar spectrum,except in the region of N—H(amideⅡ)and C—H alkyl *** conclusion,EBN fractionates with different molecular weights showed different chemical properties and antioxidant activities.

Factors influencing communication of traditional Chinese medicine use between patients and doctors:A multisite cross-sectional study

Objective: In Singapore, the use of traditional Chinese medicine(TCM) alongside Western medicine(WM)is common. There are risks of adverse herb-drug interactions when taken concurrently. Current literature suggests that TCM use is not regularly reported to WM doctors in Singapore, but the underlying reasons are not ***: A cross-sectional study was conducted across Singapore by administering questionnaires to TCM-using patients and WM-practising general practitioners(GPs). The questionnaire examined the following themes:(1) demographics and TCM use pattern;(2) respondents'(patients and GPs) knowledge and beliefs about TCM and the factors influencing the discussion of TCM during the WM consultation;and(3) respondents' qualitative suggestions to increase disclosure ***: A total of 484 patients and 334 GPs were surveyed. Factors associated with patients' initiation of TCM discussion include length of consultation(odds ratio [OR]: 2.1;P < 0.001), comfort level in discussing TCM(OR: 1.6;P < 0.001) and belief in importance of discussion(OR: 1.4;P = 0.017). Doctor's initiation of discussion(74%) was the top patient-ranked factor influencing their discussion of TCM. For doctors,knowledge of TCM indications(OR: 2.2;P < 0.001), belief in importance of discussion(OR: 2.1;P < 0.001) and comfort level in discussing TCM(OR: 1.9;P = 0.001) were associated with their initiation of TCM use discussion. Possible WM-TCM interactions(58%) was the top doctor-ranked factor influencing their discussion of ***: The discussion of TCM in a WM setting is multifactorial. Interventions include doctors' active screening for TCM use in patients and equipping doctors with TCM knowledge. Improving communication between patients and doctors is key to avoiding harmful herb-drug interactions.

T4 cervical esophageal cancer cured with modern chemoradiotherapy: A case report

Low grade gastric mucosa associated lymphoid tissue lymphoma:Treatment strategies based on 10 year follow-up

AIM:To deduce strategic guidelines of gastric mucosa associated lymphoid tissue lymphoma (MALTOMA) by evaluating the long-term outcome of patients in respect to various treatment modalities. METHODS:A total of 55 patients with MALTOMA from May 1992 to August 2002 were retrospectively reviewed. RESULTS:Complete remission was obtained in 24 (82.8%) of 29 patients treated with anti Helicobacter pylori (Hpylori) regimen *** duration to reach complete remission was 12 months (85 percentile,2-33 months).Five patients showed complete remission with radiation therapy (26-86 months).Two of them were Hpyloritreatment failure cases. CONCLUSION:Hpylorieradication is an effective primary treatment option for low grade MALTOMA and radiation therapy could be considered in patients with no evidence of Hpyloriinfection or who do not respond to Hpylorieradication therapy 12 months after successful eradication.

Myelophil, a mixture of Astragali Radix and Salviae Radix extract, moderates toxic side effects of fluorouracil in mice

AIM: To evaluate the efficacy of Myelophil, an extract containing Astragali Radix and Salviae Radix, for reducing complications induced by 5-fluorouracil (5-FU) in a gastrointestinal cancer model. METHODS: We injected 5-FU into mice and then administered Myelophil to examine the ability of the drug to treat the side effects of 5-FU in mice. Peripheral blood counts, histological examinations, and colony-forming assays of bone marrow were conducted, followed by swimming tests and assessment of survival times. RESULTS: Myelophil restored red and white blood cells and platelets in blood, and recovered cell density in bone marrow to levels comparable to those observed within the control group. In addition, Myelophil significantly increased colony-forming unit granulocyte-macrophage (CFU-GM) and CFU-erythroid (CFU-E) compared to the control group. We confirmed that interleukin-3 gene expression was upregulated by Myelophil in spleen cells. Myelophil administration also doubled the survival rate of mice that were severely myelosuppressed as a result of 5-FU injection at a lethal dose of 70%. Finally, the swimming performance of mice significantly improved as a result of Myelophil treatment. CONCLUSION: These results provide experimentalevidence in support of clinical applications of Myelophil to minimize 5-FU-induced myelosuppression and improve general post-chemotherapy health.
