该研究通过对120例吸入性损伤患者进行肺功能测定及影响因素分析,探讨烧伤合并吸入性损伤患者的肺功能康复效果。干预前,肺康复组与常规康复组患者评估结果无明显差异。肺康复组患者康复前后最大吸气压(MIP)占预计值百分比和峰值咳嗽流量变化与常规康复组相比有显著改善(P=0.04、0.01),其他参数与常规康复组比较,差异无统计学意义(P>0.05)。康复12周后,肺康复组和常规康复组患者一氧化碳弥散量占预计值百分比、MIP、MIP占预计值百分比和膈肌活动度比较,差异有统计学意义(P=0.02、0.005、0.001、0.005),其他指标相近(P>0.05)。这些结果表明,肺康复应该是烧伤患者治疗计划的基本组成部分。

Using prostate-specific antigen(PSA)for prostate cancer(PCa)screening led to overinvestigation and overdiagnosis of indolent PCa.We aimed to investigate the value of prostate health index(PHI)and magnetic resonance imaging(MRI)prostate in an Asian PCa screening program.Men aged 50–75 years were prospectively recruited from a community-based PSA screening program.Men with PSA 4.0–10.0 ng ml^(−1) had PHI result analyzed.MRI prostate was offered to men with PSA 4.0–50.0 ng ml−1.A systematic prostate biopsy was offered to men with PSA 4.0–9.9 ng ml^(−1) and PHI≥35,or PSA 10.0–50.0 ng ml^(−1).Additional targeted prostate biopsy was offered if they had PI-RADS score≥3.Clinically significant PCa(csPCa)was defined as the International Society of Urological Pathology(ISUP)grade group(GG)≥2 or ISUP GG 1 with involvement of≥30%of total systematic cores.In total,12.8%(196/1536)men had PSA≥4.0 ng ml^(−1).Among 194 men with PSA 4.0–50.0 ng ml^(−1),187(96.4%)received MRI prostate.Among them,28.3%(53/187)had PI-RADS≥3 lesions.Moreover,7.0%(107/1536)men were indicated for biopsy and 94.4%(101/107)men received biopsy.Among the men received biopsy,PCa,ISUP GG≥2 PCa,and csPCa was diagnosed in 42(41.6%),24(23.8%),and 34(33.7%)men,respectively.Compared with PSA/PHI pathway in men with PSA 4.0–50.0 ng ml^(−1),additional MRI increased diagnoses of PCa,ISUP GG≥2 PCa,and csPCa by 21.2%(from 33 to 40),22.2%(from 18 to 22),and 18.5%(from 27 to 32),respectively.The benefit of additional MRI was only observed in PSA 4.0–10.0 ng ml^(−1),and the number of MRI needed to diagnose one additional ISUP GG≥2 PCa was 20 in PHI≥35 and 94 in PHI<35.Among them,45.4%(89/196)men with PSA≥4.0 ng ml^(−1) avoided unnecessary biopsy with the use of PHI and MRI.A screening algorithm with PSA,PHI,and MRI could effectively diagnose csPCa while reducing unnecessary biopsies.The benefit of MRI prostate was mainly observed in PSA 4.0–9.9 ng ml^(−1) and PHI≥35 group.PHI was an important risk stratification step for PCa screening.

A boundary element method is developed for calculating the flare ship hull slamming problem. The nonlinear free surface elevation and the linear element assumption are employed. The method has been verified by comparisons with results for the water entry of wedges with various deadrise angles. Numerical results show that the pressure distribution varies greatly with the ship hull with different curvilinear equations, and the slamming features are also different; From the numerical simulation, the authors found that the structural damage of the flare hull might be caused by the increasing hydrodynamic pressure over an extensive area on the flare when the upper part of the flare comes into contact with water.

The asymmetric initial post-buckling corresponding to the lowest bifurcation load and the imperfection sensitivity are analysed in detail for a compressed simply supported column with an equilateral triangular cross-section in the plastic range. The effect of elastic unloading is taken into ao count in the analysis. The asymptotic relations, including high order terms, among the load, the amplitude of imperfection and the amplitude of the bifurcation mode ark realized. The results show that the maximum supported load is very sensitive to imperfection.

Abstinence from prolonged psychostimulant use prompts stimulant withdrawal syndrome.Molecular adaptations within the dorsal striatum have been considered the main hallmark of stimulant abstinence. Here we explored striatal miRNA-target interaction and its impact on circulating miRNA marker as well as behavioral dysfunctions in methamphetamine(MA) abstinence. We conducted miRNA sequencing and profiling in the nonhuman primate model of MA abstinence, followed by miRNA qPCR,LC-MS/MS proteomics, immunoassays, and behavior tests in mice. In nonhuman primates, MA abstinence triggered a lasting upregulation of miR-137 in the dorsal striatum but a simultaneous downregulation of circulating miR-137. In mice, aberrant increase in striatal miR-137-dependent inhibition of SYNCRIP essentially mediated the MA abstinence-induced reduction of circulating miR-137. Pathway modeling through experimental deduction illustrated that the MA abstinence-mediated downregulation of circulating miR-137 was caused by reduction of SYNCRIP-dependent miRNA sorting into the exosomes in the dorsal striatum. Furthermore, diminished SYNCRIP in the dorsal striatum was necessary for MA abstinence-induced behavioral bias towards egocentric spatial learning. Taken together, our data revealed circulating miR-137 as a potential blood-based marker that could reflect MA abstinence-dependent changes in striatal miR-137/SYNCRIP axis, and striatal SYNCRIP as a potential therapeutic target for striatum-associated cognitive dysfunction by MA withdrawal syndrome.

High Manganese Steel Alloying Process and Its Influence on Microstructure and Properties of the Steel

The influence of two kinds of alloying processes, adding Nb (or Ti) and N-Mn alloy as well as adding Nb (or Ti) and spraying Nz, on microstructures and properties of a high manganese steel has been studied. It has been found that adding Nb(or Ti), accompanying with N-Mn alloy, is unfavourable to microstructure compactness of the high manganese steel, but adding Nb (or Ti) and spraying N-2 into the melt is good for refining austenitic grain, forming a lot of hard particles and improving microstructure compactness. The mechanical properties of the high manganese steel have relation to the content of elements Nb or Ti. Its fracture mode will turn ductile fracture into brittle cleavage fracture gradually By X-ray and TEM analysis, it is proved that the austenite can be transformed to deformation-induced cr martensite after adding a certain amount of element Nb (or Ti). The microstructure transformation of alloying high manganese steels through deformation is one of methods for strengthening austenite matrix acid increasing the work-hardening rate as well as improving antiwear property.
