Systematic prostate biopsy has limitations,such as overdiagnosis of clinically insignificant prostate cancer and underdiagnosis of clinically significant prostate *** resonance imaging(MRI)-guided biopsy,a promising alternative,might improve diagnostic *** compare the cancer detection rates of systematic biopsy and combined biopsy(systematic biopsy plus MRI-targeted biopsy)in Asian men,we conducted a retrospective cohort study of men who underwent either systematic biopsy or combined biopsy at two medical centers(Queen Mary Hospital and Tung Wah Hospital,Hong Kong,China)from July 2015 to December *** statistics were calculated,and univariate and multivariate logistic regression analyses were *** primary and secondary outcomes were prostate cancer and clinically significant prostate cancer.A total of 1391 participants were *** overall prostate cancer detection rates did not significantly differ between the two groups(36.3%vs 36.6%,odds ratio[OR]=1.01,95%confidence interval[CI]:0.81-1.26,P=0.92).However,combined biopsy showed a significant advantage in detecting clinically significant prostate cancer(Gleason score≥3+4)in patients with a total serum prostate-specific antigen(tPSA)concentration of 2-10 ng ml−1(systematic vs combined:11.9%vs 17.5%,OR=1.58,95%CI:1.08-2.31,P=0.02).Specifically,in the transperineal biopsy subgroup,combined biopsy significantly outperformed systematic biopsy in the detection of clinically significant prostate cancer(systematic vs combined:12.6%vs 24.0%,OR=2.19,95%CI:1.21-3.97,P=0.01).These findings suggest that in patients with a tPSA concentration of 2-10 ng ml−1,MRI-targeted biopsy may be of greater predictive value than systematic biopsy in the detection of clinically significant prostate cancer.

Injuries to the central nervous system(CNS)such as stroke,brain,and spinal cord trauma often result in permanent disabilities because adult CNS neurons only exhibit limited axon *** brain has a surprising intrinsic capability of recovering itself after ***,the hostile extrinsic microenvironment significantly hinders axon *** advances have indicated that the inactivation of intrinsic regenerative pathways plays a pivotal role in the failure of most adult CNS neuronal ***,substantial evidence has convincingly demonstrated that the mechanistic target of rapamycin(mTOR)signaling is one of the most crucial intrinsic regenerative pathways that drive axonal regeneration and sprouting in various CNS *** this review,we will discuss the recent findings and highlight the critical roles of mTOR pathway in axon regeneration in different types of CNS ***,we will demonstrate that the reactivation of this regenerative pathway can be achieved by blocking the key mTOR signaling components such as phosphatase and tensin homolog(PTEN).Given that multiple mTOR signaling components are endogenous inhibitory factors of this pathway,we will discuss the promising potential of RNA-based therapeutics which are particularly suitable for this purpose,and the fact that they have attracted substantial attention recently after the success of coronavirus disease 2019 *** specifically tackle the blood-brain barrier issue,we will review the current technology to deliver these RNA therapeutics into the brain with a focus on nanoparticle *** will propose the clinical application of these RNA-mediated therapies in combination with the brain-targeted drug delivery approach against mTOR signaling components as an effective and feasible therapeutic strategy aiming to enhance axonal regeneration for functional recovery after CNS injury.

Unsupervised machine learning methods were applied on multivariate geophysical and geochemical datasets of ocean floor sediment cores collected from the South China Sea. The well-preserved and continuous core samples comprising high resolution Cenozoic sediment records enable scientists to carry out paleoenvironment studies in detail. Bayesian age-depth chronological models constructed from biostratigraphic control points for the drilling sites are applied on cluster boundaries generated from two popular unsupervised learning methods: K-means and random forest. The unsupervised learning methods experimented have produced compact and unambiguous clusters from the datasets, indicating that previously unknown data patterns can be revealed when all variables from the datasets are taken into account simultaneously. A study of synchroneity of past events represented by the cluster boundaries across geographically separated ocean drilling sites is achieved through converting the fixed depths of cluster boundaries into chronological ranges represented by Gaussian density plots which are then compared with known past events in the region. A Gaussian density peak at around 7.2 Ma has been identified from results of all three sites and it is suggested to coincide with the initiation of the East Asian monsoon. Contrary to traditional statistical approach, a priori assumptions are not required for unsupervised learning, and the clustering results serve as a novel data-driven proxy for studying the complex and dynamic processes of the paleoenvironment surrounding the ocean sediment. This work serves as a pioneering approach to extract valuable information of regional events and opens up a systematic and objective way to study the vast global ocean sediment datasets.

Unsupervised learning methods were applied to explore data patterns in multivariate geophysical datasets collected from ocean floor sediment core samples coming from scientific ocean drilling in the South China *** to studies on similar datasets,but using supervised learning methods which are designed to make predictions based on sample training data,unsupervised learning methods require no a priori information and focus only on the input *** this study,popular unsupervised learning methods including K-means,self-organizing maps,hierarchical clustering and random forest were coupled with different distance metrics to form exploratory data *** resulting data clusters were externally validated with lithologic units and geologic time scales assigned to the datasets by conventional methods. Compact and connected data clusters displayed varying degrees of correspondence with existing classification by lithologic units and geologic time scales.K-means and self-organizing maps were observed to perform better with lithologic units while random forest corresponded best with geologic time *** study sets a pioneering example of how unsupervised machine learning methods can be used as an automatic processing tool for the increasingly high volume of scientific ocean drilling data.

Background Due to the inaccuracy of the traditional geographical distance-based definition of left-behind status,data on the negative effect of left-behind status on cognitive function among older adults are *** This study examined the cross-sectional and longitudinal associations of left-behind status with cognitive function in older Chinese *** left-behind status definition was based on the frequency of face-to-face parent-child *** Data from a nationally representative sample of 8682 older adults(60+years)in 2015(5658 left behind and 3024 non-left behind),of which 6933 completed the follow-up in 2018,were obtained from the China Health and Retirement Longitudinal ***-behind older adults werebroadlydefined as those aged 60+years who had living adult children and saw their children less than once per *** cognitive function was assessed with a composite cognitive test with higher total scores indicating better cognitive *** Left-behind older adults had significantly lower cognitive test scores than non-left-behind older adults in both 2015(11.1(6.0)vs 13.2(5.9),t=15.863,p<0.001)and 2018(10.0(6.6)vs 12.4(6.7),t=14.177,p<0.001).After adjusting for demographic factors,lifestyle factors,chronic medical conditions and the baseline cognitive test score(in the longitudinal analysis only),on average,the cognitive test score of left-behind older adults was 0.628 lower than their non-left-behind counterparts in 2015(t=5.689,p<0.001).This difference in cognitive test scores attenuated to 0.322 but remained significant in 2018(t=2.733,p=0.006).Conclusions Left-behind older Chinese adults have a higher risk of poor cognitive function and cognitive decline than their non-left-behind *** efforts targeting left-behind older adults,such as encouraging adult children to visit their parents more regularly,are warranted to maintain or delay the progression of cognitive decline.

Many medication errors could be avoided if administration was more closely linked with structured monitoring. The contributory factors to administration errors occurred in one recent year were reviewed and identified contributory factors to errors as six domains of administration principles according to the report from the Quality and Safety website. The current measures including guidelines, policy, and practices to prevent the administration errors identified in the previous step were searched from the United Christian Hospital (UCH) homepage. Meanwhile, the international measures suggested in literature were identified to address the administration errors identified. 41 cases were identified as medication errors related to administration error events, with total twenty contributory factors identified according to the incident report which identified five contributory factors as common causes. Measures to prevent interruption of medication round and measures to improve individual knowledge and skills, and personal responsebility were suggested to fill the gaps. The medication administration errors should be avoided through both education reinforcing programme and preventive interventions of distraction or interruption to the procedure after comparing the existing measures to the suggested measures from literature. This study was so important to improve the current measures to prevent medication administration errors.

BACKGROUND: Cancer relapse, associated with increased drug resistance and rate of metastasis, often follows completion of chemotherapy but the cancer escape mechanisms are still incompletely understood. Percutaneous ethanol injection(PEI)has been used for treating hepatocellular carcinoma(HCC)for decades, while the recurrence after PEI treatment remains a major limitation. Recent evidence mounted that cancer cells could survive from chemical induced apoptosis, suggesting a potential route through which cancer relapse may occur. This study focuses on the consequence of HepG2 recovery from ethanol-induced apoptotic ***: The model of HepG2 recovery from ethanolinduced apoptotic event was established by live cell imaging,BrdU assay and Western blotting. MTT assay, wound healing assay and invasion assay were used to investigate the behavior of HepG2 after ***: HepG2 cells could recover from ethanol-induced apoptosis. These cells changed their behaviors such as drug resistance, mobility and invasiveness. On average, the recovered HepG2 cell clones were found to be 46% more resistant to ethanol and 84% higher in mobility. The recovered clones became 58.2% more sensitive to ***: HepG2 cells can recover from ethanolinduced apoptotic event. These cells became more resistant to ethanol and more invasive. Although the recovered cell clones were more resistant to ethanol, they became more sensitive to5-fluorouracil treatment.
