A 35-year-old mother was scheduled to be the living donor for liver transplantation to her second son,who suffered from biliary atresia complicated with biliary cirrhosis at the age of 2 *** operative plan was to recover the left lateral segment of the mother's liver for living donor *** the use of cholangiography at the time of surgery,we found the right anterior segmental duct(RASD) emptying directly into the cystic duct,and the catheter passed into the *** repairing the incision in the cystic duct,transplantation was successfully *** postoperative course was *** anatomical variations were frequently encountered,however,this variation has very rarely been *** the RASD was divided,the repair would be very difficult because the duct will not dilate sufficiently in an otherwise healthy *** preoperative evaluation of the living donor's biliary anatomy,especially using magnetic resonance cholangiography and careful intraoperative techniques,is important to prevent bile duct injury and avoid the risk to the healthy donor.

ABSTRACT The molecular signaling pathway linked to hyper-trophy of the anti-gravity/postural soleus muscle af-ter mechanical overloading has not been identified. Using Western blot and immunohistochemical analy-ses, we investigated whether the amounts of NFATc3, GSK-3?, NFATc1, and neonatal MHC change in the mechanically overloaded soleus muscle after cyc-losporine A (CsA) treatment. Adult male ICR mice were subjected to a surgical ablation of the gas-trocnemius muscle and treated with either CsA (25 mg/Kg) or vehicle once daily. They were sacrificed at 2, 4, 7, 10, and 14 days post-injury. Mechanical over-loading resulted in a significant increase in the wet weight and the cross-sectional area of slow and fast fibers of the soleus muscle in placebo-treated mice but not CsA-treated mice. After 4 days of mechanical overloading, we observed a similar co-localization of neonatal MHC and NFATc3 in several myotubes of both mice. The placebo-treated mice possessed larger myotubes with neonatal MHC than CsA-treated mice. At 7 days, mechanical overloading induced marked expression of neonatal MHC in myotubes and/or myofibers. Such neonatal MHC-positive fibers emerged less often in the hypertrophied soleus mus-cle subjected to treatment with CsA. CsA treatment did not significantly change the amount of GSK-3? protein in the soleus muscle. The modulation of growth in neonatal MHC-positive myofibers by CsA treatment may inhibit the hypertrophic process in the soleus muscle after mechanical overloading.

A quantitative analysis of capillary supply to skeletal muscle is important for understanding the upper limit of the capacity for delivery of oxygen and substrates to muscle cells. It has been well documented that the number of capillaries is altered by several factors including development, aging, and alteration of muscle activity level such as exercise training and inactivation. There is, however, a contradiction in animal studies for aging-related change in the number of capillaries. Human studies using biopsy technique also displayed an inconsistency on that point, in which capillary supply was not influenced or decreased with aging. This review discussed an inconsistency among studies for aging-related change in muscle capillary supply. In conclusion, the relationship between capillary supply and muscle fiber size is similar for both young and elderly population, and the morpho- logical balance between capillaries and each muscle fiber was maintained with advancing age.

Cleistogamy involves the shedding of pollen within an enclosed flower. In barley, this trait is determined by the presence of a recessive allele at the gene Clyl, a member of the AP2 gene family. Here we show that the Clyl ortholog in einkorn （diploid） wheat （Triticum monococcum） TmAP2 shares a similar structure and identical pattern of transcription as Clyl. The transcript abundance of TmAP2 was high in the spike around the time of anthesis, but low in the leaf, plumule and radicle. The TmAP2 transcript was cleaved at its miR172 target site. Flower gaping at anthesis in einkorn wheat is induced by the expansion of the lodicules.

Statistical manipulation of material data was conducted for probabilistic life assessment or risk-based design and maintenance for high temperature components of power plants. To obtain the statistical distribution of material properties, dominant parameters affecting material properties are introduced into normalization of statistical variables. Those parameters are hardness, chemical composition, characteristic micro structural features and so on. Creep and fatigue properties are expressed by normalized parameters and the unified statistical distributions are obtained. These probability distribution functions show good coincidence statistically with the field database of steam turbine components. It was concluded that the unified statistical baseline approach is useful for the risk management of components in power plants.

AIM:To clarify the impact of cytomegalovirus(CMV)activation and antiviral therapy based on CMV antigen status on the long-term clinical course of ulcerative colitis(UC)***:UC patients with flare-up were divided into CMV-positive and-negative groups according to the CMV antigenemia *** main treatment strategy provided for the patients in the CMV-positive group comprised a dose reduction of corticosteroids and administration of ***:The median number of days to initial remission was significantly greater for the patients in the CMV-positive group(21 d vs 16 d,P=0.009).However,the relapse rate after remission and colectomy rate during more than 30 mo of observation did not differ between the two *** analysis revealed that administration of ganciclovir was the only independent factor for avoiding colectomy in patients of the CMV-positive ***:CMV antigen status did not significantly affect the long-term prognosis in UC patients under treatment with appropriate antiviral therapy.

Pediatric living donor liver transplantation for congenital hepatic fibrosis using a mother's graft with von Meyenburg complex: A case report

This is the first report of living donor liver transplantation(LDLT) for congenital hepatic fibrosis(CHF) using a mother's graft with von Meyenburg complex. A 6-year-old girl with CHF, who suffered from recurrent gastrointestinal bleeding, was referred to our hospital for liver transplantation. Her 38-year-old mother was investigated as a living donor and multiple biliary hamartoma were seen on her computed tomography and magnetic resonance imaging scan. The mother's liver function tests were normal and she did not have any organ abnormality, including polycystic kidney disease. LDLT using the left lateral segment(LLS) graft from the donor was performed. The donor LLS graft weighed 250 g; the graft recipient weight ratio was 1.19%. The operation and post-operative course of the donor were uneventful and she was discharged on post-operative day(POD) 8. The graft liver function was good, and the recipient was discharged on POD 31. LDLT using a graft with von Meyenburg complex is safe and useful. Long-term follow-up is needed with respect to graft liver function and screening malignant tumors.

Breast cancer is the most commonly diagnosed cancer in women. A strong treatment candidate is high- intensity focused ultrasound （HIFU）, a non-invasive therapeutic method that has already demonstrated its promise. To improve the precision and lower the cost of HIFU treatment, our group has developed an ultrasound （US）-guided, five-degree-of-freedom （DOF）, robot-assisted HIFU system. We constructed a fully functional prototype enabling easy three-dimensional （3D） US image reconstruction, target seg-mentation, treatment path generation, and automatic HIFU irradiation. The position was calibrated using a wire phantom and the coagulated area was assessed on heterogeneous tissue phantoms. Under the US guidance, the centroids of the HIFU-ahlated area deviated by less than 2 mm from the planned treatment region. The overshoot around the planned region was well below the tolerance of clinical usage. Our system is considered to he sufficiently accurate for breast cancer treatment.
