Determinants and Outc.mes of Pregnanc.es among Booked versus Unbooked Patients in a Tertiary Hospital South East, Nigeria

Bac.ground: Inadequate antenatal c.re, both in c.verage and standard, has been linked with poor pregnanc. outc.mes. In a resourc. poor setting, home delivery is c.nsidered very c.eap, but the possible outc.me of suc. pregnanc.es has been a subjec. of interest. This study evaluated the assoc.ation of booking status with pregnanc. outc.mes. Objec.ives: To examine the fac.ors that influenc. booking and impac. of the booking status on pregnanc. outc.mes. Methods: This was a hospital-based prospec.ive c.hort study of 240 (120 booked and 120 un-booked) pregnant mothers selec.ed through systematic.random sampling. The study was c.rried out at the Department of Obstetric. and Gynaec.logy, Alex Ekwueme Federal University Teac.ing Hospital Abakaliki, Nigeria. c.i-square test, t-test and logistic.regression were used for analysis. Results: The mean age of the partic.pants was 30.39 ± 5.05 years and 29.05 ± 6.36 years respec.ively for the booked and unbooked. Marital status (OR = 6.35, 95% c. = 1.24 - 32.59), educ.tional status (OR = 36.40, 95% c. = 5.26-152.83), plac. of residenc. (OR = 0.15, 95% c. = 0.06 - 0.41), partner’s support (OR = 0.05, 95% c. = 0.01 - 0.37), family support (OR = 0.03, 95% c. = 0.01 - 0.13), mode of delivery (OR = 0.23, 95% c. = 0.12 - 0.46), APGAR sc.re in first minute (OR = 6.02, 95% c. = 2.45 - 14.83) and NBIc. admission (OR = 3.75, 95% c. = 1.67 - 8.43) were assoc.ated with booked status. However, being unmarried, nulliparity/grandmultiparity, low level of educ.tion, blue c.lar jobs, low inc.me, rural dwelling and poor partner and poor family support were assoc.ated with the unbooked. The booked parturients had a better perinatal outc.me c.mpared the unbooked. c.nc.usion/Rec.mmendation: There was high prevalenc. of operative deliveries, perinatal morbidity and mortality among the unbooked mothers. Girl c.ild educ.tion, employment and poverty alleviation would improve antenatal c.re utilization.

In the previous public.tion on Volume 15 No 9, September 30, 2022 of IJc., we analyzed “Data Mining as a Tec.nique for Healthc.re Approac.”. In this edition, emphasis has been made on the “Development of Data Mining Model to Detec. c.rdiovasc.lar Diseases (c.D)”. c.ngenital c.ylothorax Treated with Oral Sildenafil—c.se Report from Nigeria and Review of Literature

c.ngenital c.ylothorax, a c.ndition in whic. c.yle ac.umulates in the pleural c.vity, is extremely rare. The reported inc.denc. is 1 in 24,000 births. Medic.l treatment using oc.reotide as well as c.emic.l and surgic.l pleurodesis has been reported, but also few reports on the suc.essful use of Sildenafil exists. We herein report a c.se of c.ngenital c.ylothorax referred to our Hospital at 7 days-old. Sildenafil was introduc.d on day 6 post intubation. Before the introduc.ion of sildenafil, the peak total daily drainage was 106 ml/kg/day (340 mls/day), and after sildenafil was introduc.d, the total daily drainage dropped progressively to 10 ml/kg/day by day 11 post intubation. This work is c.nc.rned with GSM subsc.iption fraud detec.ion system using some network tec.niques. Fraud is a hitc. around the globe with huge loss of inc.me. Fraud has an effec. on the c.edibility and performanc. of telec.mmunic.tion c.mpanies. The most diffic.lt problem that fac.s the industry is the fac. that fraud is dynamic. whic. means that whenever fraudsters feel that they will be detec.ed, they devise other ways to c.rc.mvent sec.rity measures. In suc. c.ses, the perpetrators’ intention is to c.mpletely avoid or at least reduc. the c.arges for using the servic.s. Subsc.iption fraud is one of the major types of telec.mmunic.tion fraud in whic. a c.stomer obtains an ac.ount without intention to pay the bill. Thus at the level of a phone number, all transac.ions from the number will be fraudulent. In suc. c.ses abnormal usage oc.urs throughout the ac.ive period of the ac.ount;whic. is usually used for c.ll selling or intensive self usage. This provides a means for illegal high profit business for fraudsters requiring minimal investment and relatively low risk of getting c.ught. A system to prevent subsc.iption fraud in GSM telec.mmunic.tions with high impac. on long distanc. c.rriers is proposed to detec. fraud. The system employs adaptive flexible tec.niques using advanc.d data analysis like Artific.al Neural Networks (ANN). This study aims at developing a fraud detec.ion model oc.urrenc. in GSM Network. The paper also gives analysis of the fraud detec.ion Systems, fraud detec.ion and prevention, fraud prevention methods etc. Fraud affec.s us all and is of partic.lar c.nc.rn to those who manage large government and business organisations where the potential losses are greatest. c.mparative Study on the Ac.d-Base Indic.tor Properties of Natural Dye, Turmeric.Rhizome (c.rc.ma longa) and Synthetic.Dyes

Ac.d-Base Indic.tor, Turmeric.Rhizome (c.rc.ma longa) was extrac.ed from the root of a turmeric.plant. The turmeric.was peeled, washed and dried in an oven at 60°c. It was ground into powder and soaked in hot and c.ld ethanol for the extrac.ion. The extrac. was filtered and part of it was c.nc.ntrated to yield a reasonable quantity of turmeric.indic.tor. On standardization of ac.d with a base, 0.05 M base respec.ively of Sodium hydroxide (NaOH), sodium c.rbonate (Na2c.3) and Disodium borate (B4Na2O7) were used. Hot and c.ld extrac.s of turmeric.were used as indic.tors and were c.mpared with methyl orange and phenolphthalein. On the preliminary test c.rried out, hot and c.ld turmeric.indic.tor showed yellow c.lour in ac.d medium and orange c.lour in the base. Methyl orange showed red c.lour in ac.d but yellow in the base, phenolphthalein was c.lourless in ac.d but pink in the base. During titration there were c.lour c.anges at the end points in the entire test c.rried out. The average volumes at ends points were c.lc.lated, the molar c.nc.ntrations and mass c.nc.ntrations of the ac.ds used were also determined. The results showed that there was no differenc. between the natural indic.tors used and the existing synthetic.indic.tors whic. are toxic.to our environment.

Objec.ive:To c.mpare the prolac.in, progesterone, luteinizing hormone, follic.e stimulating hormone, estradiol, antisperm antibodies, tumor nec.osis fac.orα, total antioxidant c.pac.ty, prostaglandin E2 and F2α between primary and sec.ndary infertile women in ***:One hundred and two volunteers infertile women (test group), aged 20-45 years attending the infertility c.inic.in the University of c.labar Teac.ing Hospital (Uc.H) were rec.uited. Fourteen of the women had primary infertility while 88 of them had sec.ndary infertility. Sixty apparently healthy, age matc.ed women served as the c.ntrol group. Five millilitres of blood was c.llec.ed, allowed to c.ot and serum was obtained from the subjec.s and Enzyme Linked Immunosorbent Assay method was used for prolac.in, progesterone, estradiol, follic.e stimulating hormone (FSH) and luteinizing hormone (LH), prostaglandin E2and F2α, human TNF-α,c.lamydia trac.omatis IgG, antisperm antibodies assay, while serum total antioxidant c.pac.ty was assessed ***:The result shows on c.mparison that primary infertile women has a signific.ntly higher level of progesterone than those with sec.ndary infertility (P>0.05). There was a signific.nt differenc. in the levels of prolac.in and total antioxidant c.pac.ty in the primary and sec.ndary infertility when c.mpare to the c.ntrol group atP>0.05. Their mean ages were (31.10±5.37) years and (33.10±4.91) years respec.ively. There was a positive c.rrelation between TAc.and FSH, TNF and anti-sperm anti-bodies in the test group ofr=0.207 andr=0.632;P>0.05 ***: These findings suggest no alterations in levels of prostaglandin F2α, TNF and anti-oxidant between primary and sec.ndary infertile females.

Fac.ors Affec.ing the Knowledge and Prac.ic. of Drug Management among Healthc.re Workers in Primary Health c.nters in South-East, Nigeria

The knowledge of health workers regarding their management of drugs is very important in ensuring good health. One of the major indic.s of the performanc. of primary health c.re (PHc. remains improved ac.ess to essential drugs as they are the link between patients and health servic.s. c.nsequently, their availability or absenc. will c.ntribute to a positive or negative impac. on health. This was a quasi-experimental study, c.rried out in Anambra state, whic. c.mpared the intervention and c.ntrol groups following the training and provision of drug management tools to PHc.workers in the intervention group. A multi-stage sampling tec.nique was used to selec. 264 health workers from 132 health c.nters from two senatorial zones in the State, one ac.ing as a c.ntrol group and the other as an intervention group. Data was c.llec.ed using a pre-tested in-depth interview guide and semi-struc.ured interviewer administered questionnaire. Statistic.l analysis was c.nduc.ed using SPSS at a statistic.l signific.nc. level of p value less than 0.05, while qualitative data was analyzed using N-Vivo. Several fac.ors suc. as age, sex, educ.tional qualific.tion, c.dre of staff, years of prac.ic. and PHc.workers’ previous training were identified as affec.ing the knowledge and prac.ic. of drug management. The majority (72.0% and 71.2%) in intervention and c.ntrol groups respec.ively said inadequate information or lac. of knowledge was the main reason for poor prac.ic.s while the proportion with low interest of health workers for drug management was (38.5%) for intervention and (59.8%) for the c.ntrol group and the differenc. between the groups was statistic.lly signific.nt (p = 0.034). In c.nc.usion, this study has shown that age, sex, educ.tional qualific.tion, c.dre of staff, years of prac.ic. and PHc.workers’ previous training were fac.ors assoc.ated with health workers’ knowledge and prac.ic. of drug management. Urban Heat Island and Land Use/c.ver Dynamic. Evaluation in Enugu Urban, Nigeria

This study spec.fic.lly estimated the effec. of land use/c.ver c.ange (LULc. proc.sses on land surfac. temperature (LST) in Enugu urban and its suburbs. With Landsat images and supervised c.assific.tion tec.nique, four LULc.c.asses c.mprising built-up areas, vegetation, roc. outc.op, bare ground/farmland areas were delineated. The LST was extrac.ed from the thermal bands of the images. The rate of c.ange in land c.ver c.asses between 2009 and 2018 showed that from 2009 to 2013, built-up areas inc.eased from 31.65% to about 47.5%, while vegetation c.ver dec.eased from 18.43% to 11.23%. Also, the periods witnessed about 8.69 km2 of vegetation being c.nverted to other land surfac.s. The trend in the LST in Enugu urban showed the highest mean temperatures of 34.5°c.in 2018 and 32.26°c.in 2015. However, in 2013 there was a slight dec.ease in mean LST to 31.65°c.whic. further dec.eased to 31.26°c.in 2009. This c.ange in temperature suggests that urbanisation c.uld have signific.nt effec. on the mic.o-c.imate of Enugu c.ty. Result also revealed weak relationships between LULc.c.asses and the LST throughout the years. The results of the surfac. heat intensity for the urban and rural areas showed general inc.ease over the years and this suggests that rural areas are also experienc.ng high temperature whic. c.uld be due to the loss of vegetation, inc.ease in artific.al surfac.s and urban enc.oac.ment. Findings from this study c.uld be useful for effec.ive urban land-use planning, polic. development and management in Nigeria, and elsewhere.

c.ild abuse is a global problem. It inc.udes all forms of physic.l and emotional ill-treatment, sexual abuse, neglec., and exploitation that result in ac.ual or potential harm to the c.ild’s health, development or dignity. c.ild abuse and neglec. are c.mmon in Nigeria, yet poorly reported. We herein report a c.se of c.ild abuse and neglec. in a 14-year-old boy, c.mplic.ted by severe malnutrition, human immunodefic.enc. virus infec.ion and gross under ac.ievement of educ.tional goal. The soc.al and ec.nomic.fac.ors that play major roles are highlighted with the aim of sensitizing health c.re professionals, c.ild advoc.c. groups, polic. makers and other stakeholders on the impac. of c.ild abuse. We also hope that this would remind all c.nc.rned organisations and individuals of c.ild protec.ion whic. is integrated in the c.ild right ac. 2003 adopted by Nigeria.

Authentic.tion of <i>Rothmannia whitfieldii</i>Dye Extrac. with FTIR Spec.rosc.py

Extrac.ion of dye from dry fruit of Rothmannia whitfieldii was c.rried out using four different extrac.ion methods. Solvent and ac.d extrac.ion methods gave a c.lourless supernatant solution after extrac.ion time of 45 minutes at 60°c. The alkali method gave a deep brown c.loured supernatant solution while the aqueous method gave a dark c.loured supernatant solution after extrac.ion under the same c.nditions. From the result of the FTIR spec.rosc.py c.arac.erization of the c.loured solutions and the dry powder of Rothmannia whitfieldii fruit, it was observed that only the alkali method extrac.ed what c.n be c.lled a dye with likely presenc. of tannins. The result also showed that the possible func.ional groups present in the supernatant solution after aqueous extrac.ion are same with the func.ional groups present in the dry pulverized Rothmannia whitfieldii fruit. Henc., aqueous method did not extrac. any dye. Similarly, a mixture of the solution after aqueous extrac.ion with drops of alkali solution produc.d a deep brown c.loured solution indic.ting solubility of the dye c.mponent in alkali media.
