Comparing 11 early warning scores and three shock indices in early sepsis prediction in the emergency department

BACKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROC]0.75,95%CI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%CI 0.37-0.52)and 0.88(95%CI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

Identification of Ca2+ signaling components in neural stem/progenitor cells during differentiation into neurons and glia in intact and dissociated zebrafish neurospheres

The development of the CNS in vertebrate embryos involves the generation of different sub-types of neurons and glia in a complex but highly-ordered spatio-temporal manner. Zebrafish are commonly used for exploring the development, plasticity and regeneration of the CNS, and the recent development of reliable protocols for isolating and culturing neural stem/progenitor cells(NSCs/NPCs) from the brain of adult fish now enables the exploration of mechanisms underlying the induction/specification/differentiation of these cells. Here, we refined a protocol to generate proliferating and differentiating neurospheres from the entire brain of adult zebrafish. We demonstrated via RT-qPCR that some isoforms of ip3 r, ryr and stim are upregulated/downregulated significantly in differentiating neurospheres, and via immunolabelling that 1,4,5-inositol trisphosphate receptor(ip3 R) type-1 and ryanodine receptor(RyR) type-2 are differentially expressed in cells with neuron-or radial glial-like properties. Furthermore, ATP but not caffeine(ip3 R and RyR agonists, respectively), induced the generation of Ca^(2+) transients in cells exhibiting neuron-or glial-like morphology. These results indicate the differential expression of components of the Ca^(2+) -signaling toolkit in proliferating and differentiating cells. Thus, given the complexity of the intact vertebrate brain, neurospheres might be a useful system for exploring neurodegenerative disease diagnosis protocols and drug development using Ca^(2+) signaling as a read-out.

Endocrine Disrupting Chemicals(EDCs)are a group of molecules that can influence hormonal balance,causing disturbance of the reproductive system and other health *** the efforts to eliminate EDC in the environment,all current approaches are inefficient and *** previous research,studies revealed that laccase-producing microorganisms may be a potential candidate for EDC degradation,as laccases have been found to be able to degrade many kinds of EDCs effectively and ***,we created two recombinant laccases,each fused with secretion peptide,Novel Signal Peptide 4(NSP4),and expressed them in Escherichia coli(***,BL21),together with one laccase without secretion *** first optimized the culture condition of expressing these ***,we test the activity of the recombinant laccases of decolorizing of a synthetic dye,indigo ***,we confirmed the secreted can degrade one of the EDCs,β-estradiol,showing the potential of using the laccase secretion system to degrade toxic compounds.

Real-time prediction of the peak suspended sediment concentration and sediment yield of the Lao-Nung River during storms

Based on rainfall erosion of soil and suspended sediment transport in storm events,a method is proposed to predict peak suspended sediment concentration and suspended sediment yield in watersheds based on rainfall characteristics prior to peak rainfall *** rainfall characteristics factors that dominate peak suspended sediment concentration Cpare rainfall erosion factor Ref, first peak rainfall intensity of area-average rainfall ip1and antecedent precipitation index Iap;the rainfall characteristics factors that dominate suspended sediment yield Yssin storm events are total rainfall P,suspended sediment yield factor Rsfand antecedent precipitation index *** research focuses on watersheds in Liau-Kwei observation station along Lao-Nung River in southern Taiwan as the research object,and adopts the PSED-model to simulate the discharge hydrograph, suspended sediment concentration hydrograph and suspended sediment yield in 11 storm events for *** analytical results show that there is a good correlation between the above-mentioned rainfall characteristics factors and CPas well as Yss,thus enabling Cpand Yssto be predicted by using Expressions(13) and(14).These two expressions are utilized to predict CPand Yssof Typhoon Morakot in 2009,and the results are compared with those from simulation by using the *** result of comparison shows there is a good capability in *** the watersheds where it is necessary to predict CPand Yssof a storm event for the benefit of effective operation of water resource facilities,the aforesaid rainfall characteristics factors can be utilized to establish applicable models for prediction.

首先综述了处理低速稀薄气体流动的一些方法：线化Boltzmann方程方法、Lattice Boltzmann方法(LBM)、加滑移边界的Navier-Stokes方程、以及DSMC方法,并讨论它们在模拟MEMS中过渡领域低速流动特别是内部流动所遇到的困难,其中表明了LBM现有方案不适合模拟过渡领域中的MEMS流动问题。信息保存(ip)法通过保存一个模拟分子所代表的大量分子的平均信息,克服了流速低使得信息噪声比小而引起统计模拟的困难。本文给出了方法的一些理论证实。MEMS中内部流动的特点,即流速低和大的长宽比的特点,引起椭圆性问题,即出入口边界条件相互影响需要协调的问题。通过对(长约几千微米的)微槽道流动应用ip方法的算例,演示了采用守恒形式的质最守恒方程和超松弛法可成功地解决这一问题。借助同样的方法,用ip方法求解了真实长度(1000μm)硬盘驱动器读写头在过渡领域的薄膜支撑问题,压力分布与具有严格气体动理论基础的概括化Reynolds方程完全相符,而在此之前,DSMC方法只对短的读写头(5μm)与Reynolds方程做了校验．作者建议将原来用于求解读写头润滑问题的Reynolds方程退化来求解过渡领域中的微槽道流动问题,从而提供了一个有严格气体动理论品性的检验方法来验证求解MEMS内部流动的各种方法。

EKLF is an erythroid-specific,zinc finger-containing transcription factor essential for the activation of the mamma-lian beta globin gene in erythroid cells of definitive *** have prepared a polyclonal anti-mouse EKLF antibodysuitable for Western blotting and immunoprecipitation(ip)qualities,and used it to define the expression patterns of theEKLF protein during mouse erythroid *** have also used this antibody for the chromatin-immunoprecipi-tation(Chip)*** was found to bind in vivo at both the mouse beta-major-globin promoter and the HS2 site ofbeta-LCR in the mouse erythroleukemia cells(MEL)in a DMSO-inducible *** DMSO-induced bindings ofEKLF as well as three other proteins,namely,RNA polymerase Ⅱ,acetylated histone H3,and methylated histone H3,were not abolished but significantly lowered in CB3,a MEL-derived cell line with null-expression of p45/NF-E2,anerythroid-enriched factor needed for activation of the mammalian globin ***,binding of EKLF in vivowas also detected in the mouse alpha-like globin locus,at the adult alpha globin promoter and its far upstream regula-tory element alpha-MRE(HS26).This study provides direct evidence for EKLF-binding in vivo at the major regulatoryelements of the mouse beta-like globin gene clusters the data also have interesting implications with respect to the roleof EKLF-chromatin interaction in mammalian globin gene regulation.

不同形状微尺度管道(圆形、六边形、半圆形、不同宽高比的矩形)中的气体流动特性是微机电系统设计最为关心的问题之一．文中利用信息保存(ip)方法和直接模拟Monte Carlo(DSMC)方法进行研究,给出两种方法的计算结果相互符合,并与其它研究者的BGK模型方程计算结果进行了比较．对于微尺度管道中关心的低Mach数流动, ip方法的统计收敛效率明显优于DSMC方法．通过拟合ip和DSMC结果,给出了圆形、六边形、半圆形、不同宽高比的矩形截面情况下无量纲质量流率与等效Knudsen数的关系．

A new heterogeneous catalyst composed of Pd nanoparticles immobilized within a HypoG el resin has been prepared in the absence of any ligands using an extensive cross-linking *** newly developed nanocatalyst was characterized by N_2 adsorption-desorption,X-ray diffraction(XRD),transmission electron microscopy(TEM),scanning electron microscopy(SEM),energy dispersive X-ray(EDX),Fourier transform infrared spectroscopy and inductively coupled plasma-mass spectrometer(ICP-MS)*** and XRD results revealed that the Pd nanoparticles were well dispersed with diameters in the range of 4–12 nm,and an average size of about 8 *** cross-linked Pd catalyst demonstrated excellent catalytic activity towards the synthesis of a series of biaryl compounds by the reaction of various aryl halides(e.g.,bromides andiodides)with phenylboronic acid in the presence of tetrabutylammonium ***-MS analysis indicated that there was only 0.25%weight loss of Pd(0.55±0.02 ppm)from the supported catalyst after the first cycle ***,the catalyst showed excellent reusability(up to five uses)with consistently high levels of catalytic activity following its recovery by filtration.
