Parathyroidectomy restored bone mineral density in a neglected femoral neck fracture with renal osteodystrophy:A case report

BACKGROUND This case report contributes to the medical literature by highlighting the successful management of a neglected femoral neck fracture in a patient with renal osteodystrophy and secondary hyperparathyroidism(SHPTH)who was on dialysis due to end-stage renal disease(ESRD).It underscores the efficacy of parathyroidectomy(PTX)in restoring bone mineral density(BMD)and promoting fracture healing,addressing a significant complication in ESRD *** SUMMARY A 36-year-old female with renal osteodystrophy and on dialysis due to ESRD presented with a history of left patellar tendon rupture and later,a right achilles tendon avulsion *** right hip pain led to the discovery of a neglected right femoral neck fracture,which was initially overlooked due to the patient’s complex medical *** months post-achilles tendon repair,the patient underwent PTX to manage the refractory *** postoperative course included rehabilitation and weight-bearing ***,2 years after osteosynthesis,radiographic assessments indicated a solid union of the periprothesis fracture and significant improvement in BMD,showcasing the efficacy of the treatment *** PTX,combined with appropriate rehabilitation,is crucial for improving BMD and fracture healing in ESRD patients with SHPTH.

This study presents novel findings on the potential of phloretin， an apple polyphenol， to enhance the effectiveness of anti-HER2 antibody therapy in HER2-positive breast cancer patients. Our research reveals that phloretin inhibits type II glucose transporter (GLUT2) activity， significantly reducing cancer cell glucose uptake. We confirmed the overexpression of GLUT1 and GLUT2 mRNA in paired human breast tumor tissues， with GLUT2 overexpression associated explicitly with poorer survival rates in breast cancer patients. Treatment with phloretin was observed to increase the interaction between GLUT2 and HER2 proteins， attenuate glycolysis， and enhance the binding affinity of anti-HER2 antibody drugs to target human breast cancer cells. Furthermore， the efficacy of the combination therapy involving phloretin and antibody drugs was reaffirmed in a cell-derived xenograft (CDX) tumor animal model， particularly in suppressing the growth of Trastuzumab-resistant HER2+ breast cancer. These significant findings suggest that targeting GLUT2 activity with phloretin in combination with anti-HER2 antibody drugs may help mitigate the development of drug-resistant breast cancer， offering valuable insights for enhancing tumor treatment strategies and contributing to developing more effective therapies.

代码复用攻击是控制流安全面临的主要威胁之一.虽然地址分布随机化能够缓解该攻击,但它们很容易被代码探测技术绕过.相比之下,控制流完整性方法具有更好的保护效果.但是,现有的方法要么依赖于源码分析,要么采用无差别跟踪的方式追踪所有的控制流转移.前者无法摆脱对源码的依赖性,后者则会引入巨大的运行时开销.针对上述问题,本文提出一种新的控制流保护方法MCE(Micro Code Extraction).MCE的保护目标是源码不可用的闭源对象.与现有的方法相比,MCE并不会盲目地追踪所有的控制流转移活动.它实时地检测代码探测活动,并仅将被探测的代码作为保护目标.之后,MCE抽取具有潜在风险的代码片段,以进一步缩小目标对象的大小.最后,所有跳转到风险代码中的控制流都会被追踪和检测,以保护它的合法性.实验和分析表明,MCE对代码探测和代码复用攻击具有良好的保护效果,并在一般场景下仅对CPU引入2%的开销.

Ultrasound-assisted isolation: A new method to isolate stromal vascular fraction

Background: The stromal vascular fraction(SVF), a cluster of stem and progenitor cells isolated from adipose tissue, holds significant promise for application in regenerative medicine. However, the existing methods for SVF isolation are time-consuming and expensive. Thus, in this study, we explored a new method of SVF extraction—ultrasound-assisted SVF isolation(USASI)—and compared the viability and characteristics of SVF isolated using different ***: SVF extraction methods using different combinations of ultrasound power, ultrasound time, collagenase dosage, and collagenase digestion time were compared with those of the control group(collagenase digestion method). The cell yield and vitality of the SVF were evaluated via cell counting and trypan blue staining. The cell components and immunophenotypes of freshly isolated SVF were analyzed using flow cytometry. The proliferative capacity and differentiation potential of the SVF were also ***: Ultrasonication at 95 W-20 k Hz for 30 s followed by digestion with 0.15% collagenase for 30 min was identified as the most suitable parameter for the USASI method in isolating SVF, as recommended based on the evaluation of various tested conditions. The USASI method significantly reduced the collagenase dosage and shortened the digestion time. Compared to the collagenase digestion method, the USASI method had a higher cell yield and cell viability, with no adverse effects on cell components, proliferative capacity, or multipotential differentiation ***: With reduced processing time, lower collagenase dosage, and increased cell yield without impairing the viability and characteristics of SVF, USASI holds the potential to emerge as a time-saving and cost-effective method for future clinical applications.

Successful management of delirium with dexmedetomidine in a patient with haloperidol-induced neuroleptic malignant syndrome:A case report

BACKGROUND We report a case of lorazepam-induced agitated delirium treated with haloperidol,which in turn triggered the onset of neuroleptic malignant syndrome(NMS).The latter condition,a medical emergency,was effectively treated with medical treatment and dexmedetomidine,a versatile and highly selective shortacting alpha-2 adrenergic agonist with sedative-hypnotic and anxiolytic *** SUMMARY A 65-year-old man with a history of bipolar disorder presented to the emergency department with severe abdominal discomfort after binge *** his hospital stay,he received intravenous lorazepam for *** the next day,he became delirious and was thus treated with seven doses(5 mg each)of haloperidol over a 48 h *** of NMS(hyperthermia,rigidity,myoclonus of upper limbs,impaired consciousness,tachypnea,and dark urine)became apparent and haloperidol was immediately suspended and brisk diuresis was *** intensive care unit admission,he was confused,disoriented,and markedly *** infusion was started with the goal of achieving a Richmond Agitation-Sedation Scale score of-1 or *** was resolved gradually and the patient stabilized,permitting discontinuation of dexmedetomidine after 3 *** Dexmedetomidine may be clinically helpful for the management of NMS,most likely because of its sympatholytic activity.

The controversial outcomes in patients with metastatic colorectal cancer(mCRC)highlight the need for developing effective systemic neoadjuvant treatment strategies to improve clinical *** optimal treatment cycles in patients with mCRC for metastasectomy remain undeﬁ*** retrospective study compared the efﬁcacy,safety,and survival of cycles of neoadjuvant chemotherapy/targeted therapy for such ***-four patients with mCRC who received neoadjuvant chemotherapy/targeted therapy following metastasectomy were enrolled between January 2018 and April ***-eight patients received 6 cycles of chemotherapy/targeted therapy,whereas 36 patients received≥7 cycles(median,13;range,7–20).Clinical outcomes,including response,progression-free survival(PFS),overall survival(OS),and adverse events,were compared between these two *** the 64 patients,47(73.4%)were included in the response group,and 17(26.6%)were included in the nonresponse *** analysis revealed chemotherapy/targeted therapy cycle and pretreatment serum carcinoembryonic antigen(CEA)level as independent predictors of the response as well as overall survival and chemotherapy/targeted therapy cycle as an independent predictor of progression(all p0.05).The median OS and PFS were 48 months(95%CI,40.855–55.145)and 28 months(95%CI,18.952–37.48)in the≥7-cycle group and 24 months(95%CI,22.038–25.962)and 13 months(95%CI,11.674–14.326)in the 6-cycle group,respectively(both p<0.001).The oncological outcomes in the≥7-cycle group were signiﬁcantly better than those in the 6-cycle group,without signiﬁcant increases in adverse ***,prospective randomized trials are mand
