BACKGROUND Allergic bronchopulmonary aspergillosis(ABPA)is an allergic reaction to Aspergillus species that aggravates bronchial *** studies demonstrated the glucocorticoid-sparing effect of dupilumab in patients with *** is no report of complete withdrawal of glucocorticoids after *** SUMMARY The patient was a 54-year-old woman with bronchial asthma treated with inhaled corticosteroids and a long-acting beta-2 *** consulted our institution for productive cough and fever in March *** computed tomography scan revealed mucoid impaction,and the bronchial lavage fluid culture was positive for Aspergillus *** diagnosis was *** patient was treated with oral glucocorticoids from April 2017 to November *** January 2019,she had bronchial asthma exacerbation,and a chest computed tomography scan showed recurrent mucoid *** was treated with oral glucocorticoids and *** February 2020,during tapering of oral glucocorticoid,she had the third episode of bronchial asthma exacerbation and a mucoid *** patient was treated with dupilumab in addition to oral glucocorticoid and *** clinical response improved,and oral glucocorticoid was discontinued in June *** This is the first case of ABPA in which complete withdrawal of glucocorticoid was possible after treatment with dupilumab.

Obstructive and secretory complications of diverting ileostomy

This review aimed to highlight the etiology,diagnosis,treatment,and prevention of obstructive and secretory complications associated with diverting ileostomy(DI).Obstructive complications at the stoma site are termed stoma outlet obstruction(SOO)or stoma-related obstruction(SRO).The incidence of SOO/SRO is 5.4%-27.3%,and the risk factors are multifactorial;however,the configuration of the stoma limb and the thickness of the rectus abdominis muscle(RAM)may be of particular ***-stomal tube decompression is initially attempted with a success rate of 33%-86%.A thick RAM may carry the risk of *** refinement,including a wider incision of the anterior sheath and adequate stoma limb length,avoids tension and immobility and may decrease SOO/*** complications of DI are termed high output stoma(HOS).Persistent HOS lead to water and sodium depletion,and secondary hyperaldosteronism,resulting in electrolyte imbalances,such as *** incidence of HOS is 14%-24%,with an output of 1000-2000 mL/d lasting up to three *** of HOS is commenced after excluding postoperative complications or enteritis and includes fluid intake restriction,antimotility and antisecretory drug therapies,and magnesium *** monitoring and surveillance programs have been successful in decreasing readmissions for dehydration.

Hydrogen evolution was detected in an artificial system composed of light-harvesting unit of purified photosystem I, catalyst of hydrogenase, methyl viologen and electron donor under radiation. Absorption spectral features confirmed that electron transfer from electron donors to proton was via a photoinduced reductive process of methyl viologen.

Oncologic impact of colonic stents for obstructive left-sided colon cancer

Colonic stenting has had a significant positive impact on the management of obstructive left-sided colon cancer(OLCC) in terms of both palliative treatment and bridge-to-surgery(BTS). Notably, many studies have convincingly demonstrated the effectiveness of stenting as a BTS, resulting in improvements in shortterm outcomes and quality of life, safety, and efficacy in subsequent curative surgery, and increased cost-effectiveness, whereas the safety of chemotherapy after stenting and the long-term outcomes of stenting as a BTS are controversial. Several studies have suggested an increased risk of perforation in patients receiving bevacizumab chemotherapy after colonic stenting. In addition, several pathological analyses have suggested a negative oncological impact of colonic stenting. In contrast, many recent studies have demonstrated that colonic stenting for OLCC does not negatively impact the safety of chemotherapy or long-term oncological outcomes. The updated version of the European Society of Gastrointestinal Endoscopy guidelines released in 2020 included colonic stenting as a BTS for OLCC as a recommended treatment. It should be noted that the experience of endoscopists is involved in determining technical and clinical success rates and possibly oncological outcomes. This review discusses the positive and negative impacts of colonic stenting on OLCC treatment, particularly in terms of oncology.

二战后的北九州市成功克服公害,积累了丰富经验和技术,并活用这些经验推进重视环境保护的城市发展战略,建树丰硕、影响深远,在环保领域占领着日本国内无庸置疑的先驱地位。本文旨在分析高速成长期之后的北九州的环境政策的演变,列举其中代表性的措施揭示其特点,并明确今后的努力方向。从国家层面来看,战后日本环境政策可分为三个阶段,各自有不同的环境政策焦点:从公害治理对策到综合环境保全、提高环境舒适度,再到关注地球环境问题、努力建设可持续发展社会。北九州市的环境政策变迁大致遵循了相同的轨迹。本文讨论了克服公害的"北九州模式"、环境国际合作的积极推进、"生态工业园区"工程、"环境示范都市"工程等主要环保措施,阐明其中的历史背景及发展过程,并进一步探讨以环境关联产业为杠杆振兴地方经济、发展环保产业、开拓国际市场的可能,提出了今后需要致力发展的方向。

近期，由于增加烧结矿石中褐铁矿的配比，导致烧结料层透气性和烧结矿成品率降低。一般认为，虽然烧结料层是多孔的结构，但烧结过程中料层中孔隙的崩溃导致这一系列问题。一些方法如通过在烧结混合料中插入棒条等松料器控制空隙率，改善料层透气性。但却带来安装和维护费用等问题。因此，对通过改变原料颗粒粒度控制料层结构，或调节局部料层的松散密实程度等措施进行了讨论。本文中，进行了烧结混合料中配加大颗粒料改善料层透气性的烧结杯实验。并讨论了该烧结混合料的烧结行。研究发现：粉料中配加大颗粒料后，粉料部分料层的堆密度降低。因此透气性增大，烧结垂速加快。同时，增加焦粉配比可以保持烧结矿成品率维持不变。结果，烧结产能大幅度提高。

Diversity of Microflora in Colonic Mucus from Severe Ulcerative Colitis Patients Analyzed by Terminal Restriction Fragment Length Polymorphism and Clone Libraries of Bacterial 16S rRNA Gene Sequences

Although the gut microflora is thought to be an essential factor in the development of ulcerative colitis (UC), the entire gut microflora occurring in UC remains unknown. Most studies use feces to represent the microflora distribution;however, here we analyzed the bacterial diversity in colonic mucus from UC patients receiving colectomy surgery and control patients. The diversity of microflora was investigated using a combination of terminal restriction fragment length polymorphism (T-RFLP) and clone library analyses of the 16S rRNA gene sequences. In the T-RFLP analysis, the number of terminal restriction fragments (T-RFs) decreased significantly in UC patients when compared to control samples. Also in the clone library analysis, the number of operational taxonomic units (OTU) and the Shannon diversity index were reduced significantly in UC patients. These molecular analyses reveal an overall dysbiosis in UC patients. No specific pathogen was found, and a strong negative correlation in relative abundance of bacterial populations was observed between the phyla Bacteroidetes and Firmicutes in the UC patients. This is the first report showing a significant correlation between these two phyla, which may be important characteristics in the pathogenesis of UC.

Functional MRI (fMRI) is recognized as a well-established non-invasive diagnostic method to image activating cortical areas. Currently, the blood oxygenation level dependent (BOLD) contrast method is widely used in fMRI BOLD-fMRI images activating areas by detecting a reduced concentration of deoxyhemoglobin during neuronal activity, which is caused by a larger increase in O2 delivery compared with O2 consumption in normal adults. In the present study, near infrared spectroscopy, which is an optical method to evaluate cerebral blood oxygenation changes, has demonstrated an increase of deoxyhemoglobin associated with increases of oxyhemoglobin and total hemoglobin at activation areas of stroke patients, whereas BOLD-fMRI failed to image the activation areas. The present findings have serious applications for the application of BOLD-fMRI to the patients with brain disorders because BOLD-fMRI may overlook neuronal activities in these patients unless both increases and decreases of signals are considered.
