Clinical Phenotype of Japanese Patients with Dermatomyositis—Classification Based on Dermatomyositis-Specific Autoantibodies

Objectives: To correlate the precise specificity of autoantibodies in Japanese dermatomyositis (DM) patients with their clinical phenotypes. Methods: Serum samples from 94 adult DM patients (67 with classical DM and 27 with clinically amyopathic dermatomyositis, CADM) were screened for autoantibodies using immunoprecipitation assays. Patients with antibodies against aminoacyl transfer RNA synthetase (ARS), Mi-2 or who had other autoantibodies were assessed for clinical symptoms and laboratory findings. Results: Sera from 27 of 94 DM patients (29%) were found to have anti-ARS antibodies. Nineteen (20%) had anti-CADM-140/MDA5, 5 (5%) had anti-Mi-2, and 8 (6%) had anti-p155/TIF1-γ. Anti-MJ/NXP-2 was not found in our series of adult DM. Seventeen patients with anti-ARS had fever and 22 had arthritis and interstitial lung disease (ILD), compatible with a diagnosis of anti-ARS syndrome. Seventeen of 19 (89%) with anti-CADM-140/MDA5 had ILD, 16 (84%) of whom developed rapidly progressive ILD (RP-ILD). Four of 5 (80%) with anti-Mi-2 had heliotrope rash and/or Gottron’s sign/papules, and 2 (40%) had V-sign and/or shawl-sign rash, whereas no ILD or malignancy was detected. As seen with anti-Mi-2-positive patients, a low frequency of ILD (13%) was found in patients with anti-p155/TIF1-γ but 6 of 8 (75%) had malignancy during their course. The frequency of ILD was significantly higher in patients with anti-ARS or anti-CADM-140/MDA5 compared with anti-Mi-2 or anti-p155/TIF1-γ (81% and 89%, respectively). It should be noted that anti-CADM-140/MDA5-positive patients suffered significantly more RP-ILD compared to patients with anti-ARS (84% vs. 7%, P < 0.0001). On the other hand, anti-p155/TIF1-γ positive patients had a significantly higher rate of malignancy compared with anti-ARS-, anti-CADM-140/MDA5-and anti-Mi-2-positive patients (75% vs. 7%: P = 0.0004, 5%: P = 0.0006, 0%: P = 0.02, respectively). Influence of contamination on banded iron formations in the Isua supracrustal belt, West Greenland: Reevaluation of the Eoarchean seawater compositions

Banded Iron Formations（BIFs） are chemical sediments, ubiquitously distributed in the Precambrian supracrustal belts; thus their trace element compositions are helpful for deciphering geochemical evolution on the Earth through time. However, it is necessary to elucidate factors controlling the whole-rock compositions in order to decode the ancient seawater compositions because their compositions are highly variable. We analyzed major and trace element contents of the BIFs in the 3.8-3.7 Ga Isua supracrustal belt（ISB）, southern West Greenland. The BIFs are petrographically classified into four types：Black-,Gray-, Green-and White-types, respectively. The Green-type BIFs contain more amphiboles, and are significantly enriched in Co, Ni, Cu, Zn, Y, heavy rare earth element（HREE） and U contents. However,their bulk compositions are not suitable for estimate of seawater composition because the enrichment was caused by secondary mobility of metamorphic Mg, Ca and Si-rich fluid, involvement of carbonate minerals and silicate minerals of olivine and pyroxene and/or later silicification or contamination of volcanic and clastic materials. The White-type BIFs are predominant in quartz, and have lower transition element and REE contents. The Gray-type BIFs contain both quartz and magnetite. The Black-type BIFs are dominated by magnetite, and contain moderate to high transition element and REE contents. But,positive correlations of V, Ni, Zn and U contents with Zr contents suggest that involvement of detrital,volcanic and exhalative materials influences on their contents. The evidence for significant influence of the materials on the transition element contents such as Ni in the BIFs indicates the transition element contents in the Archean ocean were much lower than previously estimated. Solving the electronic structure problem with machine learning

Simulations based on solving the Kohn-Sham(KS)equation of density functional theory(DFT)have become a vital component of modern materials and chemical sciences research and development *** its versatility,routine DFT calculations are usually limited to a few hundred atoms due to the computational bottleneck posed by the KS *** we introduce a machine-learning-based scheme to efficiently assimilate the function of the KS equation,and by-pass it to directly,rapidly,and accurately predict the electronic structure of a material or a molecule,given just its atomic configuration.A new rotationally invariant representation is utilized to map the atomic environment around a grid-point to the electron density and local density of states at that *** mapping is learned using a neural network trained on previously generated reference DFT results at millions of *** proposed paradigm allows for the high-fidelity emulation of KS DFT,but orders of magnitude faster than the direct ***,the machine learning prediction scheme is strictly linear-scaling with system size.

Boosting overall water splitting by incorporating sulfur into NiFe(oxy)hydroxide

Fulminant hepatitis is a serious, life-threatening disorder and is associated with inflammatory cytokines produced by Kupffer cells. However, a number of clinical trials for the treatment of fulminant hepatitis did not show enough substantial benefits. Annual Component Analysis of Pacific Oysters （Crassostrea gigas） and Mussels （Mytilus gallopro vincialis）

To study the change in components of Pacific oysters （Crassostrea gigas） and mussels （Mytilus galloprovincialis） over a year, their annual obesity ratio, umami components （glutamic acid and glycogen）, functional components （Zn, Mn, Cu, and vitamin B12）, and Cd and noroviruses （NoVs） contents were quantified. In oysters, the obesity ratio and chemical components showed seasonal changes depending on the spawning season. Glutamic acid content increased from November to March. In contrast, glycogen content peaked in May and November. Mineral content markedly increased during spawning. From the comparison of NoVs GI and GII strains, oyster predominately infected by the GI. A remarkable difference in NoVs contents was observed depending on the oyster farms. In mussels, glutamic acid, Zn and Cu contents were stable throughout the year. Mussels＇ Cd content was about one-fifths times lower than that found in oysters. Although the NoVs strain was affected by the collection area, mussels from both areas showed decreased NoVs contents in summer, corresponding with the consumption season of mussels in Japan. Our comparative study on the seasonal variability in the chemical components and NoVs contents in pacific oysters （C. gigas） and mussels （M. galloprovincialis） clearly showed an association with physiological and environmental factors.

Effects of Denture Removal on Expiratory Upper Airway Patency during Mask Ventilation in Complete Denture Wearers under General Anesthesia

Background: Some anesthetized patients present with expiratory airway obstruction (EAO) during mask ventilation (MV). EAO may occur more frequently among edentulous patients for whom a firmer grip of the mask and the mandible is needed to avoid gas leakage. We compared EAO grades before and after denture removal and identified predictors of EAO during MV in edentulous patients. Methods: One hundred and eight denture-wearing edentulous patients were anesthetized, their mouths were wrapped with film, their nasal masks and mandibles were held with two hands, and their lungs were ventilated in a pressure-controlled mode. Their dentures were subsequently removed, mouths were re-wrapped, and nasal MV was reapplied. EAO was graded based on capnography waveforms as follows: Grade I, normal waveform (no obstruction);II, loss of the alveolar plateau of the waveform (partial obstruction);and III, no waveform (total obstruction). EAO grades were compared before and after denture removal. Predictive demographic variables for occurrence of total EAO were identified using a multivariate analysis. Results: The proportions of Grades I, II, and III were 43.5%, 26.9%, and 29.6% when dentures were worn and 35.2%, 15.7%, and 49.1%, respectively, when dentures were removed. Individual Income Status Correlates with Chronic Kidney Disease in Japan beyond Metabolic Risk Factors: Cross Sectional Study

Background: Chronic kidney disease (CKD) is a pre-condition for end-stage kidney disease (ESKD) undergoing hemodialysis, as well as a risk factor for cardiovascular disease. Therefore, it is vital that CKD prevention measures be established. In particular, socioeconomic status (SES), as represented by income, contributes to non-communicable diseases like CKD. The purpose of this study was to examine the independent association between CKD and individual SES in Japan, with consideration of other metabolic risks. Methods: In the present study, we analyzed the 2011 National Health and Nutrition Survey by the Ministry of Health, Labor, and Welfare. Specifically, we analyzed 3,557 people out of 8,762 whose serum creatinine was measured. Logistic regression analysis was used to calculate the odds ratio (OR) of CKD by income distinction, after adjustment for age and metabolic risks (obesity, diabetes mellitus, dyslipidemia, and hypertension), which were assessed at the medical examination. Results: CKD was found in 385 participants (10.8%) and was associated with greater age, obesity, higher levels of low-density lipoprotein cholesterol, triglyceride, and glycated hemoglobin, and lower levels of hemoglobin and high-density lipoprotein cholesterol. The adjusted model indicated a significant association between lower income and CKD: the OR of the low income group (<2 million yen) was 1.33 (95% confidence interval: 1.01 - 1.78) in comparison with the high income group. Conclusions: The results of the present study indicate a substantial relationship between individual lower income status and CKD in Japan, where healthcare is easy to access. To prevent ESKD and cardiovascular diseases, early detection of CKD and its metabolic risks is necessary, especially among the socioeconomically vulnerable population.
