Nomogram to predict permanent stoma in rectal cancer patients after sphincter-saving surgery

BACKGROUND Approximately 20 percent of patients with a tumour localized in the low rectum still encounter the possibility of requiring permanent stoma(PS), which can cause drastic changes in lifestyle and physical perceptions.AIM To determine the risk factors for PS and to develop a prediction model to predict the probability of PS in rectal cancer patients after sphincter-saving surgery.METHODS A retrospective cohort of 421 rectal cancer patients who underwent radical surgery at Taipei Medical University Hospital between January 2012 and December 2020 was included in this study. Univariate and multivariate analyses were performed to identify the independent risk factors for PS. A nomogram was developed according to the independent risk factors obtained in the multivariate analysis. The performance of the nomogram was assessed using a receiver operating characteristic curve and a calibration curve.RESULTS The PS rate after sphincter-saving surgery was 15.1%(59/391) in our study after a median follow-up of 47.3 mo(range 7–114 mo). Multivariate logistic regression analysis demonstrated that local recurrence, perirectal abscess, anastomosis site stenosis, perineural invasion, tumor size and operative time were independent risk factors for PS. These identified risk factors were incorporated into the nomogram, and the concordance index of this model was 0.903(95%CI: 0.851-0.955). According to the calibration curves, the nomogram represents a perfect prediction model.CONCLUSION Several risk factors for PS after sphincter-saving surgery were identified. Our nomogram exhibited perfect predictive ability and will improve a physician’s ability to communicate the benefits and risks of various treatment options in shared decision making.

Multi-Mesh-Scale Approximation of Thin Geophysical Mass Flows on Complex Topographies

This paper is devoted to a multi-mesh-scale approach for describing the dynamic behaviors of thin geophysical mass flows on complex topographies.Because the topographic surfaces are generally non-trivially curved,we introduce an appropriate local coordinate system for describing the flow behaviors in an efficient way.The complex surfaces are supposed to be composed of a finite number of triangle elements.Due to the unequal orientation of the triangular elements,the distinct flux directions add to the complexity of solving the Riemann problems at the boundaries of the triangular elements.Hence,a vertex-centered cell system is introduced for computing the evolution of the physical quantities,where the cell boundaries lie within the triangles and the conventional Riemann solvers can be applied.Consequently,there are two mesh scales:the element scale for the local topographic mapping and the vertex-centered cell scale for the evolution of the physical quantities.The final scheme is completed by employing the HLL-approach for computing the numerical flux at the interfaces.Three numerical examples and one application to a large-scale landslide are conducted to examine the performance of the proposed approach as well as to illustrate its capability in describing the shallow flows on complex topographies.

Gas Kinetic Scheme for Anisotropic Savage-Hutter Model

The gas-kinetic scheme is applied to a depth-integrated continuum model for avalanche flows,namely the Savage-Hutter model.In this method,the continuum fluxes are calculated based on the pseudo particle motions which are relaxed from nonequilibrium to equilibrium states.The processes are described by the Bhatnagar-GrossKrook(BGK)equation.The benefit of this scheme is its capability to resolve shock discontinuities sharply and to handle the vacuum state without special treatments.Because the Savage-Hutter equation bears an anisotropic stress on the tangential space of the topography,the equilibrium distribution function of the microscopic particles are shown to be bi-Maxwellian.These anisotropic stresses are the key to preserve the coordinate objectivity in the Savage-Hutter model.The effect of the anisotropic stress is illustrated by two examples:an axisymmetric dam break and a finite mass sliding on an inclined plane chute.It is found that the propagation of the flow fronts significantly depends on the orientation of the principal axes of the tangential stresses.

Optimal Shape Factor and Fictitious Radius in the MQ-RBF:Solving Ill-Posed Laplacian Problems

To solve the Laplacian problems,we adopt a meshless method with the multiquadric radial basis function(MQRBF)as a basis whose center is distributed inside a circle with a fictitious radius.A maximal projection technique is developed to identify the optimal shape factor and fictitious radius by minimizing a merit function.A sample function is interpolated by theMQ-RBF to provide a trial coefficient vector to compute the merit function.We can quickly determine the optimal values of the parameters within a preferred rage using the golden section search algorithm.The novel method provides the optimal values of parameters and,hence,an optimal MQ-RBF;the performance of the method is validated in numerical examples.Moreover,nonharmonic problems are transformed to the Poisson equation endowed with a homogeneous boundary condition;this can overcome the problem of these problems being ill-posed.The optimal MQ-RBF is extremely accurate.We further propose a novel optimal polynomial method to solve the nonharmonic problems,which achieves high precision up to an order of 10^(−11).

Variations in the dynein axonemal heavy chain gene,dynein axonemal heavy chain 6(DNAH6),lead to multiple morphological abnormalities of the flagella.Recent studies have reported that these deficiencies may result in sperm head deformation.However,whether DNAH6 is also involved in human acrosome biogenesis remains unknown.The purpose of this study was to investigate DNAH6 gene variants and their potential functions in the formation of defective sperm heads and flagella.Whole-exome sequencing was performed on a cohort of 375 patients with asthenoteratozoospermia from the First Affiliated Hospital of Anhui Medical University(Hefei,China).Hematoxylin and eosin staining,scanning electron microscopy,and transmission electron microscopy were performed to analyze the sperm morphology and ultrastructure.Immunofluorescence staining and Western blot analysis were conducted to examine the effects of genetic variants.We identified three novel deleterious variants in DNAH6 among three unrelated families.The absence of inner dynein arms and radial spokes was observed in the sperm of patients with DNAH6 variants.Additionally,deficiencies in the acrosome,abnormal chromatin compaction,and vacuole-containing sperm heads were observed in these patients with DNAH6 variants.The decreased levels of the component proteins in these defective structures were further confirmed in sperm from patients with DNAH6 variants using Western blot.After intracytoplasmic sperm injection(ICSl)treatment,the partner of one patient with a DNAH6 variant achieved successful pregnancy.Overall,novel variants in DNAH6 genes that contribute to defects in the sperm head and flagella were identified,and the findings indicated Icsl as an effective clinical treatment for such patients.

PVDF/6H-SiC composite fiber films with enhanced piezoelectric performance by interfacial engineering for diversified applications

Piezoelectric silicon carbide(SiC)has been quite attractive due to its superior chemical and physical properties as well as wide potential applications.However,the inherent brittleness and unsatisfactory piezoelectric response of piezoelectric semiconductors remain the major obstacles to their diversified applications.Here,flexible multifunctional PVDF/6H-SiC composite fiber films are fabricated and utilized to assemble both piezoelectric nanogenerators(PENGs)and stress/temperature/light sensors.The open cir-cuit voltage(V_(oc))and the density of short circuit current(I_(sc))of the PENG based on the PVDF/5 wt%6H-SiC composite fiber films reach 28.94 V and 0.24μA cm^(-2),showing a significant improvement of 240%and 300%compared with that based on the pure PVDF films.The effect of 6H-SiC nanoparticles(NPs)on inducing interfacial polarization and stress concentration in composite fiber films is proved by first-principles calculation and finite element analysis.The stress/temperature/light sensors based on the composite fiber film also show high sensitivity to the corresponding stimuli.This study shows that the PVDF/6H-SiC composite fiber film is a promising candidate for assembling high-performance energy harvesters and diverse sensors.

One-step conversion of methane and formaldehyde to ethanol over SA-FLP dual-active-site catalysts: A DFT study

One-step conversion of methane and formaldehyde into ethanol is a 100% atom-efficient process for carbon resources utilization and environment protection but still faces eminent challenges due to the lacking of efficient catalysts. Therefore, developing active and stable catalysts is crucial for the co-conversion of methane and formaldehyde. Herein, twelve kinds of “Single-Atom”-“Frustrated Lewis Pair”(SA-FLP)dual-active-site catalysts are designed for the direct conversion of methane and formaldehyde to ethanol based on density functional theory(DFT) calculations and microkinetic simulations. The results show that the SA-FLP dual active sites can simultaneously activate methane at the SA site and activate formaldehyde at the FLP site. Among the twelve designed SA-FLP catalysts, Fe1-FLP shows the best performance in the co-conversion of methane and formaldehyde to ethanol with the rate-determining barrier of 1.15 e V.Ethanol is proved as the main product with the turnover frequency of 1.32 × 10^(-4)s^(-1)at 573 K and 3 bar.This work provides a universal strategy to design dual active sites on metal oxide materials and offers new insights into the effective conversion of methane and formaldehyde to desired C_(2) chemicals.

Hepatocellular carcinoma(HCC)is a leading cause of death worldwide.Current therapies are effective for HCC patients with early disease,but many patients suffer recurrence after surgery and have a poor response to chemotherapy.Therefore,new therapeutic targets are needed.We analyzed gene expression profiles between HCC tissues and normal adjacent tissues from public databases and found that the expression of genes involved in lipid metabolism was significantly different.The analysis showed that AKR1C3 was upregulated in tumors,and high AKR1C3 expression was associated with a poorer prognosis in HCC patients.In vitro,assays demonstrated that the knockdown of AKR1C3 or the addition of the AKR1C3 inhibitor indomethacin suppressed the growth and colony formation of HCC cell lines.Knockdown of AKR1C3 in Huh7 cells reduced tumor growth in vivo.To explore the mechanism,we performed pathway enrichment analysis,and the results linked the expression of AKR1C3 with prostaglandin F2 alpha(PGF2a)downstream target genes.Suppression of AKR1C3 activity reduced the production of PGF2a,and supplementation with PGF2a restored the growth of indomethacin-treated Huh7 cells.Knockdown of the PGF receptor(PTGFR)and treatment with a PTGFR inhibitor significantly reduced HCC growth.We showed that indomethacin potentiated the sensitivity of Huh7 cells to sorafenib.In summary,our results indicate that AKR1C3 upregulation may promote HCC growth by promoting the production of PGF2α,and suppression of PTGFR limited HCC growth.Therefore,targeting the AKR1C3-PGF2a-PTGFR axis may be a new strategy for the treatment of HCC.

Constrained reinforcement learning with statewise projection: a control barrier function approach

Safety is a critical issue for reinforcement learning(RL), as it may be risky for some actual applications if the learning process involves unsafe exploration. Instead of formulating constraints as expectationbased in constrained RL, considering statewise safety in constrained RL is more meaningful. This work aims to address the issue of safe projection in RL by introducing a control barrier function that inherently learns a safe policy through a set certificate. We seek to analyze some theoretical properties of safe projection in the learning process, including convergence and performance bound, and extend the discussion into ensembles and guided controllers. Moreover, we approach analytical solutions for deterministic and stochastic system dynamics. Experimental results in different tasks show that the proposed method achieves better effects in terms of both performance and safety.

Benchmark experiment on slab^(238)U with D-T neutrons for validation of evaluated nuclear data

A benchmark experiment on^(238)U slab samples was conducted using a deuterium-tritium neutron source at the China Institute of Atomic Energy.The leakage neutron spectra within energy levels of 0.8-16 MeV at 60°and 120°were measured using the time-of-flight method.The samples were prepared as rectangular slabs with a 30 cm square base and thicknesses of 3,6,and 9 cm.The leakage neutron spectra were also calculated using the MCNP-4C program based on the latest evaluated files of^(238)U evaluated neutron data from CENDL-3.2,ENDF/B-Ⅷ.0,JENDL-5.0,and JEFF-3.3.Based on the comparison,the deficiencies and improvements in^(238)U evaluated nuclear data were analyzed.The results showed the following.(1)The calculated results for CENDL-3.2 significantly overestimated the measurements in the energy interval of elastic scattering at 60°and 120°.(2)The calculated results of CENDL-3.2 overestimated the measurements in the energy interval of inelastic scattering at 120°.(3)The calculated results for CENDL-3.2 significantly overestimated the measurements in the 3-8.5 MeV energy interval at 60°and 120°.(4)The calculated results with JENDL-5.0 were generally consistent with the measurement results.
