BACKGROUND Based on the breakthrough of genomics analysis, The Cancer Genome Atlas Research Group recently proposed an integrative genomic analysis, dividing gastric cancer(GC) into four subtypes, characterized by the chromosomal instability(CIN) status. However, the CIN status of GC is still vaguely characterized and lacking the valuable easy-to-use CIN markers to diagnosis in molecular and histological *** To explore the associations of CIN with downstream lipidomics *** We collected cancerous and noncancerous tissue samples from 18 patients with GC; the samples were divided into CIN and non-CIN types based on the system of The Cancer Genome Atlas Research Group and 409 sequenced oncogenes and tumor suppressor genes. We identified the lipidomics profiles of the GC samples and samples of their adjacent noncancerous tissues by using liquid chromatography–mass spectrometry. Furthermore, we selected leading metabolites based on variable importance in projection scores of > 1.0 and P <*** Twelve men and six women participated in this study; the participants had a median age of 67.5 years(range, 52–87 years) and were divided into CIN(n = 9)and non-CIN(n = 9) groups. The GC samples exhibited distinct profiles of lysophosphocholine, phosphocholine, phosphatidylethanolamine,phosphatidylinositol, phosphoserine, sphingomyelin, ceramide, and triglycerides compared with their adjacent noncancerous tissues. The glycerophospholipid levels(phosphocholine, phosphatidylethanolamine, and phosphatidylinositol)were 1.4-to 2.3-times higher in the CIN group compared with the non-CIN group(P < 0.05). Background and purpose NOTCH3 p.R544C mutation accounts for 5%of spontaneous intracerebral haemorrhage(ICH)in East Asian *** investigated whether certain CT features are associated with NOTCH3-related *** Patients with spontaneous ICH from a prospective stroke registry were screened for NOTCH3 p.R544C *** neuroimaging features on the initial non-contrast CT scans selected to predict NOTCH3 p.R544C mutation,including burden of white matter lesion(WML),degree of brain atrophy,number of lacunes,prominent juxtacortical WML and prominent lobar lacunes,were analysed by neuroradiologists blinded to the mutation *** Of 299 patients with spontaneous ICH(mean age,61 years;male,68%;ICH volumes,14.1±17.8 mL),13 patients(4.3%)carried NOTCH3 p.R544C *** clinical features,haematoma size and location were similar between NOTCH3 p.R544C mutation carriers and *** CT scan revealed that patients with NOTCH3 p.R544C mutation had more severe WML and more frequently had prominent juxtacortical WML(69.2%vs 17.8%,p<0.001),and the effects were not driven by ageing as seen in patients without *** juxtacortical WML(area under receiver operating characteristic curve=0.76)outperformed the total WML score and prominent lobar lacunes and significantly predicted NOTCH3 p.R544C mutation in a multivariable-adjusted model(OR,20.9;95%CI 4.94 to 88.6).Conclusion In patients with spontaneous ICH,the severity and topographic distribution of WML can help in identifying potential NOTCH3 mutation-related ICH.

Process development for producing a food-grade glucose solution from rice straws

In this paper, processes for producing a food-grade glucose solution through enzymatic hydrolysis of celluloserich solids obtained from rice straws are presented. The rice straws were pretreated by acid-catalyzed steam explosion, and the reaction efficiency, toxicity control, and process economic feasibility were studied. Mass transfer resistance to the hydrolysis reaction was reduced by grinding with glass beads. A higher glucose concentration could be obtained by feeding more cellulose in the hydrolysis reaction; however, this also resulted in the production of undesired byproducts. Thus, a soaking process for the cellulose solids in water was developed to effectively reduce the generation of byproducts in the hydrolysis reaction. The resulting food-grade glucose solution can provide 414 kilocalories per liter, and could be used during a food-shortage crisis in the *** current production cost is estimated to be 0.82 USD·L^(-1).

AIM To investigate the association between a recent gastrointestinal(GI) endoscopy and the subsequent risk of pyogenic liver abscess(PLA).METHODS We designed a nested case control study. Using the Taiwan National Health Insurance Research Database, 2135 patients with a first diagnosis of PLA were identified from 1998 to 2011. Another 10675 patients without PLA matched by age and sex were selected as reference controls. We identified and compared the possible risk factors for PLA and GI endoscopies performed before the index date(when PLA was diagnosed) between the two cohorts. Multivariate analysis was conducted to examine the risk of PLA within the 90 d after the GI *** Patients with a history of diabetes [adjusted odds ratio(a OR) = 4.92, 95%CI: 1.78-13.61], end-stage renal disease(a OR = 3.98, 95%CI: 1.45-10.91), biliary tract infection(a OR = 2.68, 95%CI: 2.11-3.40), liver cirrhosis(a OR = 2.19, 95%CI: 1.39-3.46), GI malignancies(a OR = 5.68, 95%CI: 4.23-7.64), appendicitis(a OR = 3.16, 95%CI: 2.27-4.41), diverticulitis(a OR = 1.64, 95%CI: 1.01-2.64), and recent endoscopic retrograde cholangiopancreatography(a OR = 27.04, 95%CI: 11.65-62.72) were significantly associated with an increased risk of PLA. AIM: To determine changes in the antibiotic resistance of Helicobacter pylori(H. pylori) in southern Taiwan after failure of first-line standard triple ***: We analyzed 137 H. pylori-infected isolates from patients who experienced eradication failure after standard first-line triple therapy from January2010 to December 2014. The H. pylori strains were tested for susceptibility to amoxicillin, clarithromycin,levofloxacin, metronidazole and tetracycline using the E-test method. The minimal inhibitory concentration(MIC) was determined by the agar dilution *** values of ≥ 0.5, ≥ 1, ≥ 1, ≥ 4 and ≥ 8 mg/L were considered to be the resistance breakpoints for amoxicillin, clarithromycin, levofloxacin, tetracycline and metronidazole, ***: A high resistance rate was found for clarithromycin(65%-75%) and metronidazole(30%-40%)among patients who failed first-line standard *** resistance levels to amoxicillin and tetracycline remained very low; however, levofloxacin resistance was as high as 37.5% in 2010 but did not increase any further during the past 5 years. The rates of resistance to these antibiotics did not show a statistically significant upward or downward ***: Antibiotic resistance of H. pylori remains a problem for the effective eradication of this pathogen and its associated diseases in Taiwan. High clarithromycin resistance indicated that this antibiotic should not be prescribed as a second-line H. pylori eradication therapy. Moreover, levofloxacin-based second-line therapy should be used cautiously, and the local resistance rates should be carefully monitored.

随着深度学习技术的发展,开放域对话系统作为人机对话系统的重要分支也得到了快速发展。但目前开放域对话模型生成的回复语句依然存在同理心较差、多样性较低等问题。对此,提出一种融合情感和常识知识的对话生成模型。首先依据情感词典和常识知识图谱获取每个单词对应的常识知识向量,然后将该向量和单词本身的词嵌入向量一同输入编码器中进行编码,接着通过两阶段解码来生成回复语句:第一个解码阶段预测要生成单词的情感强度,并据此获得该单词对应的情感向量,第二阶段解码结合第一阶段编码的结果和已生成单词的词嵌入向量及其对应的常识知识向量作为输入,预测要生成的单词。实验结果表明,该模型生成的回复语句更具同理心和多样性,并且在PPL,BLEU,ACC和DISTINCT等指标上相比基线模型都有一定提升。

AIM To compare the one-week clinical effects of single doses of dexlansoprazole and esomeprazole on grades A and B erosive *** We enrolled 175 adult patients with gastroesophageal reflux disease(GERD). The patients were randomized in a 1:1 ratio into two sequence groups to define the order in which they received single doses of dexlansoprazole(n = 88) and esomeprazole(n = 87) for an intention-to-treat analysis. The primary endpoints were the complete symptom resolution(CSR) rates at days 1, 3, and 7 after drug *** Thirteen patients were lost to follow-up, resulting in 81 patients in each group for the per-protocol analysis. The CSRs for both groups were similar at days 1, 3 and 7. In the subgroup analysis, the female patients achieved higher CSRs in the dexlansoprazole group than in the esomeprazole group at day 3(38.3% vs 18.4%, P = 0.046). An increasing trend toward a higher CSR was observed in the dexlansoprazole group at day 7(55.3% vs 36.8%, P = 0.09). In the esomeprazole group, female sex was a negative predictive factor for CSR on post-administration day 1 [OR =-1.249 ± 0.543; 95%CI: 0.287(0.099-0.832), P = 0.022] and day 3 [OR =-1.254 ± 0.519; 95%CI: 0.285(0.103-0.789), P = 0.016]. Patients with spicy food eating habits achieved lower CSRs on day 1 [37.3% vs 21.4%, OR =-0.969 ± 0.438; 95%CI: 0.380(0.161-0.896), P = 0.027]. CONCLUSION The overall CSR for GERD patients was similar at days 1-7 for both the dexlansoprazole and esomeprazole groups, although a higher incidence of CSR was observed on day 3 in female patients who received a single dose of dexlansoprazole.
