Aberrant activation of nuclear factor of activated T cell 2 in lamina propria mononuclear cells in ulcerative colitis

AIM:To investigate the role of nuclear factor of activated T cell 2(NFAT2),the major NFAT protein in peripheral T cells,in sustained T cell activation and intractable inflammation in human ulcerative colitis(UC). METHODS:We used two-dimensional gel-electrophoresis, immunohistochemistry,double immunohistochemical staining,and confocal microscopy to inspect the expression of NFAT2 in 107,15,48 and 5 cases of UC, Crohn's disease(CD),non-specific colitis,and 5 healthy individuals,respectively. RESULTS:Up-regulation with profound nucleo- translocation/activation of NFAT2 of lamina propria mononuclear cells(LPMC)of colonic mucosa was found specifically in the affected colonic mucosa from patients with UC,as compared to CD or NC(P<0.001,Kruskal- Wallis test).Nucleo-translocation/activation of NFAT2 primarily occurred in CD8+T,but was less prominent in CD4+T cells or CD20+B *** was strongly associated with the disease activity,including endoscopic stage (τ=0.2145,P=0.0281)and histologic grade(τ=0.4167, P<0.001). CONCLUSION:We disclose for the first time the nucleo-translocation/activatin of NFAT2 in lamina propria mononuclear cells in ulcerative *** of NFAT2 was specific for ulcerative colitis and highly associated with disease *** activation of NFAT2is implicated in an auto-regulatory positive feedback loop of sustained T-cell activation and NFAT proteins play key roles in the calcium/calcineurin signaling pathways,our results not only provide new insights into the mechanism for sustained intractable inflammation,but also suggest the calcium-calcineurin/NFAT pathway as a new therapeutic target for ulcerative colitis.

Microbial diversity of soil bacteria in agricultural field contaminated with heavy metals

In this study we evaluated the bacterial diversity in a soil sample from a site next to a chemical industrial factory previously contaminated with heavy metals. Analysis of 16S rDNA sequences amplified from DNA directly extracted from the soil revealed 17 different bacterial types （genera and/or species）. They included Polyangium spp., Sphingomonas spp., Variovorax spp., Hafina spp., Clostridia, Acidobacteria, the enterics and some uncultured strains. Microbes able to tolerate high concentrations of cadmium （500μmol/L and above） were also isolated from the soil. These isolates included strains of Acinetobacter （strain CD06）, Enterobacter sp. （strains CD01, CD03, CD04 and CD08）（similar strains also identified in culture-independent approach） and a strain of Stenotrophomonas sp. The results indicated that the species identified from direct analysis of 16S rDNA of the soil can be quite different from those strains obtained from enrichment cultures and the microbial activities for heavy metal resistance might be more appropriately addressed by the actual isolates.

Effect of transcutaneous electrical nerve stimulation through acupoints of Pucan (BL 61) and Shenmai (BL 62) on intraocular pressure in patients with glaucoma： a randomized controlled trial

OBJECTIVE： To investigate the effects of transcutaneous electrical nerve stimulation （TENS） through acupoints on the intraocular pressure （lOP） in patients with glaucoma. METHODS： A single-blinded, randomized experimental design study was conducted. Patients from the North of Taipei Ophthalmology outpatient department diagnosed with intraocular hypertension were selected and were further enrolled if they met the research subject criteria. The patients who met all criteria, and who met no exclusion criteria became the participants. Participants were randomly assigned into the treated group, wherein the TENS electrodes （Qing Ming Medical Device Co., Ltd., New Taipei City, China） were applied with direct current （DC） on the acupoints Pucan （BL 61） and Shenmai （BL 62） for 20 min. The control group received no TENS electrode treatments. Measurements of lOP were taken four times using a pneumotonometer （Clinico Co., Ltd., New Taipei City, China）, before, immediately after, 30 min after, and 60 min after TENS electrode treatments. RESULTS： Eighty-two participants completed the experiment, lOP decreased more in the group treated with DC （P 〈 0.01） than in the control *** lOPs were not affected by the age of the patients and their usage of medications. CONCLUSION： TENS of acupoints of Pucan （BL 61） and Shenmai （BL 62） could significantly reduce the lOP in patients with glaucoma.

Clinical outcome of intersphincteric resection for ultra-low rectal cancer

AIM： To analyze oncological outcome of intersphincteric resection （ISR） in ultra-low rectal cancer with intent to spare colostoma. METHODS： From 1995 to 1998, patients with a nonfixed rectal adenocarcinoma （tumor stage T2） preserving the lower margin at 1-3 cm above the dentate line without distant metastasis was enrolled （period I）. ISR was practiced in eight patients, and their postoperative followup was at least 5 years. In addition, from 1999 to 2003, another 10 patients having the same tumor location as period Ⅰ underwent ISR （period Ⅱ）. Among those, 6 patients with T3-4-staged tumor received preoperative chemoradiotherapy. RESULTS： All patients received ISR with curative intention and no postoperative mortality. In these case series at period Ⅰ, local recurrence rate was 12.5% and metastasis rate 25.0%; the S-year survival rate was 87.5% and disease-free survival rate 75.0%. There was no local recurrence or distant metastases in 10 patients with a median follow-up of 30 （range, 18-47） mo at period Ⅱ. CONCLUSION： As to ultra-low rectal cancer, intersphincteric resection could provide acceptable local control and cancerrelated survival with no permanent stoma in early-staged tumor （tumor stage T2）; more- over, preoperative concurrent chemoradiotheraw would make ISR feasible with surgical curative intent in more advanced tumors （tumor stages T3-4）.

Peanut consumption and reduced risk of colorectal cancer in women:A prospective study in Taiwan

AIM： To examine whether peanut consumption is associated with a reduced risk of colorectal cancer in a prospective cohort with a 10-year follow-up. METHODS： In 1990-1992, residents （12026 men and 11917 women aged 30 to 65 years） in 7 townships, Taiwan, were interviewed and recruited into a cancerscreening cohort and annually followed up. Colorectal cancer cases in this cohort were identified from cancer registry and death certificates. Incidence rates of this disease by the end of 2001 were calculated by gender for the primary study variable and covariates. The dietary intake was assessed by means of weekly food frequency measures, including frequently consumed food groups and folk dishes including sweet potato, bean products, peanut products, pickled foodstuffs, nitrated or smoked foodstuffs. RESULTS： During the study period, 107 new colorectal cancer cases （68 men and 39 women） were confirmed. The multivariate Cox＇s proportional hazard model showed that the relative risk （RR） of peanut consumption was 0.73 [95% confidence interval （CI） = 0.44-1.21] for men and 0.42 （95% CI = 0.21-0.84） for women. However, frequent intake of pickled foodstuffs was harmful for women （RR= 2.15, 95% CI=0.99-4.65）. The risk of colorectal cancer was also elevated among cigarette smokers but not significant （P〈 0.05）.CONCLUSION： This study suggests that frequent intake of peanut and its products may reduce colorectal cancer risk in women, demonstrating the anti-proliferating effect of peanut intake.

Meteorological influences on dengue transmission in Pakistan

Objective: To identify the influences of local and regional climate phenomena on dengue transmission in Lahore District of Pakistan, from 2006 to 2014. Methods: Time-series models were applied to analyze associations between reported cases of dengue and climatic parameters. The coherence trend of regional climate phenomena(IOD and ENSO) was evaluated with wavelet analysis. Results: The minimum temperature 4 months before the dengue outbreak played the most important role in the Lahore District(P=0.03). A NINO 3.4 index 9 months before the outbreaks exhibited a significant negative effect on dengue transmission(P=0.02). The IOD exhibited a synchronized pattern with dengue outbreak from 2010 to 2012. The ENSO effect(NINO 3.4 index) might have played a more important role after 2012. Conclusions: This study provides preliminary results of climate influences on dengue transmission in the Lahore District of Pakistan. An increasing dengue transmission risk accompanied by frequent climate changes should be noted. Integrating the influences of climate variability into disease prevention strategies should be considered by public health authorities.

AIM:To identify the mucosal patterns of Helicobacter pylori(*** )-related gastritis in the gastric corpus using standard endoscopy and to evaluate their ***:A total of 112 consecutive patients underwent upper gastrointestinal *** endoscopists classified the endoscopic findings into 4 *** the second part of the study,90 images were shown to 3 endoscopists in order to evaluate the inter-observer and intra-observer variability in image ***:The mucosal patterns of the gastric bodywere categorized into 4 *** 1 pattern was defined as cleft-like appearance,type 2 as regular arrangement of red dots,type 3 pattern as the mosaic mucosal pattern and type 4 pattern as the mosaic pattern with a focal area of *** 1 and type 2 mucosal patterns were statistically significant in predicting *** status as compared with other mucosal types(χ 2 = 12.79 and 61.25 respectively,P < 0.01).Type 3 and type 4 mucosal patterns were statistically significant in predicting a ***-positive status as compared with other mucosal types(χ 2 = 21.22 and 11.02 respectively,P < 0.01).Furthermore,the sensitivity,specificity,positive and negative predictive values of type 3 plus type 4 patterns for predicting ***-positive gastric mucosa were 100%,86%,94%,and 100%,*** mean κ values for inter-and intra-observer agreement in assessing the various endoscopic patterns were 0.808(95% CI,0.678-0.938) and 0.826(95% CI,0.727-0.925) ***:Our study suggests that mucosal patterns in ***-infected gastric mucosa without atrophy can be reliably identified using standard endoscopy in the gastric corpus.

Dynamic Gain-Clamped Amplifier with Backward-Injection of a Fabry-Perot Laser

We have been experimentally demonstrated an active control technique of dynamic gain-clamped spectra for theerbium-doped fiber amplifiers (EDFA's) by a backward-injected Fabry-Perot (F-P) laser into this EDFA. In addition, employing a short length erbium-doped fiber (EDF) that not cause any gain saturation for preamplification in front of this amplifier module, it can reduce the noise figure degradation and simultaneously achieve gain variation from 11.1 dB to 0.5 dB for 10 dB input power level change.

A high-fidelity design methodology using LES-based simulation and POD-based emulation:A case study of swirl injectors

Engineering design is undergoing a paradigm shift from design for performance to design for affordability, operability, and durability, seeking multi-objective optimization. To facilitate this transformation, significantly extended design freedom and knowledge must be available in the early design stages. This paper presents a high-fidelity framework for design and optimization of the liquid swirl injectors that are widely used in aerospace propulsion and power-generation systems. The framework assembles a set of techniques, including Design Of Experiment(DOE), high-fidelity Large Eddy Simulations(LES), machine learning, Proper Orthogonal Decomposition(POD)-based Kriging surrogate modeling(emulation), inverse problem optimization, and uncertainty quantification. LES-based simulations can reveal detailed spatiotemporal evolution of flow structures and flame dynamics in a high-fidelity manner, and identify important injector design parameters according to their effects on propellant mixing, flame stabilization, and thermal *** a given a space of design parameters, DOE determines the number of design points to perform LES-based simulations. POD-based emulations, trained by the LES database, can effectively explore the design space and deduce an optimal group of design parameters in a turn-around time that is reduced by three orders of magnitude. The accuracy of the emulated results is validated, and the uncertainty of prediction is quantified. The proposed design methodology is expected to profoundly extend the knowledge base and reduce the cost for initial design stages.
